1.Analyzing the heart-oriented view of the treatment of mental illness in Synopsis of Golden Chamber based on the theory of five-spirit-viscera

Mental state is an important part of the normal life activities of the human body, and it is also the most external expression and the most easily obtained information of the physical condition. The normal activities of the mind depend on the normal operation of the viscera, qi, and blood, and are a unified whole that prospers together and suffers together. The theory of the five-spirit-viscera in the Yellow Emperor’s Inner Classic revealed that the normal mental activities of the human body were dominated by the five internal organs, that is, the five internal organs were the body and the five spirits were the function. And it highlighted the viewpoint that the five internal organs store the spirits and are actually one. The heart governs the spirit and belongs to the four internal organs. On this basis, Synopsis of Golden Chamber used the internal organs to diagnose and treat mental diseases, integrating the theory of the five spirits into it, forming a unique method of diagnosis and treatment with the heart as the leading factor and regulating the qi and blood of the four internal organs. It identified the pathogenesis of diseases such as pathogenic crying, lily disease, and hysteria from five levels: heart deficiency and weak qi, heart-lung disharmony, heart-liver disharmony, the heart of the loss of the spleen nourishment, and disharmony between heart and kidney. The treatment was mainly to replenish the deficiency of the viscera and eliminate the pathogens, reflecting the characteristics of regulating the mind and calming the four internal organs. This unique view on diagnosis and treatment has profoundly influenced the diagnosis and treatment theories of mental illnesses by later doctors, and is of great significance to the current clinical treatment of such illnesses.
2.Efficacy of anlotinib in the treatment of advanced radioactive iodine-refractory differentiated thyroid cancer and the effect on iodine uptake of lesions

Objective:To study the efficacy and safety of anlotinib in the treatment of advanced radioactive iodine-refractory differentiated thyroid cancer (RAIR-DTC) and the effect of anlotinib on iodine uptake of lesions.Methods:A retrospective analysis was performed on 23 patients (10 males and 13 females, age (59.1±8.7) years) with advanced RAIR-DTC who were treated with anlotinib in Liaocheng Hospital Affiliated to Shandong First Medical University between January 2019 and February 2023 and met the enrollment criteria. Thyroid function, serum thyroglobulin (Tg) and Tg antibody (TgAb) were determined every 6 weeks during the treatment with anlotinib, and maximum diameter of target lesion (TL) was monitored by CT every 12 weeks, in order to evaluate the therapeutic efficacy, and treatment-related adverse reactions were observed. Diagnostic 131I whole body scan (Dx-WBS) was performed in some patients to evaluate the changes in iodine uptake of lesions after anlotinib treatment. In this study, the posttreatment changes of patients within 24 weeks during the treatment were analyzed. The maximum diameter of TL and Tg at different time points were compared by Friedman test, and were further compared in pairs with P values corrected by Bonferroni method. Results:After 24 weeks of treatment with anlotinib, 8 of 23 patients achieved partial response, 15 had stable disease, and no patients achieved complete response. Serum Tg levels at 6, 12, 18, 24 weeks after anlotinib treatment were 189.5(85.0, 483.3), 127.7(52.4, 319.8), 82.0(40.2, 213.5) and 80.1(39.9, 205.0) μg/L, all of which were lower than the baseline level of Tg (384.5(210.9, 1 605.0) μg/L; χ2 values: 4.23-7.86, all P<0.001). Tg level at 18 weeks after treatment was statistically different from that at 12 weeks after treatment ( χ2 =3.06, P<0.001), but was not statistically different from that at 24 weeks after treatment ( χ2 =12.57, P=0.059). The maximum TL diameters of lung and cervical lymph nodes were significantly reduced at week 12 and 24 of anlotinib treatment compared with baseline ( χ2 values: 14.76-31.12, all P<0.001), while there was no significant difference in TL maximum diameter at 12 and 24 weeks of treatment ( χ2 values: 5.65, 9.02, P values: 0.314, 0.070). Common adverse reactions included hypertension, hand-foot syndrome, hyperacylglyceremia and proteinuria. No adverse reactions above grade 4 or death related to adverse reactions occurred. Dx-WBS evaluation was performed in 7 patients after anlotinib treatment, and no change in iodine uptake was found. Conclusions:Anlotinib has a clear effect on advanced RAIR-DTC with less adverse reactions. The efficacy of anlotinib reaches the strongest at around 12-18 weeks and becomes stable at 24 weeks. No effect of anlotinib on inducing redifferentiation of RAIR-DTC cells and enhancing iodine uptake is found.
3.Relationship between α-synuclein and ApoA1 expression and cognitive function in elderly patients with cerebral small vessel disease

