1.Clinical efficacy of a novel autologous blood recovery device during ECMO weaning

Objective: To evaluate the clinical efficacy of a novel autologous blood recovery device during the weaning process from extracorporeal membrane oxygenation (ECMO). Methods: A total of 16 patients who received ECMO support and underwent blood recovery during the weaning process from January 2022 to September 2024 at our hospital were included in the experimental group. In contrast, 58 patients who did not receive blood recovery during the weaning process were assigned to the control group. Transfusion components, costs, and changes in routine blood tests and coagulation functions were compared between the two groups from the day of weaning until 48 hours post-weaning. Results: Significant differences were observed in the volumes of red blood cell transfusions, plasma transfusions, and transfusion costs between the two groups from the day of weaning to 48 hours post-weaning (P<0.05). Additionally, in the experimental group, significant differences were noted in hemoglobin (Hb), platelet (Plt), and activated partial thromboplastin time (APTT) results when comparing values before and after extubation (P<0.05). Conclusion: The application of a novel autologous blood recovery device during ECMO weaning reduces patient costs, minimizes wastage of autologous blood, decreases reliance on exogenous blood transfusions, and mitigates the risks associated with allogeneic blood transfusion. This approach merits further promotion for clinical use.
2.Effects of imidapril on blood gas, oxidative stress and inflammatory fac-tors in rats with acute lung injury induced by ammonium chloride

AIM:In this study, the rat lung injury model was induced by ammonium chloride for studying the effect of imidapril on blood gas, serum TNF-α, IL-6 and MDA concentrations, and AngⅡand CD54 protein expression in rat lung tissue.METHODS:Male rats were randomly divided into 3 groups:control group, lung injury model group and drug group.The rats in control group were given saline (2 mL/kg), while the rats in lung injury model group were given 6% ammonium chloride (2 mL/kg).In drug group, imidapril (3 mg· kg-1· d-1) was given to the rats once daily for 1 week by intragastric gavage after given 6%ammonium chloride.On the 7th day, the rats were anesthetized with 2% so-dium pentobarbital.Abdominal aorta blood, venous blood and lung tissue were collected.The blood gas indexes and serum TNF-α, IL-6 and MDA concentrations were determined.The lung tissues were fixed and sliced, and the expression of AngⅡ and CD54 proteins was detected by immunohistochemistry.RESULTS: The PaCO2 increased in lung injury model group compared with control group and drug group (P<0.05).The expression of AngⅡand CD54, and the concentrations of TNF-α, IL-6 and MDA also increased significantly ( P<0.01) in model group.Pulmonary edema, inflammation, alveo-lus congestion, hemorrhage and hyperplasia in model group were obvious compared with control group and drug group. CONCLUSION:Imidapril improves blood gas indexes, and reduces lipid peroxidation and inflammatory responses in the rats with lung injury induced by ammonium chloride.
3.The study of cylindrical abdominal perineal resection for rectal cancer and pelvic floor reconstruction improvement

Objective To study the abdominal perineal resection for rectal cancer and clinical application of pelvic floor recon-struction .Methods 30 cases of rectal cancer patients from 2008-2011 were randomly divided into experimental group and the con-trol group ,30 cases in each group .Experimental group patients by columnar abdominal perineal resection of rectal cancer treatment , postoperative pelvic floor defect for dermal matrix reconstruction .The control group patients for colorectal cancer resection by lapa-rotomy .Results Compared with the traditional open resection ,the operation time ,intraoperative blood loss ,postoperative patholog-ical column T3 N0 M0 positive rate ,incidence of complications and incision aspect of cylindrical abdominal perineal resection were ob-vious advantages ,the difference was statistically significant (P< 0 .05) .Conclusion Compared with Miles surgery ,cylindrical ab-dominal perineal resection for rectal cancer can reduce the incidence of postoperative incisal edge positive rate and bowel perfora -tion .Human decellularized dermal matrix basin redevelopment can significantly reduce the surgical risk and difficulty of surgical op-erations .
