1.Herbal Textual Research on Inulae Flos in Famous Classical Formulas

In this paper, by referring to ancient and modern literature, the textual research of Inulae Flos has been conducted to clarify the name, origin, production area, quality evaluation, harvesting, processing and others, so as to provide reference and basis for the development and utilization of famous classical formulas containing this herb. After textual research, it could be verified that the medicinal use of Inulae Flos was first recorded in Shennong Bencaojing of the Han dynasty. In successive dynasties, Xuanfuhua has been taken as the official name, and it also has other alternative names such as Jinfeicao, Daogeng and Jinqianhua. The period before the Song and Yuan dynasties, the main origin of Inulae Flos was the Asteraceae plant Inula japonica, and from the Ming and Qing dynasties to the present, I. japonica and I. britannica are the primary source. In addition to the dominant basal species, there are also regional species such as I. linariifolia, I. helianthus-aquatili, and I. hupehensis. The earliest recorded production areas in ancient times were Henan, Hubei and other places, and the literature records that it has been distributed throughout the country since modern times. The medicinal part is its flower, the harvesting and processing method recorded in the past dynasties is mainly harvested in the fifth and ninth lunar months, and dried in the sun, and the modern harvesting is mostly harvested in summer and autumn when the flowers bloom, in order to remove impurities, dry in the shade or dry in the sun. In addition, the roots, whole herbs and aerial parts are used as medicinal materials. In ancient times, there were no records about the quality of Inulae Flos, and in modern times, it is generally believed that the quality of complete flower structure, small receptacles, large blooms, yellow petals, long filaments, many fluffs, no fragments, and no branches is better. Ancient processing methods primarily involved cleaning, steaming, and sun-drying, supplemented by techniques such as boiling, roasting, burning, simmering, stir-frying, and honey-processing. Modern processing focuses mainly on cleaning the stems and leaves before use. Regarding the medicinal properties, ancient texts describe it as salty and sweet in taste, slightly warm in nature, and mildly toxic. Modern studies characterize it as bitter, pungent, and salty in taste, with a slightly warm nature. Its therapeutic effects remain consistent across eras, including descending Qi, resolving phlegm, promoting diuresis, and stopping vomiting. Based on the research results, it is recommended that when developing famous classical formulas containing Inulae Flos, either I. japonica or I. britannica should be used as the medicinal source. Processing methods should follow formula requirements, where no processing instructions are specified, the raw products may be used after cleaning.

3.Establishment and validation of a risk prediction model for nonattainment plasma concentration of zonisamide in children with epilepsy

OBJECTIVE To construct a risk prediction model for nonattainment plasma concentration of zonisamide in children with epilepsy, and to provide evidence for individualized dosing.METHODS Children with epilepsy who received standardized zonisamide treatment orally in Kunming Children’s Hospital were retrospectively selected as research subjects. Those treated from May 2022 to May 2024 were included in the modeling cohort, and those treated from June 2024 to January 2025 were included in the external validation cohort. According to plasma concentration, children were divided into therapeutic group and nonattainment group. Independent influencing factors for nonattainment plasma concentration of zonisamide were screened by collinearity diagnosis, univariate analysis, Lasso regression and random forest, based on which a nomogram risk prediction model was constructed and validated. Finally, the Shapley additive explanations (SHAP) method was used to interpret the features of the nomogram risk prediction model.RESULTS A total of 945 children with epilepsy were included in this study. There was no statistically significant difference in baseline data between the modeling cohort (531 cases) and the external validation cohort (414 cases) ( P >0.05), indicating comparability. Through multi-stage feature screening, seven influencing factors were finally included to construct the prediction model, namely gender, age, dosage, aspartate transferase, total bilirubin, uric acid and vitamin D. The corrected area under the curve of the model was 0.799 in the modeling cohort and 0.762 in the external validation cohort, showing good calibration and clinical applicability. SHAP analysis showed that gender, dosage, total bilirubin, age and aspartate transferase were the top five important variables affecting model prediction.CONCLUSIONS The nomogram risk prediction model for nonattainment plasma concentration of zonisamide constructed in this study has stable predictive performance. Clinically, the risk of nonattainment plasma concentration can be quickly assessed by combining the child’s gender, age, dosage, liver and kidney function, and vitamin D, and the dosing regimen can be adjusted in time.

