This study assessed the role and mechanism of the recombinant Bacillus Calmette-Gue?rin vaccine(rBCG)with c-di-AMP adjuvant in regulating metabolism and immunity in epididymal white adipose(eWAT)in mice.Male C57BL/6 mice were intravenously immunized with BCG and rBCG,and their body weights were monitored.eWAT was isolated from the mice, and the stromal vascular fractions(SVFs)cell number was counted with a hemocytometer.Sections of mouse adipose tissue were prepared, and the size, number, and morphology of eWAT adipocytes and crown-like structure(CLS)formation were compared under a microscope after HE staining. The transcription levels of lipid metabolism-associated factors, cytokines and aging-associated genes in each group were determined with qRT-PCR.The body weights of mice gradually increased after immunization with BCG and rBCG.The proportions of eWAT increased, and the SVFs cell number decreased,in rBCG immunized mice.HE staining indicated that BCG immunization promoted hyperplasia, whereas rBCG immunization promoted hypertrophy of eWAT adipocytes;moreover, both BCG and rBCG immunization induced CLS formation in eWAT.The qRT-PCR results indicated that rBCG immunization inhibited the expression of genes associated with lipolysis and energy expenditure in eWAT.BCG immunization had little effect on cytokine transcription, whereas rBCG significantly induced the transcription of IFN-γ and IL-1Ra,and inhibited that of IL-15 and IL-2,but did not induce the expression of aging-associated genes. Thus,rBCG immunization induced eWAT adipocyte hypertrophy, which was associated with the inhibition of eWAT lipolysis and the regulation of cytokine expression.

Objective:To compare the clinical efficacy of O-arm navigation-assisted percutaneous vertebroplasty (PVP) versus C-arm-guided PVP in the treatment of postoperative recurrent vertebral fractures following Kümmell′s disease. Methods:A retrospective cohort study was conducted to analyze the clinical data of 48 patients with postoperative recurrent vertebral fractures following Kümmell′s disease who were admitted to Zhengzhou Orthopedic Hospital from January 2021 to September 2024, including 16 males and 32 females, aged 51-85 years [(69.8±6.6)years]. Among them, 21 patients had stage I Kümmell′s disease and 27 stage II. Fractured vertebrae involved T 8-T 10 in 4 patients, T 11-L 2 in 29, and L 3-L 5 in 15. Twenty-five patients underwent O-arm navigation-assisted PVP (O-arm-assisted group) and 23 underwent C-arm-guided PVP (C-arm-guided group). The two groups were compared in terms of the operative time, intraoperative blood loss, bone cement volume, and bone cement filling saturation rate in the injured vertebral body. The visual analogue scale (VAS) scores and Oswestry disability index (ODI) values were also compared before operation, at 1 day, 1 month, 6 months after operation, and at the last follow-up. The excellent-good rate based on the MacNab criteria at the last follow-up and incidence of postoperative complications were detected. Results:All the patients were followed up for 6-24 months [(13.3±3.5)months]. There were no significant differences in the operative time, operative blood loss or bone cement volume between the two groups ( P>0.05). The O-arm-assisted group demonstrated a bone cement filling saturation rate of 96% (24/25) in the fractured vertebrae, significantly higher than 65% (15/23) in the C-arm-guided group ( P<0.05). The VAS scores before operation, at 1 day, and 1 month after operation were (8.4±1.0)points, (1.9±0.7)points, and (1.8±0.6)points, respectively in the O-arm-assisted group, while they were (8.3±0.8)points, (2.0±0.6)points, and (1.9±0.5)points, respectively in the C-arm-guided group ( P>0.05). The ODI values before operation, at 1 day, and 1 month after operation were 76.6±8.2, 20.4±4.5, and 19.8±4.1, respectively in the O-arm-assisted group, and 74.9±9.1, 21.3±3.6, and 20.9±3.2, respectively in the O-arm-assisted group ( P>0.05). At 6 months after operation and at the last follow-up, the VAS scores were (1.4±0.5)points and (1.5±0.5)points in the O-arm-assisted group, with significant improvement compared to (1.8±0.4)points and (1.9±0.3)points in the C-arm-guided group ( P<0.01); the ODI values were 17.8±3.2 and 18.2±3.5 in the O-arm-assisted group, with significant improvement compared to 19.9±3.1 and 21.3±4.0 in the C-arm-guided group ( P<0.05). Both groups demonstrated significant improvements in VAS scores and ODI values at 1 day, 1 month, 6 months after operation, and at the last follow-up, compared to those preoperatively ( P<0.05), while no statistically significant differences were found in VAS scores or ODI values at any postoperative timepoints ( P>0.05). According to the MacNab criteria, the O-arm-assisted group had a 100% (25/25) excellent-good rate, compared to 74% (17/23) in the C-arm-guided group ( P<0.05). The complication rate was 4% (1/25) in the O-arm-assisted group, significantly lower than 35% (8/23) in the C-arm-guided group ( P<0.05). Conclusion:O-arm navigation-assisted PVP for postoperative recurrent vertebral fractures following Kümmell′s disease offers advantages in precise cement delivery with sufficient dispersion, enhanced pain relief, functional recovery, improved quality of life, and reduced complication rates when compared to C-arm navigation-assisted PVP.

