ObjectiveTo observe the effects of electroacupuncture with "tonifying the kidney and dispelling stasis" acupoint prescription on sexual function in diabetic erectile dysfunction (DMED)model rats, and to explore its possible mechanism of action. MethodsSPF male SD rats were randomly divided into 10 each in blank group, model group, Tadalafil group, and electroacupuncture group. DMED rat model was prepared by high glucose and high fat diet combined with intraperitoneal injection of streptozotocin. After successful modelling, rats in the electroacupuncture group were given electroacupuncture intervention of "tonifying the kidney and dispelling stasis" acupoint prescription once every other day; Tadalafil group was given Tadalafil solution 0.5 mg/kg·d by gavage, and the blank group, model group and electroacupuncture group were given 10 ml/kg pure water by gavage once a day. Each group was intervened for 30 days. The body mass and blood glucose level of the rats were detected on the 1st, 8th, 15th, 22nd days and at the end of the intervention, respectively. At the end of the intervention, the penile erection of the rats was observed by using the apomorphine test; the level of plasma endothelial cell microparticles (EMPs)was detected by flow cytometry; the histopathological morphology of the penile cavernous body was observed by HE staining, and the pathological morphology of the endothelial cells of the penile vasculature was observed by electron microscopy. Serum sex hormones including testosterone (T), follicle stimulating hormone (FSH), luteinising hormone (LH) and vascular endothelial function-related factors including vascular endothelial growth factor (VEGF), endothelin-1 (ET-1), lectin-like oxidized low-density lipoprotein receptor-1 (LOX-1), soluble E-selectin (sE-selectin), and soluble intercellular adhesion molecule-1 (sICAM-1) were measured by ELISA method. ResultsCompared with blank group, the model group, Tadalafil group, and electroacupuncture group all had lower body mass and higher blood glucose levels at each time of testing (P<0.01). Compared with the blank group, the number of penile erections reduced in the model group, the level of CD31+ EMPs increased, the levels of serum T, FSH, LH, and VEGF reduced, and the levels of serum ET-1, LOX-1, sE-selectin, and sICAM-1 increased (P<0.01). Compared with the model group, the Tadalafil group and the electroacupuncture group showed an increased number of penile erections decreased level of CD31+ EMPs, increased levels of serum T, FSH, LH, and VEGF, and decreased levels of serum ET-1, LOX-1, sE-selectin, and sICAM-1 (P<0.01). Compared with the Tadalafil group, serum T, FSH, LH, VEGF levels increased and ET-1, LOX-1 levels decreased in the electroacupuncture group (P<0.05 or P<0.01). HE staining and electron microscopic observation revealed that there was severe pathological damage to the cavernous tissue of the penis and vascular endothelial cells of the rat in the model group, which was ameliorated to a certain degree in both Tadalafil group and electroacupuncture group. ConclusionThe electroacupuncture prescription of "tonifying the kidney and dispelling stasis" can improve the erectile dysfunction of DMED rats, which is comparable to the effect of Tadalafi. Its mechanism of action may be related to the regulation of vascular endothelial function.

