Objective To investigate the effects of microRNA-34a(miR-34a-5p)on the progression of chronic lymphocytic leukemia(CLL)through the Wnt/β-catenin signaling pathway.Methods Human chronic B-cell leukemia MEC-1 cells were selected for experimentation.MEC cells were divided into two groups,group one:the p53 agonist group and the control group;group two:the control group,the miR-34a-5p mimic group and its corresponding negative control,the miR-34a-5p inhibitor group and its corresponding negative control,as well as the miR-34a-5p inhibitor+Wnt inhibitor XAV-939 group.The expression levels of miR-34a-5p in each group were measured using real-time fluorescence quantitative PCR(qPCR).Cell proliferation was assessed by CCK-8 assay,while cell migration ability was evaluated using Transwell migration assay.Dual-luciferase reporter assay was employed to validate the targeting relationships between p53 and miR-34a-5p,as well as between miR-34a-5p and Wnt1.Western blot analysis was used to detect the protein expressions of β-catenin and Cyclin D1,which were key components of the Wnt/β-catenin signaling pathway.Results In MEC-1 cells:① compared with the control group,there was a increased miR-34a-5p expression and inhibited cell proliferation in the p53 agonist group(P<0.05).Dual-luciferase reporter assay confirmed a negative regulatory correlation between miR-34a-5p and p53.② the miR-34a-5p mimic group showed significantly upregulated miR-34a-5p expression compared to the control group,leading to suppressed cell proliferation,reduced migration capability and decreased protein expressions of β-catenin and Cyclin D1(P<0.05).Conversely,the miR-34a-5p inhibitor group demonstrated significantly downregulated miR-34a-5p expression,resulting in enhanced cell proliferation,increased migratory capacity and upregulated protein levels of β-catenin and Cyclin D1 compared to those of the control group(P<0.05).③ A targeting relationship was observed between miR-34a-5p and Wnt1.④ Compared with the miR-34a-5p inhibitor group,the XAV-939 group exhibited significantly upregulated miR-34a-5p expression,markedly decreased numbers of migrated cells and substantially reduced protein expression levels of β-catenin and Cyclin D1(P<0.05).Conclusion miR-34a plays the role of a tumor suppressor gene in CLL.Overexpression of miR-34a can inhibit the Wnt/β-catenin signaling pathway,reduce the proliferation activity and migration ability of cells,and promote cell apoptosis.
4.Clinical Observation of Jin's Triple-Needle Therapy Combined with Bilateral High-Frequency Repetitive Transcranial Magnetic Stimulation in the Treatment of Post-stroke Hemiplegia

Objective To observe the clinical efficacy of Jin's triple-needle therapy combined with bilateral high-frequency repetitive transcranial magnetic stimulation in the treatment of post-stroke hemiplegia.Methods A total of 116 patients diagnosed with post-stroke hemiplegia and admitted to the wards and outpatient clinics of Yan'an City Hospital of Traditional Chinese Medicine from January 2023 to June 2024 were selected as the study subjects.The patients were randomly divided into an observation group and a control group using a random number table,with 58 patients in each group.The control group received bilateral high-frequency repetitive transcranial magnetic stimulation,while the observation group received Jin's triple-needle therapy in addition to the control group's treatment.After 30-day treatment,the clinical efficacy of the two groups was evaluated.Changes in traditional Chinese medicine(TCM)syndrome scores,Fugl-Meyer Assessment(FMA)scores for limb motor function,and Stroke-Specific Quality of Life(SS-QOL)scores were observed before and after treatment.Changes in neuron-specific enolase(NSE),glial fibrillary acidic protein(GFAP),and central nervous system-specific protein(S100-β)levels were compared between the two groups before and after treatment.The safety and incidence of adverse reactions in both groups were also evaluated.Results(1)The total effective rate in the observation group was 91.38%(53/58),compared to 75.86%(44/58)in the control group.The observation group showed significantly better efficacy than the control group,with a statistically significant difference(P<0.05).(2)After treatment,both groups showed significant improvements in TCM syndrome scores and FMA scores for limb motor function(P<0.05),and the observation group was significantly superior to the control group in improving TCM syndrome scores and FMA scores,with a statistically significant difference(P<0.05).(3)After treatment,both groups showed significant improvementss(P<0.05)in NSE,GFAP,and S100-β level,and the observation group was significantly superior to the control group in improving NSE,GFAP,and S100-β levels,with a statistically significant difference(P<0.05).(4)After treatment,both groups showed significant improvements in SS-QOL scores for quality of life(P<0.05),and the observation group was significantly superior to the control group in improving SS-QOL scores,with a statistically significant difference(P<0.05).(5)The incidence of adverse reactions in the observation group was 12.07%(7/58),compared to 8.62%(5/58)in the control group.There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Jin's triple-needle therapy combined with bilateral high-frequency repetitive transcranial magnetic stimulation can significantly improve clinical symptoms,enhance limb motor function,reduce levels of brain injury markers,and improve the quality of life in patients with post-stroke hemiplegia.The treatment is safe and shows significant efficacy.
5.The expression level and clinical value of ALKBH5 and PAK5 in high-risk HPV positive cervical cancer tissue

