1.Progress on drug treatment for corrosive esophageal stenosis in children

Children who ingest corrosive substances by mistake usually have a small amount of exploratory ingestion,so perforation caused by injury is rare,and emergency surgical treatment is rarely required in the acute stage.But it is very easy to cause esophageal stenosis in the long term.Esophageal stenosis can last for weeks to months,and the children's difficult swallowing can lead to chronic pain in the esophagus and long-term malnutrition.At present,the main clinical treatment is endoscopic esophageal dilation,which requires multiple dilation treatments.The treatment period of corrosive esophageal stenosis in children is long,if supplemented with effective drugs during treatment,it is expected to reduce the degree of esophageal stenosis,reduce the number of dilation,and shorten the treatment period.On the other hand,early administration of protective drugs can promote benign wound healing after esophageal injury.This article reviews the research progress on drug treatment for corrosive esophageal stenosis in children.
2.Prediction of very early recurrence of pancreatic head cancer after radical surgery by preoperative enhanced CT imaging features and postoperative pathology

Objective:To predict very early recurrence after radical resection in patients with pancreatic head cancer by combining preoperative enhanced CT imaging findings and clinicopathological features.Methods:The clinicopathological and preoperative enhanced CT imaging features of 241 patients who underwent radical pancreaticoduodenectomy at Peking University Third Hospital from Jan 2010 to Dec 2022 were retrospectively collected and analyzed.Results:Using recurrence within 3 months after surgery as the definition for very early recurrence, 49 out of 241 patients experienced very early recurrence. Multivariate analysis showed that preoperative BMI<18.5 kg/m2( P=0.040), rim enhancement( P=0.028), lymphovascular invasion( P<0.001), and poor tumor differentiation ( P=0.013) were independent risk factors for very early recurrence. Conclusion:Enhanced CT imaging features combined with clinicopathological characteristics can predict very early recurrence after resection of pancreatic head cancer.
3.Optimization of Molding Process and Determination of Volatile Components Assay in Compound Shexiang Huangqi Dropping Pills by A Central Composite Design-Response Surface Method

Objective The molding process of compound Shexiang Huangqi dropping pills was optimized by central composite design and response surface method,the determination of volatile components in the compound by gas chromatography was established in order to improve the quality standard of the compound.Methods Single factor method was used to select the optimum range of matrix type,the ratio of matrix to liquid,and drop distance of compound Shexiang Huangqi dropping pills;Appearance traits,a difference of pill weight,and dissolution time were used as evaluation indexes,the optimum forming process conditions of compound Shexiang Huangqi dropping pills were optimized by central composite design and response surface method;Three batches of compound Shexiang Huangqi dropping pills were taken as test samples and determined by gas chromatography;The gas chromatographic column was HP-5 sillica capillary column,the inlet temperature was 260 ℃,the temperature was programmed,and the detector temperature was 300 ℃,the split ratio was 10∶1,nitrogen was selected as the carrier gas with a flow rate of 1.0 mL·min-1.Results The optimum forming process conditions of compound Shexiang Huangqi dropping pills were selected by central composite design and response surface method as a matrix:matrix=0.99,drug:matrix=0.55,drop distance=6.00,and the comprehensive score were 0.845 2.Under these conditions,the quality of the prepared dropping pills was the best;The chromatographic peaks of borneol,muscone,and ligustilide reached the baseline separation;the linear ranges of the three components were 0.327-1.962,0.140-0.840,0.710 5-4.263 μg(all r>0.999);The average recoveries were 92.30%(RSD=1.65%,n=6),101.28%(RSD=0.81%,n=6)and 98.99%(RSD=0.65%,n=6);The average contents of the three components were 0.201 7,0.084 7 and 1.382 9 mg·g-1,respectively.Conclusions The forming process is stable and feasible,which can provide a reference for the development and application of compound Shexiang Huangqi dropping pills;The gas chromatography method established can simultaneously determine the contents of borneol,muscone,and ligustilide in compound Shexiang Huangqi dropping pills,which can be used for quality control of compound Shexiang Huangqi dropping pills.
4.Preliminary Study on Influencing Factors of Textbook Outcome After Minimally Invasive Pancreaticoduodenectomy