Objective:This study aims to investigate the expression levels of α-synuclein(α-synuclein)and apolipoprotein A-1(ApoA1)in elderly patients with cerebral small vessel disease(CSVD)and their association with cognitive function.Methods:A total of 176 elderly patients diagnosed with CSVD and admitted to Huludao Central Hospital between September 2022 and February 2024 were retrospectively selected as research subjects.Based on their cognitive function at the time of admission, patients were categorized into a disorder group comprising 71 cases and a normal group consisting of 105 cases.General information was collected from the patients, and the serum expression levels of α-synuclein and ApoA1 were measured.Pearson correlation analysis was employed to assess the relationship between the expression levels of α-synuclein and ApoA1 and the Montreal Cognitive Assessment(MoCA)scores.Furthermore, multi-factor logistic regression analysis was conducted to identify influencing factors, and receiver operating characteristic(ROC)curves were generated to evaluate the diagnostic value of serum levels.Results:Patients in the disorder group were older than those in the normal group.Additionally, the MoCA score and serum α-synuclein levels were significantly higher in the disorder group, while ApoA1 levels were lower(all P<0.05).Pearson correlation analysis indicated that the MoCA score was negatively correlated with α-synuclein expression and positively correlated with ApoA1 expression(all P<0.05).Multivariate logistic regression analysis revealed that age, smoking history, and α-synuclein levels are risk factors for cognitive dysfunction in patients with CSVD, whereas ApoA1 is identified as a protective factor for cognitive function in this population.Furthermore, ROC curve analysis demonstrated that serum α-synuclein and ApoA1 levels possess diagnostic value for cognitive dysfunction in patients.The area under the curve(AUC)for the combined predictions was 0.836, with sensitivity and specificity rates of 76.14% and 78.14%, respectively. Conclusions:Elderly patients with CSVD and cognitive dysfunction exhibit abnormally high levels of α-synuclein and abnormally low levels of ApoA1.Both factors are influential in the context of cognitive dysfunction among elderly patients with CSVD and contribute to improved diagnostic efficiency.

Neck dissection(ND)is one of the main treatment methods for oral and maxillofacial malignancies.Although ND type is in con-stant improvement,but intraoperative peal-pull-push injury of the accessory nerve,muscle,muscle membrane,fascia and ligament induced shoulder syndrome(SS)is still a common postoperative complication,combined with the influence of radiochemotherapy,not only can cause pain,stiffness,numbness,limited dysfunction of shoulder neck and arm,but also may have serious impact on patient's life quality and phys-ical and mental health.At present,there is still a lack of a systematic evaluation and rehabilitation management program for postoperative SS of oral and maxillofacial malignant tumors.Based on the previous clinical practice and the current available evidence,refer to the relevant lit-erature at home and abroad,the experts in the field of maxillofacial tumor surgery and rehabilitation were invited to discuss,modify and reach a consenusus on the etiology,assessment diagnosis,differential diagnosis,rehabilitation strategy and prevention of SS,in order to provide clinical reference.
5.Relationship between SII,RAR and severity of disease and respiratory failure in patients with AECOPD

