1.Screening and quantitative analysis of Q-Marker for anti-renal fibrosis of Shenqi shenshuai mixture based on untargeted metabolomics and bioinformatics

OBJECTIVE To screen and quantitatively analyze the quality marker (Q-Marker) associated with anti-renal fibrosis in Shenqi shenshuai mixture (SQSS), so as to provide references for the analysis of pharmacodynamic substances and new drug transformation of SQSS. METHODS The chemical components of SQSS were characterized by untargeted metabolomics. Combined with network pharmacology, gene expression omnibus (GEO) and connectivity map (CMAP) databases, the anti-renal fibrosis Q-Markers of SQSS were screened. HPLC-MS/MS was applied to determine the contents of Q-Markers in 10 batches of SQSS. RESULTS A total of 1 319 compounds were identified from SQSS via untargeted metabolomics,and 84 active ingredients with anti-renal fibrosis activity were further screened out, such as formononetin. Nine anti-renal fibrosis Q-Markers were obtained by network pharmacology and GEO database, including berberine, quercetin, honokiol, nicotinamide, daidzein, coumarin, kaempferol, formononetin and amygdalin. The quantitative results showed that the average contents of the above components (excluding amygdalin) in 10 batches of SQSS were 2.523, 1.942, 26.848, 1.415, 0.692, 0.171, 0.374, 7.401 μg/mL, respectively. CONCLUSIONS Nine anti-renal fibrosis Q-Markers of SQSS were screened in this study, and the contents of eight among them were determined. The results can provide a basis for elucidating the pharmacodynamic material basis and promoting the new drug transformation of SQSS.

3.Review of radiation effects of implantable medical electronic devices

With the performance improvement of the medical electronics and the progress of social development,the utilization rate of implantable medical electronic devices in China shows an increasing trend in recent years,and will maintain a growing trend in the future as population aging is accelerating.Even though implantable medical electronic devices have extremely low failure rates,the explicit clinical accidents caused by their reliability issues deserve sufficient attention in consideration of the large quantity of patients.Compared with other countries,there is lack of domestic researches on application risk of implantable medical electronic devices due to radiation therapy and diagnosis,which is reflected in not only the clinical research,but also the researches on the underlying physical damage mechanism and macro performance of the devices.Therefore,it is crucial and urgent to explore the application risk of implantable medical electronic devices caused by radiation therapy and diagnosis,which has high clinical and scientific significance.The study conducts a literature survey on the risks of medical electronic devices in the radiation environment generated by clinical treatment,summarizes the risks encountered in the aspects of total dose effect,electromagnetic compatibility and instantaneous effect,analyzes the above behaviors from the physical mechanism and perspective,and puts forward some meaningful suggestions for medical electronic engineering and clinical treatment.

To improve the effectiveness of bedside localization of nasointestinal tube(NIT)and facilitate the placement of nasointestinal tube into jejunum,we established a procedure and composed a teaching verse for bedside placement of nasointestinal tube based on relevant classical literature and our own practices.Verse content:enteral nutrition means a successful strategy to improve the outcome in critically ill patient management,never hesitate to place nasointestinal tubes when necessary.There are several methods to deal with it,but popularizing it remains a long way off.Half-sitting and swallowing into the esophagus,freely withdrawing signifies the stomach cavity.Passing through the pylorus using light tension on the tube in the right lateral decubitus position.Arriving at the jejunum with low resistance in the left lateral decubitus position.What are the signs of intragastric coiling?Tube return out of nose is the initial observation,Failure of air insufflation indicates tube coiling.Dyeing location surpasses imaging.Vacuum test is the most sensitive,Sequential change from acid to base is specific.Methylene blue test is dramatical for localization.Combining three methods is enough to navigate.Abdominal plain film is the goldan standard and can still be used in ultrasonic era.3-D image establishes overall view.CT reveals the tube route exactly.The teaching verse has become a powerful tool for clinical teaching of manual nasointestinal tube placement in a concise and easy-to-remember form.
5.Anti-inflammatory Effect and Mechanismof Chrysanthemum Indicum Decoction on RAW264.7 Inflammatory Cell Model

