ObjectiveTo investigate the effect and potential mechanism of Zhishi Daozhi Formula （枳实导滞方， ZDF） on inflammation in rats with sepsis-induced gastrointestinal dysfunction based on the Toll-like receptor 4/myeloid differentiation factor 88/nuclear factor-κB （TLR4/MyD88/NF‑κB） signaling pathway and intestinal flora. MethodsSixty SD rats were randomly divided into sham‑operation group， model group， ulinastatin group， and low‑， medium‑ and high‑dose ZDF groups， with 10 rats in each group. Except for the sham‑operation group， the rat model of sepsis‑induced gastrointestinal dysfunction was established by cecal ligation and puncture （CLP）. Three days before modeling and 12 h after modeling， the low-， medium- and high-dose ZDF groups were intragastrically administered with ZDF solution at doses of 3.80， 7.60 and 15.20 g·kg⁻¹·d⁻¹， respectively. The ulinastatin group received intraperitoneal injection of ulinastatin at 100,000 U/kg， while the sham-operation and model groups were given normal saline at 1 ml/100 g by gavage. All interventions were performed once daily for 4 consecutive days. Tow hours after the last intervention， gastric emptying rate and small intestinal propulsion rate were detected. Gastrointestinal function indicators including D‑lactic acid （D‑LA） and intestinal fatty acid binding protein （i‑FABP）， as well as inflammatory indicators including procalcitonin （PCT） and C‑reactive protein （CRP） were measured by ELISA 24 h after modeling. The counts of Escherichia coli， Bifidobacterium and Lactobacillus in the intestine were compared. The histopathological morphological changes of the rat intestinal mucosa were observed by HE staining. The protein and mRNA expressions of IκB kinase （Ikk）， TLR4， MyD88 and NF‑κB in ileal tissues were detected by western blot and qRT‑PCR， respectively. ResultsCompared to the sham-operation group， the model group showed decreased gastric emptying rate and small intestinal propulsion rate， increased intestinal Escherichia coli count， elevated serum D‑LA， i‑FABP， PCT and CRP levels， increased protein and mRNA expressions of Ikk， TLR4， MyD88 and NF-κB in ileal tissue， decreased counts of Lactobacillus and Bifidobacterium （P＜0.05）， accompanied by obvious pathological injury of ileal mucosa. Compared to the model group， all medication groups exhibited significant improvements in the above mentioned indicators and alleviated intestinal mucosal injury （P＜0.05）. Compared to the ulinastatin group， except for D-LA in the medium-dose ZDF group， the differences in the above indicators between the low- and medium-dose ZDF groups were statistically significant （P＜0.05）， while the differences between the high-dose ZDF group and the ulinastatin group were not statistically significant （P＞0.05）. With increasing dosage， the gastric emptying rate， small intestinal propulsion rate， Lactobacillus and Bifidobacterium counts in the rats of all three ZDF groups increased， while the levels of serum D-LA， PCT， CRP， and i-FABP， the protein levels of Ikk， TLR4， MyD88， and NF-κB in intestinal tissue， and the Escherichia coli count decreased （P＜0.05）. ConclusionZDF can ameliorate intestinal mucosal injury， maintain intestinal flora balance and inhibit inflammatory response in rats with sepsis‑induced gastrointestinal dysfunction， with the high-dose group showing the best efficacy， which may be achieved by down‑regulating the TLR4/MyD88/NF-κB signaling pathway.

Cholelithiasis is one of the most common diseases of the digestive system,and the accurate preoperative diagnosis of cholelithiasis is important to guiding treatment and improving prognosis.Dual-energy CT based on material decomposition and post-processing techniques can generate virtual monoenergetic images,virtual noncontrast images,calcium-and fat-based material decomposition images and Rho/Z images which are helpful for detecting isoattenuating gallstones,and offer objective evidence for diagnosing gallstone disease and its complications.This article reviews the principle of dual-energy CT and its progress in cholelithiasis.

Objective Under the operation background of Diagnosis-Intervention Packet(DIP),whether there is interaction between reducing medical cost and average length of stay combined with pre-hospitalization mode,and whether there is difference between different departments and diseases in interaction.Methods Based on real-world data from 71 453 patients admitted to Guizhou Provincial People's Hospital from July to December 2021,a two-way analysis of variance was employed.When the interaction effect was statistically significant,parameter estimation was used to determine the magnitude and direction of the interaction effect,followed by subgroup analyses by department and disease.Results Without adjustment,both total medical costs and average length of stay exhibited a negative interaction effect(P＜0.05).Subgroup analyses revealed that in terms of total medical costs,the effect size for the surgical system was 0.18％,lower than that for the internal medicine system(0.70％);for core diseases,it was 6.62％,lower than that for comprehensive diseases(7.71％).Regarding average length of stay,the effect size for the surgical system was 0.55％,better than that for the internal medicine system(0.22％);for core diseases,it was 8.70％,higher than that for comprehensive diseases(2.90％).Conclusion The combination of DIP payment reform and pre-admission management model demonstrates a synergistic effect,effectively reducing patients' medical costs and length of stay.This effect is influenced by disease complexity and the standardization of diagnostic and treatment processes.