Objective To investigate the relationship between systemic immune-inflammation index(SII),red blood cell distribution width(RDW),albumin(ALB)ratio(RAR)and severity of disease and respiratory failure in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 110 patients with AECOPD were divided into the severe group(n=37)and the non-severe group(n=73).They were also divided into the respiratory failure group(n=26)and the non-respiratory failure group(n=84)according to whether they had respiratory failure.Platelet count(PLT),neutrophil count(NEU),lymphocyte count(LYM),RDW,ALB,SII and RAR were compared between different groups.Multiple Logistic regression method was used to analyze influencing factors of severe and concurrent respiratory failure.Receiver operating characteristic(ROC)curves were plotted to analyze the predictive value of indicators for severe condition and respiratory failure.Results Compared with the non-severe group,there were increased NEU,SII,RDW and RAR,and decreased PLT and LYM in the severe group(P<0.05).Compared with the non-respiratory failure group,there were increased NEU,SII,RDW and RAR,and decreased LYM and ALB in the respiratory failure group(P<0.05).Multivariate Logistic regression analysis showed that increased SII and RAR were independent risk factors for severe condition or respiratory failure in patients with AECOPD.ROC curves indicated that the area under the curve(AUC)of SII combined with RAR for predicting severe condition and respiratory failure in patients with AECOPD were 0.882(0.806-0.935)and 0.908(0.837-0.954),both of which were higher than those of SII or RAR alone(P<0.05).Conclusion Combination of SII and RAR can effectively help to evaluate the condition and the occurrence of respiratory failure in patients with AECOPD.
6.Targeting TRIM63 to regulate oxidative stress pathways and improve blood-brain barrier injury and neurological recovery after acute stroke

Objective To investigate the effect of targeting TRIM63 in regulating oxidative stress pathways and im-proving blood-brain barrier injury and neurological recovery after acute stroke.Methods Adult male C57 mice were ran-domly divided into Sham group,Vehicle group,and Myomed-205 group,and a mouse model of transient middle cerebral artery occlusion(tMCAO)was constructed.After removal of the suture,the mice in the Myomed-205 group and the Ve-hicle group were given intraperitoneal injection of TRIM63 inhibitor,and those in the Sham group were given intraperito-neal injection of an equal volume of solvent.On day 3 of reperfusion,TTC staining was used to observe cerebral infarct vol-ume,neurological score was used to evaluate neurological recovery,and the dry-wet weight method was used to measure brain water content.Reactive oxygen species(ROS),superoxide dismutase(SOD),glutathione(GSH),and malondial-dehyde(MDA)kits were used to measure the content of oxidative stress indices in brain.The Evans Blue method was used to observe blood-brain barrier injury,and Western blotting was used to measure the protein expression levels of ZO-1 and Occludin.Results Compared with the Vehicle group,the Myomed-205 group had a significant reduction in cerebral infarct volume(P<0.05),and compared with the control group,the 100 mg/ml Myomed-205 treatment group had signifi-cant reductions in neurological score(P<0.05)and brain water content(P<0.05).Compared with the Vehicle group,the Myomed-205 group had a significant reduction in the number of ROS-positive cells on days 3,7,and 14 after stroke(P<0.05),and compared with the control group,the Myomed-205 group had a significant reduction in the expression level of the pro-oxidative stress index MDA(P<0.05)and significant increases in the expression levels of the antioxidant indices GSH and SOD(P<0.05).Compared with the Vehicle group,the Myomed-205 group had a significant reduction in EB leakage on days 7 and 14 after stroke(P<0.05)and significant increases in the protein expression levels of ZO-1 and Occludin(P<0.05).Conclusion Inhibition of the TRIM63 signaling pathway can promote neurological recovery after cerebral ischemic stroke in mice,which may be achieved by alleviating oxidative stress and blood-brain barrier injury.
7.Metastasis patterns and survival analysis of 572 patients with metastatic cervical cancer:a hospital-based real world study

Background and purpose:Effective treatment for cervical cancer patients is one of the global strategies to eliminate cervical cancer.By analyzing the metastasis characteristics and survival status of patients with distant metastasis of cervical cancer from a hospital-based cancer registry data,our study provided real-world evidence for better survival of cervical cancer and finally eliminating cervical cancer.Methods:A total of 572 cervical cancer patients who had metastasis cancer at the initial diagnosis or developed distant metastasis during follow-up in Fudan University Shanghai Cancer Center from 2008 to 2017 were included in this study.Medical records review,telephone visits and death registry data linkage were applied in collecting endpoint data.The first follow-up date was the diagnose date of metastasis,and the last follow-up date was November 1,2020.Kaplan-Meier method was applied in evaluating the 1-,3-and 5-year overall survival(OS)rates for overall and site-specific patients.Results:The median follow-up time was 38.93 months,and 348 cases died during the follow-up.72.55%were single site metastasis,and 27.45%were multiple metastases.Among all metastatic sites,the proportion of lung metastasis was the highest,41.26%,15.21%to bone,and 11.54%to liver.After metastasis,the 1-year,3-year and 5-year OS rates were 62.29%(95%CI:62.25-62.33),33.13%(95%CI:33.08-33.18)and 23.42%(95%CI:23.37-23.47),respectively.In single site metastasis,1-year OS was the highest after metastasis to the lung(72.52%).Besides,there was no significant difference among different metastatic sites,both in 3-year and 5-year OS.Conclusion:The most frequent distant metastatic sites of cervical cancer are lung,bone and liver.The survival rate after metastasis is poor.Further research with systematic treatment strategy is required for better survival.
8.Application and practice of indocyanine green-guided laparoscopic radical gastrec-tomy for gastric cancer

