1.Effect of the nitroglycerin-controlled low central venous pressure technique on cerebral metabolic markers and cerebral blood oxygen saturation in patients undergoing laparoscopic hepatectomy for liver cancer

ObjectiveTo investigate the effect of the nitroglycerin-controlled low central venous pressure (CLCVP) technique on brain metabolic markers and cerebral blood oxygen saturation in patients undergoing laparoscopic hepatectomy for liver cancer, and to reduce the risk of neurological complications. MethodsA total of 105 patients who underwent elective laparoscopic hepatectomy for liver cancer in Haikou Hospital Affiliated to Xiangya Hospital of Central South University from April 2020 to May 2023 were enrolled and randomly divided into CLCVP group with 54 patients and non-CLCVP group with 51 patients. The patients in the CLCVP group were treated with the nitroglycerin CLCVP technique during surgery, while those in the non-CLCVP group were given conventional surgical treatment. The two groups were compared in terms of the following indicators: perioperative indicators; hemodynamic parameters and cerebral oxygen metabolism before anesthesia induction (T0), at 5 minutes after anesthesia induction (T1), at 5 minutes after the beginning of liver parenchyma dissection (T2), at 5 minutes after the end of hepatectomy (T3), and immediately after the end of surgery (T4); the changes in liver function parameters after surgery; the incidence rate of adverse reactions. The independent-samples t test was used for comparison of continuous data between two groups, and the chi-square test was used for comparison of categorical data between two groups; the analysis of variance with repeated measures was used for comparison between multiple time points. ResultsCompared with the non-CLCVP group, the CLCVP group had significantly lower intraoperative blood loss and intraoperative fluid infusion volume (t=5.408 and 7.220, both P<0.05), while there were no significant differences between the two groups in time of operation, anesthesia time, extubation time, resuscitation time and intraoperative urine volume (all P>0.05). Compared with the data at T0, both groups had significant reductions in mean arterial pressure, heart rate, and central venous pressure during surgery (all P<0.05), and compared with the non-CLCVP group, the CLCVP group had significantly lower mean arterial pressure and central venous pressure (P<0.05) and a significantly higher heart rate (P<0.05) at T2 and T3. Compared with the data at T0, both groups had a significant reduction in Ca-jvDO2 at T2 — T4 time points (all P<0.05), while there was no significant difference in Ca-jvDO2 between the two groups at each time point (all P>0.05). Compared with the data at T0, the CLCVP group had a significant reduction in rSO2 at T2 — T4 time points (all P<0.05), and the CLCVP group had a significantly lower level of rSO2 than the non-CLCVP group at T2 — T3 time points (both P<0.05); there were no significant changes in CERO2 and Djv-aBL in either group at each time point (all P>0.05). At 3 and 7 days after surgery, both groups had significant increases in the liver function parameters of aspartate aminotransferase (AST), alanine aminotransferase (ALT), and total bilirubin (TBil) (all P<0.05), and the CLCVP group had significantly lower levels of AST and ALT than the non-CLCVP group (all P<0.05); there was no significant difference in TBil between the two groups at each time point (all P>0.05). There was no significant difference in the incidence rate of perioperative complications between the two groups (χ2=0.729, P=0.394). ConclusionThe application of the nitroglycerin CLCVP technique in laparoscopic hepatectomy for liver cancer can reduce the amount of intraoperative blood loss in patients, but it is necessary to further enhance the monitoring of cerebral blood oxygen saturation during surgery, so as to reduce the risk of neurological complications as much as possible.
2.Design and baseline characteristics of a population-based birth cohort in Shanghai

ObjectiveTo introduce the basic design, development plan and objectives of a population-based birth cohort in Shanghai, and further present the main data and baseline characteristics of enrolled participants in the cohort, and to provide key information for reproductive health-related studies. MethodsThe Shanghai population-based birth cohort initiated on January 1, 2005, included newborns born in Shanghai every year and their parents, and collected information on reproductive health, reproductive treatment, birth characteristics, growth and development status, as well as the incidence, treatment and death of diseases by employing data linkage technology and investigations. This formed a birth cohort spanning the entire life cycle. ResultsAs of October 2022, a total of 2 978 538 newborns and their parents were included in the cohort. Among them, 2 905 135 (97.54%) were naturally conceived (NC), and 73 403 (2.46%) were born through assisted reproductive technologies (ART). The average age of parents was (32.56±4.12) years old for females and (34.62±5.34) years old for males in the ART group, which was higher than (28.02±4.71) years and (30.07±5.54) years for parents in the NC group. Among parents, females and males aged 30 and above accounted for 77.12% and 85.08%, respectively, which were higher than that of parents (35.28% for females and 49.66% for males) in the NC group. Furthermore, the percentage of parents with a college degree or above in the ART group was 73.23% for females and 73.66% for males, which were higher than those in the NC group (49.98% and 50.91%, respectively). The multiple births rate in the ART group was 33.81%, which was higher than that in the NC group (1.88%). The incidence of premature birth and low birth weight in the ART group were 24.47% and 19.08%, respectively, which was higher than that in the NC group (5.47% and 3.73%). ConclusionThe comprehensive collection of reproductive health-related information in the birth cohort in Shanghai can provide essential resources to determine the influence of genetics, environment, reproductive treatment and other related factors on the health of offspring after birth.
3.The influence of adopting CBL teaching based on real patients of otorhinolaryngology on the clinical practice ability of general practice

