1.Pharmacodynamic Substances and Mechanisms of Xinglou Chengqi Tang in Treating Post-stroke Complications: A Review

Stroke is the leading cause of death and disability among adults in China, and its common complications include digestive system abnormalities, cognitive impairment, depression, stroke-associated pneumonia, and hemiplegia. The combination of traditional Chinese and Western medicine has great potential in treating post-stroke complications. Xinglou Chengqitang (XLCQT) is a representative prescription of alleviating the disease in the upper part by treating the lower part. It has definite therapeutic effect and high safety. Clinically, XLCQT is often used to treat stroke and its complications. However, the quantity and quality of clinical trials of XLCQT in treating post-stroke complications need to be improved. Additionally, since the basic research is weak, the material basis and multi-target mechanism for the efficacy of this prescription are unknown. This article reviews XLCQT in terms of the pharmacodynamic basis, medicinal properties, safety evaluation, and progress in clinical research and mechanisms in treating post-stroke complications. This article summarizes 22 key active ingredients of XLCQT in treating acute stroke complicated with syndrome of phlegm heat and fu-organ excess. Among these key active ingredients, resveratrol, kaempferol, luteolin, chrysoeriol, apigenin, (+)-catechin, and adenosine have good pharmacokinetic properties and high bioavailability. The mechanisms of XLCQT in treating post-stroke complications are complex, including inflammatory response, brain-gut axis, hypothalamic-pituitary-adrenal (HPA) axis, intestinal flora, neurotrophic factors, autophagy, oxidative stress, and free radical damage. This review helps to deeply understand the pharmacodynamic basis and mechanisms of XLCQT in treating post-stroke complications and provides a theoretical basis for the clinical application of XLCQT against post-stroke complications and the development of drugs.

Ferroptosis is a key driver of the onset and progression of chronic obstructive pulmonary disease (COPD). By exploring the role of ferroptosis in COPD from the perspective of "qi deficiency with retention and stagnation", it is considered that mitochondrial dysfunction and imbalanced antioxidant defenses are the microscopic manifestations of "qi deficiency", whereas iron accumulation and lipid peroxide deposition constitute the pathological basis of "retention and stagnation". In traditional Chinese medicine (TCM), the treatment principle is tonifying deficiency and benefiting qi, scattering retention and unblocking stagnation. Its mechanism involves improving the antioxidant system and mitochondrial function to enhance cellular resistance to ferroptosis, as well as relieving pulmonary iron overload, excessive lipid peroxidation, and inflammatory factor release to reduce the accumulation of pathological products, thereby exerting therapeutic effects on COPD.
3.Role of cardiometabolic comorbidities in association between dietary behavior and mild cognitive impairment

Objective To investigate the association between dietary behavior and mild cognitive impairment(MCI)in older adults,and to further analyze the role of cardiometabolic comorbidities in this relationship.Methods A total of 6599 older adults were recruited from 3 communities and 48 villages in Dawu County between 2018 and 2023 as part of the Hubei Elderly Memory Cohort Study.Dietary behaviors were assessed using a food frequency questionnaire and a dietary behavior questionnaire.Latent class analysis was performed to categorize the participants into healthy eat-ing behavior(HEB),sub-healthy eating behavior(SHEB),and unhealthy eating behavior(UEB).Cardiovascular-metabolic diseases were diabetes,hypertension,coronary heart disease,and cere-brovascular disease,which all diagnosed by physicians.MCI was diagnosed by a team of clinical experts according to Peterson's criteria.Multivariable logistic regression model was used to ana-lyze the impact of cardiometabolic comorbidities on the association between dietary behavior and MCI.Results The morbidity rate of MCI was 24.3%,and that of HEB,SHEB and UEB was 16.6%,24.3%and 31.3%,respectively.The incidence of MCI was higher in the participants who were female,over ≥75 years old,unmarried,and lack physical exercise,SHEB and UEB groups,had low educational level,lived in rural areas,had no stable income,had abnormal BMI,and more types of cardiometabolic diseases(P<0.05,P<0.01).After adjusting for confounders,multivari-able logistic regression analysis indicated that both UEB(OR=1.220,95%CI:1.004-1.418,P=0.045)and SHEB(OR=1.592,95%CI:1.345-1.883,P=0.001)were positively correlated with MCI risk in older adults.Further stratified analysis by cardiometabolic comorbidities revealed that for the patients in the HEB group,those suffering from hypertension+diabetes+coronary heart disease had the highest risk for MCI(OR=4.220,95%CI:1.913-9.309,P=0.001),while for the SHEB group,the following comorbidities were significantly associated with increased MCI risk:hypertension+diabetes(OR=1.640,95%CI:1.157-2.322,P=0.005),hypertension+cerebro-vascular disease(OR=1.454,95%CI:1.041-2.031,P=0.028),hypertension+diabetes+cere-brovascular disease(OR=2.064,95%CI:1.246-3.419,P=0.005),and hypertension+diabetes+coronary heart disease+cerebrovascular disease(OR=1.974,95%CI:1.036-3.760,P=0.039).Conclusion Older adults with SHEB or UEB have a higher risk of developing MCI,and the pres-ence of cardiometabolic comorbidities further exacerbates this risk.
4.Study on the effectiveness and safety of a novel intravascular shock wave balloon for pre-treatment of severe coronary artery calcification lesions

