Mesenchymal stem cells (MSCs) have been widely used in the treatment of various autoimmune and inflammation-related diseases due to their potent immunomodulatory properties. Several studies have demonstrated that MSC-mediated immunomodulation is complex and bidirectional, with the in vivo microenvironment influencing the direction of this modulation. Indoleamine-2,3-dioxygenase (IDO), an immunosuppressive factor, has been identified as a key "switch" in the immunomodulatory role of MSCs. In this review, we explore how IDO functions as a critical regulator of MSC immunoregulatory plasticity. We delve into the mechanisms by which changes in IDO expression affect the function of various immune cells, summarize relevant research and clinical advances regarding the role of IDO expression in MSC-based therapies for various diseases, and discuss potential therapeutic strategies that target IDO to enhance the stability of MSC therapeutic effects. This provides a theoretical foundation for optimizing MSCs as safer and more effective clinical therapeutic agents.
2.Analysis on rate of adverse pregnancy outcomes in HIV-infected pregnant women and influencing factors in some regions of China, 2017-2023

Objective:To understand the incidence of adverse pregnancy outcome in HIV-infected pregnant women and influencing factors in China and provide reference for the improvement of the health status of HIV-infected pregnant women and their newborns.Methods:Based on a mother-child cohort of HIV-infected pregnant women and children (PMTCT-MC-2005) established in Guangxi Zhuang Autonomous Region, Yunnan Province and Xinjiang Uygur Autonomous Region, this study enrolled pregnant women with or without HIV infection as study subjects from January 2017 to June 2023, a total of 1 646 pregnant women (558 HIV-infected and 1 088 HIV-uninfected) were included, and 34 cases with missing data were excluded. The χ2 test was used to analyze the difference in the incidence adverse pregnancy outcome between two groups, and used logistic regression model to identify the influencing factors of adverse pregnancy outcome in HIV-infected pregnant women. Results:A total of 1 612 pregnant women were included in the study, in whom 541 were infected with HIV and 1 071 were not infected with HIV. The incidence of adverse pregnancy outcome was 18.8% (303/1 612), the incidence of adverse pregnancy outcome was 33.1% (179/541) in the HIV-infected pregnant women and 11.6% (124/1 071) in the pregnant women without HIV infection. The results of multivariable logistic regression analysis showed that the influencing factors of adverse pregnancy outcome were age <35 years at delivery (a OR=0.64, 95% CI: 0.43-0.95) compared with the age ≥35 years and the duration of antiviral treatment over 10 years (a OR=0.43, 95% CI: 0.23-0.79) compared with less than one year. Conclusions:The incidence of adverse pregnancy outcome in HIV-infected pregnant women was high in some regions of China during 2017-2023. It is necessary for HIV-infected women to get pregnancy at appropriate time based on antiretroviral treatment effect and strengthen self-care to reduce the incidence of adverse pregnancy outcome.
3.Symptoms and impact on quality of life of patients with Ménière′s disease

Objective:To explore the symptoms of Ménière′s disease patients and their impact on quality of life.Methods:A cross-sectional study was conducted, consecutively enrolling patients of Ménière′s disease who visited the Otolaryngology Clinic at the Eye & ENT Hospital of Fudan University from October 2014 to December 2022. The Chinese version of the Ménière′s Disease Outcomes Questionnaire (MDOQ) and a Ménière′s disease-specific symptom checklist were utilized for assessment. SPSS 25.0 software was employed to perform multiple linear regression analysis to determine the impact of Ménière′s disease symptoms on patients′ quality of life.Results:A total of 790 patients with a definitive diagnosis of Ménière′s disease who met the inclusion and exclusion criteria were analyzed. The cohort comprised 418 males and 372 females, with a mean age of (54.8±13.1) years and a diagnosis duration ranging from 0 to 72 months, with a median of 3 months. The total score of the Chinese MDOQ was 61.0±12.0. The symptomatic presentations of the enrolled patients included hearing changes, tinnitus, aural fullness, drop attacks, vertigo episodes, visual instability, dizziness, and headache. Multiple linear regression analysis revealed that age, tinnitus, aural fullness, frequency of drop attacks, visual instability, dizziness, and headache were significant factors affecting the quality of life in Ménière′s disease patients ( F=145.50, P<0.05). Conclusions:The quality of life in Ménière′s disease patients requires improvement. Attention to the specific symptoms of Ménière′s disease and their impact on patients′ quality of life, along with targeted symptom interventions based on these findings, will be a focal point in future disease management.
4.Mediating role of self-compassion in psychological distress and self-management behaviors among patients with type 2 diabetes

