1.Species of sandflies and prevalence of Leishmania infections in sandflies in selected areas of northern and northwestern China

Objective To investigate the species of sandflies and the prevalence of Leishmania infections in sandflies from selected areas of northern and northwestern China, so as to provide insights into identification of leishmaniasis vectors and assessment of epidemiological trends of leishmaniasis in China. Methods Sandfly samples were collected from Mentougou District of Beijing Municipality, Xiangning County in Linfen City of Shanxi Province, Ejin Banner in Alxa League of Inner Mongolia Autonomous Region, and Payzawat County of Karamay District of Karamay City, Gaochang District of Turpan City in Xinjiang Uygur Autonomous Region from July 2023 to July 2024. Approximately 100 intact female sandfly samples were randomly selected from each site and the species of sandflies was identified according to morphological characteristics and molecular assays. Female sandflies originating from the same habitat were grouped into pools of 10 individuals. Leishmania infection was detected using polymerase chain reaction (PCR) assay targeting the internal transcribed spacer 1 (ITS-1) gene, and the prevalence of Leishmania infection was calculated in sandflies from different sampling sites using the minimum infection rate (MIR) method. In addition, positive amplicons were sequenced and subjected to phylogenetic analysis. Results A total of 6 155 sandflies were collected from different environments at sampling sites across the six aforementioned regions from July 2023 to July 2024. Phlebotomus chinensis (96.00%) was the dominant sandfly species in Mentougou District, Beijing Municipality, with a small proportion of Ph. sergenti (4.00%), and only Ph. chinensis was found in Xiangning County, Linfen City, Shanxi Province. Ph. wui was the only sandfly species detected in Ejin Banner, Alxa League, Inner Mongolia Autonomous Region, and Payzawat County, Kashgar City, Xinjiang Uygur Autonomous Region, and Ph. caucasicus (97.70%) was the dominant sandfly species in Karamay District, Karamay City, Xinjiang Uygur Autonomous Region, with a small proportion of Ph. wui (2.30%), while Ph. alexandri was the only species in Gaochang District, Turpan City, Xinjiang Uygur Autonomous Region. A total of 40, 60, 34, 18, 18, and 22 pools of sandfly samples were tested from Mentougou District in Beijing Municipality, Xiangning County in Linfen City of Shanxi Province, Ejin Banner in Alxa League of Inner Mongolia Autonomous Region, Payzawat County in Kashgar City, Karamay District in Karamay City, and Gaochang District in Turpan City of Xinjiang Uygur Autonomous Region, respectively. L. infantum was detected in Ph. chinensis samples from Mentougou District in Beijing Municipality, and Xiangning County of Linfen City in Shanxi Province, with MIR of 0.25% to 1.00%, and L. donovani was detected in Ph. wui from Ejin Banner in Alxa League of Inner Mongolia Autonomous Region, and Payzawat County in Kashgar City of Xinjiang Uygur Autonomous Region, with MIR of 0.56% to 0.88%; however, no Leishmania infection was detected in Ph. caucasicus from Karamay District in Karamay City or Ph. alexandri from Gaochang District in Turpan City of Xinjiang Uygur Autonomous Region. Phylogenetic analysis showed that the Leishmania ITS-1 gene sequences obtained from Mentougou District in Beijing Municipality and Xiangning County in Linfen City of Shanxi Province were clustered into the same clade with the reference sequences of L. infantum ITS-1 gene, while the Leishmania ITS-1 gene sequences obtained from Ejin Banner in Alxa League of Inner Mongolia Autonomous Region and Payzawat County in Kashgar City of Xinjiang Uygur Autonomous Region were clustered into the same clade with the reference sequences of L. donovani ITS-1 gene. Conclusions There are variations in sandfly species in selected areas of northern and northwestern China, and variations in the species of Leishmania infecting sandflies. Improved surveillance of sandfly vectors and targeted control strategies with adaptations to geographical features and leishmaniasis vectors are recommended.
2.Herbal Textual Research on Inulae Flos in Famous Classical Formulas

