ObjectiveTo observe the clinical efficacy of electroacupuncture in treatment of simple obesity patients and its effects on the metabolism-inflammation network. MethodsSeventy-two patients with simple obesity were randomly divided into a electroacupuncture group and a sham electroacupuncture group, with 36 patients in each group. In the electroacupuncture group, conventional acupuncture was applied at Zhongwan (CV 12), bilateral Tianshu (ST 25), bilateral Daheng (SP15), Qihai (CV 6), Guanyuan (CV 4), bilateral Zusanli (ST 36) and Fenglong (ST 40). The sham electroacupuncture group used placebo needle at same acupoints as the electroacupuncture group. Both groups were given sparse-dense waves with a frequency of 2/10 Hz, 30 minutes per session, 3 times a week, for a total of 8 weeks of treatment. The changes of human body parameters including body weight, body mass index (BMI), waist circumference, and body fat percentage (BFP), metabolic factors including serum fasting blood glucose (FBG), triglyceride (TG), total cholesterol (TC), high density lipoprotein (HDL), low density lipoprotein (LDL), leptin, and ghrelin, and inflammatory factors including interleukin-6 (IL-6) and tumor necrosis factor α (TNF-α) were compared before and after treatment. Correlations between changes in leptin, ghrelin and inflammatory factors were further analyzed. ResultsWaist circumference was significantly reduced after treatment in both groups compared with baseline (P<0.05), and the post-treatment waist circumference was lower in the electroacupuncture group than in the sham electroacupuncture group (P<0.05); body weight, BMI and BFP decreased significantly after treatment compared to pretreatment levels in the electroacupuncture group (P<0.05). Serum leptin, IL-6 and TNF-α declined significantly after treatment in both groups versus baseline (P<0.05); the electroacupuncture group had lower post-treatment levels of leptin, IL-6 and TNF-α as well as larger pre-post changes than the sham group (P<0.05); ghrelin increased markedly after treatment in both groups (P<0.05), with higher post-treatment ghrelin and greater pre-post variation observed in the electroacupuncture group (P<0.05). In the electroacupuncture group, changes in ghrelin were negatively correlated with IL-6 and positively correlated with TNF-α (P<0.01); in the sham electroacupuncture group, ghrelin changes were negatively correlated with both IL-6 and TNF-α, whereas leptin changes were positively correlated with the two inflammatory markers (P<0.01). ConclusionElectroacupuncture can effectively improve clinical manifestations, as well as regulate serum metabolic and pro-inflammatory factors in patients with simple obesity, and discrepant correlation patterns of metabolism-inflammation are observed between the two groups.

This study aims to understand the biological characteristics of Streptococcus dysgalacti-ae of yak origin.Bacterial isolation and identification, drug susceptibility test, virulence gene test and pathogenicity test were carried out on milk samples of yaks from Naqu City to evaluate the bi-ological characteristics of the isolated strains.Meanwhile, molecular biological information such as virulence factors and drug resistance genes were analyzed by whole genome sequencing, and viru-lence genes were verified by PCR.The results showed that a strain of Streptococcus dysgalactiae was isolated from the milk of yak,and its colony morphology was pinpoint size, smooth edge and milky white. This strain is sensitive to many antibiotics(penicillin G,cephalosporin,ciprofloxacin,tetracycline,erythromycin,etc.).Virulence gene test results showed that the strain carries six key virulence genes(cyl, eno, scpB,bca, bac and napr),which may be closely related to its pathoge-nicity.In the pathogenicity test, the mice were listless and less active after infection, but no death occurred during the observation period. The pathological changes of spleen, kidney, liver and lung tissue were found, suggesting that the strain had certain pathogenic potential but not high lethali-ty.Whole genome sequencing data showed that the gene length of this strain was 4 079 280 bp,the GC content was 39.41%,3 964 coding genes were predicted,604 of which were annotated as viru-lence factors, and another 28 gene mutations may enhance its pathogenic ability.Through annota-tion of CARD database, two Pat A resistance genes and two lmrp resistance genes were found,re-vealing their potential resistance mechanism.Through whole genome sequencing technology and bioinformatics analysis method, this study revealed the genomic characteristics, drug resistance and pathogenicity mechanism of Streptococcus dysgalactiae of yak origin. The findings provide impor-tant scientific evidence for further exploration of the pathogenicity, drug resistance mechanisms, and molecular evolution of yak-derived Streptococcus agalactiae.