Spine fracture and dislocation are common traumatic spinal conditions that often require surgical intervention due to compromised spinal stability. Surgical approaches include anterior, posterior, and combined anterior-posterior spinal procedures. According to the specific surgical requirements, patients may be placed in the prone position or repositioned between prone and supine positions during surgery. Intraoperative repositioning has become an essential step in patient positioning. However, during repositioning, patients with spinal fracture and dislocation are at increased risk for complications such as hemodynamic instability, nerve injury, and pressure injuries to the skin and soft tissue. Notably, due to the instability of the spinal cord, even minor manipulations can further exacerbate the damage, potentially leading to severe outcomes like paraplegia. Although the current clinical guidelines provide instructive recommendations for standard position, there remains no specific protocols for intraoperative repositioning in patients with spine fracture and dislocation. With a concern for the lack of clinical studies on positioning techniques, risk prevention, and operational norms for special patients, no applicable guidelines or standards are available. A consensus was required to provide clinical reference, meet the requirements of surgical treatment, and minimize the safety risks of patients caused by improper placement of positions. Professional Committee of Operating Room Nursing of Shaanxi Nursing Association organized experts in nursing management and operating room nursing from major hospitals across China to formulate Expert consensus on intraoperative repositioning for patients with spinal fracture and dislocation ( version 2025). The consensus provides 11 recommendations covering pre-repositioning preparation, intraoperative maneuvers, and post-repositioning observation, aiming to provide references for clinical standardization of the intraoperative repositioning process and protection of patients′ safety.
5.Correlation between serum soluble intercellular adhesion molecule-1 and the severity of white matter hyperintensities

Objective:To investigate the correlation between serum soluble intercellular adhesion molecule-1 (sICAM-1) and the severity of white matter hyperintensities (WMHs).Methods:Medical examinees in the Department of Neurology, Suzhou Hospital Affiliated to Anhui Medical University from February 2021 to October 2022 were selected as the research subjects. Head MRI was performed and the severity of WMHs was assessed by Fazekas scale. The enzyme-linked immunosorbent assay was used to detect serum sICAM-1 expression level. Multivariate logistic regression analysis was used to determine the independent influencing factors of the severity of WMHs. Results:A total of 171 subjects were enrolled, including 112 males (65.5%), aged 65.50±9.07 years. There were 92 patients (53.8%) with no WMH or mild WMHs and 79 patients (46.2%) with moderate to severe WMHs. The serum sICAM-1 levels in the moderate to severe WMH group were significantly higher than those in the no or milds WMH group ( t=5.679, P<0.001). Ordinal multivariate logistic regression analysis showed that after adjusting for confounding factors, higher sICAM-1 level was independently correlated with the severity of WMHs (compared to the 1 st quartile group, the 3 rd quartile group: odds ratio 3.149, 95% confidence interval 1.147-8.645, P=0.026; the 4 th quartile group: odds ratio 6.087, 95% confidence interval 2.051-18.061; P=0.001). Conclusion:Elevated serum sICAM-1 level is an independent influencing factor of WMHs.
6.Correlation analysis between the relative quantification of miR-200a in peritoneal dialysis effluent and peritoneal function in peritoneal dialysis patients

