This paper summarizes professor ZHOU Zhongying's clinical experience in differentiating and treating miscellaneous internal diseases using Erjia Longgu Decoction （二加龙骨汤）. It is proposed that Erjia Longgu Decoction exerts the effects of clearing the upper and warming the lower， nourishing yin and suppressing yang， and harmonizing yin and yang， applicable for miscellaneous internal diseases characterized by the core pathogenesis of insufficient ying-yin （营阴） and floating deficient yang. In clinical practice， treatment is tailored according to individual patient conditions， with emphasis placed on gentle and flexible medication use. Erjia Longgu Decoction is used as the foundational formula with appropriate modifications， and has been widely applied in the management of miscellaneous internal diseases， such as dizziness， palpitations， insomnia， nocturnal emission， cold intolerance， and sweating disorders， offering new perspectives for the expanded application of classical formulas.

Objective·[18F]F-FMISO and[18F]F-FLT are specific PET imaging agents for detecting the hypoxia microenvironment and cell proliferation,respectively.This study aims to visualize and monitor the impact of drug resistance in pancreatic cancer on the hypoxia microenvironment and cell proliferation through[18F]F-FMISO and[18F]F-FLT PET/CT dual-nuclide imaging,with the goal of providing a theoretical basis for clinical application.Methods·The CCK-8 assay was conducted to assess drug resistance in the PANC-1/R(PR)pancreatic cancer cell line compared to the parental PANC-1(P)cell line.Subcutaneous xenograft models of pancreatic cancer were established by injecting male BALB/c nude mice with pancreatic cancer cells into the left axillary subcutaneous region.Subgroups were treated with gemcitabine(GEM)chemotherapy starting on day 18(18D-G group)or day 12(12D-G group)after inoculation of tumor cells.[18F]F-FMISO and[18F]F-FLT PET/CT imaging were performed before and after treatment to obtain semi-quantitative parameters(maximum standardized uptake value,SUVmax).ΔSUVmax was calculated by using the following equation:ΔSUVmax=(SUVmax of second imaging-SUVmax of first imaging)/SUVmax of first imaging.Receiver operating characteristic(ROC)curves were used to determine the optimal threshold for the semi-quantitative parameters to assess pancreatic cancer drug resistance.Results·The CCK-8 assay confirmed that the PR cells exhibited high resistance to GEM,with a resistance index of 4.24(n=5).In vivo experiments showed that GEM chemotherapy significantly inhibited tumor growth and prolonged survival in the parental pancreatic cancer group(12D-G group,P=0.025),whereas GEM chemotherapy accelerated tumor growth and shortened survival(18D-G and 12D-G,P=0.025)in the drug-resistant pancreatic cancer group.In addition,in the non-chemotherapy group,ΔSUVmax-FLT might be negatively correlated with survival time,while in the chemotherapy group,both ΔSUVmax-FMISO and ΔSUVmax-FLT were negatively correlated with survival time(P=0.050,P=0.006).In the 18D-G and chemotherapy group,the second imaging showed significantly lower ΔSUVmax-FMISO and ΔSUVmax-FLT in P tumors compared to PR tumors(P=0.045,P=0.050).In the 12D-G and chemotherapy group,the second imaging showed slightly lower ΔSUVmax-FLT in P tumors compared to PR tumors(P=0.051).ROC analysis identified the optimal threshold for assessing pancreatic cancer drug resistance:when ΔSUVmax-FLT=0.45 in the non-chemotherapy group,the sensitivity and specificity were 100.00%and 50.00%,respectively;when ΔSUVmax-FMISO=0.37 and ΔSUVmax-FLT=0.36 in the chemotherapy group,the sensitivity and specificity were 100.00%and 83.33%,respectively.Conclusion·[18F]F-FMISO and[18F]F-FLT PET/CT dual-nuclide imaging can be used to assess drug resistance in pancreatic cancer.The comparison of[18F]F-FMISO and[18F]F-FLT PET differences before and after chemotherapy provides the most accurate prediction of drug resistance and survival time.

Objective To analyze the influence of trimetazidine combined with atorvastatin on ser-um angiopoietin-2(Ang-2)and chitinase protein 40(YKL-40)levels and ultrasound parameters in elderly patients with unstable angina pectoris(UAP).Methods A total of 133 elderly UAP pa-tients admitted in Hospital of the 81st Group of PLA Army from January 2021 to March 2024 were enrolled,and according to their treatment methods,they were divided into control group(64 cases,simple atorvastatin)and trimetazidine group(69 cases,atorvastatin+trimetazidine).After 4 weeks of treatment,the clinical efficacy was observed in the two groups.Seattle Angina Question-naire(SAQ)score and nitroglycerin dosage,lipid metabolic indicators(TC,TG,LDL-C and HDL-C),cardiac ultrasound parameters[LVEF,LVEDD and left ventricle mass index(LVMI)],serum in-dicators[Ang-2,YKL-40 and von Willebrand factor(vWF),matrix metallo proteinase-9(MMP-9)]were compared between the two groups before treatment and after 4 weeks of treatment.The ad-verse reactions during treatment were also recorded in the two groups.Results The total effective rate was significantly higher in the trimetazidine group than the control group(84.06％vs 67.19％,P＜0.05).The SAQ score,HDL-C level and LVEF value were significantly risen in the two groups after treatment(P＜0.05),and these indicators were obviously higher in the trimetaz-idine group than the control group(P＜0.01).The nitroglycerin dosage,levels of TC,TG and LDL-C,LVEDD and LVMI values and serum contents of Ang-2,YKL-40,vWF and MMP-9 were significantly reduced in the two groups after treatment when compared with those before treat-ment(P＜0.05),and these indicators in the trimetazidine group were lower than those in the con-trol group(P＜0.01).There was no statistical difference in the total incidence of adverse reactions between the two groups(P＞0.05).Conclusion Trimetazidine combined with atorvastatin can en-hance the efficacy and improve the lipid metabolism and cardiac function in the treatment of elder-ly UAP patients,which might be due to down-regulating the serum levels of Ang-2,YKL-40,vWF and MMP-9.