In this paper, by referring to ancient and modern literature, the textual research of Inulae Flos has been conducted to clarify the name, origin, production area, quality evaluation, harvesting, processing and others, so as to provide reference and basis for the development and utilization of famous classical formulas containing this herb. After textual research, it could be verified that the medicinal use of Inulae Flos was first recorded in Shennong Bencaojing of the Han dynasty. In successive dynasties, Xuanfuhua has been taken as the official name, and it also has other alternative names such as Jinfeicao, Daogeng and Jinqianhua. The period before the Song and Yuan dynasties, the main origin of Inulae Flos was the Asteraceae plant Inula japonica, and from the Ming and Qing dynasties to the present, I. japonica and I. britannica are the primary source. In addition to the dominant basal species, there are also regional species such as I. linariifolia, I. helianthus-aquatili, and I. hupehensis. The earliest recorded production areas in ancient times were Henan, Hubei and other places, and the literature records that it has been distributed throughout the country since modern times. The medicinal part is its flower, the harvesting and processing method recorded in the past dynasties is mainly harvested in the fifth and ninth lunar months, and dried in the sun, and the modern harvesting is mostly harvested in summer and autumn when the flowers bloom, in order to remove impurities, dry in the shade or dry in the sun. In addition, the roots, whole herbs and aerial parts are used as medicinal materials. In ancient times, there were no records about the quality of Inulae Flos, and in modern times, it is generally believed that the quality of complete flower structure, small receptacles, large blooms, yellow petals, long filaments, many fluffs, no fragments, and no branches is better. Ancient processing methods primarily involved cleaning, steaming, and sun-drying, supplemented by techniques such as boiling, roasting, burning, simmering, stir-frying, and honey-processing. Modern processing focuses mainly on cleaning the stems and leaves before use. Regarding the medicinal properties, ancient texts describe it as salty and sweet in taste, slightly warm in nature, and mildly toxic. Modern studies characterize it as bitter, pungent, and salty in taste, with a slightly warm nature. Its therapeutic effects remain consistent across eras, including descending Qi, resolving phlegm, promoting diuresis, and stopping vomiting. Based on the research results, it is recommended that when developing famous classical formulas containing Inulae Flos, either I. japonica or I. britannica should be used as the medicinal source. Processing methods should follow formula requirements, where no processing instructions are specified, the raw products may be used after cleaning.

4.Research progress on oral microecological imbalance and intervention strategies after radiotherapy for head and neck tumors

Radiotherapy is a crucial treatment modality for head and neck tumors. However, while effectively killing tumor cells, it significantly disrupts the homeostasis of the oral microecology, which is closely associated with various complications such as radiation-induced oral mucositis. Literature review indicates that as radiotherapy doses accumulate and treatment durations extend, the richness and diversity of the oral microbiota show a declining trend, with the genus Streptococcus decreasing most markedly. In contrast, radiotherapy selectively promotes the proliferation of bacterial phyla such as Proteobacteria and Bacteroidetes, which are rich in opportunistic pathogens. Mechanistically, radiotherapy activates the nuclear factor-kappa B pathway, triggering chronic inflammation and oxidative stress, damaging the epithelial barrier, suppressing local immunity, and causing damage to organs such as the salivary glands. It can also induce systemic diseases via the oral-gut axis, forming a multi-level, interconnected pathogenic network. In terms of interventions, treatment strategies including probiotics and prebiotics have shown promising efficacy against side effects such as radiation-induced oral mucositis. Saliva-based oral microbiota transplantation is an emerging strategy that is expected to become widely utilized for restoring oral microecological balance. Existing interventions provide preliminary pathways for clinical practice, but this field still faces several key scientific questions. The association between oral microecology and systemic diseases remains largely correlative, lacking causal evidence. Furthermore, critical parameters for oral microbiota transplantation, such as donor screening criteria, transplantation protocols, and long-term safety, are not yet well-defined. Therefore, future research should focus on conducting large-scale clinical trials to establish standardized protocols and safety evaluation systems for oral microecological interventions, and explore combined treatment therapies such as probiotics, prebiotics, and microbiota transplantation to advance the development of personalized precision modulation. These will enable more effective management of radiotherapy-induced oral microecological dysbiosis and improve treatment outcomes and quality of life for patients with head and neck tumors.

