1.Reliability and validity of the Chinese version of Biological Rhythms Interview of Assessment in Neuropsychiatry in patients with major depressive disorder

Objective:To verify the reliability and validity of the Chinese Version of Biological Rhythms Interview of Assessment in Neuropsychiatry (BRIAN-C) in patients with major depressive disorder(MDD).Methods:A total of 122 patients with MDD and 122 controls were recruited and measured with the BRIAN-C, 16-Item Quick Inventory of Depressive Symptomatology Self-Report (QIDS-SR16), 7-Item Generalized Anxiety Disorder Scale(GAD-7) and 6-Item Short Form Health Survey(SF-6). The index D and correlation analysis were conducted to evaluate the discrimination and homogeneity of the items, respectively. The Cronbach′s α coefficient, Spearman-Brown coefficient, intraclass correlation coefficient ( ICC) and retest correlation coefficient were conducted to evaluate the reliability.Confirmatory factor analysis, correlation analysis and the receiver operating characteristic (ROC) curve were conducted to evaluate the construct validity, criterion validity and prediction validity, respectively. Results:The item distinction and homogeneity analysis showed significant differences ( P<0.05). The Cronbach′s α coefficient and the Spearman-Brown coefficient were 0.911 and 0.807, while the ICC and the correlation coefficient r of test-retest reliability were 0.639 (95% confidence interval( CI):0.375-0.729) and 0.43 ( P=0.001), respectively. Confirmatory factor analysis showed that the fit indexes χ 2/dF=1.937, goodness-of-fit index(GFI)=0.822, comparative fit index (CFI)=0.877, Tucker-lewis index (TLI)=0.855, and root mean square error of approximation (RMSEA)=0.088. The score of BRIAN-C was significantly correlated with that of QIDS-SR16, GAD-7 or SF-6 ( r=0.74, 0.46, -0.69, all P<0.05).The ROC curve suggested that the area under curve (AUC) was 0.939 (95% CI: 0.909-0.970, P<0.05) for distinguishing MDD patients from controls. The optimal cut-off was 29, with 0.861 sensitivity and 0.926 specificity. Conclusions:The BRIAN-C scale has good reliability and validity, which is supposed to be used in Chinese cultural background to evaluate the biorhythms in patients with MDD.

3.Diagnosis of multiple primary malignancies complicated with hepatocellular carcinoma

Objective To explore the clinical diagnosis method of multiple primary malignancies (MPM) complicated with hepatocellular carcinoma (HCC). Methods Clinical data of 68 MPM patients complicated with HCC treated in the Sun Yat-sen University Cancer Center from January 1989 to April 2008 were retrospectively analyzed. There were 61 males and 7 females, aged from 32 to 82 years with a median age of 60 years. The informed consents of all patients were obtained and the local ethical committee approval had been received. The patients undergoing no surgery were diagnosed by imaging examination combined with detection of serum AFP level. Ultrasound-or CT-guided pathological biopsy was performed further on the suspected cases. The diagnosis was conifrmed by pathological examination in patients undergoing surgery.Results Of the 68 patients, 22 complicated with HCC simultaneously and 46 metachronously. The age of the ifrst onset of malignancy ranged from 31 to 76 years with a median age of 57 years. The age of onset of secondary malignancy ranged from 32 to 82 years with a median age of 60 years. Hepatitis B surface antigen (HBsAg) was detected in 45 patients, whereas hepatitis C antibody was negative in all cases. Fifty-one cases were complicated with liver cirrhosis. AFP≤25 g/L was detected in 30 cases and>25 g/L in 38 cases. Sixteen cases had a family history of malignant tumors. Thirty of the 33 patients undergoing no surgery were conifrmed based upon the typical HCC manifestations of imaging ifndings and AFP levels, including 16 cases were conifrmed by positron emission computed tomography (PET/CT). The remaining 3 suspected cases were conifrmed by ultrasound-or CT-guided liver biopsy. Thirty-ifve patients undergoing hepatic resection received pathological examination including 30 cases with single cancerous nodule and 5 with multiple cancerous nodules, 12 with tumor diameter<5 cm and 23 with tumor diameter≥5 cm. Twenty-seven patients were complicated with cirrhosis, 32 with vascular invasion, and 15 cases were found with surgical margin≥2 cm. Extrahepatic malignancies were distributed in different organs including head and neck tumors in 23 cases, digestive system tumors in 18 cases, urinary system tumors in 5 and other organ tumors in 22 cases. Conclusions The clinical characteristics of MPM complicated with HCC patients are similar to those of primary liver cancer patients. PET/CT probably possesses specific advantages in identifying MPM. Liver biopsy contributes to conifrming the diagnosis of suspected cases.
4.CT morphometric assessment of the acromion and clavicle hook plate

Objective To measure the morphological parameters of the acromion with CT and to analyze their match with the hook plate. Methods From October 2009 to February 2010,spiral CT scanning (with Somatom Emotion16) and three-dimensional reconstruction of bilateral shoulders were conducted in 61 Chinese subjects.They were 24 men and 37 women,aged from 20 to 83 years (average,45.2 years).The thickness,length and width of the acromion were measured and the subacromial shape was observed with software of the CT system to analyze the match between the hook plate and the acromion.Results The mean thickness of the acromion was 0.85 ± 0.13 cm in all subjects,0.94 ± 0.12 cm in males and 0.79 ± 0.10 cm in fe males,with a significant difference between males and females ( t =2.382,P =0.202).The mean acromion length was 2.08 ± 0.20 cm in all subjects,2.09 ± 0.21 cm in males and 2.06 ± 0.18 cm in females,with no significant difference between males and females( t =1.541,P =0.129).The mean acromion width was 3.81 ± 0.52 cm in all subjects,4.34 ± 0.32 cm in males and 3.47 ± 0.25 cm in females,with a significant difference between males and females ( t =2.296, P =0.025 ). Conclusions The acromial morphology varies significantly between genders in Chinese population,particularly in the thickness and width.It is,therefore,necessary to modify the morphological parameters of the hook plate to match better the gender difference in Chinese population.It is optimal that the hook plate should allow individualized pre-moulding to enhance its therapeutic efficacy.
