2.Associations of Exposure to Typical Environmental Organic Pollutants with Cardiopulmonary Health and the Mediating Role of Oxidative Stress: A Randomized Crossover Study.

OBJECTIVE:
The study aim was to investigate the effects of exposure to multiple environmental organic pollutants on cardiopulmonary health with a focus on the potential mediating role of oxidative stress.
METHODS:
A repeated-measures randomized crossover study involving healthy college students in Beijing was conducted. Biological samples, including morning urine and venous blood, were collected to measure concentrations of 29 typical organic pollutants, including hydroxy polycyclic aromatic hydrocarbons (OH-PAHs), bisphenol A and its substitutes, phthalates and their metabolites, parabens, and five biomarkers of oxidative stress. Health assessments included blood pressure measurements and lung function indicators.
RESULTS:
Urinary concentrations of 2-hydroxyphenanthrene (2-OH-PHE) ( β = 4.35% [95% confidence interval ( CI): 0.85%, 7.97%]), 3-hydroxyphenanthrene ( β = 3.44% [95% CI: 0.19%, 6.79%]), and 4-hydroxyphenanthrene (4-OH-PHE) ( β = 5.78% [95% CI: 1.27%, 10.5%]) were significantly and positively associated with systolic blood pressure. Exposures to 1-hydroxypyrene (1-OH-PYR) ( β = 3.05% [95% CI: -4.66%, -1.41%]), 2-OH-PHE ( β = 2.68% [95% CI: -4%, -1.34%]), and 4-OH-PHE ( β = 3% [95% CI: -4.68%, -1.29%]) were negatively associated with the ratio of forced expiratory volume in the first second to forced vital capacity. These findings highlight the adverse effects of exposure to multiple pollutants on cardiopulmonary health. Biomarkers of oxidative stress, including 8-hydroxy-2'-deoxyguanosine and extracellular superoxide dismutase, mediated the effects of multiple OH-PAHs on blood pressure and lung function.
CONCLUSION
Exposure to multiple organic pollutants can adversely affect cardiopulmonary health. Oxidative stress is a key mediator of the effects of OH-PAHs on blood pressure and lung function.

3.Establishment and optimization of a high-performance size-exclusion chromatography method for quantifying the classical swine fever virus E2 protein.

This study aims to establish a high-performance size-exclusion chromatography (HPSEC) method for determining the content of the classical swine fever virus (CSFV) E2 protein and screen the optimal stabilizer to enhance the stability of this protein. The optimal detection conditions were determined by optimizing the composition of the mobile phase, and characteristic chromatographic peaks were identified by SDS-PAGE and Western blotting. The specificity, repeatability, precision, linearity, limit of detection (LOD), and limit of quantitation (LOQ) of the method were assessed. The method established was used to determine the content of CSFV E2 protein antigen and vaccine. Differential scanning fluorimetry (DSF) was employed to screen the buffer system, pH, and salt ion concentrations, and sugar, amino acid, and alcohol stabilizers were further screened. The results showed that using a 200 mmol/L phosphate buffer provided the best column efficiency. An antigen-specific chromatographic peak appeared at the retention time of 18 min, which was identified as the CSFV E2 protein by SDS-PAGE and Western blotting. The method exhibited high specificity for detecting the CSFV E2 protein, with no absorbance peak observed in the blank control. The relative standard deviation (RSD) of the peak area for six repeated injections of the CSFV E2 protein was 0.74%, indicating good repeatability of the method. The RSD for repeated detection of two different concentrations of CSFV E2 protein samples by different operators at different time points was less than 2%, suggesting good intermediate precision of the method. The peak area of the CSFV E2 protein was linearly related to its concentration, with the regression equation showing R2 of 1.000. The LOD and LOQ of the method were 14.88 μg/mL and 29.75 μg/mL, respectively. Application of the developed method in the detection of three batches of CSFV E2 protein antigen and three batches of vaccine demonstrated results consistent with those from the bicinchoninic acid (BCA) assay, which meant that the method could accurately determine the content of CSFV E2 protein antigen and vaccine. The DSF method identified 50 mmol/L Tris-HCl at pH 8.0 as the optimal buffer, and the addition of sugar and alcohol stabilizers further improved the stability of the CSFV E2 protein. The HPSEC method established in this study is simple, fast, and exhibits good accuracy and repeatability, enabling precise measurement of the CSFV E2 protein content. It is expected to play a crucial role in the quality control of the CSFV E2 vaccine. Furthermore, the strategy for improving the CSFV E2 protein stability, identified through DSF screening, has significant implications for enhancing the stability of the CSFV E2 vaccine.

