Party affairs cadres in public hospitals play a significant role in promoting high-quality development within hospitals.Their competence and quality directly influence the level of Party construction work in public hospitals and affect the comprehensive development of hospitals.They also serve as an important window for the Party to connect with and serve the peo-ple.Based on competency theory,this study selects Party affairs cadres in public hospitals as the research subjects.Through lit-erature review and interviews,competency elements are summarized,and a competency model for Party affairs cadres in public hospitals is constructed.From four dimensions-innovating selection and training mechanisms,developing career planning and mentorship systems,strengthening performance management and incentive mechanisms,and fostering a learning-oriented organi-zational atmosphere-this study proposes innovative strategies for building a team of Party affairs cadres in public hospitals.

Medical ultrasound technology is widely used for diagnosis and therapy in clinical practice.Ultrasound probes,which are directly contact with patients,pose a potential risk of pathogen transmission.This expert consen-sus was developed by a multidisciplinary team based on international guidelines,standards in China,and the results of a national survey,aiming to reduce the risk of healthcare-associated infection through standardizing reprocessing of medical ultrasound probes,and formulating consensus recommendations with the Delphi method.The consensus clarifies the reprocessing principles for three types of ultrasound probes of different infection risks:external-use ul-trasound probes,interventional percutaneous ultrasound probes,and internal-use ultrasound probes,puts forward systematic suggestions on the reprocessing standards and disinfection levels of ultrasound probe isolation covers and coupling agents,the reprocessing procedures and methods of ultrasound probes,as well as architectural layout and management of reprocessing,so as to provide a scientific prevention and control framework for ensuring ultrasound diagnosis and therapy safety.

4.The impact of intrapartum fever severity associated with epidural labor analgesia on maternal and neonatal perinatal outcomes

Objective To investigate the impact of different degrees of fever during epidural labor analgesia on maternal and neonatal perinatal outcomes.Methods A retrospective analysis was conducted on 240 pregnant women who received epidural labor analgesia.Based on the peak temperature during labor,the participants were divided into the control group(<37.3℃,160 cases),the A group(37.3 to<38℃,55 cases)and the B group(≥38℃,25 cases).The differences in maternal prenatal data,labor progression and neonatal outcomes were compared between the three groups.Results There were no significant differences in mode of delivery,onset of labor and membrane rupture method between the three groups(P>0.05).The time interval from the initiation of epidural analgesia to delivery and the time from membrane rupture to delivery were longer in the A group and the B group compared to the control group(P<0.05).Among women who delivered vaginally,the first stage of labor was longer in the A group and the B group compared to the control group(P<0.05),and the second stage and total duration of labor were longer in the A group than those in both the control group and the B group(P<0.05).There were no significant differences in neonatal Apgar scores,birth weight,umbilical blood pH and the incidence of neonatal complications between the three groups(P>0.05).Conclusion Longer epidural analgesia duration and a prolonged first stage of labor may increase the risk of labor-associated fever.Higher degree of fever during labor promotes more active intervention in the second stage of labor,but it does not significantly affect neonatal outcomes.
5.Advances in role of tissue-resident memory T cells in SARS-CoV-2 infec-tion

Tissue resident memory T(TRM)cells are a class of memory T cells with long life span and non-lymphoid resident characteristics.TRM cells are the first line of defense against secondary infections in human.TRM cells can restrict the transmission of viruses as well as combat against viruses.Rapid evolution of severe acute respiratory syn-drome coronavirus 2(SARS-CoV-2)and post-COVID-19 syndrome could lead to a series of symptoms such as pulmonary fi-brosis,resulting in long-term fall in quality of life.Many studies focused on enhancing the ability to cope with the mutation of SARS-CoV-2 and reducing the incidence of post-COVID-19 syndrome.The relationship between TRM cells and SARS-CoV-2 infection or therapy still remain unknown.It is significant to investigate TRM cells divergence and functions for the development of vaccines.We review the functions and residence properties of TRM cells,as well as their roles in SARS-CoV-2 infection and post-COVID-19 syndrome.In this review,we are dedicated to deepen our understanding of the immune process in SARS-CoV-2 infection and provide a new prospect for the prevention and treatment of post-COVID-19 syndrome.
6.Related factors and predictive value of intrauterine infection during delivery in pregnant women with gestational diabetes mellitus

