1.One case of civil aviation pilot with nasopharyngeal non-Hodgkin lymphoma and literature review

Objective:To investigate the diagnosis, treatment, prognosis and aeromedical assessment of nasopharyngeal non-Hodgkin lymphoma (NHL) in civil aviation pilots.Methods:The clinical data of a civil pilot with nasopharyngeal NHL was retrospectively analyzed while related literature was reviewed. The clinical characteristics, diagnosis, treatment, prognosis and identification of nasopharyngeal NHL were traced.Results:A civil pilot was diagnosed with nasopharyngeal NHL in January 2016 before being grounded by aviation physicians. After treatment with systematic chemoradiation and chemotherapy by the Department of Oncology, West China Hospital of Sichuan University, the patient′s results of blood routine tests, blood biochemistry, EB virus DNA detection, electrical audiometry, and sound immittance returned to normal as evidenced by multiple outpatient examinations between June 2016 and April 2017. No signs of malignant tumor were found by fiber nasopharyngoscopy, nasopharyngeal MRI or positron emission tomography/computed tomography. In June 2017, he applied for specially-permitted assessment for the first time, and the aeromedical assessment conclusion was waiver to flight, but his flight was limited to 60 h or less. The pilot was able to perform flight duties ever since. EB virus DNA detection, nasopharyngeal MRI, fiber nasopharyngoscopy and other related examinations were repeated in each physical examination, and the pilot was given specially-permitted eligibility for fight missions annually for 6 consecutive years, but the monthly flying hours was limited to 60 h or less. So far, the disease neither recurred nor progressed.Conclusions:Nasopharyngeal NHL is rare in civil aviation pilots, so missed diagnosis and misdiagnosis should be avoided. Patients with nasopharyngeal NHL who have recovered after treatment may apply for specially-permitted health assessment and resume their work after comprehensive assessment by related experts.
2.Normal values for vestibular function of video head impulse test in civilian student pilots

Objective To establish normal reference values of video head impulse test(vHIT)for civil avia-tion student pilots to provide a reference basis for the accurate assessment of the vestibular function of civil aviation student pilots.Methods Forty civil aviation student pilots in the theoretical training phase were randomly selected for vHIT.The average gain of the vestibulo-ocular reflex(VOR)and the asymmetry ratios of the semicircular ca-nals were compared.Results The VOR values for the right horizontal,anterior,and posterior semicircular canals were 1.05±0.07,0.92±0.13,0.81±0.09,respectively.For the left horizontal,anterior,and posterior semicir-cular canals,the values were 1.00±0.06,0.85±0.10,0.86±0.10,respectively.There was no compensatory sac-cade in any of the subjects.The asymmetry ratios of bilateral horizontal,left anterior-right posterior,and right an-terior-left posterior semicircular canals were 5.56%±3.64%,9.65%±4.74%,8.68%±7.07%,respectively.Conclusion This study establishes vHIT normal reference values for civil aviation student pilots,providing an ob-jective basis for their selection and vestibular function evaluation.

5. Effects of 2-APB in skin wound healing in mice through down-regulation of TRPM7

AIM: To explore the promoting effect of 2-APB on skin wound healing in mice and its potential mechanism. METHODS: KM mice were divided into 5 groups: control group, DMSO group, low (50 mg/L), medium (100 mg/L) and high (200 mg/L) concentration 2-APB group. On the back of each mouse's skin use a circular punch about 1 cm on both sides of the midline of the spine to make a skin wound with a diameter of 10 mm and as deep as the fascia. The control group was only wrapped with gauze and no drugs were applied; the DMSO group was applied 1 g DMSO/Vaseline ointment per day; in the 2-APB group, apply 1 g of 2-APB/Vaseline ointment at a corresponding concentration every day. Pictures were taken the next day to observe the healing, and the material was taken on the 21st day, HE staining was used to observe the pathological morphology of the wound and western blot to detect TRPM7, TGF-β, collagen-I and IL-1β expression. RESULTS: Compared with the control group and the DMSO group, different concentrations of 2-APB could significantly promote skin wound healing in mice (P<0.01), but there was no significant difference in wound healing rate between the DMSO group and the control group group. The results of HE staining showed that, compared with the control group group and the DMSO group, 2-APB could increase the collagen content of the wound and the thickness of the dermis (P<0.01), but there was no significant difference between the DMSO group and the control group group. At the same time, 2-APB could also significantly increase the expression of TGF-β and Col-I on the wound, and inhibit the expression of TRPM7 and IL-1β. CONCLUSION: Different concentrations of 2-APB (50, 100 and 200 mg/L) can promote skin wound healing, and its mechanism may be related to the inhibition of TRPM7.
6.Recommendations for diagnosis and treatment of psoriatic arthritis in China