Objective In order to explore the influence and effect of case teaching method based on real patients in oto-rhinolaryngology on clinical practice ability of general practice.Methods 96 trainees of general practice in otolaryngology de-partment from January 2018 to January 2021 were randomly divided into two groups:a CBL group and a control group.In the CBL group,CBL teaching method was adopted based on real patients in Otorhinolaryngology.Conventional teaching method was used in the control group.Results The theoretical scores of written test and the examination of clinical skill operation scores of the CBL group and the control group were analyzed.There was significant difference between the two groups(P<0.05).The students in CBL group were more satisfied with clinical thinking ability,analysis and problem solving ability,active learning abil-ity,learning efficiency,teacher-student interaction,doctor-patient communication ability and consultation skills than the control group.There was significant difference between the two groups(P<0.05).Conclusion The CBL teaching method can obvi-ously improve the learning enthusiasm and clinical practice ability of general resident training doctors.The teaching quality has been significantly improved.It is of great value to cultivate excellent general practitioners.
4.Giant extralobar pulmonary sequestration with severe pleural effusion in neonatal period: A case report

Giant extralobar pulmonary sequestration in newborns is still relatively rare in pulmonary diseases, and there are few relevant studies published. A neonate with the giant extralobar pulmonary sequestration accompanied by severe pleural effusion was reported here. After 12 days of birth, the diseased lung tissue was surgically extracted. The patient had an uneventful postoperative recovery and was discharged from the hospital. The case shows the advantage of early surgical treatment to extralobar pulmonary sequestration with severe pleural effusion in neonatal period.

Diabetes, hypertension, and dyslipidemia, collectively referred to the " Three Highs, " represent increasingly prevalent metabolic risk factors in China. Many individuals experience all three conditions concurrently, significantly heightening the risk of cardiovascular disease and mortality. Although the National Metabolic Management Center(MMC) has been established for over eight years and has its unique features, the awareness, treatment, and control rates of these diseases in China remain low, and the efficiency of community management is insufficient. According to the previous two editions of management guidelines and the most recent domestic and international diagnostic and treatment guidelines, this paper conducts an in-depth analysis of the operational experience and management strategies of the MMC. Its aim is to improve the efficiency of grassroots MMC mode management for " Three Highs" patients and ensure that patients receive more standardized management.

Objective To investigate the inhibitory effect and elucidate the molecular mechanism of 6-gingerol on human multiple myeloma cells.Methods The human multiple myeloma cell lines RPMI 8226 and ARH77 were cultured in vitro,followed by treatment with varying concentrations(50,100,200,300,400 μmol/L)of 6-gingerol.The inhibitory effect on cell proliferation was assessed using the CCK-8 assay.Flow cytometry was employed to evaluate cell apoptosis and cycle distribution.Additionally,qRT-PCR and Western blotting techniques were utilized to analyze gene and protein expression levels.Results The proliferation of RPMI 8226 and ARH77 cells was dose-and time-dependently inhibited by 6-gingerol,leading to the induction of apoptosis with statistically significant differences(P<0.05).Further mechanistic investigations revealed that treatment with 6-gingerol arrested RPMI 8226 cells in the G0/G1 phase,resulting in a significant increase in Bax levels and a decrease in Bcl-2 mRNA and c-Myc mRNA levels(P<0.05).Additionally,it significantly upregulated the expression of Bax,Cleaved-PARP,Cleaved-caspase3,P53,and p-AKT proteins while down regulating the expression of Bcl-2 protein(P<0.05).Conclusions The compound 6-Gingerol exhibits inhibitory effects on the proliferation and induction of apoptosis in MM cells,as well as cell cycle arrest at the G0/G1 phase.Its mechanism of action is likely associated with the suppression of the AKT signaling pathway,downregulation of Bcl-2 family protein expression,and inhibition of c-Myc expression.

9.Clinical analysis of 11 cases of otogenic intracranial complications treated by multidisciplinary collaboration.

