1.Postoperative corneal topography generation based on attention mechanism and Pix2Pix network

AIM:To explore the use of attention mechanism and Pix2Pix generative adversarial network to predict the postoperative corneal topography of age-related cataract patients undergone femtosecond laser arcuate keratotomy.METHODS:In this retrospective case series study, the 210 preoperative and postoperative corneal topographies from 87 age-related cataract patients(105 eyes)undergoing femtosecond laser arcuate keratotomy at Shanxi Eye Hospital between March 2018 and March 2020 were selected and divided into a training set(180)and a test set(30)for model training and testing. The peak signal-to-noise ratio(PSNR), structural similarity(SSIM)and Alpins astigmatism vector analysis were used to compare the accuracy of postoperative corneal topography prediction under different attention mechanisms.RESULTS:The model based on attention mechanism and Pix2Pix network can predict postoperative corneal topography, among which the model based on Self-Attention mechanism has the best prediction effect, with PSNR and SSIM reaching 16.048 and 0.7661, respectively. There were no statistically significant differences in the difference vector, difference vector axis position, surgically induced astigmatism, and correction index between real and generated corneal topography on the 3mm and 5mm rings(all P>0.05).CONCLUSION:Based on the Self-Attention mechanism and Pix2Pix network, the postoperative corneal topography can be well predicted, which can provide reference for the surgical planning and postoperative effects of ophthalmic clinicians.
3.Network Analysis of Y-STR in Six Ethnic Populations in Guangxi and Its Forensic Significances.

Objective To explore the distribution of genetic structure of Y-SNP and Y-STR genetic markers in different ethnic groups and its application in forensic science. Methods SNaPshot minisequencing was used to detect the polymorphisms of 12 Y-SNP loci in 439 males from 6 ethnic groups, including Guangxi Han, Guangxi Jing, Guangxi Miao, Guangxi Yao, Guangxi Zhuang and Guangxi Dong. DNATyperTM Y26 kit was used to multiplex-amplify 26 Y-STR loci. The PCR products were analyzed by 3130xl genetic analyzer. The network analysis of Y-STR haplotype under the same Y-SNP haplogroup was analyzed by Network 5.0 software. Results Six haplogroups defined by 12 Y-SNP loci were detected in 6 ethnic groups, and 362 haplotypes were detected in 26 Y-STR loci. The haplotype diversity was 0.996 6. In the C haplogroup, the samples from Guangxi Yao, Guangxi Zhuang and Guangxi Dong were clustered on different branches; in the O1 haplogroup, those from Guangxi Zhuang, Guangxi Miao and Guangxi Jing were relatively independent and clustered separately; in the O2 haplogroup, some samples from Guangxi Miao and Guangxi Yao were gathered in a cluster. Conclusion Based on the Y-STR network analysis of samples with identical haplogroup of Y-SNP, some ethnic groups can be preliminarily distinguished, which could be used to infer male suspects' ethnic group through detecting their genetic markers left in the crime scene.

4.Development of clinical practice guidelines for the management of adherence to highly active antiretroviral therapy

Objective To develop the clinical practice guidelines for the management of medication adherence to highly active antiretroviral therapy (HAART) in China. Methods The development methods included qualitative interview of 31 stakeholders, questionnaire survey of 423 PLHIV, adaptation of 30 clinical practice guidelines related to AIDS care, and overviews of reviews of 44 systematic reviews/Meta-analysis. Results 10 clinical practice guidelines and 10 systematic reviews/Meta-analysis were included. The clinical practice guidelines for the management of HAART were formed. Conclusions The formed clinical practice guidelines showed better applicability and higher general quality. It is recommended to use the guidelines in AIDS care.
5.Arthroscopic treatment of symptomatic anterior cruciate ligament cysts of the knee.

OBJECTIVETo explore the clinical symptom and effect of arthroscopic treatment of symptomatic anterior cruciate ligament (ACL) cysts of the knee.
METHODSClinical data from 12 symptomatic ACL cysts patients from January 2005 to December 2010 were retrospectively analyzed,including 8 males and 4 females,with an average age of (33.7±9.5) years old (ranged, 19 to 53 years old). The locations were the left knee in 5 cases and the right knee in 7 cases. The disease duration ranged from 3 to 48 months,with a mean of (15.8±13.2) months. All cysts were arthroscopically resected. Range of motion was measured preoperatively and postoperatively, and Lysholm scoring system was used to evaluate the knee function.
RESULTSAll the incisions healed by first intention, and no complications occurred. Twelve patients were followed up for an average of (32.3±6.6) months(ranged, 24 to 48 months). The symptoms of arthralgia,swelling and interlocking of the affected knees disappeared. There was no recurrence during the follow-up. There were significant differences in the range of motion and Lysholm score between pre-operation and post-operation.
CONCLUSIONArthroscopic surgery, showing its advantages of minimal invasion and rapid recovery,is an effective measure in the treatment of ACL cysts.