Psoriatic arthritis is a chronic systemic autoimmune disease, characterized by psoriasis skin lesions and inflammation of the spine and joint. It has complicated clinical manifestations and individual variations. Nearly half of the patients will have joints erosion in two years, which is crippling. The severity of the skin and joint disease frequently do not correlate with each other. Currently, the understanding of the disease is insufficient in China with the lack of standardized diagnosis and treatment. Therefore, researchers from the Chinese Rheumatology Association formulated this specification based on the diagnosis and management experience together with guidelines at home and abroad. The specification summarizes the present situation of domestic diagnosis and treatment, aiming to standardize the diagnosis process and treatment protocols of psoriatic arthritis. Furthermore, it can reduce misdiagnosis and missed diagnosis, as well as improve the prognosis.
7.Peroxidase from foxtail millet bran exerts anti-colorectal cancer activity via targeting cell-surface GRP78 to inactivate STAT3 pathway.

Molecular targeted therapy has become an emerging promising strategy in cancer treatment, and screening the agents targeting at cancer cell specific targets is very desirable for cancer treatment. Our previous study firstly found that a secretory peroxidase of class III derived from foxtail millet bran (FMBP) exhibited excellent targeting anti-colorectal cancer (CRC) activity in vivo and in vitro, whereas its underlying target remains unclear. The highlight of present study focuses on the finding that cell surface glucose-regulated protein 78 (csGRP78) abnormally located on CRC is positively correlated with the anti-CRC effects of FMBP, indicating it serves as a potential target of FMBP against CRC. Further, we demonstrated that the combination of FMBP with the nucleotide binding domain (NBD) of csGRP78 interfered with the downstream activation of signal transducer and activator of transcription 3 (STAT3) in CRC cells, thus promoting the intracellular accumulation of reactive oxygen species (ROS) and cell grown inhibition. These phenomena were further confirmed in nude mice tumor model. Collectively, our study highlights csGRP78 acts as an underlying target of FMBP against CRC, uncovering the clinical potential of FMBP as a targeted agent for CRC in the future.
8.The effect of hyperbaric oxygen therapy combined with repetitive transcranial magnetic stimulation on patients with cognitive dysfunction after cerebral infarction

Objective To observe the effect of hyperbaric oxygen (HBO) therapy combined with repeated transcranial magnetic stimulation (rTMS) on patients with cognitive dysfunction after cerebral infarction.Methods A total of 81 patients with cognitive dysfunction after cerebral infarction were randomly divided into a control group,an HBO group and a combined treatment group,each of 27.In addition to basic medication and traditional rehabilitation therapy,the HBO group was also treated with hyperbaric oxygen,while the combined treatment group received both HBO and rTMS.The mini-mental state examination (MMSE) and the Montreal cognitive assessment (MoCA) were administered before and after the four weeks of treatment.Results The average MMSE scores of the HBO and combined treatment groups were both significantly higher than that of the control group,with the former significantly higher than the latter.Moreover,the average MoCA score of the HBO group was also significantly higher than that of the combined treatment group,and both were significantly higher than that of the control group.Conclusion Hyperbaric oxygen therapy can more effectively improve the cognitive function of patients with cognitive dysfunction after the cerebral infarction when it is combined with repeated transcranial magnetic stimulation.
9.Sampling study on auditory status of civil aviation air traffic controllers

