In view of the problem of slow development of intensive care medicine in Xizang, the research team made full use of the national partner assistance to Xizang, gathered resources across all cities in Xizang, and formed a national academic platform for critical care medicine in plateau areas. Adhering to the academic orientation with hemodynamics as the main topic, critical care ultrasound as the bedside dynamic monitoring and evaluation method, and blood flow-oxygen flow resuscitation as the core connotation, we have achieved the goals of improving the critical care talent echelon throughout Xizang, driving the overall progress of intensive care medicine in Xizang, making a figure in China, and focusing on training of top-notch talents.
2.Chidamide plus prednisone, cyclophosphamide, and thalidomide for relapsed or refractory peripheral T-cell lymphoma: A multicenter phase II trial

Background::Although the treatment of peripheral T-cell lymphoma (PTCL) has undergone advancements during the past several years, the response rate and long-term effects with respect to patients with PTCL remain unsatisfactory—particularly for relapsed or refractory (R/R) patients. This phase II trial was designed to explore the efficacy and safety of an all-oral regimen of chidamide plus prednisone, cyclophosphamide, and thalidomide (CPCT) for R/R PTCL patients who could not tolerate the standard chemotherapy for a variety of reasons.Methods::We conducted a multicenter phase II clinical trial in which we combined chidamide (30 mg twice weekly) with prednisone (20 mg daily after breakfast), cyclophosphamide (50 mg daily after lunch), and thalidomide (100 mg daily at bedtime) (the CPCT regimen) for a total of fewer than 12 cycles as an induction-combined treatment period, and then applied chidamide as single-drug maintenance. Forty-five patients were ultimately enrolled from August 2016 to April 2021 with respect to Chinese patients at nine centers. Our primary objective was to assess the overall response rate (ORR) after the treatment with CPCT.Results::Of the 45 enrolled patients, the optimal ORR and complete response (CR)/CR unconfirmed (CRu) were 71.1% (32/45) and 28.9% (13/45), respectively, and after a median follow-up period of 56 months, the median progression-free survival (PFS) and overall survival (OS) were 8.5 months and 17.2 months, respectively. The five-year PFS and OS rates were 21.2% (95% confidence interval [CI], 7.9-34.5%) and 43.8% (95% CI, 28.3-59.3%), respectively. The most common adverse event was neutropenia (20/45, 44.4%), but we observed no treatment-related death.Conclusion::The all-oral CPCT regimen was an effective and safe regimen for R/R PTCL patients who could not tolerate standard chemotherapy for various reasons.Trial Registration::ClinicalTrials.gov, NCT02879526.
3.Changes and clinical significance of NLRP3 inflammasomes and related factors in patients with spinal fracture com-plicated with acute spinal cord injury