Left ventricular hypertrophy(LVH)is one of the common target organ damage of hypertension."Yang leading to qi and yin leading to formation"is the basic form of human material and energy metabolism and disease development. The pathogenesis of hypertension LVH is inseparable from the imbalance of yin and yang. The deficiency of"yang leading to qi"leads to the decline of zang-fu function, deficiency of qi and blood, and the excess of"yin leading to formation"leads to the accumulation of pathological products such as phlegm and fluid, both of which are important pathogenesis of hypertensive LVH.Hypertensive LVH can be seen at the beginning of yin deficiency and yang hyperactivity, liver hyperactivity and wind movement,qi and blood deficiency, and later yin and yang deficiency, phlegm and blood stasis interjunction and other symptoms. In the treatment of hypertensive LVH,the early stage should be based on nourishing yin and extinguishing wind, regulating qi and blood, and the later stage should be based on supplementing yin and yang, expelling phlegm and promoting blood,so as to improve the clinical efficacy of TCM treatment of this disease.

Objective:To analyze the clinical features and genetic sequencing results of a patient with Kallmann syndrome(KS) carrying digenic mutations who initially presented with osteoporosis, and to enhance awareness of this disease phenotype. Methods:Clinical data were collected, and peripheral blood DNA was extracted for whole-exome sequencing. Relevant literature was reviewed to summarize phenotypes associated with digenic/oligogenic variants involving CHD7.Results:The patient exhibited back pain, delayed development of secondary sexual characteristics, and hyposmia. Laboratory tests revealed reduced sex hormones and gonadotropin levels, while pituitary imaging was unremarkable. Bone mineral density imaging confirmed osteoporosis, and thoracolumbar X-rays showed multiple vertebral compression fractures. Genetic analysis identified a heterozygous splice-site mutation in CHD7(c.2698-1G>T) and a heterozygous missense mutation in WDR11(c.439G>A: p.D147N). According to ACMG criteria, the CHD7 mutation was classified as pathogenic, while the WDR11 variant was defined as a variant of uncertain significance(VUS). Literature review indicated that 40% of KS patients with digenic/oligogenic variants involving CHD7 presented with hearing or ocular abnormalities. Conclusion:This study reports a novel CHD7 mutation and a previously undescribed digenic combination of CHD7 and WDR11 variants in a KS patient. CHD7 variants may be implicated in auditory or ocular involvement in KS cases with digenic/oligogenic inheritances. KS patients may also manifest skeletal abnormalities in addition to hypogonadotropic hypogonadism. Tailored management of sex hormones and osteoporosis therapies across life stages is essential for optimizing bone health in KS.

Objective:To conduct a comprehensive review on the current research status of postpartum follow-up for patients with hypertensive disorders of pregnancy (HDP), and provide a basis and guidance for the management of postpartum follow-up for HDP patients. Methods:Guided by the methodological framework of the scoping review, the literatures on postpartum follow-up of HDP patients in Cochrane Library, PubMed, Web of Science, Embase, China Biomedical Literature Database, China National Knowledge Infrastructure, VIP and Wanfang databases were retrieved, and search for relevant literature by the snowballing method. The search time range is from the establishment of the database to February 1, 2025. The included literature was analyzed and summarized. Results:A total of 32 literatures were included, including 31 in English and 1 in Chinese. The participation rate of postpartum follow-up for HDP patients is generally low, and the disease has a significant impact on long-term health. In addition, there is considerable variation in the content, format and time of follow-up. The follow-up content includes the patient's physical health status, long-term health risks and the availability of medical resources, etc. Conclusions:There are still deficiencies in the current implementation of postpartum follow-up for HDP patients. Existing follow-up studies lack high-quality randomized controlled trials as evidence support, and the influencing factors are complex and diverse. In the future, it is necessary to strengthen cooperation among multiple disciplines, closely integrate clinical guidelines with practical needs, and systematically evaluate the effectiveness of follow-up in clinical practice to enhance the scientific and effectiveness of follow-up and provide better health management support for HDP patients.

Objective:To develop and validate a Self-care Ability Scale for Arteriovenous Fistula (AVF) in Maintenance Hemodialysis (MHD) patients. Methods:Guided by Orem's self-care theory, the initial item pool of the scale was developed through a literature review, semi-structured interviews, and group discussions. The initial scale was finalized after two rounds of expert consultation using the Delphi method. A convenience sampling method was used to recruit 418 MHD patients using AVF in January 2024 for item analysis, exploratory factor analysis and reliability testing. Another 293 MHD patients using AVF were recruited in March 2024 for confirmatory factor analysis. Results:The self-care ability scale for AVF in MHD patients included four dimensions: knowledge and skills of AVF self-care, willingness and attitude toward AVF self-care, recognition and prevention of AVF complications, and patient self-adjustment and adaptation, comprising 38 items. The content validity index at the scale level was 0.98. Exploratory factor analysis extracted four common factors with a cumulative variance contribution rate of 84.706%. Confirmatory factor analysis indicated good model fit, strong convergent validity, and ideal discriminant validity. The total Cronbach's α coefficient of the scale was 0.987; the split-half reliability coefficient was 0.902, and the test-retest reliability coefficient was 0.979.Conclusions:The Self-care Ability Scale for AVF in MHD patients demonstrates excellent reliability and validity, making it a suitable tool for assessing patients' ability to self-care for their AVF.