Objective:To investigate the awareness levels of doctors from various departments regarding fertility preservation to provide reference and suggestions for further improving awareness and promoting multidisciplinary development in fertility preservation. Methods:From November to December 2023, the China Alliance of Fertility Preservation initiated this survey covering 88 hospitals in 23 cities across 20 provinces in China. The survey focused on the degree of understanding of basic concepts, technological perceptions, treatment concepts, and related challenges regarding fertility preservation. Descriptive statistics and Fisher's exact test were used to analyze the difference in awareness levels across various departments. Results:There was a significant difference in the awareness of fertility preservation guidelines among physicians from different departments ( P<0.001). Reproductive center physicians had the highest level of understanding [68.3% (41/60)]. Doctors in the reproductive centers had a higher awareness rate of the five fertility preservation techniques (ovarian tissue cryopreservation, oocyte cryopreservation, sperm cryopreservation, testicular tissue cryopreservation, and embryo cryopreservation) than that in other departments (all P<0.05). Regarding treatment concepts, more reproductive center physicians believed that psychological counselling about fertility preservation before oncological treatment was necessary ( P=0.016). Hematologists had more worries and concerns about fertility preservation treatment. Additionally, 96.7% (58/60) of reproductive center physicians expressed the need to establish effective referral mechanisms within the same city, province, or across provinces. However, 55.6% (35/63) of oncologists, 64.6% (31/48) of hematologists, and 77.8% (7/9) of pediatricians considered treating primary tumors far more important than fertility preservation. Conclusion:Doctors from different departments have varying levels of understanding regarding fertility preservation. It is necessary to strengthen the dissemination and training of fertility preservation knowledge among physicians from various departments, as well as to promote a multidisciplinary treatment model to improve efficiency, to establish a comprehensive referral mechanism to improve patients' opportunities for fertility preservation.

Seizure characteristics and survival were assessed through video monitoring, and respiratory parameters were monitored. Immunofluorescence was used to detect colocalization of tryptophan hydroxylase 2(TPH2) and c-Fos in DRN. Statistical analysis was performed using SPSS 24.0 and GraphPad Prism 9.0. The independent sample t-test or Mann-Whitney test was used for inter-group comparison. Results:(1) In the fluoxetine intervention experiment: the survival rates between the model group and fluoxetine group showed no statistically significant difference ( χ2=2.23, P>0.05). As for the frequency of grade Ⅳ seizures, the model group (1.50(1.25, 2.35)min) demonstrated higher frequency than the fluoxetine group (0.43(0.20, 0.67)min) ( t=-3.40, P<0.05).With respect to respiratory parameters, the model group demonstrated shorter expiratory time ((0.10±0.02) s) and inspiratory time ((0.15±0.02) s) compared to the fluoxetine group ((0.16±0.05) s, (0.19±0.04) s) ( t=-3.47, -3.73, both P<0.01). The respiratory rate in the model group ((269.96±44.84) times/min) was significantly higher compared to the fluoxetine group ((195.04±52.37) times/min) ( t=3.44, P<0.01). The tidal volume in the model group ((0.10±0.02) mL) was significantly lower than the fluoxetine group ((0.13±0.04) mL) ( t=-2.19, P<0.05).The number of TPH2+ /c-Fos+ co-expressing cells in the model group (11.00±4.00) was lower than that in the fluoxetine group (33.00±8.39)( t=-4.16, P<0.05). (2) In the chemogenetic activation experiment: compared to the vehicle group, the chemogenetic activation group demonstrated significantly enhanced survival rates ( χ2=5.83, P<0.05). As for the frequency of grade Ⅴ seizures, the vehicle group (2.11(1.62, 3.44) times/min) showed higher frequency compared to the chemogenetic activation group (0.81(0.00, 1.62) times/min) ( t=18.00, P<0.05). In terms of respiratory parameters, the vehicle group showed shorter expiratory time ((0.10±0.01) s) and inspiratory time ((0.14±0.01) s) compared to the chemogenetic activation group ((0.12±0.01) s, (0.15±0.01) s) ( t=-2.78, -2.50, both P<0.05). The respiratory rate in the vehicle group ((208.37±9.73) times/min) was significantly higher than the chemogenetic activation group ((191.85±8.83) times/min) ( t=3.98, P<0.01). The tidal volume in the vehicle group ((0.09±0.01) mL) was significantly lower than the chemogenetic activation group ((0.12±0.02) mL) ( t=-4.77, P<0.001).The number of TPH2+ /c-Fos+ co-expressing cells in the vehicle group (9.00±3.46) was lower than that in the chemogenetic activation group (43.00±11.02)( t=-5.20, P<0.01). Conclusion:Specific activation of serotonergic neurons in the DRN can ameliorate heat-induced epileptic symptoms, improve respiratory function, and prolong survival time in Scn1a + /- mice.

