1.Interpretation of the " Shang Guan Shang" Pulse in Treatise on Cold Pathogenic and Miscellaneous Diseases

The Treatise on Cold Pathogenic and Miscellaneous Diseases (Shanghan Zabing Lun) records a unique system of pulse diagnosis, pioneering the " site for taking wrist pulse (Cunkou)-Anterior tibial (Fuyang)-Shaoyin" three-part pulse diagnosis method. This system emphasizes the application of the wrist pulse site, classifying pulse conditions into two major categories, yin and yang, thereby laying a solid foundation for the development of traditional Chinese medicine diagnostics. Studying the pulse theory of Shanghai Zabing Lun is of great significance for understanding, inherting, and developing ZHANG Zhongjing′s medicine and clinical diagnosis. However, owing to the passage of time, the precise meaning of certain pulse conditions described by ZHANG Zhongjing, such as the " shang guanshang" pulse, remains unclear. This article explores the location and clinical significance of the " shangguan shang" pulse as referenced in Treatise on Cold Pathogenic and Miscellaneous Diseases and Synopsis of Golden Chamber (Jingui Yaolue). Through textual research, it investigates this pulse and related ones based on medical texts before the Tang Dynasty, such as Inner Canon of Huangdi (Huangdi Neijing) and Classic of Questioning (Nanjing). The findings indicate that the " shangguan shang" pulse is positioned one division before the Guan pulse site. Further analysis of the clinical significance suggests the existence of a pulse-taking method involving three body regions and nine pulse-taking sites, subdividing the Cun, Guan, and Chi positions into three distinct sections. This pulse diagnostic method enriches the ways of pulse diagnosis, but further exploration and verfication are needed.

3.Conceptual clarification of the disease and syndrome concept in the Chapter of Simultaneous Treatment of Pulse and Syndrome of Water Qi Disease of Synopsis of Golden Chamber

The Simultaneous Treatment of Pulse and Syndrome of Water Qi Disease chapter of Synopsis of Golden Chamber is regarded as one of its most challenging sections. Although nominally focused on water qi disease, this chapter also discusses yellowish sweating disease, qifen disease, and other diseases. This multiplicity of topics led to the misconception that all these diseases are water qi diseases, complicating the diagnosis and treatment strategies. By distinguishing water qi as both a pathogenic factor and a disease entity, this paper redefines the concept, linking it to the abnormal accumulation of liquid and gaseous water in the body, akin to the disrupted water cycle of the nature. It demonstrates that ZHANG Zhongjing recognizes the primary syndrome, pathogenesis, and therapeutic principles of water qi disease from the generation, aggregation, and dissipation of vaporous water. The study further differentiates water qi disease, yellowish sweating disease, and qifen disease as distinct entities. An analysis of their etiology, pathogenesis, syndromes, and treatment approaches establishes their independence while exploring their interrelations. Moreover, the relationships among the qifen, xuefen, and water phase diseases are clarified. ZHANG Zhongjing′s discussion in the Simultaneous Treatment of Pulse and Syndrome of Water Qi Disease identifies the three diseases around the three " disease of water phase." The clarification of the concepts and relationships of the diseases in the Simultaneous Treatment of Pulse and Syndrome of Water Qi Disease will help to systematically and thoroughly elucidate ZHANG Zhongjing′s principles and thoughts on identifying and treating water qi disease.

6.A preliminary study on the causes of olfactory dysfunction following aesthetic rhinoplasty