In response to the high risk of recurrence and metastasis after breast cancer surgery, this paper proposes a six-dimensional integrated preventive and therapeutic model, namely the "disease-syndrome-constitution-trend-strategy-efficacy" framework, including disease identification, syndrome differentiation, constitution differentiation, trend assessment, strategy implementation, and effect evaluation. This model builds upon the traditional three-dimensional approach of disease identification, syndrome differentiation and constitution differentiation, and introduces "trend" to dynamically capture the evolving tendencies of disease progression and the transformation of syndrome and constitution, thereby operationalizing the concept of "treating before disease onset". The "strategy" dimension enables the organic integration and individualized regulation of TCM and western medical interventions, while the "efficacy" dimension establishes a multidimensional evaluation system integrating TCM and western medicine to assess the efficacy of the "strategy", so as to promote the systematic, dynamic, and precise development of treating before disease onset in TCM.

The Expert Consensus on Clinical Application of Qinbaohong Zhike Oral Liquid in Treatment of Acute Bronchitis and Acute Attack of Chronic Bronchitis (GS/CACM 337-2023) was released by the China Association of Chinese Medicine on December 13th, 2023. This expert consensus was developed by experts in methodology, pharmacy, and Chinese medicine in strict accordance with the development requirements of the China Association of Chinese Medicine (CACM) and based on the latest medical evidence and the clinical medication experience of well-known experts in the fields of respiratory medicine (pulmonary diseases) and pediatrics. This expert consensus defines the application of Qinbaohong Zhike oral liquid in the treatment of cough and excessive sputum caused by phlegm-heat obstructing lung, acute bronchitis, and acute attack of chronic bronchitis from the aspects of applicable populations, efficacy evaluation, usage, dosage, drug combination, and safety. It is expected to guide the rational drug use in medical and health institutions, give full play to the unique value of Qinbaohong Zhike oral liquid, and vigorously promote the inheritance and innovation of Chinese patent medicines.
7.5-Aza-CdR reverses pericyte-myofibroblast transition by inhibiting Epo gene promoter hypermethylation in rat primary renal myofibroblasts

Objective To examine the effect of the demethylating agent 5-aza-2'-deoxycytidine(5-Aza-CdR)on pericyte-myofibroblast transition(PMT)in primary rat renal myofibroblasts.Methods Rat primary renal myofibroblasts were treated with 5-Aza-CdR 250 ng/mL for 72 h,and the degree of Epo promoter methylation was detected by pyrosequencing.Protein expression levels of α-smooth muscle actin(α-SMA),platelet-derived growth factor receptor-β(PDGFRβ)and DNA methyltransferase(Dnmt3a)were detected by immunofluorescence and Western blot,and erythropoietin(EPO)levels in the supernatant were detected.Results Compared with the control group,5-Aza-CdR treatment significantly decreased the expression of Dnmt3a and hypermethylation of the Epo promoter,and subsequently decreased the expression of α-SMA and the expression ratio of α-SMA to PDGFRβ in myofibroblasts.Meanwhile,5-Aza-CdR treatment increased the level of EPO in the cell supernatant.Conclusions 5-Aza-CdR can reverse PMT by inhibiting Epo promoter hypermethylation in primary renal myofibroblasts.
8.Research status and prospects of treatment for malignant pleural mesothelioma