Objective To analyze the expression levels of RNA demethylase alkB homolog 5(ALKBH5)and p21 activated kinase 5(PAK5)in high-risk human papillomavirus(HR-HPV)positive cervical cancer tissue,and explore their clinical significance.Methods A total of 96 patients with HR-HPV positive cervical cancer were selected as the research object.The expressions of ALKBH5 and PAK5 mRNA and protein in cancer tissue and adjacent tissue were analyzed by real-time fluorescent quantitative PCR,immunohistochemistry and Western blot assay.The expression levels of ALKBH5 and PAK5 proteins in patients with HR-HPV positive cervical cancer with different clinicopathological characteristics were compared.Multivariate Cox regression analysis was used to analyze influencing factors of death in patients with HR-HPV positive cervical cancer.Kaplan-Meier curve was used to analyze the effects of ALKBH5 and PAK5 protein expression on the 5-year survival rate of patients.Results The results of real-time fluorescent quantitative PCR,immunohistochemistry and Western blot assay showed that expression levels of ALKBH5 and PAK5 mRNA and protein in HR-HPV positive cervical cancer tissue were higher than those in adjacent tissue(P<0.05).The positive rates of ALKBH5 and PAK5 in cancer tissue of HR-HPV positive cervical cancer patients with FIGO stage ⅡA and lymph node metastasis were increased(P<0.05).ALKBH5 positive(HR=1.467,95%CI:1.104-1.949),PAK5 positive(HR=1.435,95%CI:1.093-1.884),FIGO stage ⅡA(HR=1.435,95%CI=1.065-1.933)and lymph node metastasis(HR=1.652,95%CI:1.053-2.593)were risk factors affecting the prognosis of HR-HPV positive cervical cancer patients(P<0.05).The 5-year cumulative survival rate was lower in the ALKBH5 positive group[72.58%(45/62)vs.91.18%(31/34)]than that of the ALKBH5 negative group(log rank χ2=4.481,P=0.034).The 5-year overall survival rate of the PAK5 positive group[71.67%(43/60)vs.91.67%(33/36)]was lower than that of the PAK5 negative group(log rank χ2=4.153,P=0.043).Conclusion The expression levels of ALKBH5 and PAK5 are significantly up-regulated in HR-HPV positive cervical cancer,both of which are related to the poor clinicopathological features of patients,and are helpful to evaluate the prognosis.
6.Summary of best evidence on discharge preparation services for adult patients with bronchial asthma

Objective:To search, evaluate, and summarize the best evidence regarding discharge preparation services for adult patients with bronchial asthma, providing references for clinical practice and guideline development.Methods:A computer search was conducted in BMJ Best Practice, UpToDate, PubMed, Australia Joanna Briggs Institute Evidence-based Healthcare Center Database, Cochrane Library, Embase, CINAHL, Web of Science, China National Knowledge Infrastructure, Wanfang Database, and China Biology Medicine disc, as well as relevant guideline and professional websites. The search period covered from the establishment of the databases to June 30, 2024. Two researchers independently conducted quality assessments of the included literature, evidence extraction, and synthesis.Results:A total of 14 articles were included, comprising five guidelines, four expert consensus, and five systematic reviews. Twenty-three pieces of evidence were summarized across six aspects: discharge preparation objectives, discharge preparation principles, health guidance during hospitalization, pre-discharge preparation, discharge instructions, and post-discharge follow-up.Conclusions:The best evidence for discharge preparation services for adult patients with bronchial asthma summarized in this study can be applied by clinical healthcare providers in China, considering the specific clinical context, to help adult patients with bronchial asthma manage their condition.

10.Regulatory effect of human umbilical cord mesenchymal stem cells on intestinal barrier function in diabetic nephropathy rats

BACKGROUND:Diabetic nephropathy is an important cause of end-stage renal disease,and intestinal barrier damage plays an important role in the occurrence and development of diabetic nephropathy. OBJECTIVE:To observe the protective effect of human umbilical cord mesenchymal stem cells on the intestinal barrier in rats with diabetic nephropathy. METHODS:Thirty 8-week-old male SD rats were randomly assigned to healthy control group,model group and human umbilical cord mesenchymal stem cell group,with 10 rats in each group.Rats in the human umbilical cord mesenchymal stem cell group were injected with 1×106 human umbilical cord mesenchymal stem cells through the tail vein once a week for 4 weeks after the model establishment of diabetic nephropathy.Rats in the healthy control group and the model group were injected with an equal volume of PBS at the same time.1 week after the last injection,the histomorphological changes in the kidney and colon were observed under a light microscope.The expressions of ZO-1 and Occludin in the colon tissue of rats were detected by immunohistochemistry.Serum D-lactic acid and lipopolysaccharide levels were detected by ELISA.In addition,the distribution of human umbilical cord mesenchymal stem cells labeled with DiR dye in rats was observed by in vivo imaging system.The expression of human mesenchymal stem cell surface marker antigens CD44 and CD90 in colon tissue was detected by immunohistochemistry. RESULTS AND CONCLUSION:(1)Compared with the model group,human umbilical cord mesenchymal stem cell transplantation significantly inhibited the increase of urea nitrogen,serum creatinine,24-hour urine protein level and urinary albumin/creatinine ratio in diabetic nephropathy rats(all P<0.05).(2)The expression of human mesenchymal stem cell surface markers CD44 and CD90 was found in the colon of diabetic nephropathy rats.(3)Compared with the healthy control group,the expression levels of tight junction proteins Occludin and ZO-1 in the colon tissue of the model group were significantly reduced,while the expressions of Occludin and ZO-1 were significantly increased after treatment with human umbilical cord mesenchymal stem cells.(4)Compared with the model group,human umbilical cord mesenchymal stem cell transplantation significantly reduced serum D-lactic acid and lipopolysaccharide levels in diabetic nephropathy rats.(5)The results suggest that human umbilical cord mesenchymal stem cells may protect the intestinal barrier function by enhancing the expression of intestinal tight junction proteins in diabetic nephropathy rats.