Objective: To study the effects of different concentrated sulfuric acid etching durations on the shear bond strength between polyether-ketone-ketone (PEKK) and dentin, providing a scientific basis for the clinical bonding procedures of PEKK prosthesis. Methods: Forty-four PEKK specimens were prepared and randomly divided into four groups: group A was the control group, which was only polished with abrasive papers, group B, group C and group D were experimental groups, which were etched by 98% concentrated sulfuric acid for 5 s, 30 s and 60 s, respectively. In addition, one sample was randomly selected from each group, and the profile was prepared by a slow cutting machine. The surface morphology of the profile was observed under SEM. After the four groups of specimens and dentin were bonded by resin, they were soaked in distilled water at 37 ℃ for 24 h. After the shear bonding strengths were measured, the fracture interfaces of the specimens were examined by the scanning electron microscopy and stereomicroscopy, and failure models of bonding were analyzed. Results: After acid etching treatments, the cross-sectional images in group B presented uniform spongy shapes, while the cross-sectional images in group C and group D showed destructive pore structures. The shear bond strengths of group B (16.84 ± 1.84) MPa, group C (12.33 ± 1.22) MPa and group D (6.44 ± 1.18) MPa were higher than that of group A (3.99 ± 1.06) MPa (P < 0.05). The highest shear bond strength was observed in group B (16.84 ± 1.84) MPa. Conclusion The surface treatment of 98% sulfuric acid etching for 5 s manifested the best bond strength between PEKK and dentin.

ObjectiveTo evaluate the clinical effects of surgery using scar flap in situ combined with radiotherapy in 24 hours in the restorative treatment of keloids on the auricula and the preventive effects of the therapy in the recurrence of the keloids.MethodsThe scar flap in situ was designed,its size was large enough for covering the wound of the keloid on the auricula.The keloids along the designed lines were excised using local anesthesia,the flap was clipped into the one with even thickness and suitable size which covered the wound tensionlessly to ensure that the scar flap in situ survived well,and then the wound was bandaged with pressure and drained when necessary.18-24 hours after the surgery the wound was perpendicularly irradiated by the 5 MeV high energy electron beam (beta particle) of the Siemens Primus linear accelerator.After the dressing change was performed and the drain was removed; the wound was exposed to the irradiation,3-4 Gy segmentation dose per time,and the wound was then bandaged with pressure.The radiation was performed every two days and four times altogether with a total irradiation dose of 12-16 Gy.Stitches were removed 8-10 days after the surgery.ResultsThere were no avascular necrosis in the 25 scar flaps in situ and the wounds were all primary healing with normal color and fine appearance.All the patients were satisfied with the surgery.There was no recurrence of the 23 patients during the 8 to 42 months' follow-up,but there was a tendency to recur in 2 patients after 4-6 months,and the recurrence was controlled after the beta methasone was locally injected for 2-4 times.ConclusionsIt is not necessary to harvest the flaps on the other sites applying the sear flap in situ in the restorative treatment of keloids on the auricula,and therefore it prevents the formation of the keloids on the donor sites.Furthermore,the surgery is simple and the appearance of the auricula is fine,and it presents satisfactory clinical effects to irradiate the wound in 24 hours after the surgery.

Objective To investigate the morbility and epidemic characteristics of hair dye dermatitis in individuals who dyed their hair in Tianjin.Methods Questionnaires were distributed to the outpatients in the Gerneral Hospital of Tianjin Medical University,students and teachers in Tianjin Medical University,residents in the community and customers in barber shop from Aug.2007 to Mar.2008.The personal data including the methods and site of coloring hair and something correlated to hair dyes were investigated.Results A total of 597 cases with the history of coloring hair were enrolled in the study,including 485 women and 112 men,with mean age of 41 years (ranged from 16- 74years).Among 597 cases,69 cases had allergic reactions to hair dye,including 51 women and 18 men,with mean age of 44 years (ranged from 19-65 years).The median age of the first coloring hair was 40 years (ranged from 3-50 years).The comparison between the sites of coloring hair had no statistic significance (P ＞0.05),but there was a significant difference between black dyed hairs and col or dyed hairs (P ＜0.05).Conclusions People with black hair dye are prone to be allergic.

Objective To assess polymerase chain reaction(PCR)combined with restriction fragment length polymorphism(RFLP)and gene sequencing technologies in the detection and typing of HPV DNA.Methods Tissue specimens were collected from skin diseases and venereal disease in perianal or genitals.PCR was performed with HPV DNA general primers(MY09/11)in tissue samples. Positive fragments of HPV DNA were purified and digested by restriction enzymes.The digested fragments were typed by po]yacrylamide gel electrophoresis(PAGE).The Resultswere verified by direct sequencing.Results In 50 clinical samples there were 35 HPV DNA positive,including 26 from patients with condyloma acuminatum,8 from patients with bowenoid papulosis,and 1 from patients with squamous cell carcinoma.In HPV DNA positive samples,19 were HPV6,3 were HPV11,8 were HPV16,4 were HPV6 and HPV 11,and I was HPV62.Sequencing Resultswere in accordance with the PCR-RFLP Results .Conclusion PCRRFLP method is effective in the detection and typing of HPV DNA.
