Nonunion of osteoporotic vertebral fractures (OVF), predominantly affecting the elderly, can lead to intractable pain, vertebral collapse, progressive kyphotic deformity, and neurological impairment, significantly compromising patients′ quality of life. There exists considerable debate on diagnosis and management of OVF, encompassing key issues such as clinical diagnosis and staging criteria for nonunion, surgical indications and procedure selection, and postoperative rehabilitation planning. Currently, there lacks standardized clinical guideline and expert consensus on the diagnosis and management of OVF nonunion in China. To address this gap, Minimally Invasive Surgery Group of Chinese Orthopedic Association, Osteoporosis Committee of Chinese Association of Orthopedic Surgeons, Prevention and Rehabilitation Committee for Osteoporosis of Chinese Association of Rehabilitation Medicine and Minimally Invasive Orthopedic Surgery Branch of China Association for Geriatric Care jointly organized domestic experts in spinal surgery, endocrinology, and rehabilitation to formulate the Clinical guideline for the diagnosis and treatment for nonunion of osteoporotic vertebral fractures ( version 2025), based on existing literature and clinical experience and adhering to principles of scientific rigor and practicality. The guideline provided 13 evidence-based recommendations encompassing diagnosis and treatment of OVF nonunion, aiming to standardize its clinical management.

Objective:To explore the value of droplet digital PCR (ddPCR) and next-generation sequencing (NGS) technology in the pathogenetic diagnosis of sepsis patients, and to provide a reference basis for the early diagnosis of sepsis. Methods:A retrospective analysis was used to collect the clinical data of 53 patients with suspected sepsis admitted to the intensive care unit (ICU) of the Second Affiliated Hospital of Xi'an Jiaotong University from February to August 2023, and the blood was collected for blood culture, ddPCR and NGS detection simultaneously. Results:After excluding viral infections, the blood culture positive rate was 18.87%(10/53), the ddPCR positive rate was 47.17% (25/53), and the NGS positive rate was 41.51%(22/53). When using the ddPCR detection range as a reference, the ddPCR positivity rate was 98.11% (52/53), while the NGS positivity rate was 84.91%(45/53). There was a statistically significant difference in the positivity rate between the two groups ( P<0.05). Using blood culture as a reference, the sensitivity (60.00% vs 70.00%) and specificity (65.11% vs 69.77%) of ddPCR and NGS were in good agreement. In terms of pathogen detection, NGS had a wider detection range than ddPCR (34 species vs 21 species), and the difference was statistically significant (χ 2=55.000, P<0.001). In terms of time-consumption, blood culture took 66.93 h on average, while ddPCR was faster than NGS (about 4 h vs 20 h ). In terms of antimicrobial resistance (AMR) gene detection, five resistant strains were detected by both ddPCR and NGS, ddPCR detected two AMR genes, namely blaKPC and mecA, while NGS detected five AMR genes. Among them, except for blaKPC which was detected outside the target range in ddPCR, the other four AMR genes were also detected by ddPCR. Conclusions:Compared with blood culture, ddPCR and NGS have good application value in the etiological diagnosis of sepsis. Specifically, ddPCR has a higher detection rate of pathogens and takes less time. On the other hand, NGS has a wider detection range, especially for the discovery of some rare bacteria or pathogens that are difficult to be cultured routinely.

BACKGROUND:Intervertebral disc degeneration is the basis of spinal degenerative diseases;however, there is no effective treatment. OBJECTIVE:To investigate whether sinomenine can inhibit interleukin-1β-induced apoptosis in nucleus pulposus cells and its molecular mechanism. METHODS:Rat nucleus pulposus cells were cultured in vitro by trypsin combined with type II collagenase digestion, and the cell growth curve was plotted. An appropriate sinomenine concentration was determined using the cell counting kit-8 kit.Nucleus pulposus cells were divided into control group, sinomenine group,interleukin-1β group,sinomenine+interleukin-1β group, zinc protoporphyrin group, zinc protoporphyrin+sinomenine group, zinc protoporphyrin+interleukin-1β group, and sinomenine+zinc protoporphyrin+interleukin-1β group.Proliferative activity, reactive oxygen species content, apoptosis rate, and heme oxygenase-1 expression in nucleus pulposus cells were detected. RESULTS AND CONCLUSION:The rat nucleus pulposus cells cultured in vitro were polygonal, triangular, and short wedge-shaped,and the cell growth showed an"S"curve. The cells grew slowly in the first 3 days of culture, rapidly in 4-6 days, and slowly again in 7-8 days. The cells then entered the"platform stage"where the number of cells no longer increased. The proliferative activity of myeloid cells showed no significant changes when the concentration of sinomenine was≤80 μmol/L(P>0.05).Interleukin-1β significantly reduced the proliferative activity of nucleus pulposus cells, increased the content of reactive oxygen species and led to apoptosis(P<0.01).Sinomenine intervention not only promoted heme oxygenase-1 expression(P<0.05)but also inhibited interleukin-1β-induced decrease in proliferative activity and increase in reactive oxygen species content and apoptosis rate in nucleus pulposus cells(P<0.05).These effects could be reversed by zinc protoporphyrin(P<0.01).