This study was a single-center retrospective study, to investigate the correlation between the relative level of miR-200a in exfoliated cells of peritoneal dialysis (PD) effluent and clinical indicators of peritoneal function in PD patients. Clinical data of PD patients who underwent treatment in the Department of Nephrology, the First Affiliated Hospital of Nanchang University, from January 2024 to December 2024 were collected, and the relative level of miR-200a in exfoliated cells of PD effluent was assessed. Patients were divided into three groups according to dialysis duration (Group A:≤1 year; Group B:>1 to <5 years; Group C:≥5 years), and differences in clinical data among the groups were compared. The correlation between the relative level of miR-200a and serum albumin, serum calcium, serum phosphorus, serum creatinine, hemoglobin, the neutrophil/high-density lipoprotein cholesterol ratio, the monocyte/high-density lipoprotein cholesterol ratio, the lymphocyte/high-density lipoprotein cholesterol ratio, and 24-hour urine volume in PD patients was analyzed. The results showed that there were no statistically significant differences among the three groups in gender, age, creatinine clearance rate, or Kt/V (all P>0.05). However, statistically significant differences were found among the three groups in peritoneal transport type ( χ2=13.518, P=0.001) and ultrafiltration volume ( H=6.905, P=0.032). Based on the 4-hour PD effluent creatinine/blood creatinine in the peritoneal equilibration test, patients were divided into the low transporter/low average transporter group (L group) and the high transporter/high average transporter group (H group). A statistically significant difference in the relative level of miR-200a was observed between the two groups [11.70 (1.09, 20.49) vs. 0.73 (0.46, 1.98), t=4.545, P<0.001]. The relative level of miR-200a was positively correlated with ultrafiltration volume and 24-hour urine output ( r=0.328, P=0.030; r=0.516, P<0.001), and negatively correlated with dialysis vintage ( r=-0.496, P<0.001). These results indicate that miR-200a level in PD effluent may, to some extent, reflect patients' peritoneal membrane function.

Objective:To investigate the effectiveness of combined teaching method integrating case-based teaching method(CBL),problem-based teaching method(PBL),and team-based teaching method(TBL)in clinical nursing teaching in pediatric intensive care unit(PICU).Methods:A total of 60 nursing students who trained in our PICU from Jan 2021 to Jun 2022 were selected as the control group,and 60 nursing students who trained in our PICU from Jul 2022 to Jun 2024 were selected as the research group.The control group received conventional nursing teaching mode,while the research group received combined teaching mode of CBL,PBL,and TBL.The scores of academic performance,logical thinking ability,self-learning ability,and clinical nursing comprehensive ability between the two groups were compared.Result:The scores of theoretical knowledge,operational skills,post-teaching Critical Thinking Disposition Inventory-Chinese Version(CTDI-CV)scale,self-directed learning readiness scale for nursing,and Mini-clinical evaluation exercise scale(Mini-CEX)in the research group were higher than those in the control group(P<0.05).Conclusion:The combined teaching method of CBL,PBL,and TBL can effectively improve clinical nursing teaching results and comprehensive abilities of nursing students.

10.Influence of nurses' proactive personality on work engagement:testing the effects of job crafting and inclusive leadership

Objective To explore the influence of nurses' proactive personality on their work engagement and analyze the mediating role of job crafting and the moderating effect of inclusive leadership.Methods Questionnaires were designed and disseminated via the Questionnaire Star platform.The survey adopted a general information questionnaire,the Proactive Personali-ty Scale,the Job Crafting Scale,the Work Engagement Scale and the Inclusive Leadership Scale for the investigation among all the nurses in two public hospitals in Yangquan.Totally,490 valid samples were collected.Data analysis was conducted using hi-erarchical multiple regression and simple slope analysis.Results Job crafting was found to serve as a mediator in the relation-ship between proactive personality and work engagement.Inclusive leadership positively moderated the relationship between pro-active personality and job crafting,with statistically significant difference(β=0.067,P<0.05).Conclusion Proactive per-sonality enhances nurses' work engagement indirectly by fostering job crafting behaviors.Moreover,in teams characterized by higher levels of inclusive leadership,the positive influence of proactive personality on job crafting is amplified.