6.Study of psychological intervention on alleviating anxiety of patients during high active antiretroviral therapy

Objective To explore the effect of psychological intervention on the alleviating anxiety of patients on high active antiretroviral therapy (HAART).Methods Forty AIDS patients were recruited and were randomly divided into the treatment group (n =20) which received psychotherapy,and control group(n =20) which received traditional care.Self-rating anxiety scale (SAS) was used to investigate patients before and after intervention.Results The SAS score of the treatment group before intervention ( 48.8 ± 1.8 ) was significantly higher than that after the intervention (36.8 ± 1.7),(t =4.825 0,P < 0.001 ).No difference was found in the SAS score in control group before(49.4 ± 1.96) and after(49.2 ± 1.9) the intervention(t =0.073 1,P>0.05).After intervention,the SAS score of intervention group was lower than that of control group (t =- 4.860 1,P < 0.01 ),and the adherence,follow-up compliance of treatment group were significantly improved (P < 0.05 or P < 0.01 ).Conclusions Psychological intervention can effectively alleviate the anxiety of patients on HAART.
7.Clinical study of early hyperbaric oxygen therapy for cerebral resuscitation

Objective To observe the effect of early hyperbaric oxygen (HBO) therapy on cerebral resuscitation and to provide good evidence and method for the treatment of cerebral resuscitation.Methods The patients responded to cardiac pulmonary resuscitation (CPR) were divided into 2 groups,the early HBO group( within one week of onset) and the delayed HBO group (after 1 week of onset).All the patients were given routine medicinal treatment coupled with HBO therapy.The exposure pressure of HBO and the course of treatment were all the same,only the time point of HBO intervention was different.Evaluation was made by using "the Revised 2001 PVS Criteria for Diagnosis and Treatment" ( the Nanjing Criteria).Consciousness of the patients was evaluated both before HBO therapy and 6 months after onset.Results Of the 18 patients who received early HBO therapy,4 patients recovered and returned to normal life (effective),5 patients became fully conscious,but with minor disability (effective),and 9 patients became vegetable (ineffective),with a total effective rate of 50.0%.Of the 53 patients with delayed HBO therapy,9 patients recovered and returned to normal life (effective),15 patients became fully conscious,but with minor disability (effective),and 29 patients became vegetable (ineffective),with a total effective rate of 45.3%.No significant differences could be seen in effective rates between the 2 groups (P >0.05).However,statistical significance could be noted,when effective rates of the 2 groups were compared with those presented in domestic and international reports (2%-10% ) ( P < 0.05 ).Conclusions In-time HBO therapy could improve prognosis of some patients following cardiac resuscitation,protect neural cells,and improve the intelligence of patients,however,early and ultra-early HBO therapy could not increase the effective rate of cerebral resuscitation.

9.Analysis of anxiety symptoms among AIDS patients in Shanghai

Objective To understand the anxiety state and its influencing factors among AIDS patients in Shanghai.Methods Self-rating Anxiety Scale,SAS was used in this study.The general information and the scores of SAS form 254 patients with AIDS were collected.Results Anxiety scores were no significantly different in different gender and different education level of AIDS patients.However,the mean anxiety scores and route of infection of patients over 40-year-old were different from patients below 40-year-old.Conclusions The anxiety symptoms of AIDS patients from different infection routes are not same.The community attitude to AIDS patients impacts on the patients' quality of life.It is important to enhance health education and improve social support for AIDS patients.
10.Expression of heparin-binding epidermal growth factor-like growth factor in patients with gastric carcinoma.

OBJECTIVETo investigate the expression of heparin-binding epidermal growth factor-like growth factor (HB-EGF) in patients with gastric carcinoma in different stages.
METHODSThe expressions of HB-EGF protein and mRNA in normal gastric tissues, metaplasic intestinal mucosa, early-stage gastric cancer and advanced-stage gastric cancer tissues were detected by immunohistochemistry and in situ hybridization.
RESULTSHB-EGF expression was only detected in the parietal cells of the gastric fundic glands and in gastrin cells of the pyloric glands in normal gastric tissues. Weak HB-EGF expression was detected in the epithelial cells of the gastric mucosa in intestinal metaplasic mucosa, and the expression increased in all layers of the gastric mucosa in early-stage gastric cancer. Intense HB-EGF expression was observed in advanced gastric cancer.
CONCLUSIONIncreased HB-EGF expression may be implicated in the pathogenesis and development of gastric carcinoma.