Objective:To analyze the clinical diagnosis, treatment ,and surgical timing of otogenic intracranial complications. Methods:The clinical data of 11 patients with intracranial complications with ear symptoms as the first manifestation in Department of Otorhinolaryngology Head and Neck Surgery, Qilu Hospital of Shandong University(Qingdao) from December 2014 to June 2022 were collected, including 8 males and 3 females, aged from 4 to 69 years. All patients had complete otoendoscopy, audiology, imaging and etiology examination, and the diagnosis and treatment plan was jointly developed through multidisciplinary consultation according to the critical degree of clinical symptoms and imaging changes. Among the 11 patients, 5 cases were treated with intracranial lesions first in neurosurgery department and middle ear lesions later in otolaryngology, 3 cases of meningitis, were treated with middle ear surgery after intracranial infection control, 1 case was treated with middle ear lesions and intracranial infection simultaneously, and 2 cases were treated with sigmoid sinus and transverse sinus thrombosis conservatively. They were followed up for 1-6 years. Descriptive statistical methods were used for analysis. Results:All the 11 patients had ear varying symptoms, including ear pain, pus discharge and hearing loss, etc, and then fever appeared, headache, disturbance of consciousness, facial paralysis and other intracranial complication. Otoendoscopy showed perforation of the relaxation of the tympanic membrane in 5 cases, major perforation of the tension in 3 cases, neoplasia in the ear canal in 1 case, bulging of the tympanic membrane in 1 case, and turbidity of the tympanic membrane in 1 case. There were 4 cases of conductive hearing loss, 4 cases of mixed hearing loss and 3 cases of total deafness. Imaging examination showed cholesteatoma of the middle ear complicated with temporal lobe brain abscess in 4 cases, cerebellar abscess in 2 cases, cholesteatoma of the middle ear complicated with intracranial infection in 3 cases, and sigmoid sinus thrombophlebitis in 2 cases. In the etiological examination, 2 cases of Streptococcus pneumoniae were cultured in the pus of brain abscess and cerebrospinal fluid, and 1 case was cultured in streptococcus vestibularis, Bacteroides uniformis and Proteus mirabilis respectively. During the follow-up, 1 patient died of cardiovascular disease 3 years after discharge, and the remaining 10 patients survived. There was no recurrence of intracranial and middle ear lesions. Sigmoid sinus and transverse sinus thrombosis were significantly improved. Conclusion:Brain abscess, intracranial infection and thrombophlebitis are the most common otogenic intracranial complications, and cholesteatoma of middle ear is the most common primary disease. Timely diagnosis, multidisciplinary collaboration, accurate grasp of the timing in the treatment of primary focal and complications have improved the cure rate of the disease.

10.Risk prediction of low birth weight infants in Shanghai

ObjectiveTo investigate the risk factors of fertility behaviors with preterm birth and low birth weight, and to develop a nomogram model to predict the occurrence of low birth weight. MethodsBirth registration information in Shanghai from 2010 to 2020 was collected, and ANOVA and Chi-square tests were used to compare the differences in reproductive behavior factors and newborn health status across time. The odds ratio (OR) value and 95%CI were calculated by a multi-classification logistic regression model to determine the association between reproductive behavior factors and preterm birth or low birth weight infants. A nomogram model was established based on logistic model and the area under the ROC curve was used to assess the effect of the model. ResultsThis analysis included 2 089 384 live newborns. The incidence of full-term low birth weight, preterm normal weight and preterm low birth weight in Shanghai was 0.94%, 2.48% and 2.01%, respectively. From 2010 to 2020, 40.00% women had a history of abortion, the proportion of women who gave birth at age ≥40 years old increased from 1.05% to 2.24%, the proportion of fathers aged ≥40 years increased from 4.79% to 7.48%, and the proportion of women with postgraduate or above increased from 4.81% to 11.74%. The incidence of preterm low birth weight in Shanghai showed an increasing trend over time. Logistic regression analysis showed that the risk of preterm low birth weight was lower in female than in male infants (OR=0.97, 95%CI: 0.95‒0.98), and the risk of full-term low birth weight was higher than in male infants (OR=1.85, 95%CI: 1.80‒1.90). The risk of preterm birth and low birth weight was lower for couples of childbearing age with higher education. The risk of preterm low birth weight in newborns tended to increase with maternal age at childbirth >30 years, paternal age ≥40 years, and the number of abortions >2 times. Mother <25 or >35 years, father aged 30‒34 years, and the number of abortions >3 times were the risk factors of full-term low birth weight infants. ConclusionCouples of childbearing age who choose to have children at too high or too low age may increase the risk of preterm birth or low birth weight, so it is necessary to strengthen population awareness and promote age-appropriate childbirth. Multiple abortions are also associated with preterm birth and low birth weight, and it is advisable to popularize the scientific knowledge of contraception and birth control to reduce unnecessary abortions. The nomogram in the study can visualize the risk of full-term and low birth weight infant at different levels of factors, which can assist couples preparing for pregnancy in making decisions about the timing of childbirth and understanding the level of risk.