Objective:This study aims to evaluate the nasal structural and electrophysiological features of patients with postoperative olfactory dysfunction following aesthetic rhinoplasty.Methods:We retrospectively analyzed the clinical features of 30 outpatients (females, aged 33±6 years) from Beijing Anzhen Hospital and China-Japan Friendship Hospital between 2014 and 2023, who complained of olfactory dysfunction following aesthetic rhinoplasty. The control group was 30 healthy females aged 32±9 years. Psychophysical olfactory test (Sniffin′ Sticks, SS), olfactory and trigeminal event-related potentials (oERPs and tERPs), and acoustic rhinometry were used for evaluating the olfactory function and nasal structure in patients and healthy controls. SPSS 17.0 software was used to compare the difference in olfactory function and nasal structure between the two groups and to analyze the factors related postoperative olfactory dysfunction.Results:There was a significant difference in the scores on psychophysical olfactory test between the patients and controls (10.78±3.90 vs. 33.66±2.42, t=-23.35, P<0.001). ERPs could be evoked in all patients and controls. Patients showed higher amplitudes of N 1 waves in both oERPs and tERPs than controls ( P<0.05 for all), but no differences in the latencies of N 1 and P 2 waves or in the amplitudes of P 2 waves were observed between the two groups ( P>0.05 for all). There was no difference in nasal structure between the two groups ( P>0.05). However, after nasal decongestant, mucosal congestion in the cross-sectional area (CSA) from the nostril to 6 cm level was found more significantly in patients than controls (nasal congestion index 40.00% vs. 1.00%, t=2.09, P=0.047). Better olfactory function was associated with increasing nasal volumes, increasing nasal threshold and anterior nasal turbinate plane CSA( P<0.05 for all). Conclusion:The important factor related to olfactory dysfunction following aesthetic rhinoplasty may be attributed to local mucosal congestion, rather than nasal structural alteration or neurophysiologic deficits in the olfactory pathway.
7.Textual research of"heavy abdomen as if carrying a load of five thousand coins"in Synopsis of Golden Chamber

Medical experts across different dynasties have interpreted the phrase"heavy abdomen as if carrying a load of five thousand coins,"found in the Synopsis of Golden Chamber,in various ways.The primary viewpoint is that the phrase uses exaggerated rhetoric to vividly describe the clinical characteristics of abdominal or low back heaviness in patients with kidney stagnation.However,why ZHANG Zhongjing used"money"as a metaphor,what specific type of coins"money"referred to,why the quantity"five thousand"was selected,and whether this description focused more on the weight or the shape,have not been further discussed by the academic community.This article systematically answers these questions based on the history of the development of ancient Chinese coins,the versions of the Synopsis of the Golden Chamber,and its annotations.Using the metaphor of"five thousand coins"for feeling heavy in the abdomen originates from the ancient people′s general understanding of money.The money in"five thousand coins"specifically refers to the wu zhu qian of the Han Dynasty,and the amount of"five thousand coins"is the savings of common people′s families at that time,and has universal reference significance."Five thousand coins"emphasizes the weight of"five thousand coins"rather than its shape.This study provides a novel perspective and theoretical basis for understanding the original text of the Synopsis of Golden Chamber.
8.Comparison of the prognostic value of 15 nutritional/inflammatory indicators in postoperative cancer patients

Objective To explore and identify the nutritional/inflammatory indicator with the highest predictive potential for overall survival(OS)in postoperative tumor patients so as to provide guidance for postoperative rehabilitation of tumor patients.Methods Data from 3 191 surgical patients were collected,including 15 nutritional/inflammatory indicators.The maximum selection rank statistic method was used to calculate the optimal cut-off values for continuous indicators.The Kaplan-Meier method was used to assess OS,and Cox proportional hazards models were used to analyze the association between the aforementioned 15 indicators and survival.The predictive value of these 15 indicators was evaluated with receiver operating characteristic(ROC)curves and C-index.Results Multivariate analysis showed that all 15 indicators were significantly associated with poorer OS in surgical patients(P<0.05 for all).Time-dependent area under the curve(AUC)and C-index analysis indicated that 3 indicators with the highest predictive potential in OS in postoperative tumor patients were the nutritional risk index(NRI)(C-index:0.597),C-reactive protein-to-albumin ratio(CAR)(C-index:0.587),and C-reactive protein-to-lymphocyte ratio(CLR)(C-index:0.587).The optimal cut-off value for NRI was determined to be 104.31(i.e.,NRI<104.31 suggests malnutrition)with the maximum selection rank statistic method,the optimal cut-off value for CAR to be 0.05(i.e.,CAR≥0.05 suggests a strong inflammatory response,often accompanied by malnutrition),and the optimal cut-off value for CLR to be 1.18(i.e.,CLR≥1.18 suggests a strong inflammatory response).Subgroup analysis indicated that NRI,CAR,and CLR had good correlation with tumor staging,and there were significant differences between tumor node metastasis(TNM)Ⅲ/Ⅳ stage patients and TNM Ⅰ/Ⅱ stage patients when there was a strong inflammatory response or malnutrition.Conclusion In postoperative tumor patients,NRI,CLR,and CAR have high prognostic value.Combining these with the patient's clinical stage,it enables more precise guidance for clinical diagnosis and treatment strategies.
9.Construction of a gene-phenotypic association network of cell adhesion molecule-1-mediated sleep deprivation-induced anxiety-like behaviors with systems genetics