OBJECTIVE To investigate and analysis of hearing status and characteristics of China civil aviation air traffic controllers(ATC). METHODS With cluster random sampling, air conduction threshold data of 1498 ATC, who had finished the class Ⅲa medical assessment this year in a certain area were studied. The subjects were tested by pure tone audiometry, the prevalence rate of speech and high frequency hearing loss between gender groups were compared; After age correction, the threshold of different frequencies were compared between age groups. RESULTS The prevalence rate of hearing loss at speech frequency was 6.68% in male and 1.97% in female. The result of high frequency was 7.87% and 1.23% respectively. 10.Significance of peripheral perfusion index in early diagnosis and goal-directed therapy of septic shock patients: a prospective single-blind randomized controlled trial

Objective To investigate the application of peripheral perfusion index (PPI) in early diagnosis and goal-directed therapy of septic shock, and to provide reference for the early clinical diagnosis and treatment of septic shock. Methods A prospective single-blind randomized controlled trial (RCT) was conducted. Adult patients with sepsis admitted to emergency medical department and intensive care unit (ICU) of the First People's Hospital of Lianyungang City in Jiangsu Province from January 2013 to December 2016 were enrolled. The patients were randomly divided into two groups (n = 46). The PPI group was defined using PPI < 1.4 as diagnosis of septic shock standard, and PPI > 2 as treatment guide target. Control group was defined according to the traditional diagnostic criteria of shock which systolic blood pressure was less than 90 mmHg (1 mmHg = 0.133 kPa) or systolic blood pressure value decrease> 40 mmHg baseline and bundle treatment was performed. The volume of fluid resuscitation, organ dysfunction, the sequential organ failure score (SOFA), acute physiology and chronic health evaluationⅡ (APACHE Ⅱ) score,continuous renal replacement therapy (CRRT) time, mechanical ventilation (MV) time, the length of ICU stay and 28-day mortality were observed. Results There were 39 and 27 septic shock patients in PPI group and control group respectively. The diagnostic criteria of traditional septic shock with blood pressure as "gold standard", the sensitivity of PPI < 1.4 for septic shock was 94.3%, the specificity was 28.2%, the authenticity was 66.3%, the positive predictive value was 64.1%, the negative predictive value was 78.6%, the positive likelihood ratio was 1.31, the negative likelihood ratio was 0.18. The per capita fluid replacement within 24 hours in the PPI group was significantly higher than that in the control group (mL: 4 601±1 250 vs. 3 458±1 006, P < 0.01), but there was no significant difference in the per capita volume of the patients diagnosed as septic shock (mL: 4 596±1 320 vs. 4 205±1 058, P > 0.05). Compared with the control group, the PPI group treated patients within 48 hours with less vascular active drugs (cases: 6 vs. 15), APACHE Ⅱand SOFA score were lower (48 hours: APACHE Ⅱ was 10.2±2.1 vs. 12.0±3.2; 72 hours: SOFA was 5.1±1.8 vs. 6.0±2.1, APACHE Ⅱ was 8.9±1.8 vs. 9.8±2.2), the period of CRRT and the length of ICU stay were shorter [the period of CRRT (days): 3.0±0.9 vs. 3.6±1.4, the length of ICU stay (days): 5.2±2.1 vs. 6.3±2.9), the difference was statistically significant (all P < 0.05). There was no significant difference in the liver and kidney function index, arterial blood lactic acid (Lac), MV time (days: 3.3±1.4 vs. 3.5±1.2) and 28-day mortality (15.22% vs. 19.57%) between two groups (all P > 0.05). Conclusions The inadequacy of microcirculatory perfusion by oximetry-derived PPI is more sensitive to the diagnosis of septic shock than hypotension of systemic circulation. With PPI guiding the fluid resuscitation of septic shock patients, vasopressors can be withdrawn earlier and the duration of the CRRT and ICU can be decreased.