Objective To investigate the anti-inflammatory effect and molecular mechanism of Chrysanthemum indicum decoction(C.indicum decoction,CID)through NF-κB signaling pathway based on RAW264.7 macrophages model induced by lipopolysaccharide(LPS).Methods MTT was used to select the appropriate concentration of CID on the activity of RAW 264.7 macrophages;Griess method and enzyme-linked immunosorbent assay(ELISA)were used to measure the release of nitric oxide(NO),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in cells treated with 50,100 and 200 μg·mL-1 CID,respectively;and the relative contents expression levels of cyclooxygenase-2(COX-2)and inducible nitric oxide synthase(iNOS)in each group were analyzed by real-time fluorescent quantitative polymerase chain reaction(Real-time PCR);the relative production of nuclear factor-kappa B p65(NF-κB p65),inhibitor kappa B(IκB-α)and phosphorylated IκB-α(p-IκB-α)in each group was observed by Western blotting(WB).Results 50-200 μg·mL-1 CID significantly decreased the relative production of NO,TNF-α,and IL-6 in RAW264.7 macrophages induced by LPS(P<0.01),down-regulated the expression of COX-2 and iNOX mRNA(P<0.01),down-regulated the relative contents of p-IκB-α,total NF-κB p65 and nuclear NF-κB p65(P<0.01),and up-regulated the relative contents of IκB-α and cytoplasmic NF-κB p65(P<0.01).Conclusion CID could effectively inhibit the release of inflammatory factors from RAW 264.7 macrophages induced by LPS,and the mechanism may be related to the inhibition of inflammation by reducing the expression of TNF-α and other key proteins and regulating the inflammatory signaling pathway such as NF-κB.
6.Optimal Ratios and Their Spectral-efficacy Relationship of Baitong Decoction in the Treatment of Ulcerative Colitis

Objective To explore the optimal ratio of Baitong decoction based on efficacy,clarify its spectrum-effect relationship,and identify its potential quality markers.Methods An ulcerative colitis(UC)model in mice was established using dextran sulfate sodium.The efficacy of Baitong decoction with varying drug ratios was assessed by evaluating the apparent score,pathological score and inflammatory factor changes of UC in each group of experimental animals.The fingerprints of Baitong decoction with different ratios were established by high performance liquid chromatography(HPLC),and the relationship between the content of each substance and its efficacy was analyzed by partial least squares regression to determine the potential quality markers of Baitong decoction.Results Baitong decoction was most effective in relieving ulcerative colitis when the mass ratio of Fuzi,Ganjiang and Congbai was 1∶2∶2.The fingerprinting identified 14 common peaks across 7 ratios,with 9 peaks were found to be associated with the remission of ulcerative colitis by partial least squares regression analysis.Conclusion The optimal ratio of Fuzi,Ganjiang and Congbai for treating UC is 1∶2∶2.The spectrum-effect relationship analysis suggested that the quality markers of Baitong decoction may be the substances represented by peak 2(benzoylaconine),3,5,6,8(mesaconine),9(aconitine),10(hypaconitine),13(10-gingerol)and 14.
7.Impact of hyperoxia on the phenotype of pulmonary artery smooth muscle cells

Objective:To investigate the influence of varied oxygen (O 2) concentration environments on the phenotypic transformation of pulmonary artery smooth muscle cells (PASMC) and the mechanism of pulmonary hypertension. Methods:Primary rat PASMC were isolated and cultured through the process of enzymatic digestion. Following identification, the stable passaged PASMC were subjected to a 6-hour incubation in sealed containers with normal O 2 content (group C) and relative O 2 content comprising 55% (group H55), 75% (group H75), and 95% (group H95). mRNA and protein expression of α-Actin (α-SMA), smooth muscle 22α (SM22α), osteopontin (OPN), and matrix metalloproteinase-2 (MMP-2) were measured using real-time quantitative PCR and western blot analysis. Results:The H55 group displayed no significant difference from the C group in terms of mRNA and relative protein expression levels for α-SMA, SM22α, OPN, and MMP-2 (all P>0.05). On the other hand, groups H75 and H95 exhibited a reduction in mRNA and relative protein expression of α-SMA and SM22α, along with an increase in mRNA and relative protein expression of OPN and MMP-2 when compared with both the C and H55 groups (all P<0.05). The H95 group showed a higher relative mRNA expression of MMP-2 as compared to the H75 group ( P<0.05). Conclusions:Oxygen concentration environments of 75% or higher can serve as the foundation for the pathogenesis of pulmonary hypertension, essentially by inducing a phenotypic transformation in PASMC towards adopting a robust secretory function. This induction is contingent upon the concentration of oxygen present.