Objective To evaluate the efficacy and safety of a novel intravascular lithotripsy(IVL)balloon—Vesscrack shockwave balloon—for vascular preparation before stent implantation in patients with severe coronary artery calcification(CAC).Methods This was a prospective,single-arm,multicenter study conducted in China from June 2022 to October 2022.Patients with severe CAC were treated with the Vesscrack shockwave balloon for lesion preparation,followed by drug-eluting stent(DES)implantation.Of these,33 patients underwent optical coherence tomography(OCT).The primary endpoint was procedural success,defined as successful stent implantation with residual stenosis≤30%and the absence of in-hospital major adverse events,including cardiac death,target vessel-related myocardial infarction,or target lesion revascularization.Results A total of 170 patients[mean age:(65.9±7.9)years,116 males]were enrolled.After treatment with IVL and DES,the minimum lumen diameter increased significantly compared to baseline[(2.34±0.40)mm vs.(0.95±0.33)mm,P<0.001],the degree of stenosis was significantly reduced[(13.24±6.60)%vs.(65.18±10.59)%,P<0.001].Procedural success was achieved in 100%of cases,and device success was 98.8%.The 30-day patient-related cardiovascular clinical composite endpoint(POCE)rate was 0.0,with no target lesion failure,no confirmed or potential thrombotic events were observed.The shockwave energy generator demonstrated excellent stability and ease of use.Among the 33 patients assessed with OCT,after IVL intervention,the maximum calcified area of the lumen[(3.51±1.51)mm2 vs.(2.85±1.80)mm2,P<0.001],and the minimum lumen area within the target lesion[(3.08±1.04)mm2 vs.(2.02±0.75)mm2,P<0.001],and after DES intervention,the luminal area of the largest calcified site[(6.59±1.64)mm2 vs.(2.85±1.80)mm2,P<0.001]and the minimum luminal area within the target lesion[(6.19±1.45)mm2 vs.(2.02±0.75)mm2,P<0.001]were significantly increased,and the differences were statistically significant.Conclusions The Vesscrack shockwave balloon is effective and safe for vascular preparation in patients with severe CAC prior to stent implantation.It achieves significant calcified plaque modification,high procedural success rates,and minimal complications.

Opium is obtained by air-drying the milky latex extracted from the unripe capsules of the opium poppy(Papaver somniferum).This latex is rich in benzylisoquinoline alkaloids(BIA),with major active compounds in-cluding morphine,codeine,thebaine,papaverine,and noscapine.Compound licorice oral solution and compound licorice tablets are derivative drugs containing opium.Initially classified as over-the-counter(OTC)medications,both formulations were later reclassified as prescription drugs by the National Medical Products Administration(NMPA),restricting their purchase without proper authorization.Although the national pharmacopeia standards specify the morphine content in the opium raw materials used for compound licorice oral solution and tablets,they lack mandatory requirements for the detection and quantification of the other four major alkaloids.Given the unique nature of opium raw materials and the stringent regulatory requirements for such drugs,it is imperative to enhance and refine simultaneous detection and control methods for all alkaloid components in these products.Furthermore,the establishment of scientific and reasonable detection and control standards for preservatives in compound licorice formulations is crucial to improving overall product quality management and ensuring drug safety and efficacy.This study analyzes and discusses the quality standards,detection methods,and research progress for opium and com-pound licorice preparations,aiming to explore the potential for technological innovation and ensure the safe use of these medications.
6.Observation on the efficacy of Huoluo Xiaotong capsules in treating cervical spondylosis radiculopathy:a real-world study

