1.Meta-analysis of anterior cervical decompression and fusion ROI-CTM self-locking system in treatment of degenerative cervical spondylosis

OBJECTIVE:Anterior cervical decompression and fusion is a classic surgical method for the treatment of degenerative cervical spondylosis.The use of nail plates increases the fusion rate and stability and indirectly leads to adjacent vertebral degeneration and postoperative dysphagia.In this paper,the clinical results and complications of ROI-CTM self-locking system and traditional cage combined with screw-plate internal fixation in the treatment of degenerative cervical spondylosis were compared by meta-analysis to provide evidence-based support for the selection of internal fixation methods in anterior cervical decompression and fusion. METHODS:CNKI,WanFang,VIP,PubMed,Cochrane Library,Web of Science,and Embase databases were searched for Chinese and English literature on the application of ROI-CTM self-locking system and fusion cage combined with screw plate internal fixation in the treatment of degenerative cervical spondylosis.The retrieval time range was from inception to July 2023.Two researchers selected the literature strictly according to the inclusion and exclusion criteria.The Cochrane bias risk tool was used to evaluate the quality of randomized controlled trials.Newcastle-Ottawa Scale was used to assess the quality of cohort studies.Meta-analysis was performed using RevMan 5.4 software.Outcome indicators included operation time,intraoperative blood loss,Japanese Orthopaedic Association score,Neck Disability Index,C2-C7 Cobb angle,fusion rate,incidence of adjacent vertebral degeneration,cage subsidence rate,and incidence of dysphagia. RESULTS:Thirteen articles were included,including eleven retrospective cohort studies and two randomized controlled trials,with 1 136 patients,569 in the ROI-C group,and 567 in the cage combined with the nail plate group.Meta-analysis results showed that the operation time(MD=-15.52,95%CI:-18.62 to-12.42,P<0.000 01)and intraoperative blood loss(MD=-24.53,95%CI:-32.46 to-16.61,P<0.000 01)in the ROI-C group and the fusion device combined with nail plate group.Postoperative adjacent segment degeneration rate(RR=0.40,95%CI:0.27-0.60,P<0.000 01)and postoperative total dysphagia rate(RR=0.18,95%CI:0.13-0.26),P<0.000 01)were statistically different.The two groups had no significant difference in Japanese Orthopaedic Association score,Neck Disability Index,C2-C7 Cobb angle,fusion rate,or cage subsidence rate(P≥0.05). CONCLUSION:Applying an ROI-CTM self-locking system and traditional cage combined with plate internal fixation in anterior cervical decompression and fusion can achieve satisfactory clinical results in treating degenerative cervical spondylosis.The operation of the ROI-CTM self-locking system is more straightforward.Compared with a cage combined with plate internal fixation,the ROI-CTM self-locking system can significantly reduce the operation time and intraoperative blood loss and has obvious advantages in reducing the incidence of postoperative dysphagia and adjacent segment degeneration.The ROI-CTM self-locking system is recommended for patients with skip cervical spondylosis and adjacent vertebral disease.However,given its possible high settlement rate,using a fusion cage combined with screw-plate internal fixation is still recommended for patients with degenerative cervical spondylosis with multiple segments and high-risk factors of fusion cage settlement,such as osteoporosis and vertebral endplate damage.
2.Clinical effect of personalized pars plana vitrectomy for proliferative diabetic retinopathy