In order to understand the current status of the knowledge, attitude and practice (KAP) about prevention and control of COVID -19 in college students, and to provide theoretical basis for prevention and control work in college campus. This study investigated the KAP of COVID -19 of 1 847 college students in Shaanxi province by questionnaire using the convenience sampling method. Chisquare test and multivariate logistic regression analysis were used to analyze the influencing factors for the KAP of COVID -19. The results demonstrated that 48.3% of the students had a higher knowledge level of COVID -19, 11.7% had a fear attitude and 39.6% had good protective practices. Logistic regression results showed that female and urban household college students had higher cognitive level of COVID -19. The college students with anxiety state were more likely to have fear attitude. Students of female, urban household, anxiety, higher cognition and fear attitude showed better protective practices. The above results indicated that the knowledge level of COVID -19 in college students are not enough, and the attitude and protective practices need to be further improved. Therefore, relevant departments should follow the rules of KAP, carry out targeted propaganda and education on COVID -19 for college students, to improve their ability to cope with public health emergencies.
9.Treatment of male hypogonadotropic hypogonadism and application of recombinant follicle stimulating hormone
Chinese Journal of Applied Clinical Pediatrics 2024;39(11):874-877
Male hypogonadotropic hypogonadism (HH), caused by decreased gonadotropin (Gn), is a group of disorders characterized by lack of pubertal development, cessation or degeneration of secondary sex characteristics, and infertility.Hormone replacement therapy is the primary treatment for this disease and can promote the development of secondary sex characteristics in patients, but the induction of fertility in HH patients with microrchidia is a challenging problem currently. The application of testosterone-based preparations and human chorionic gonadotropin (hCG) alone is not sufficient to improve fertility, while combined treatment of hCG with human menopausal gonadotropin (hMG) injection and gonadotropin-releasing hormone (GnRH) pump can promote testicular sperm production.Recent studies have shown that recombinant follicle stimulating hormone (rFSH) has some positive effects.Especially for patients with severe HH, rFSH pretreatment and sequential treatment are more scientific and effective. This review summarizes the treatment of male HH, focusing on the application of rFSH in this disease, to provide more options for improving male fertility.

Background and Aims:Accurate early pathogen diagnosis is a breakthrough for improving the prognosis of infectious pancreatic necrosis(IPN)patients. However, there is currently a lack of efficient methods for early identification of IPN in clinical settings. This study was performed to assess the application value of next-generation sequencing technology based on metagenomic capture(MetaCAP)in the pathogen diagnosis of IPN. Methods:A prospective study was conducted on 29 patients suspected of having acute necrotizing pancreatitis at Xiangya Hospital of Central South University between January and July 2024.Blood samples were tested using MetaCAP and conventional pathogen culture. The results of peritoneal fluid pathogen culture were used as the gold standard to compare the diagnostic efficacy of the two methods. Results:Due to three cases lacking peritoneal fluid culture results,a total of 26 cases were included in the final analysis. The overall mortality rate was 23.1%(6/26).During hospitalization,9 cases(34.6%)were diagnosed with IPN.The sensitivity and negative predictive value of MetaCAP for diagnosing IPN were significantly higher than those of conventional pathogen culture(77.8%vs.11.1%,P=0.031;86.7%vs.65.2%,P=0.032),while the differences in specificity(76.5%vs.88.2%,P=0.689)and positive predictive value(63.6%vs.33.3%,P=0.347)between the two methods were not statistically significant. The average detection time for MetaCAP was 33(20-49)h,while microbial culture took 125(45-142)h,with a significant difference(P<0.001).The average cost for blood MetaCAP testing was 2 500 yuan per case, but it accounted for only 1.19%of the average hospitalization cost. Conclusion:MetaCAP has significant value in the early pathogen diagnosis of IPN,with a shorter detection time, good testing efficacy, and health-economic value, demonstrating a promising clinical application prospect.