Objective To investigate the changes and clinical significance of NOD like receptor protein 3(NLRP3)in-flammasomes and related factors in patients with spinal fractures complicated with acute spinal cord injury(SCI).Methods Eighty-six spinal fracture patients complicated with acute SCI admitted to hospital from June 2019 to March 2022 were selected as SCI group,There were 48 males and 38 females,with an average age of(43.48±6.58)years old.And 100 healthy volunteers who underwent physical examination during the same time were selected as control group,including 56 males patients and 44 females patients,with an average age of(45.13±6.43)years old.Peripheral blood mononuclear cell(PBMC)were collected,and the mRNA expressions of NLRP3 and Caspase-1 were detected.Serum was collected and the levels of interleukin(IL)-1β,IL-18 were detected.According to Frankel's grade,the SCI group was divided into complete injury patients and incomplete injury patients,and according to the Japanese Orthopedic Society(JOA)grade,the SCI group was divided into good prognosis group and poor prognosis group.The difference of NLRP3,Caspase-1,IL-1β,IL-18 among groups were compared,the influ-encing factors for poor prognosis in SCI patients was analyzed by Logistic regression.Results The mRNA expression levels of NLRP3(1.41±0.33)and Caspase-1(1.44±0.35)in PBMC and the levels of IL-1β(45.34±13.22)pg·ml-1,IL-18(40.95±8.77)pg·ml-1 in serum of SCI group were higher than those of the control group[(1.00±0.19),(1.00±0.16),(16.58±4.24)pg·ml-1,(12.57±3.68)pg·ml-1](P<0.05).The mRNA expression levels of NLRP3(1.63±0.34)and Caspase-1(1.67±0.27)in PBMC and the levels of IL-1β(51.09±11.10)pg·ml-1,IL-18(47.65±7.93)pg·ml-1 in serum of patients with complete injury in the SCI group were higher than those of patients with incomplete injury[(1.31±0.27),(1.34±0.33),(42.85±13.36)pg·ml-1,(38.05±7.48)pg·ml-1](P<0.05).The mRNA expression levels of NLRP3(1.66±0.31)and Caspase-1(1.72±0.31)in PBMC and the levels of IL-1β(51.21±11.31)pg·ml-1,IL-18(45.70±7.25)pg·ml-1 in serum,the proportion of complete injury(21 patients),and the proportion of spinal cord edema or bleeding of patients(15 patients)with poor prognosis in the SCI group were higher than those of patients with good prognosis[(1.28±0.26),(1.37±0.36),(42.79±13.25)pg·ml-1、(38.90±8.63)pg·ml-1,5、20 cases](P<0.05).Complete injury and the mRNA expression of NLRP3 in PBMC were the influencing fac-tors for poor prognosis in the SCI group(P<0.05).Conclusion The activation of NLRP3 inflammasomes in patients with spinal fractures complicated with acute SCI is associated with worsening injury and poor prognosis,and NLRP3 expression can serve as a marker for evaluating prognosis.
4.Preparation of chitin/hyaluronic acid/collagen hydrogel loaded with mouse adipose-derived stem cells and its effects on wound healing of full-thickness skin defects in rats

Objective:To prepare the chitin/hyaluronic acid/collagen hydrogel loaded with mouse adipose-derived stem cells and to explore its effects on wound healing of full-thickness skin defects in rats.Methods:The research was an experimental research. Chitin nanofibers were prepared by acid hydrolysis and alkaline extraction method, and then mixed with hyaluronic acid and collagen to prepare chitin/hyaluronic acid/collagen hydrogels (hereinafter referred to as hydrogels). Besides, the hydrogels loaded with mouse adipose-derived stem cells were prepared. Thirty male 12-week-old guinea pigs were divided into negative control group, positive control group, and hydrogel group according to the random number table, with 10 guinea pigs in each group. Ethanol, 4-aminobenzoic acid ethyl ester, or the aforementioned prepared hydrogels without cells were topically applied on both sides of back of guinea pigs respectively for induced contact and stimulated contact, and skin edema and erythema formation were observed at 24 and 48 h after stimulated contact. Adipose-derived stem cells from mice were divided into normal control group cultured routinely and hydrogel group cultured with the aforementioned prepared hydrogels without cells. After 3 d of culture, protein expressions of platelet-derived growth factor-D (PDGF-D), insulin-like growth factor-Ⅰ (IGF-Ⅰ), and transforming growth factor β 1 (TGF-β 1) were detected by Western blotting ( n=3). Eight male 8-week-old Sprague-Dawley rats were taken and a circular full-thickness skin defect wound was created on each side of the back. The wounds were divided into blank control group without any treatment and hydrogel group with the aforementioned prepared hydrogels loaded with adipose-derived stem cells applied. Wound healing was observed at 0 (immediately), 2, 4, 8, and 10 d after injury, and the wound healing rate was calculated at 2, 4, 8, and 10 d after injury. Wound tissue samples at 10 d after injury were collected, the new tissue formation was observed by hematoxylin-eosin staining; the concentrations of interleukin-1α (IL-1α), IL-6, IL-4, and IL-10 were detected by enzyme-linked immunosorbent assay method; the expressions of CD16 and CD206 positive cells were observed by immunohistochemical staining and the percentages of positive cells were calculated. The sample numbers in animal experiment were all 8. Results:At 24 h after stimulated contact, no skin edema was observed in the three groups of guinea pigs, and only mild skin erythema was observed in 7 guinea pigs in positive control group. At 48 h after stimulated contact, skin erythema was observed in 8 guinea pigs and skin edema was observed in 4 guinea pigs in positive control group, while no obvious skin erythema or edema was observed in guinea pigs in the other two groups. After 3 d of culture, the protein expression levels of PDGF-D, IGF-I, and TGF-β 1 in adipose-derived stem cells in hydrogel group were significantly higher than those in normal control group (with t values of 12.91, 11.83, and 7.92, respectively, P<0.05). From 0 to 10 d after injury, the wound areas in both groups gradually decreased, and the wounds in hydrogel group were almost completely healed at 10 d after injury. At 4, 8, and 10 d after injury, the wound healing rates in hydrogel group were (38±4)%, (54±5)%, and (69±6)%, respectively, which were significantly higher than (21±6)%, (29±7)%, and (31±7)% in blank control group (with t values of 3.82, 3.97, and 4.05, respectively, Pvalues all <0.05). At 10 d after injury, compared with those in blank control group, the epidermis in wound in hydrogel group was more intact, and there were increases in hair follicles, blood vessels, and other skin appendages. At 10 d after injury, the concentrations of IL-1α and IL-6 in wound tissue in hydrogel group were significantly lower than those in blank control group (with tvalues of 8.21 and 7.99, respectively, P<0.05), while the concentrations of IL-4 and IL-10 were significantly higher than those in blank control group (with tvalues of 6.57 and 9.03, respectively, P<0.05). The percentage of CD16 positive cells in wound tissue in hydrogel group was significantly lower than that in blank control group ( t=8.02, P<0.05), while the percentage of CD206 positive cells was significantly higher than that in blank control group ( t=7.21, P<0.05). Conclusions:The hydrogel loaded with mouse adipose-derived stem cells is non-allergenic, can promote the secretion of growth factors in adipose-derived stem cells, promote the polarization of macrophages to M2 phenotype in wound tissue in rats with full-thickness skin defects, and alleviate inflammatory reaction, thereby promoting wound healing.
5.Feasibility of left atrial appendage closure in patients with persistent left atrial appendage thrombus:a systematic analysis