Chinese Journal of Medical Physics 2025;42(10):1289-1297
Objective To achieve efficient compression of on-board radiotherapy projection images using dynamic video encoding algorithms. Methods The on-board radiotherapy imaging system primarily provides 2D X-ray projection images for patient positioning verification and 3D tomographic image reconstruction.Since multiple projection images acquired continuously exhibit strong spatiotemporal correlations, their similarities could be used to eliminate redundant information, thereby improving the image compression ratio.During image compression, the image sets obtained at different times were arranged into an image sequence which was input into a video encoder and output as a video file.During image decompression, the video file was input into a video decoder and output as an image sequence, and the images in the sequence were then assigned back to their original image sets.Three current dynamic video encoding algorithms(AVC,HEVC,and AV1)and the classic static image coding algorithm(JPEG 2000)were tested on a database of 2D projection images. The performance of various compression algorithms was evaluated using indicators such as compression ratio(CR),peak signal-to-noise ratio(PSNR),and structural similarity(SSIM).Moreover, visual comparison of projection images before and after compression was evaluated by clinical radiation oncologists. Results Dynamic video encoding algorithms achieved higher CR than the static image coding algorithm. The average CR of the 3 dynamic video encoding algorithms was as followed:CRAVC=11.50,CRHEVC=30.74,and CRAV1=27.10,while the average CR of the static image coding algorithm(JPEG 2000)was 5.28.For abdominal projection images,well-defined contours and textural details were preserved even when the CR reached 42.37.For head-neck projection images, although mild contour blurring emerged at a CR of 20.71,subsequent evaluation by clinical radiation oncologists confirmed that the reconstructed CBCT images still satisfied clinical requirements. Conclusion These dynamic video encoding algorithms effectively utilize the strong correlation information between multiple projection images, reduce the storage of redundant information, and greatly improve the image CR.

9.One case of VAV-ECMO treatment of a patient with severe cardiopulmonary insufficiency caused by inhalation of dilute nitric acid liquid and aerosol
Chinese Journal of Industrial Hygiene and Occupational Diseases 2025;43(3):232-236
This article analyzed the use of veno-arterio-venous extracorporeal membrane oxygenation (VAV-ECMO) in the treatment of a patient with severe cardiopulmonary insufficiency caused by inhalation of dilute nitric acid liquid and aerosol. Combined with relevant literature, the pathogenesis was discussed and the treatment experience was summarized. The inhalation of a large amount of dilute nitric acid liquid and aerosol caused severe chemical pneumonia in the patient, which resulted in severe cardiac insufficiency due to the dysregulation of the ventricular artery coupling. After the administration of VAV-ECMO, the well-oxygenated blood was effectively supplied to the systemic circulation and pulmonary circulation, and the load of the heart was reduced, providing valuable time for the recovery of the patient's cardiopulmonary function.