4.Promotive effect of high expression of nerve growth factor in Schwan-like cells induced by adipose-derived stem cells on growth of rat dorsal root ganglion cell protrusion

Objective:To discuss the promotive effect of nerve growth factor(NGF),which is highly expressed in the adipose-derived stem cell(ADSC)-induced Schwann-like cells(SCLCs),on the growth of dorsal root ganglion(DRG)cell processes in the rats,and to clarify its mechanism.Methods:The ADSCs were extracted from the epididymal adipose tissue of the SD rats,and their multidirectional differentiation potential was identified through osteogenic,adipogenic,and chondrogenic induction.The ADSCs were induced to differentiate into the SCLCs,and the expression levels of glial fibrillary acidic protein(GFAP)and S100 calcium-binding protein β(S100β)protein in the ADSCs and SCLCs were detected by immunofluorescence staining and Western blotting methods.The DRG cells were isolated and cultured,and immunofluorescence staining was used to detect the βⅢ-tubulin expression in the DRG cells for identification.The SCLCs were co-cultured with the DRG cells(co-culture group),the single-culture DRG cells were regared as DRG group and toluidine blue staining was used to observe and measure the length of DRG cell processes under the optical microscope in co-culture group and DRG group.Small interfering RNA(siRNA)transfection was used to knock down NGF,and plasmid transfection was used to over-express NGF.Real-time fluorescence quantitative PCR(RT-qPCR)method was used to detect the NGF mRNA expression levels in the cells in various groups;enzyme-linked immunosorbent assay(ELISA)method was used to detect the NGF protein levels in the cell supernatants.The transfected SCLCs were co-cultured with DRG cells and divided into control group,siNC/vector group,NGF knockdown group(si-NGF group),and NGF over-expression group(oe-NGF group).The lengths of DRG cell processes in various groups were observed.Results:The primary ADSCs adhered within 24 h after seeding,with a small number of lipid droplets remaining.After 3 d of culture,the cells were mostly short spindle-shaped,fusiform,or polygonal,growing rapidly in a vortex pattern.After passaging,the cells exhibited a uniform morphology,appearing as long spindles arranged in a fish-school pattern.After 14 d of adipogenic induction,the cell morphology changed from spindle-shaped to flat-round,with translucent lipid droplets forming in the cytoplasm,which were stained red by Oil Red O.After 28 d of osteogenic induction,the cells appeared sand-like with blurred morphology,and calcified nodules were observed,which were stained red by Alizarin Red and deposited in the extracellular matrix.After 28 d of chondrogenic induction in a 3D culture system,millet-sized chondrogenic spheres formed.Frozen sections of the spheres were stained with Alcian Blue,and acidic mucopolysaccharides in the cartilage tissue were stained blue under the microscope.Under the fluorescence microscope,the third-passage purified ADSCs showed positive expression of CD29[fluorescein isothiocy anate(FITC)-labeled green fluorescence]and CD44(Cy3-labeled red fluorescence).The immunofluorescence staining results showed that GFAP was labeled with FITC(green fluorescence),and S100β was labeled with Cy3(red fluorescence).The Western blotting results showed that compared with ADSCs,the expression levels of S100β and GFAP proteins in the SCLCs were increased(P<0.05).The primary DRG cells began to adhere 6 h after conventional culture,and after 3 d,the cell bodies appeared round and bright,with two linear processes extending from them.Under fluorescence microscope,the cells positively expressed the neuron-specific marker βⅢ-tubulin,confirming that the isolated cells were DRG cells.Compared with the ADSCs,the NGF protein expression level in the SCLCs was increased(P<0.05).Compared with DRG group,the length of DRG cell processes in co-culture group was the highest when DRG cells and SCLCs were co-cultured at a 1∶2 ratio(P<0.05).The RT-qPCR results showed that compared with si-NC group,the expression levels of NGF mRNA in the cell supernatant in si-NGF-1,si-NGF-2,and si-NGF-3 groups were significantly decreased(P<0.05),with si-NGF-1 showing the highest knockdown efficiency,which was selected for subsequent experiments.The ELISA results showed that compared with si-NC group,the NGF levels in the cell supernatant of si-NGF-1,si-NGF-2,and si-NGF-3 groups were decreased(P<0.05).Compared with Vector group,the expression level of NGF mRNA and NGF protein level in the supernatant in oe-NGF group were increased(P<0.05).Compared with control group and siNC/vector group,the length of DRG cell processes in si-NGF group was decreased(P<0.05),while the length of DRG cell processes in oe-NGF group was increased(P<0.05).Conclusion:ADSCs can be directionally differentiated into SCLCs,and the differentiated cells highly express NGF.Knockdown or overexpression of NGF can affect the growth of DRG cell processes.
5.Effectiveness analysis of midline versus paramedian approaches for ultrasound-guided combined spinal-epidural anesthesia