Objective:To investigate the status of psychological distress, self-compassion, and self-management behaviors among patients with type 2 diabetes mellitus (T2DM), and explore the mediating role of self-compassion in the relationship between diabetes distress and self-management behaviors, and to provide a theoretical basis for the psychological intervention of patients with T2DM.Methods:A convenience sampling method was adopted to select 221 patients with T2DM hospitalized in the Department of Endocrinology and Metabolism, the First Hospital of China Medical University from November 2023 to April 2024. The General Information Form, the Diabetes Distress Scale-17, the Self-Compassion Scale and the Summary of Diabetes Self-Care Activity were utilized for investigation. A structural equation model was constructed with AMOS 24.0 to analyze the relationships among psychological distress, self-compassion, and self-management behaviors, and the mediating effect of self-compassion was tested using the Bootstrap method.Results:A total of 221 patients with T2DM were included, including 138 males and 83 females, aged (54.52 ±10.42) years old. The total score of psychological distress of patients with T2DM was (34.55 ± 18.82) points, the total score of self-compassion was (94.22 ± 26.04) points, and the total score of self-management behavior was (40.81 ± 22.64) points. Psychological distress in T2DM patients was significantly negatively correlated with both self-compassion ( r=-0.850, P<0.01) and self-management behaviors ( r=-0.736, P<0.01). In contrast, self-compassion was significantly positively correlated with self-management behaviors ( r=0.743, P<0.01). Self-compassion partially mediated the relationship between diabetes-related psychological distress and self-management behaviors, with a mediation effect size of -0.456. Conclusions:Psychological distress among T2DM patients is at a moderate level, and their self-management behaviors require urgent improvement. Psychological distress not only directly impacts self-management behaviors but also indirectly influences them through self-compassion. Healthcare professionals should prioritize enhancing patients′ self-compassion levels to mitigate the negative effects of psychological distress on their health behaviors.

8.Clinical evaluation for rapid detection of carbapenemase produced by Klebsiella pneumoniae and Pseudomonas aeruginosa u-sing Autof MS 1000 mass spectrometry identification system

Objective To investigate the clinical value of matrix-assisted laser desorption/ionization time of flight mass spectrometry(MALDI-TOF MS)in rapid detection of carbapenemase produced by Klebsiella pneumoniae and Pseudomonas aeruginosa.Methods A total of 60 strains of Klebsiella pneumoniae and 80 strains of Pseudomonas aeruginosa isolated from Pizhou People's Hospital affiliated to Xuzhou Medical University from January 2022 to October 2023 were collected,including 30 strains of carbapenem-resistant Klebsiella pneumoniae(CRKP),30 strains of carbapenem-sensitive Klebsiella pneumoniae(CSKP),50 strains of carbapenem-resistant Pseudo-monas aeruginosa(CRPA)and 30 strains of carbapenem-sensitive Pseudomonas aeruginosa(CSPA).Three detection methods were applied,i.e.,modified carbapenem inactivation method(mCIM),colloidal gold immunochromatography and Autof MS 1000 mass spectrometry identification system to evaluate the ability of Autof MS 1000 mass spectrometry identification system in detecting carbape-nase production of Klebsiella pneumoniae and Pseudomonas aeruginosa.Results The results of Autof MS 1000 mass spectrometry iden-tification system were consistent with those of both mCIM and colloidal gold immunochromatography.Carbapenemase was detected in 28 of the 30 CRKP strains,and it was negative in 2 CRKP strains.Carbapenamase was detected in 15 of the 50 CRPA strains and it was negative in 35 CRPA strains.Thirty strains of CSKP and 30 strains of CSPA were all Carbapenemase negative.The coincidence rate of the results of the three methods in the detection for carbapenase was 100%.Conclusion The result of Autof MS 1000 mass spectrome-try identification system has been consistent with those of mCIM and colloidal gold immunochromatography.It not only has the charac-teristics of cost-saving compare with of mCIM method,but also hold the advantages of fast speed and high accuracy of colloidal gold im-munochromatography method.Thus,Autof MS 1000 system can be used for the rapid identification of carbapenemase produced by Kleb-siella pneumoniae and Pseudomonas aeruginosa.

10.Effects of peripelvic multi-joint comprehensive exercise in late pregnancy in pregnant women

Objective:To explore the effect of peripelvic multi-joint comprehensive exercise in late pregnancy in pregnant women.Methods:From July 2020 to June 2021, 110 first-time pregnant women who were registered at the People's Hospital of Bozhou were selected as the study subject using convenience sampling. According to the random number table method, study subjects were randomly divided into a study group and a control group, with 55 cases in each group. Pregnant women in the study group underwent peripelvic multi-joint comprehensive exercise, while those in the control group underwent routine walking. We compared the adverse events, pain, pelvic ligament elasticity, Bishop score, pregnancy days, and fetal position after exercise between two groups of pregnant women.Results:The study group ultimately included 52 pregnant women, while the control group ultimately included 51 pregnant women. Both groups of pregnant women did not experience abnormal bleeding after exercise. The number of pregnant women in the study group experiencing pain after exercise one day before delivery was lower than that in the control group, with a statistically significant difference ( P<0.05). The pelvic ligament elasticity of pregnant women in the study group during delivery was better than that of the control group, and the Bishop score was higher than that of the control group, with statistically significant differences ( P<0.05). There was no statistically significant difference in the number of pregnancy days between the two groups of pregnant women ( P>0.05). The number of fetuses in the occipital anterior position during admission to labor in the study group was higher than that in the control group, with a statistically significant difference ( P<0.05) . Conclusions:Prenatal multi-joint and deep muscle and ligament exercises through peripelvic multi-joint comprehensive exercise can strengthen the elasticity of the pelvic ligaments in pregnant women, adjust the fetal pelvic entry angle, and reduce the occurrence of post exercise pain, which is worthy of clinical practice.