Objective To investigate the feasibility of left atrial appendage closure(LAAC)in patients with atrial fibrillation combined with persistent left atrial appendage(LAA)thrombus.Methods The authors searched PubMed,EBSCO,CNKI and WANFANG DATA for all abstracts or full-text articles investigating LAAC in patients with atrial fibrillation combined with persistent LAA thrombus from 1 January 2000 to 1 June 2023.Baseline clinical characteristics,procedure-related data and perioperative complications,and clinical outcomes at follow-up were collected.The pooled data were analysed.Results 278 patients with atrial fibrillation combined with persistent LAA thrombus were retrospectively included.The average age was 73.2 years,58.6%male,the mean CHA2DS2-VASc and HAS-BLED scores was(4.62±1.23)and(3.28±0.94),respectively.In patients with previous stroke,ischaemic stroke/transient ischaemic attack(TIA)accounted for 74.8%.Most of the patients had a distally located thrombus in the LAA.All cases underwent successful implantation of LAAC devices with some procedural modifications.Amulet was the most commonly used device(65%).A cerebral protection device was used in 118(42.4%)patients.Most LAAC procedures were underwent with some technical modifications.One TIA(0.36%)and 3(1.08%)mild pericardial effusion that did not require drainage occurred post-procedure.There were 3 major bleeding events,and 2 of them required blood transfusion.The mean follow-up was(6.2±7.1)months,with 238 patients followed up.The all-cause mortality was 1.68%(4/238).Four ischemic strokes(4/238,1.68%)and 12(12/238,5.04%)device-related thrombus were noted during the follow-up.Conclusions Percutaneous LAAC procedures appear to be feasible in patients with persistent LAA thrombus when performed by experienced operators with some technical modifications and chose appropriate type of closure device according to the location of thrombus in the LAA.
6.Long-term efficacy of catheter-based renal sympathetic denervation for hypertension treatment