Objective To compare the early efficacy and safety of Huoluo Xiaotong capsules and Jingtong granules in treating cervical spondylosis radiculopathy(CSR).Methods A real-world study was conducted among CSR patients who visited three tertiary-grade-A hospitals in Changsha between January 2024 and February 2025.Patients were divided into a control group(Jingtong granules,4 g,po,tid)and an observation group(Huoluo Xiaotong capsules,1.4 g,po,tid);both groups were treated for 2 weeks.Baseline characteristics were balanced with inverse probability of treatment weighting(IPTW).Numeric rating scale(NRS)scores,overall response rates,and traditional Chinese medicine(TCM)symptom scores were compared at baseline,1 week,and 2 week.Results A total of 120 patients were ultimately included,with 30 in the control group and 90 in the observation group.After IPTW adjustment,at 1 week of treatment,the clinical efficacy rate of observation group(98.85%)were significantly exceeded that of control group(80.14%)(P<0.001);at 2 weeks,the clinical efficacy rate of the two groups were both 100.00%.NRS and TCM syndrome scores showed no intergroup differences at baseline or 1 week(P>0.05),at 2 weeks,the observation group demonstrated significantly lower NRS and TCM syndrome scores versus controls(P<0.001).No adverse reactions occurred in either group.Conclusion Huoluo Xiaotong capsules demonstrate favorable short-term efficacy and safety in CSR,significantly alleviating pain and improving TCM symptoms within 2 weeks.High-quality randomized controlled trials are warranted to confirm these findings.
7.Tissue-Bone Homeostasis Manipulation Restores Quadriceps-Patellar Ligament Biomechanical Homeostasis and Functional Performance in Knee Osteoarthritis:A Randomized Controlled Trial

OBJECTIVE To investigate the intervention effects of tissue-bone homeostasis manipulation(TBHM)on peripatellar biomechanical parameters and knee joint function in knee osteoarthritis(KOA)patients.METHODS Sixty patients with KOA(Kell-gren-Lawrence grade Ⅱ-Ⅲ)were recruited from the Acupuncture-Moxibustion Rehabilitation Department,Anhui University of Chi-nese Medicine between October 2024 and May 2025.Participants were randomized into a TBHM group(n=30)or a transcutaneous e-lectrical neuromuscular stimulation(TENS)group(n=30).Using two-way repeated measures ANOVA,biomechanical indicators,in-cluding rectus femoris tension,vastus medialis tension,vastus lateralis tension,patellar ligament tension,lateral patellar displacement(LPD),medial patellar displacement(MPD),normalized patellar mobility(LPD/patellar width[PW],MPD/PW),knee flexion range of motion,and functional indicators,including KOOS subscales,time up and go test(TUGT),were compared between groups at baseline and after 6 weeks of intervention.RESULTS After intervention,all biomechanical and knee joint function indicators in the TBHM group were significantly improved(P<0.05,P<0.01),while only the vastus medialis tension,TUGT and KOOS Pain,ADL and QoL scores in the control group were significantly improved(P<0.01).The improvement amplitudes of biomechanical indicators in the TBHM group,including rectus femoris tension,vastus lateralis tension,patellar ligament tension,MPD/PW,LPD/PW and knee flexion range of motion were better than those in the control group(P<0.05,P<0.01).In the functional evaluation,the interaction effects of the TBHM group in all dimensions of the KOOS score and TUGT were statistically significant(P<0.05,P<0.01).Post-hoc simple effect analysis confirmed that there were significant differences in the above indicators between the two groups after intervention(P<0.05),and all indicators showed a significant main effect of time(P<0.01),suggesting that the intervention measures had contin-uous and cumulative curative effects.CONCLUSION TBHM effectively improves joint function and quality of life in KOA patients by restoring dynamic equilibrium in soft tissue tension and patellar mobility,ultimately achieving the therapeutic goal of concurrent tis-sue-bone management.
8.Mediating Effect of 25-Hydroxyvitamin D3 on the Relationship between Psychological Status and Cognitive Function in Elderly Obese Patients