OBJECTIVE To explore the possible molecular mechanism through which amygdala cell adhesion molecule-1(CADM1)is involved in acute sleep deprivation-induced anxiety.METHODS Sixteen 8-week-old C57BL/6J mice were randomly divided into the control group and para-chlorophe-nylalanine(PCPA)-induced acute sleep deprivation experimental group.The PCPA mice were intraperi-toneally injected with PCPA suspension(at a dose of 300 mg?kg-1)between 8∶00 and 9∶00 am for 2 consecutive days while the control mice were injected with the same dose of normal saline.The sleep latency and sleep duration of mice were detected via the righting reflex test.Anxiety-like behaviors were detected by the open field test and elevated plus maze test.The expression level of CADM1 in the mouse amygdala was detected by Western Blot and Immunofluorescence staining.The GeneNet-work database was used to analyze the association between Cadm1 genes and other genes in the mouse amygdala.The key candidate regulatory genes were screened,and the Cadm1-anxiety behavior phenotype network was constructed.The mRNA expression levels of the key candidate regulatory genes were analyzed via qPCR analysis.RESULTS Compared with the control mice,the sleep latency of PCPA mice was significantly prolonged(P<0.01)while the sleep duration was significantly shortened(P<0.01).The activity time and distance of PCPA mice in the open field center were significantly shorter than those of the control group(P<0.05).The elevated plus maze experiment showed that the percentage of the number of times PCPA mice entered the open arm and the percentage of residence time in the open arm were significantly lower than those of the control group(P<0.05).Western Blot and immuno-fluorescence staining showed that the expression of CADM1 protein in the amygdala from PCPA mice was down-regulated compared with the control mice(P<0.05).Based on gene-behavioral association network analysis,Cadm1 was significantly associated with 25 anxiety-like behavior-related genes.The enrichment analysis of Cadm1 co-expression genes showed that Cadm1 was associated with γ-amino-butyric acid GABAergic synaptic pathway(P=4.31e-09),and that the key genes were huntingtin associ-ated protein 1(Hap1)(r=0.705,P=1.09e-08)、inositol 1,4,5-triphosphate receptor type 1(Itpr1)(r=-0.751,P=3.34e-10)、gamma-aminobutyric acid type A receptor subunit delta(Gabrd)(r=-0.836,P=3.93e-14)、γ-aminobutyric acid A receptor β1 subunit gene(Gabrb1)(r=0.732,P=1.50e-09)and adrenoceptor alpha 2A(Adra2a)(r=0.759,P=1.73e-10).The results of qPCR analysis showed that the mRNA levels of Hap1(P<0.05)、Gabrb1 and Adra2a were significantly up-regulated(P<0.01)while those of Itpr1 and Gabrd were significantly down-regulated(P<0.01).CONCLUSION Acute sleep deprivation leads to down-regulation of Cadm1 expression in the amygdala,and induces anxiety-like behaviors by affecting the expression of GABAergic synaptic signaling pathways Hap1,Gabrb1,Adra2a,Itpr1 and Gabrd.
10.Metaphorical analysis of tongue coating terminology and its connotation changes

The terminology and connotation changes of tongue coating are closely associated with the development of the theory of tongue inspection.Previous literature research on tongue inspection has primarily focused on tracing the origins;however,it lacks an analysis of the connotations of terminology changes.This article uses the theory of metaphorical cognition to elucidate the corresponding relationship between tongue coating terminology and its connotation changes.In the process of terminology change from"tongue fur"to"tongue moss,"tongue coating has evolved from a purely pathological concept to one with physiological significance.The relationship between tongue coating and body has changed from the attachment of tongue coating to the surface of the tongue body to the inseparability of tongue coating and body.The characteristic terminology"fur"used by ZHANG Zhongjing contains architectural metaphors,reflecting the characteristic of the tongue coating being flat and slightly higher than the tongue body.During the Ming and Qing dynasties,medical practitioners gradually used the term"moss"to illustrate the organic connection between tongue coating and the tongue body through plant metaphors.They constructed metaphors of water and fire,as well as plants,to explain how tongue coating forms in both physiological and pathological states.Therefore,compared to the"fur,"the"moss"has a richer metaphorical meaning,corresponding to the connotations of tongue coating terminology,which helps explain related physiological and pathological manifestations,and is a key factor affecting the evolution of tongue coating terminology.