Objective To comprehensively analyze the circulatory RNA-long non-coding RNA-microRNA-messenger RNA(circRNA-lncRNA-miRNA-mRNA)network in cervical cancer and construct a prognostic model.Methods Differential and key genes were ana-lyzed using bioinformatics based on data from The Cancer Genome Atlas(TCGA)and Gene Expression Omnibus(GEO)databases.Prognostic mRNA models were constructed based on TCGA database using univariate,Least Absolute Shrinkage and Selection Operator(LASSO),and multivariate Cox regression analyses and validated using the GEO database.The R package and Cystoscape software were used to construct a nomogram model and competing endogenous(ceRNA)network of circRNA-lncRNA-miRNA-mRNA in cervical cancer.Results A prognostic model including five mRNAs was constructed using univariate,LASSO,and multivariate Cox regression analyses,which had area under the receiver operating characteristic curve AUC values of 0.71,0.71,and 0.70 at 1,2,and 3 years,respec-tively,indicating its sensitivity and specificity in cervical cancer prognosis.Predictive results were validated using the GSE44001 dataset.The C-index of the nomogram model for this prognostic model was 0.707.In this study,a ceRNA network comprising 39 circRNAs,27 lncRNAs,12 miRNAs,and five mRNAs was constructed.Conclusion The network constructed in this study can help comprehensively elucidate the mechanism of ceRNAs in cervical cancer,and the construction of prognostic and Nomogram models can predict patient prog-nosis.

Objective: To investigate the characteristics of HIV-1 molecular transmission network among HIV/AIDS patients in parts of Jiangxi Province, so as to provide insights into guiding AIDS prevention and intervention. Methods: The HIV/AIDS patients newly reported from January to June 2018 in Shangrao City, Yichun City and Ganzhou City were recruited, and demographic information and infection routes were collected. Blood samples were obtained to extract HIV RNA, and HIV-1 pol gene was amplified and sequenced using reverse transcription PCR and nested PCR assays. Gene subtypes were analyzed by constructing a phylogenetic tree. Molecular transmission network was constructed using gene-set distance, and the clustering patterns and the characteristics of HIV/AIDS patients within the clusters were analyzed. Results: A total of 305 pol gene sequences were obtained successfully, including 231 males (75.74%), 184 patients aged 50 years and above (60.33%), and 288 patients with heterosexual contact (94.43%). The main subtypes were CRF07_BC and CRF01_AE, accounting for 51.48% and 29.18%, respectively. Ganzhou City had the most genetic subtypes, with 8 types. Under the 1.0% gene distance threshold, 27 molecular clusters were established, with 107 nodes and 150 edges, and the molecular clustering rate was 35.08%. The largest molecular cluster involved 30 patients from 7 counties (districts) of Shangrao City. All of them were CRF07_BC subtypes, had an average age of (63.03±9.46) years, and were infected through heterosexual contact. Among the 17 patients with high transmission risk (degree value≥4), 10 patients came from Yushan County. Conclusions The main subtypes of HIV-1 are CRF07_BC and CRF01_AE in parts of Jiangxi Province, and the subtypes in Ganzhou City are diversified. There may be potential transmission risk points in Yushan County.

ObjectiveTo explore the effect of Qingre Huayu Jianpi prescription （QHJ） on colitis-associated colorectal cancer （CAC） in mice， and its related mechanism. MethodC57BL/6 mice were randomly divided into four groups including the normal， model， QHJ low-dose （QHJ-L， 10 g·kg^-1）， and QHJ high-dose （QHJ-H， 40 g·kg^-1） groups. Azoxymethane （AOM） and dextran sodium sulfate （DSS） were combined to chemically build a CAC mouse model for 14 weeks. Each drug group was given intragastrically from the 5^th week to the 14^th week， once per day. An equal volume of water was fed to the normal and model groups. The mouse survival rate， colon length， weight， and pathological alterations were assessed. The protein expressions of Wnt-3a protein signaling （Wnt3a）， β-catenin， Non-phosphor-β-catenin （Non-p-β-catenin）， and cholesterol-binding glycoproteins 133 （CD133） were detected by Western blot. The localization and expression of the cluster of differentiation （CD） 80 and CD11 antigen-like family member B （CD11b） were detected by immunohistochemistry （IHC）. The colon organoids derived from CAC mice were isolated and cultured to detect the expression of Wnt signaling pathway-related proteins. ResultThe survival rate of the CAC mice was improved by QHJ treatment and the number of colon tumors was inhibited significantly. Compared with those in the normal group， the expression levels of Wnt3a， β-catenin， Non-p-β-catenin， and CD133 in colon tissues in the model group were significantly increased （P<0.05， P<0.01）. Compared with those in the model group， the levels of Wnt3a and β-catenin in the QHJ-L group were significantly decreased （P<0.01）， and the protein levels of Wnt3a， β-catenin， Non-p-β-catenin， and CD133 in the QHJ-H group were significantly decreased （P<0.05， P<0.01）. Meanwhile， the expression level of CD11b in the model group was significantly increased compared with that in the normal group while the CD80 level was significantly decreased （P<0.05， P<0.01）. Compared with those in the model group， CD11b in QHJ-L and QHJ-H groups was significantly decreased， and CD80 was significantly increased（P<0.05， P<0.01）. The expressions of Non-p-β-catenin and CD133 in colonic organoids of CAC model mice were significantly increased， while QHJ treatment could inhibit the expressions of Non-p-β-catenin and CD133 in colonic organoids （P<0.01）. ConclusionQHJ could inhibit the inflammation-cancer development in CAC mice， the mechanism of which might be related to regulating the microenvironment and inhibiting the over-activation of Wnt signaling.