Malignant pleural mesothelioma(MPM)is strongly associated with a history of asbestos exposure and is characterized by high malignancy,high mortality,and poor prognosis.Current treatments for MPM are limited and generally suboptimal,resulting in a median overall survival(OS)of approximately one year for MPM patients.However,advancements in treatment options,including surgery,radiotherapy,chemotherapy,immunotherapy and targeted therapy,have brought new hope to patients with MPM.For early-stage MPM patients categorized under the TNM staging system,surgical treatment is feasible and can improve survival rates and quality of life.However,there is still debate regarding the optimal surgical approach for MPM.In addition to surgery,radiotherapy plays a vital role in MPM treatment.It is often used as prophylactic treatment or for alleviating local symptoms in advanced stages.Radiotherapy can also serve as neoadjuvant or adjuvant therapy in surgical contexts.For patients experiencing local progression or isolated distant metastases after systemic treatment,radiotherapy is a viable option.The advent of advanced radiotherapy techniques,such as intensity-modulated radiotherapy(IMRT)and volumetric intensity-modulated arc therapy(VMAT),has significantly improved the precision and efficacy of radiotherapy while minimizing damage to healthy tissues.Furthermore,brachytherapy can relieve pain or act as a localized supplemental therapy.Chemotherapy remains the standard treatment for MPM.The combination of pemetrexed and platinum-based drugs is widely applied as first-line therapy and has been shown to significantly extend survival.However,commonly used second-line regimens often yield suboptimal results.In recent years,immunotherapy has developed rapidly.Dual immunotherapy with nivolumab and ipilimumab has demonstrated impressive clinical efficacy and safety.The combination of immunotherapy and chemotherapy has also notably extended patients'median survival.Multiple clinical trials have confirmed that this combination therapy benefits patients.Currently available targeted therapies for MPM primarily focus on anti-angiogenesis.Bevacizumab combined with chemotherapy has established its position as a first-line treatment.Research on ramucirumab and apatinib suggests that these drugs have certain efficacy and safety profiles.Beyond conventional treatment options,the UV1 cancer vaccine combined with dual immunotherapy offers new hope for patients.Chimeric antigen receptor T(CAR-T)cell therapy is an emerging treatment method being investigated in MPM patients,with phase Ⅰ clinical trials demonstrating good antitumor effects.Additionally,some antibody-drug conjugates are becoming therapeutic options for MPM through precise targeting.Tumor treating fields combined with chemotherapy has also shown efficacy in extending survival.Despite the increasing variety of treatment options for MPM,its diagnosis and treatment still face numerous challenges,including difficulties in early detection,treatment resistance,and a lack of large-scale evidence-based clinical studies.Future research should focus on improving early diagnosis rates,developing new treatment strategies,overcoming resistance,and advancing personalized therapy.Strengthening the integration of basic research and clinical trials will also be essential.Through multidisciplinary collaboration and continuous innovation,it is hoped that more effective and safer treatment options will become available,ultimately improving the prognosis of MPM patients.
9.Associations of metabolic dysfunction-associated steatotic liver disease and cardio-metabolic risk factor abnormalities with adverse pregnancy outcomes