The morbidity and mortality of hypertension have increased in recent years,threatening human cardiovascular health and increasing the global health burden.Renal sympathetic denervation(RDN),which reduces blood pressure by inhibiting renal sympathetic nerve activity,is a promising and novel treatment for hypertension.Among them,catheter-based renal sympathetic denervation has already been studied in several large-scale clinical studies,and the long-term follow-up results have been published,which has triggered a new thinking about the long-term efficacy of RDN.Therefore,this article focuses on the long-term efficacy of RSD and its potential influencing factors,aiming to provide a reference for the clinical application of RDN.
7.Current status of research on diagnosis,prevention and control of Surra

Surra is a bloodborne protozoan disease caused by Trypanosoma evansi(T.evansi)that infects most mammals and is widely distributed worldwide.The Ministry of Agriculture of China classifies Surra as a category Ⅲ animal disease,it can lead to anemia,wasting,and even death in e-quine animals,camels,buffaloes,etc.,causing huge economic losses to the stock raising industry.In addition,T.evansi may also have zoonotic potential,making its accurate diagnosis crucial.Howev-er,the close affinity between the subgenus of trypanosomes,as well as the presence of antigenic variation and immune escape,make the accurate diagnosis of T.evansi difficult.In this paper,we review the pathogens,immune mechanism,diagnosis,prevention and control of Surra,aiming to provide scientific references for the accurate diagnosis and prevention of this disease.
8.Effects of perioperative use of renin-angiotensin system inhibitor on renal function and clinical outcomes in patients undergoing coronary artery bypass grafting surgery

Objective:To analyze the effects of preoperative renin-angiotensin system inhibitor (RASi) use on postoperative renal function and short-term and long-term prognosis in patients undergoing coronary artery bypass grafting (CABG).Methods:A retrospective cohort analysis was conducted. Based on the registration study data of CABG patients at Fuwai Hospital, Chinese Academy of Medical Sciences, the clinical data of adult patients who underwent CABG from January 2013 to December 2022 were analyzed. Preoperative use of RASi (PreRASi) was defined as receiving RASi treatment within 48 hours before surgery. Postoperative acute kidney injury (AKI) was defined using the diagnostic criteria of Kidney Disease: Improving Global Outcomes (KDIGO). Demographic characteristics, past medical history, comorbidities, preoperative medication, preoperative laboratory test results, specific information on surgical procedures, and postoperative treatment related data were extracted. The primary endpoint was the incidence of postoperative AKI. Secondary endpoints included in-hospital all-cause mortality and all-cause mortality within the longest follow-up period. According to whether RASi was used before surgery, the patients were divided into PreRASi group and No-PreRASi group. The baseline data of the two groups were balanced by propensity score matching (PSM). Logistic regression model and Cox proportional hazards model were used to assess the correlation between PreRASi and postoperative AKI and clinical outcomes, and analyze the subgroups of hypertension and heart failure with preserved ejection fraction (HFpEF) in the cohort.Results:A total of 33?884 patients who underwent CABG were included, with a mean follow-up duration of (3.0±2.4) years and the longest follow-up duration up to 8.5 years. There were 9?128 cases (26.94%) in the PreRASi group and 24?756 cases (73.06%) in the No-PreRASi group. The incidence of postoperative AKI in the PreRASi group was 47.61% (4?346 cases), compared to 52.37% (12?964 cases) in the No-PreRASi group. Two groups were matched with 5?094 patients each. Compared to the No-PreRASi group, both before and after PSM, PreRASi was associated with a reduction of risk of postoperative AKI [before PSM: odds ratio ( OR) = 0.834, 95% confidence interval (95% CI) was 0.793-0.877, P < 0.001; after PSM: OR = 0.875, 95% CI was 0.808-0.948, P = 0.001]. Subgroup analysis of hypertensive and HFpEF patients showed that PreRASi was associated with a decreased risk of postoperative AKI before and after PSM. The in-hospital mortality for the PreRASi and No-PreRASi groups were 0.61% (56 cases) and 0.49% (121 cases), respectively. Analysis of the overall cohort and subgroups with hypertension and HFpEF showed no correlation between PreRASi and in-hospital mortality or longest follow-up mortality. Conclusion:The perioperative use of RASi can reduce the risk of postoperative AKI in patients undergoing CABG, has a certain renal protective effect, but is not associated with short-term or long-term death risk after surgery.
9.Related factors of hypothyroidism induced by programmed death(PD)-1 treatment in elderly patients with cancer