Objective To explore the clinical effect of personalized pars plana vitrectomy(PPV)for proliferative di-abetic retinopathy(PDR).Methods In this retrospective case study,76 patients(86 eyes)diagnosed with PDR and re-ceiving PPV in the Department of Ophthalmology of Songjiang Hospital affiliated to Shanghai Jiao Tong University School of Medicine,from October 2019 to November 2022,were divided into the observation group(40 patients,46 eyes)and the control group(36 patients,40 eyes).Patients in the obseration group were treated with personalized PPV,while patients in the control group were treated with conventional PPV,After treatment,all patients were followed up for 12 months.The operation time,intraoperative use of heavy water and silicone oil,incidence of iatrogenic retinal tears and heavy water resi-dues,proportion of scleral buckling,preoperative and postoperative best corrected visual acuity(BCVA)and intraocular pressure(IOP),retinal reattachment rate at 12 months after surgery,and the incidence of post-vitrectomy vitreous hemor-rhage(PVH),diabetic macular edema(DME)and neovascular glaucoma(NVG)were compared between the two groups.Results The operation time of patients in the observation group was shorter than that in the control group(P<0.05).Intraoperative use of heavy water and silicone oil in the observation group was lower than that in the control group(both P<0.05).The incidence of iatrogenic retinal tears and heavy water residues and the proportion of scleral buckling showed no statistically significant difference between the two groups(all P>0.05).There was no statistically significant difference between the two groups in BCVA preoperatively,3,6 and 12 months postoperatively(all P>0.05).BCVA in the observa-tion group was better than that in the control group at 1 day,1 week and 1 month after surgery(all P<0.05).Compared with the preoperative value,BCVA increased in the observation group at 1 day,1 week,1 month,3 months,6 months,and 12 months after surgery(all P<0.05);in the control group,BCVA increased slightly at 1 day and 1 week(both P>0.05)and then increased significantly at 1 month,3 months,6 months,and 12 months after surgery(all P<0.05).The two groups showed no statistically significant difference in IOP at 1 day,1 week,1 month,3 months,6 months,and 12 months postoperatively(all P>0.05).There was no statistically significant difference in the retinal reattachment rate and the inci-dence of complications such as PVH,DME,and NVG between the two groups at 12 months postoperatively(all P>0.05).Conclusion Personalized PPV can shorten the operation time,reduce the intraoperative use of heavy water and silicone oil,enhance the efficiency of the operation,and rapidly improve the visual acuity of PDR patients.
3.Research progress of lactate dehydrogenase in the diagnosis and treatment of gastric cancer

China is the rank 1st of gastric cancer in the world,and the incidence and mortality of gastric cancer rank the 3rd among various malignant tumors in China.It has been found that tumor cells have their own energy metabolism characteristics.Even in the presence of sufficient oxygen,tumor cells are more inclined to use glycolysis to produce energy,also known as"aerobic glycoly-sis".Aerobic glycolysis can lead to an increase in lactate,promoting the acceleration of tumor cell proliferation and invasiveness,and the key enzyme driving this phenomenon is lactate dehydrogenase(LDH).LDH levels are significantly elevated in patients with gas-tric cancer,and LDH can contribute to the occurrence and development of gastric cancer in many ways.This article will discuss the role of LDH in tumor glycolysis,which is correlated with gastric cancer as well as its clinical application value.It will also discuss the research progress of LDH in targeted therapy for gastric cancer.
4.Study on the Safety and Influencing Factors of Home Medication for Elderly Patients with Chronic Diseases in Urban Areas of Henan:A Cross-sectional Survey

Objective To evaluate the status of home medication safety among elderly patients with chronic diseases and analyze its influencing factors in urban and rural areas of Henan Province.Methods Convenient sampling method was adopted.Data was collected through a designed and optimized questionnaire.Pharmacists conducted in-home surveys.Excel and SPSS 26.0 software were used for data analysis.Results A total of 352 valid questionnaires were analyzed.Most respondents were aged 60~70 years,with a female proportion of 56.2%,and 52.0%of them had a junior high school education or lower.The top-ranked chronic diseases were hypertension(62.2%)and hyperlipidemia(33.2%),and 61.1%of patients suffered from multiple chronic diseases.Antipyretic and analgesic drugs were most common in home medicine cabinets(90.1%).The proportion of drugs being stored in accordance with the instructions(46.9%)was slightly low,and 93.8%of the elderly handled expired drugs improperly.Awareness of medication guidance services was low(34.9%).Logistic regression analysis showed that factors such as education level and monthly income had significant effects on the understanding of drug package insert,rational drug storage,proper disposal of expired drugs,and awareness of medication guidance services(P<0.05).Conclusion There are still some potential safety risks in the home medication use of elderly patients with chronic diseases in urban and rural areas of Henan Province.It is necessary to enhance education on drug storage and medication guidance,particularly for low-income and less-educated groups,to improve the safety of home medication use.
5.Efficacy of Ortho-Bridge system in the treatment of scapular fractures