Objective:The purpose of this study is to explore the mediating effect of 25-hydroxyvitamin D3[25(OH)D3]on the relationship between psychological status and cognitive function in elderly obese patients.Methods:130 elderly obese patients who visited Hongdu Traditional Chinese Medicine Hospital in Nanchang from January 2023 to June 2024 were selected,they were divided into cognitive impairment group(n=27)and cognitive normal group(n=103)based on the Mini Mental State Examination(MMSE)score.General information were prospectively collected,their psychological status used Hamilton Depression Rating Scale(HAMD)and Hamilton Anxiety Rating Scale(HAMA)to evaluated,25(OH)D3 levels were detected by enzyme-linked immunosorbent assay.General information,psychological status scores,and 25(OH)D3 levels were compare between two groups.Analysis of the correlation between 25(OH)D3 and psychological status and cognitive function by Pearson method.Analysis of the mediating effect of 25-hydroxyvitamin D3[25(OH)D3]on the relationship between psychological status and cognitive function in elderly obese patients.Results:Compared with cognitive normal group,cognitive impairment group had higher age and lower years of education(P<0.05).HAMA and HAMD scores in the cognitive impairment group were significantly higher than those in the cognitive normal group(P<0.05),and the 25(OH)D3 level was significantly lower than that in the cognitive normal group(P<0.05).Pearson correlation analysis showed that,25(OH)D3 was negatively correlated with HAMA score and HAMD score,and positively correlated with MMSE score(P<0.05);HAMA score,HAMD score,and MMSE score were all negatively correlated(P<0.05).The structural equation model shows good fit[x2/df=1.87,root mean square error approximate=0.062,comparative fit index=0.945,Tucker-Lewis index=0.931].The direct effect of psychological status(HAMA+HAMD)on cognitive function was-0.325(95%confidence Interval(CI):-0.454~-0.216,P<0.05),and the mediating effect through 25(OH)D3 was-0.182(95%CI:-0.271~-0.112,P<0.05),accounting for 36.00%of the total effect.Conclusion:In elderly obese patients,25(OH)D3 is positively correlated with cognitive function and negatively correlated with psychological status,moreover,25(OH)D3 plays a significant mediating role in the relationship between psychological status and cognitive function,and is an important mediating factor affecting the relationship between the two.
9.Genome sequencing and biological characteristics analysis of a Streptococcus dys-galactiae from yak

This study aims to understand the biological characteristics of Streptococcus dysgalacti-ae of yak origin.Bacterial isolation and identification,drug susceptibility test,virulence gene test and pathogenicity test were carried out on milk samples of yaks from Naqu City to evaluate the bi-ological characteristics of the isolated strains.Meanwhile,molecular biological information such as virulence factors and drug resistance genes were analyzed by whole genome sequencing,and viru-lence genes were verified by PCR.The results showed that a strain of Streptococcus dysgalactiae was isolated from the milk of yak,and its colony morphology was pinpoint size,smooth edge and milky white.This strain is sensitive to many antibiotics(penicillin G,cephalosporin,ciprofloxacin,tetracycline,erythromycin,etc.).Virulence gene test results showed that the strain carries six key virulence genes(cyl,eno,scpB,bca,bac and napr),which may be closely related to its pathoge-nicity.In the pathogenicity test,the mice were listless and less active after infection,but no death occurred during the observation period.The pathological changes of spleen,kidney,liver and lung tissue were found,suggesting that the strain had certain pathogenic potential but not high lethali-ty.Whole genome sequencing data showed that the gene length of this strain was 4 079 280 bp,the GC content was 39.41%,3 964 coding genes were predicted,604 of which were annotated as viru-lence factors,and another 28 gene mutations may enhance its pathogenic ability.Through annota-tion of CARD database,two Pat A resistance genes and two lmrp resistance genes were found,re-vealing their potential resistance mechanism.Through whole genome sequencing technology and bioinformatics analysis method,this study revealed the genomic characteristics,drug resistance and pathogenicity mechanism of Streptococcus dysgalactiae of yak origin.The findings provide impor-tant scientific evidence for further exploration of the pathogenicity,drug resistance mechanisms,and molecular evolution of yak-derived Streptococcus agalactiae.