Objective:To investigate the association between metabolic dysfunction-associated steatotic liver disease(MASLD)and the risk of adverse pregnancy outcomes,and to analyze the impact of the type and severity of cardiometabolic risk factor(CMRF)abnormalities on this association.Methods:A retrospective cohort study was conducted among primiparous women with singleton pregnancies who had registered at Beijing Friendship Hospital from March 10,2020,to December 31,2022.A total of 2 623 women were included.Basic characteristics and delivery outcomes were documented,liver ultrasound and relevant prenatal examinations were performed,and adverse pregnancy outcomes were diagnosed.Modi-fied Poisson regression models were used to analyze the association between MASLD and adverse pregnan-cy outcomes.The relationship between the type or severity of CMRF abnormalities in MASLD and the risk of adverse pregnancy outcomes was also explored.Results:After adjusting for confounding factors including age,gestational weight gain,and education level,MASLD was associated with an increased risk of cesarean section(RR=1.531,95%CI:1.304-1.799,P<0.001),gestational diabetes melli-tus(GDM;RR=2.409,95%CI:1.948-2.979,P<0.001),pregnancy-associated hypertension(PAH;RR=3.062,95%CI:2.069-4.533,P<0.001),preterm birth(RR=2.145,95%CI:1.342-3.429,P=0.001),and large for gestational age(LGA;2.224,95%CI:1.599-3.095,P<0.001).However,no significant associations were found for small for gestational age or postpartum hemorrhage.After adjusting for other CMRF abnormalities,the risk of adverse pregnancy outcomes varied among MASLD pregnant women with different CMRF abnormalities:the body mass index abnormal group had higher risks of cesarean section,GDM,PAH,preterm birth,and LGA;the glucose abnormal group had an increased risk of GDM;the blood pressure abnormal group had a higher risk of PAH;the high density lipoprotein cholesterol abnormal group had higher risks of cesarean section,GDM,and PAH;and the tri-glyceride abnormal group was associated with higher risks of GDM and preterm birth.Additional,as the severity of CMRF abnormalities increased,the risks of cesarean section(RR=1.199,95%CI:1.112-1.292,P<0.001),GDM(RR=1.478,95%CI:1.345-1.624,P<0.001),PAH(RR=1.626,95%CI:1.367-1.934,P<0.001),preterm birth(RR=1.384,95%CI:1.120-1.710,P=0.003),and LGA(RR=1.422,95%CI:1.224-1.650,P<0.001)continued to rise.Conclusion:MASLD during pregnancy is associated with an increased risk of multiple adverse pregnancy outcomes,and the type and severity of CMRF abnormalities significantly influence this association.These results suggest that attention should be paid to the specific CMRF abnormalities when diagnosed MASLD,as this may help to facilitate targeted interventions and reduce the risk of adverse pregnancy outcomes.
10.Genetic evolution characteristics and their influence on disease transmission in sandflies in various environments in China

This study analyzed the genetic evolutionary characteristics of sandflies and their effects on the spread of kala-azar in various environments in endemic provinces in China,to provide a scientific basis for kala-azar disease prevention and control.Sand-flies were collected in kala-azar endemic areas such as southern Xinjiang,the large hilly areas of southern Gansu,the northern Sich-uan and Taihang Mountains,and surrounding small hills.The cytochrome c oxidase subunit I and cytochrome b gene fragments of mito-chondrial DNA were amplified to identify sandfly species.The COI and Cytb gene sequences of sandflies from southern Xinjiang and Si-chuan recorded in NCBI were also collected.The intraspecific and interspecific genetic differences of sandflies were calculated in MEGA11.0,and a phylogenetic tree was constructed through the neighbor-joining method,for analysis of the genetic and evolutionary characteristics of sandfly populations and their effects on disease transmission.A total of 155 sandflies were collected from nine sam-pling sites in seven provinces of China;the species included Phlebotomus chinensis,Phlebotomus wui,and Sergentomyia squamirostris.Five sandfly species belonging to two genera were collected:P.chinensis,P.wui,and Phlebotomus alexandri in the genus Phleboto-mus,and S.squamirostris in the genus Sergentomyia.Genetic evolution analysis based on COI and Cytb gene sequences indicated intra-specific genetic distances of 0-0.062 and 0-0.056,respectively,and interspecific genetic distances of 0.126-0.176 and 0.110-0.171,respectively.The phylogenetic tree indicated that P.wui,P.alexandri,Phlebotomus longiductus,and S.squamirostris clus-tered into one branch.The sequences of P.chinensis in the large and small hilly areas clustered into two geographical clades.In the small hilly areas,the sequences of P.chinensis aggregates showed small genetic differences,the pathogen infection was consistent,and the cases showed an epidemic spread trend.Large genetic differences at the molecular level were observed among sandflies in dif-ferent ecological regions,thus indicating key effects on leishmaniasis transmission.On the basis of these findings,prevention and con-trol strategies should be adapted to local conditions,and precise and effective prevention and control measures should be formulated according to the genetic evolution characteristics of sandflies in different regions,to better control the transmission of Kala-azar.