Objective:To investigate the clinical effects of Ortho-Bridge system in the treatment of scapular fractures.Methods:A retrospective analysis was performed for the 23 patients with scapular fracture who had been treated with Ortho-Bridge system and followed up completely at Department of Orthopedics, Suzhou Kowloon Hospital, Shanghai Jiao Tong University School of Medicine from May 2020 to May 2023. They were 19 males and 4 females with an age of (40.3±11.6) years. The modified Judet approach was adopted in 9 cases and minimally invasive approach in 14 cases in the surgery. The time from injury to surgery was 3.0 (0, 4.0) days. Related perioperative indicators were recorded, including surgery time, intraoperative bleeding, postoperative complications, and fracture healing time. Postoperatively, Constant shoulder score (CSS) and the quick disabilities of the arm, shoulder and hand (quickDASH) were used to quantify the postoperative efficacy.Results:All the 23 patients were followed up for (30.1±12.0) months. The surgery time averaged (136.3±32.0) minutes and the intraoperative bleeding (178.3±50.3) mL. The rate of postoperative fracture healing was 100% (23/23), and the fracture healing time 4.0 (3.0, 4.0) months. After surgery, one patient developed superficial infection which was cured by oral antibiotics and dressing change. No other adverse complications occurred. The CSS of the shoulder at the last follow-up was (91.7±3.0) points. The quickDASH score of the upper limb function at the last follow-up averaged (10.4±3.6) points, giving an excellent to good rate of 91.3% (21/23).Conclusion:In the treatment of scapular fractures, internal fixation with Ortho-Bridge system can result in excellent clinical effects and a low incidence of postoperative complications, because it is simple to handle, limitedly traumatic, flexible and versatile in combination of internal fixation and very personalized.
6.The application value of multimodal MRI in the preoperative staging of cervical cancer

Objective To explore the application value of multimodal MRI in the preoperative staging of cervical cancer.Methods The preoperative imaging data of 60 patients with cervical cancer confirmed by surgical pathology or biopsy were analyzed retro-spectively.The results of preoperative staging by multimodal MRI and postoperative pathological staging were compared.The diag-nostic accuracy of multimodal MRI in staging of cervical cancer was evaluated.Results Among the 60 cases,the preoperative staging results of multimodal MRI were 25 ⅠB,20 ⅡA1,9 ⅡA2,and 6 ⅢC.While the postoperative pathological staging results were 27 Ⅰ B,18 ⅡA1,9 ⅡA2,and 6 ⅢC.The staging results achieved by preoperative multimodal MRI and postoperative pathological staging were in good agreement.With the postoperative pathological staging as the gold standard,the diagnostic accuracy of multimodal MRI in preoperative staging was 100.00%(25/25),85.00%(17/20),77.78%(7/9),and 83.33%(5/6),respectively.Conclusion Multimodal MRI has a high accuracy in the preoperative staging of cervical cancer which can be used as a routine evaluation tool for the accurate preoperative staging of cervical cancer in clinical work.
7.The mechanism of miR-10b targeting TGFBR1/SMAD3 pathway on chondrocyte proliferation and hypertrophy in idiopathic short stature

Objective To investigate the effect and mechanism of microRNA-10b(miR-10b)on idiopathic short stature(ISS).Methods A total of 54 children with ISS and 54 healthy children were collected.The serum expression of miR-10b was detected by RT-qPCR,and the relationship between serum miR-10b expression and clinical data of children with ISS was analyzed.miR-10b inhibitor,si-TGFBR1 and their negative control transfection C28/I2 cells were used.CCK-8 experimental detection was used to detect C28/I2 cell proliferation.Western blot assay was used to detect gnome related transcription factor 2(RUNX2),collagen type X alpha 1 chain(COL10A1),transforming growth factor beta receptor 1(TGFBR1),SMAD3 and pSMAD3 protein expression.The target of miR-10b was screened in StarBase database,and the targeting relationship between miR-10b and TGFBR1 was verified by dual luciferase reporter gene assay.Results The serum expression of miR-10b was higher in the ISS group than that of the healthy control group,and the higher the miR-10b expression,the more obvious the decrease of child height,IGF-1 and alkaline phosphatase(P<0.05).Compared with the NC group,the cell proliferation ability and RUNX2,COL10A1,TGFBR1,and pSMAD3 protein expression were up-regulated in the miR-10b inhibitor group(P<0.05).StarBase database suggested that miR-10b had a binding site of TGFBR1,and dual luciferase reporter gene assay confirmed that TGFBR1 interacted with miR-10b(P<0.05).Compared with the si-NC group,the expression of TGFBR1 was down-regulated and the cell proliferation ability was decreased in the si-TGFBR1 group(P<0.05).Conclusion miR-10b inhibits chondrocyte proliferation and hypertrophy in idiopathic short stature by targeting TGFBR1/SMAD3 pathway.
8.Augmented renal clearance in septic patients with antibiotic blood concentration

Augmented renal clearance(ARC)can be caused by the disease itself,inflammatory state,or therapeutic interventions,and is often associated with severe infections and trauma.Sepsis is one of the most important diseases with high morbidity and mortality in the world.Current studies suggest that ARC is significantly associated with inadequate antimicrobial therapy concentration in patients with sepsis,increasing the risk of clinical treatment failure.It is important to optimize the drug administration strategy for patients with ARC in sepsis,although the current clinical screening of ARC is not yet perfect.This review summarized the literature on changes in blood concentration of antibiotics and administration strategies in patients with sepsis with ARC,in order to provide appropriate medication and clinical guidance for ARC patients.
9.Comparison of the predictive value of four critical scoring systems in predicting the prognosis of children with sepsis