Objective To explore related factors of intra-amniotic infection(IAI)during the delivery period in pregnant women with gestational diabetes mellitus(GDM),and to analyze the predictive value of these related factors for IAI.Methods A retrospective study was conducted on 360 GDM pregnant women,including 120 cases with IAI(IAI group)and 240 pregnant women with GDM who did not develop IAI(control group).Clinical data,fetal monitoring during labor,placental pathology,pregnancy outcomes and glycosylated hemoglobin(HbA1c)results were collected for both groups.Multivariate Logistic regression was used to investigate influencing factors of IAI.The predictive value of combined HbA1c levels,parity and rupture time for IAI was evaluated using receiver operating characteristic(ROC)curves,and the area under the curve(AUC)was calculated.Results In the IAI group,the proportion of primigravida,first-time delivery,artificial rupture of membranes,oxytocin use,rupture time and HbA1c levels were significantly higher than those in the control group(P<0.05).The IAI group had a smaller gestational age at delivery,higher rates of forceps-assisted delivery,cesarean section and amniotic fluid contamination(P<0.05).In the IAI group,the proportion of abnormal intrapartum baseline fetal heart rate and the incidence of late decelerations were higher than those in the control group(P<0.05),while the incidence of variable deceleration and the proportion of category Ⅱ fetal heart rate monitoring were lower in the IAI group than those in the control group(P<0.05).There were no significant differences in baseline variability or acceleration of fetal heart rate between the two groups(P>0.05).The proportions of mild neonatal asphyxia and neonatal transfer to intensive care were high in the IAI group than those of the control group(P<0.05).Multivariate Logistic regression showed that elevated HbA1c levels(OR=3.342,95%CI:1.539-7.257)and prolonged rupture time(OR=1.001,95%CI:1.000-1.001)were independent risk factors for IAI in GDM pregnant women,while multiparity was a protective factor(OR=0.113,95%CI:0.046-0.278).The AUC for predicting IAI during labor by combining HbA1c,parity and rupture time was 0.819(95%CI:0.772-0.865),with a sensitivity of 0.742 and specificity of 0.767.Conclusion HbA1c levels,rupture time and parity are independent influencing factors for IAI during labor in pregnant women with GDM.The combination of these three factors has high clinical value in predicting IAI in GDM patients.
7.Clinical study of Cuofeng San in the treatment of anterior circulation cerebral infarction in patients with syndrome of wind and phlegm blocking collaterals

Objective To observe the efficacy of Cuofeng San in the treatment of anterior circulation cerebral infarction(ACCI)of patients with syndrome of wind and phlegm blocking collaterals.Methods A total of 150 ACCI patients were collected in this study,and they were divided into the control group and the treatment group using a random number table method,with 75 cases in each group.The control group was given standardized intervention according to the stroke diagnosis and treatment guidelines,while the treatment group was treated with modified Cuofeng San on the basis of the control group.Both groups were treated continuously for 2 weeks.The treatment effects were compared between the two groups before and after treatment,including the National Institutes of Health Neurological Impairment Scale(NIHSS)score,Barthel Index(BI)score of daily activity ability,scores of syndrome of wind and phlegm blocking collaterals,serum levels of neuron specific enolase(NSE),S100 calcium binding protein B(S100B),monocyte chemoattractant protein-1(MCP-1)and vascular endothelial cell calcium binding protein(VE-cadherin).Results The total effective rate of ACCI patients was higher in the treatment group than that in the control group(93.3%vs.81.3%,P<0.05).After 1 week and 2 weeks of treatment,the NIHSS score and wind phlegm obstruction syndrome score decreased in both groups compared to those before treatment,while the BI score increased,and the improvement was more significant in the treatment group(P<0.05).After the end of the treatment course,serum levels of NSE,S100B,MCP-1 and VE-cadherin decreased compared to those before treatment,and the decrease was more significant in the treatment group(P<0.05).Conclusion The modified Cuofeng San has a significant effect on the treatment of wind phlegm obstruction syndrome in patients with ACCI acute phase.It can improve neurological function and enhance daily activity ability.

10.Clinical advances in lateral nail fold formation in complete syndactyly

Syndactyly is a deformity that the soft tissues and (or) bones of two or more fingers are joined together. Based on the severity of interphalangeal skin-soft tissue juxtaposition, syndactyly is categorized as: incomplete syndactyly and complete syndactyly. Complete syndactyly correction includes interphalangeal separation, web-space reconstruction, coverage of skin defects on the body of the finger, and fingertip formation. The key step is the formation of the lateral nail fold after the separation of the syndactyly, and the shaping technique is closely related to the postoperative appearance of the fingertip. By reviewing the literatures, this article presents an overview of the relevant anatomy, nail juxtaposition typing, and the principles and applications of the complete juxtaposition nail lateral nail fold molding procedure.