Objective:To investigate the factors related to hypothyroidism induced by programmed death(PD)-1 treatment in elderly patients with cancer.Methods:A total of 193 older patients(≥60 years old)with advanced solid tumors who received PD-1 treatment between January 2018 and January 2021 at the Department of Oncology of Xiangyang Central Hospital were included in this study.The patients were divided into two groups based on whether they were diagnosed with hypothyroidism after PD-1 treatment: the hypothyroidism group(36 cases)and the non-hypothyroidism group(157 cases).The clinical data of both groups, including age, gender, Eastern Cooperative Oncology Group performance status(ECOG PS), PD-1 inhibitors, thyroid function, and thyroid antibody, were compared to analyze the risk factors associated with hypothyroidism.Results:Among the 193 patients, 36(18.7%)were diagnosed with hypothyroidism.The study found no significant differences between the two groups in terms of age, gender, ECOG PS, tumor type, and PD-1 type(all P>0.05).However, significant differences were observed in the baseline levels of thyroid stimulating hormone(TSH)and thyroid antibody subgroups(both P<0.05).The results of multivariate Logistic regression analysis revealed that the presence of baseline anti-thyroid peroxidase antibody(TPOAb)( OR=20.256, 95% CI: 5.709-71.868, P<0.001), the presence of both baseline thyroglobulin antibody(TGAb)and TPOAb( OR=5.853, 95% CI: 1.475-23.227, P=0.012), and an increase in baseline TSH levels( OR=3.065, 95% CI: 1.049-8.959, P=0.041)were identified as risk factors for hypothyroidism induced by PD-1 treatment.On the other hand, there was no significant association between the presence of baseline TGAb and the occurrence of hypothyroidism( OR=1.373, 95% CI: 0.353-5.341, P=0.648). Conclusions:The incidence rate of hypothyroidism induced by PD-1 inhibitors is high among elderly patients with cancer.Additionally, the risk of hypothyroidism is higher in patients with elevated baseline TSH and positive TPOAb.Therefore, it is crucial to remain vigilant for the occurrence of hypothyroidism during PD-1 treatment.Timely diagnosis and treatment of hypothyroidism are necessary to minimize the incidence of adverse events.
10.Serum myostatin and follistatin as biomarkers of sarcopenia in elderly women

Objective:This study aimed to explore the clinical value of myostatin(MST) and follistatin(FST) as biological biomarkers in evaluating sarcopenia in elderly women.Methods:This was a retrospective cross-sectional study that enrolled 350 females aged 20-89 years who underwent physical examinations in Shanghai Huadong Hospital in 2021. Demographic characteristics, muscle mass, fat mass, bone mineral density, hand grip strength, gait speed, and serum indices of MST and FST were collected.Results:The serum levels of MST did not change significantly with age. However, the serum levels of FST increased with age. In women aged≥60 years, MST was positively correlated with total lean mass and appendicular skeletal muscle index(ASMI; r=0.236, P=0.041; r=0.289, P=0.014), while FST was negatively correlated with ASMI( r=-0.265, P=0.030). In multivariate stepwise regression analysis, after adjusting for age, body mass index, hip bone mineral density, and total fat mass, only FST was independently correlated with ASMI( β=-0.238, P=0.006), while MST was not correlated with ASMI. The receiver operating characteristic curve was plotted using muscle mass reduction as the state variable and serum FST level as the test variable. The area under the curve was 0.753. And when the FST cutoff value was 17.49 ng/mL, the maximum Jordan index was 0.46, with a sensitivity of 77.3% and a specificity of 68.7%. Women aged ≥60 years were divided into three groups based on serum FST levels. Compared to the upper third of the serum FST level group, the low third of the FST level group had a significantly reduced risk of suffering from sarcopenia( OR=0.098, P =0.036). Conclusions:Serum FST lever has a better correlation with muscle mass among elderly women, making it a promising biomarker for evaluating muscle mass.