9.Observation on the preventive effect of indwelling pancreatic duct guide wire through nasobiliary drainage tube for post-ERCP pancreatitis in cholelithiasis patients with small-caliber pancreatic duct

Objective:To evaluate the preventive effect of indwelling pancreatic duct guide wire through nasobiliary drainage catheter for post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) in cholelithiasis patients with small-caliber pancreatic duct.Methods:The clinical data of 127 patients with gall bladder and common bile duct stones undergoing ERCP and elective laparoscopic cholecystectomy in the Cholelithiasis Center of Shanghai East Hospital Affiliated to Tongji University from January 2019 to June 2023 were analyzed retrospectively, including 55 males and 72 females, aged (56.95±10.86) years old. According to the preventive methods of PEP, patients were divided into the guide wire group (indwelling pancreatic duct guide wire through nasobiliary drainage catheter), stenting group (nasobiliary catheter with 5Fr 5 cm single pigtail pancreatic duct stent) and conventional group (nasobiliary catheter). The incidence of post-ERCP hyperamylasemia (PEH) and PEP were compared.Results:The incidence of PEH in the guide wire group was lower than that in the conventional group [17.8% (8/45) vs. 43.5% (10/23), P=0.023], and also lower than that in the stenting group [17.8% (8/45) vs. 32.2% (19/59)], despite no statistical significance ( P=0.337). The incidences of PEH were comparable in the stenting group and conventional group [32.2% (19/59) vs. 43.5% (10/23), P=0.096)]. The incidence of PEP in the guide wire group was lower than that in both the stenting group [6.7% (3/45) vs. 23.7% (14/59), P=0.030]. and conventional group [6.7% (3/45) vs. 30.4% (7/23), P=0.025]. The incidences of PEP were comparable in the stenting and conventional group [23.7% (14/59) vs. 30.4% (7/23), P=0.532]. Conclusion:Compared to the preventive pancreatic duct stenting, indwelling pancreatic duct guide wire through nasobiliary drainage catheter can effectively prevent the PEH and PEP in high-risk patients with a small-caliber pancreatic duct.
10.Effects of orexin A on morphine-induced gastrointestinal dysfunction in mice

Objective:To evaluate the effect of orexin A on morphine-induced gastrointestinal dysfunction in mice.Methods:Forty SPF C57B/6 mice, aged 6-8 weeks, half male and half female, weighing 18-22 g, were divided into 5 groups ( n=8 each) using a random number table method: control group (group C), morphine group (group M) and morphine + different doses of orexin A groups (MOH, MOM and MOL groups). Normal saline 8 ml/kg was subcutaneously injected daily in group C, morphine 6 mg/kg was subcutaneously injected daily in the other four groups, and orexin A 75, 50 and 25 μg/kg were subcutaneously injected daily for 10 days at the same time in MOH, MOM and MOL groups.The fetal water content was calculated and averaged daily.After the last administration, the mice were gavaged with black nutrient paste, and the gastric emptying rate and small intestinal propulsion rate were detected 30 min later.Blood samples were collected from the orbit, and the concentration of serum gastrin (GAS) was detected by enzyme-linked immunosorbent assay.The mice were then sacrificed, and colon tissues were removed for determination of c-kit positive cell area (by immunohistochemistry) and expression of c-kit, substance P (SP) and neural nitric oxide synthase (nNOS) in colon tissues (by Western blot). Results:Compared with group C, the rate of fecal water content, gastric emptying rate, small intestinal propulsion rate and serum GAS concentration were significantly decreased, the area of c-kit positive cells was decreased, and the expression of c-kit and SP was down-regulated, and the expression of nNOS was up-regulated in group M ( P<0.05). Compared with group M, the small intestinal propulsive rate and serum GAS concentration were significantly increased, and the area of c-kit positive cells was increased, and the expression of c-kit was up-regulated in group MOH, the rate of fecal water content, gastric emptying rate, small intestinal propulsion rate and serum GAS concentration were significantly increased, the area of c-kit positive cells was increased, and the expression of c-kit and SP was up-regulated, and the expression of nNOS was down-regulated in group MOM, and the serum GAS concentration and c-kit positive cell area were significantly increased in group MOL ( P<0.05). Conclusions:Orexin A 50 μg/kg can effectively alleviate the gastrointestinal dysfunction induced by morphine in mice, and the mechanism may be related to promotion of GAS secretion, interstitial cells of Cajal growth and SP release and inhibition of NO release.