Objective To investigate influencing factors of textbook outcome(TO)in patients with pancreatic ductal adenocarcinoma undergoing minimally invasive pancreaticoduodenectomy(MIPD).Methods A retrospective analysis was conducted on the clinical data of 101 cases of pancreatic ductal adenocarcinoma treated with MIPD in our hospital from January 2020 to December 2022.According to the inclusion and exclusion criteria,89 cases were ultimately included in this study,of which 61 cases reached TO(TO group)and 28 cases did not reach TO(non-TO group).Variables with P<0.05 in univariate analysis were included in multivariate logistic regression analysis to identify independent prognostic factors of TO.Results Univariate analysis showed that there were significant differences in pancreatic duct dilation>3 mm,preoperative neutrophil lymphocyte ratio(NLR),extended hospital stay,postoperative hospitalstay,and drain fluid amylase(DFA)>1100 U/L at1-3 d aftersurgery(P<0.05).Multivariate logistic regression analysis showed that the independent prognostic factors affecting TO were:pancreatic duct dilation>3 mm(OR=7.290,95%CI:1.485-35.786,P=0.014),postoperative hospital stay(OR=0.862,95%CI:0.751-0.989,P=0.034),and the DFA on the first postoperative day>1100 U/L(OR=0.052,95%CI:0.005-0.545,P=0.014).Conclusions The outcome of TO in patients after MIPD is not related to the surgical approach(robot assisted minimally invasive pancreaticoduodenectomy or laparoscopic pancreaticoduodenectomy).Pancreatic duct dilation>3 mm,postoperative hospital stay,and DFA on the first postoperative day>1100 U/L are independent prognostic factors of TO after MIPD in patients with pancreatic ductal adenocarcinoma.

8.Comparison of value of three methods alone and their combination in diagnosis of thyroid micronodules

Objective To compare the value of two-dimensional high-frequency ultrasound, acoustic pulsed radiant force imaging (ARFI)-acoustic palpation tissue quantification (VTQ) technology, and strain elastography alone and their combination in diagnosis of benign and malignant thyroid micronodules. Methods A retrospective analysis was conducted in 99 patients (107 thyroid micro-solid nodules with diameters ≤ 10.0 mm) who were treated at Baoji People's Hospital in Shaanxi Province from June 2021 to June 2023. All patients underwent two-dimensional high-frequency ultrasound, ARFI-VTQ technology, and strain elastography to obtain ultrasound data scores and data. Using biopsy and surgical pathology as the diagnostic "gold standard", the diagnostic efficacy of the three ultrasound methods alone and their combination was compared for thyroid micro-nodules. Results A total of 62 patients (68 nodules) underwent fine-needle aspiration biopsy (FNAB), and 37 patients (39 nodules) underwent surgery. Among 107 thyroid micro-nodules, 45 were diagnosed as malignant, and 62 as benign. 9.Recent advance in mild traumatic brain injury

Mild traumatic brain injury (mTBI) is a traumatic disease that has transient impairment of consciousness and memory function after an external force to the head with basically normal neurological function and neuroimaging examination results. At present, research on mTBI is insufficient, resulting from early diagnosed difficulty of mTBI patients (due to mild clinical manifestations) and not suitable traditional diagnosis and treatment evaluation methods. This article reviews the research progress in the pathogenesis, auxiliary examinations, treatment principles and related diseases of mTBI, aiming to provide new theoretical basis for diagnosis and treatment of mTBI.
10.Over the top reconstruction combined with modified Lemaire technique in the treatment of anterior cruciate ligament injury with pivot-shift positive

Objective:To investigate the clinical efficacy of over-the-top reconstruction combined with the modified Lemaire technique in the treatment of anterior cruciate ligament (ACL) injuries with pivot-shift positive.Methods:From March 2020 to October 2021, a total of 46 patients with ACL injury and pivot-shift test grade II or above were admitted to Xinhua Hospital Affiliated to Dalian University. There were 28 males and 18 females, aged 28.0±10.5 years (range, 15-45 years). All cases were unilateral, including 17 cases of left knee and 29 cases of right knee. The pivot-shift test showed that 30 cases were grade II and 16 cases were grade III, and the cause of injury was sports injury. The semitendinosus muscle and gracilis muscle were harvested, and the ACL was reconstructed with the over-the-top combined modified Lemaire technique. The International Knee Documentation Committee (IKDC) score, Lysholm score and KT-2000 side-to-side difference before and after operation were compared.Results:All patients successfully completed the operation and were followed up for 26.6±2.3 months (range, 24-28 months). The Lysholm scores of the patients at 3 months and 24 months after operation were 73.6±4.3 and 91.6±2.8, which were higher than those before operation (58.5±4.6), and the difference was statistically significant ( F=18.351, P<0.001). The IKDC scores of patients at 3 months and 24 months after operation were 59.0±2.0 and 91.8±3.2, respectively, which were higher than those before operation 50.3±2.8, and the difference was statistically significant ( F=17.290, P<0.001). The side-to-side difference of KT-2000 was 1.7±0.8 mm and 1.5±0.4 mm at 3 and 24 months after operation, respectively, which was lower than that before operation (5.9±1.1 mm), and the difference was statistically significant ( F=14.192, P<0.001). At 24 months after operation, 3 patients had pivot shift test grade I and 4 patients had Lachman test grade I, but they complained of good knee stability and did not receive further treatment. At the last follow-up, there were no complications such as incision and intra-articular infection, deep vein thrombosis, knee stiffness, quadriceps musculus ossificans myositis, and reconstruction ligament rupture. All patients returned to sports with an average time of 15.7±2.6 months (range, 12-24 months). Conclusion:Over-the-top reconstruction combined with the modified Lemaire technique for the treatment of ACL injury with positive pivot shift test effectively improves knee function and promotes the patient's return to sports, with a low incidence of surgical complications.