Objective:To analyze the predictive value of four scoring systems,Pediatric Clinical Illness Score (PCIS),Pediatric Logistic Organ Dysfunction Score 2 (PELOD-2),Pediatric Risk of Mortality Ⅲ (PRISM Ⅲ),and Pediatric Sequential Organ Failure Assessment (pSOFA),in the severity and prognosis in pediatric sepsis.Methods:The medical records of 159 children with sepsis who admitted to PICU at the First Hospital of Jilin University between August 2015 and December 2020 were retrospectively analyzed.The PCIS,PELOD-2,PRISM Ⅲ and pSOFA scores were calculated according to the worst value of each index within 24 h of admission.The children were divided into survival group and death group based on clinical outcomes during hospitalization.The children were stratified according to the severity of sepsis into sepsis (no organ dysfunction) group ( n=18) and severe sepsis group ( n=141).The receiver operator characteristic (ROC) curve was used to evaluate the value of PCIS,PELOD-2,PRISM Ⅲ and pSOFA scores in predicting the severity and prognosis in pediatric sepsis. Results:There were 97 (61.01%) cases in the survival group and 62(38.99%) cases in the death group.There were 141(88.68%) children with severe sepsis.The areas under curve (AUCs) of PCIS,PELOD-2,PRISM Ⅲ and pSOFA scores for predicting severe sepsis in children with sepsis were 0.869,0.875,0.672 and 0.933,respectively (all P <0.05).The AUCs of PCIS,PELOD-2,PRISM Ⅲ and pSOFA scores for predicting the death of children with sepsis in PICU were 0.687,0.697,0.716 and 0.656,respectively (all P<0.05). Conclusion:Among the four scoring systems,the pSOFA scoring system had the highest predictive power for the severity of sepsis.The PRISM Ⅲ score had higher value in assessing the risk of death in all patients with sepsis.
10.Effects of GW501516 on the injury of pulmonary artery endothelial cells induced by hypoxia and its mechanism

OBJECTIVE To investigate the effects of the peroxisome proliferator-activated receptors δ (PPARδ) agonist GW501516 on the injury of pulmonary artery endothelial cells (PAECs) induced by hypoxia and its mechanism. METHODS The cytotoxic effects of GW501516 were observed by detecting the relative survival rate of PAECs; the protein expression of PPARδ was determined by Western blot assay. The cellular model of PAECs injury was established under hypoxic conditions; using antioxidant N-acetylcysteine (NAC) as positive control, the effects of GW501516 on cell injury and reactive oxygen species (ROS) production were investigated by detecting cell apoptotic rate, cell viability, lactate dehydrogenase (LDH) activity and ROS levels. Using nuclear factor erythroid 2-related factor 2(Nrf2) activator dimethyl fumarate (DMF) as positive control, PAECs were incubated with GW501516 and/or Nrf2 inhibitor ML385 under hypoxic conditions; the mechanism of GW501516 on PAECs injury induced by hypoxia was investigated by detecting cell injury (cell apoptosis, cell viability, LDH activity), the levels of superoxide dismutase (SOD), glutathione peroxidase (GPx), catalase (CAT), malondialdehyde (MDA) and ROS, the expressions of Nrf2, heme oxygenase-1 (HO-1) and cleaved-caspase-3 (C-caspase-3) protein. RESULTS The results demonstrated that hypoxia inhibited the protein expression of PPARδ (P<0.05), while GW501516 promoted the protein expression of PPARδ in hypoxia- exposed PAECs without obvious cytotoxic effects. GW501516 inhibited the apoptosis of PAECs, improved cell viability, and reduced LDH activity and ROS levels. GW501516 could up-regulate the protein expression of HO-1 in PAECs and the levels of SOD, GPx and CAT, while down-regulated the levels of MDA and ROS by activating the Nrf2 pathway (P<0.05); but Nrf2 inhibitor ML385 could reverse the above effects of GW501516 (P<0.05). GW501516 exerted similar effects to Nrf2 activator DMF in down-regulating the expression of C-caspase-3 and inhibiting the injury of PAECs under conditions of hypoxia (P<0.05). Moreover, Nrf2 inhibitor ML385 reversed the 163.com inhibition effects of GW501516 on PAECs injury (P<0.05). CONCLUSIONS GW501516 can relieve the hypoxia-induced injury of PAECs via the inhibition of oxidative stress, the mechanism of which may be associated with activating Nrf2.