Indocyanine green(ICG)near-infrared imaging technology has significant research value in laparoscopic radical gastrectomy and has garnered widespread attention and research both domestically and internationally.However,the application of ICG near-infrared ima-ging technology in laparoscopic radical gastrectomy is still in the exploratory stage in current clinical practice,with no unified standards es-tablished as yet.This review introduces the mechanism of ICG fluorescence imaging and discusses the indications and contraindications of ICG use in laparoscopic radical gastrectomy.The application methods and procedures of ICG molecular fluorescence imaging technology in laparoscopic radical gastrectomy and its clinical applications are elaborated upon.Finally,the practical applications of ICG-guided laparo-scopic radical gastrectomy is summarized and generalized to help facilitate its promotion and further standardization.
9.Advances in minimally invasive surgery for locally advanced gastric cancer

Gastric cancer(GC)is one of the most common malignancies worldwide.In our country,the majority of patients are diagnosed with GC at alocally advanced stage.Fortunately,clinical studies have demonstrated that minimally invasive radical gastrectomy,particularly laparoscopic surgery,is just as effective as traditional open surgery in terms of both short-and long-term outcomes.Consequently,minim-ally invasive surgery has become the primary surgical approach for patients with locally advanced gastric cancer(LAGC).Advancements in surgical techniques and technologies are enabling even more precise and minimally invasive approaches to GC surgery.Based on recent do-mestic and international studies,this study aimed to review and elaborate on the development of minimally invasive surgery for LAGC.
10.Application of iliac vein molding and stent implantation through the ipsilateral great saphenous vein approach in daytime treatment mode

Objective:To explore the feasibility and recent efficacy of iliac vein molding and stenting in daytime treatment mode in patients with iliac vein stenosis.Methods:Medical records of iliac vein molding and stenting performed in the ipsilateral great saphenous vein approach conducted from February 2017 to March 2022 were retrospective reviewed.There were 21 cases,6 males and 15 females.Age ranged from 37 to 79 years[(62.5±10.2)years].The stenosis in the 21 limbs simply involved the common iliac veins in 16 patients,2 patients had the simple and external iliac veins,and both the total and external iliac veins in 3 patients.Both iliac vein molding and iliac vein stenting were performed through the ipsilateral great saphenous vein approach.The patients with simple iliac vein stenosis with great saphenous vein valve insufficiency also underwent radiofrequency closure of great saphenous vein and flexural vein sclerosis therapy simultaneously.Regular postoperative direct oral anticoagulants therapy and stress therapy were followed.All the patients were hospitalized for less than 24 h.Results:All the 21 patients operations were successful(the success rate was 100%),without any intraoperative compli-cations.Immediate postoperative complications were puncture point bleeding in 1 case.The bandage gauze was completely wet.The bleeding was stopped after 5min of recompression.All the patients were hospitalized for less than 24 h.Follow-up results:The 3-month follow-up rate after operation was 100%.Absolute effective 18 cases(18/21,85.7%).Relatively effective(postmentation still after surgery,but with less extent)in 3 cases(3/21,14.3%).The iliac vein stents were unobstructed,and the trunk of the great saphenous vein was well closed in the patients with great saphenous vein radiofrequency treat-ment.The 6-month follow-up rate after operation was 71.4%(15/21).Of these,14 cases(14/15,93.3%)were absolutely effective.Relatively effective(postmentation still after surgery,but with less ex-tent)in 1 case(1/15,6.7%).The iliac vein stents were no restenosis or obstruction,and the trunk of the great saphenous vein was well closed in the patients with great saphenous vein radiofrequency treat-ment.Conclusion:The interventional treatment technique of iliac vein stenosis is feasible in the daytime treatment mode,with clear advantages and satisfactory recent efficacy.