Journal of Modern Urology 2023;28(9):742-747
【Objective】 To summarize and analyze the clinical data of fumarate hydrase-deficient renal cell carcinoma (FH-RCC), so as to improve the understanding of the disease. 【Methods】 General clinical data of 12 FH-RCC patients treated during Mar.2019 and Dec.2021 were retrospectively analyzed, including the imaging, pathological, genetic testing, surgical and adjuvant therapy, and follow-up results. 【Results】 All cases were confirmed with pathology or genetic tests, including 1 case complicated with uterine fibroids, 3 cases with renal cysts, 4 with family history of uterine fibroids and 2 with family history of renal carcinoma. Cysticular irregular density shadows with an enhancement of 25.8 Hu were detected in 4 cases. Of the 7 cases undergoing genetic testing, 2 had embryo and system mutation, 1 had germ line mutation, 1 had system mutation, and 3 had no germ line or system mutation. Radical nephrotomy was performed in 9 cases. The average operation time was 3.8 h, and the average amount of blood loss was 625 mL. Immunotherapy with targeted therapy was conducted in 10 cases, surgery with postoperative adjuvant therapy in 7 cases, and tyrosine kinase inhibitor with immune checkpoint inhibitor (TKI/ICI) in 1 case. All 12 cases were followed up, 3 died of tumor, 1 had no recurrence, 8 showed progress on imaging. 【Conclusion】 FH-RCC is a rare and highly malignancy prone to metastasis. Imaging shows cystic solid space occupying lesions. FH immunohistochemical staining is negative. Genetic testing is needed to confirm the diagnosis. TKI/ICI combination therapy has a good survival benefit.

Nucleotide binding oligomeric dome-like receptor protein 3 (NLRP3) inflammasome is an intracellular sensing protein complex, and it is an important player in the innate immune system, capable of sensing foreign pathogens and endogenous danger signals. After tissue injury, the activation of the NLRP3 inflammasome induces the release of the pro-inflammatory cytokines interleukin (IL)-1β and IL-18, while promoting gasdermin D-mediated pyroptosis. Existing studies have shown that NLRP3 inflammasome plays a key role in the occurrence and development of common bone and joint diseases such as osteoporosis, osteoarthritis, rheumatoid arthritis, and gouty arthritis by inducing inflammatory cascade reaction and accelerating bone resorption and cartilage destruction. Therefore, blocking the NLRP3 inflammasome signaling pathway may be an effective strategy to treat or prevent bone and joint diseases. Currently, researchers have developed and tested several drugs that selectively target the NLRP3 inflammasome in animal and clinical studies, but the progress has been poor due to obvious side effects and high prices. Traditional Chinese medicine (TCM) has been widely recognized in the treatment of bone and joint diseases due to its unique advantages of multi-target, multi-pathway, multi-mechanism synergism, low price, and low side effects. With the deepening of research, the targeted intervention of NLRP3 inflammasome by TCM in the treatment of bone and joint diseases has attracted wide attention. In this paper, the mechanism of NLRP3 inflammasome in osteoarthritis was summarized by analyzing relevant literature in China and abroad in recent years, and the progress of targeted intervention of NLRP3 inflammasome by TCM in the treatment of bone and joint diseases was systematically reviewed, so as to provide new ideas and theoretical basis for the treatment of bone and joint diseases.

ObjectiveTo investigate a cluster epidemic of coronavirus disease 2019 (COVID-19) infections in a school in Longchuan County, Yunnan Province, and further guide the prevention and control of COVID-19 in the border area. MethodsAccording to the Protocol on Prevention and Control of Novel Coronavirus Pneumonia (8th Edition), an epidemiological investigation was performed on all COVID-19 cases to collect the information on demographics, onset, diagnosis and treatment, prognosis, and epidemiological history. Close contacts were also tracked to determine the transmission chains. ResultsIn this cluster epidemic, a total of 37 COVID-19 cases were identified, including 32 females and 5 males aged from 13 to 25 years, who were 35 students and 2 teachers. The student cases were found in four classes of two grades. Furthermore, gene sequencing showed that all cases had been infected with delta variants, belonging to the same transmission chain that was not related to the previous epidemics in Dehong Prefecture. In additionally, a total of 2 127 close contacts were found. After 21 days of centralized quarantine for medical observation, all close contacts tested negative for SARS-CoV-2. In the COVID-19 cases, only one case remained positive for SARS-CoV-2, while the other 36 cases were successfully treated and became negative. ConclusionThis school cluster is caused by the border villagers who contacted the water polluted with SARS-CoV-2. It warrants more strict management of students from border villages and their belongings to prevent similar epidemics in school settings.