Objective To investigate the clinical efficacy of the median and paramedian approaches in ultrasound-guided combined spinal-epidural anesthesia for patients with hemorrhoidectomy and to evaluate the differences between the two techniques.Methods A retrospective analysis was performed on 90 AIDS patients who underwent hemorrhoidectomy under combined spinal-epidural anesthesia from February 2024 to March 2025.The patients were categorized into two groups based on the ultrasound-guided puncture approach:a midline approach group and a paramedian approach group.In the midline approach group,anatomical landmarks such as the spinous process space,facet joint,and transverse process were identified in the transverse axial plane for accurate localiza-tion.The puncture site was determined as the midpoint of the interspinous space along the spinal midline,and an out-of-plane puncture technique was utilized.In the paramedian approach group,positioning was performed by scanning the lamina in the sagittal oblique plane.The dorsal complex structure space with the clearest visibility was centered under the probe,and an in-plane puncture technique was applied,with the needle advanced from the caudal to the cephalad direction to complete the combined spinal-epidural procedure.Data regarding puncture performance,complications,and anesthetic efficacy were systematically recorded and assessed for both groups.Results Intergroup analysis demonstrated that the paramedian approach group exhibited significantly fewer total puncture attempts and a higher success rate on the first attempt(P<0.05).No significant differences were observed between the two groups regarding the need to change puncture levels or modify the puncture approach(P>0.05).The midline approach group showed a significantly higher incidence of paresthesia and a greater frequency of post-operative puncture site tenderness at two days post-surgery compared to the paramedian approach group(P<0.05).Anesthetic efficacy was comparable between the two groups,with no statistically significant difference detected(P>0.05).Conclusions In hemorrhoidectomy procedures for AIDS patients,the paramedian approach demon-strates advantages over the median approach in terms of puncture success and reduced complications associated with ultrasound-guided combined spinal-epidural anesthesia.However,no significant difference in anesthetic efficacy is observed between the two approaches.
6.Arthroscopic suture and transoral tunnel Versalok anchor fixation for acute posterior cruciate ligament femoral insertion avulsion fractures

Objective:To investigate the early clinical efficacy of arthroscopic suture and transoral tunnel Versalok anchor fixation in the treatment of acute posterior cruciate ligament (PCL) femoral avulsion fractures in adults.Methods:A retrospective study was conducted to analyze the clinical data of 4 patients with acute PCL femoral insertion avulsion fracture who had been treated at Sports Medicine Center, Xi'an Honghui Hospital from January 2019 to June 2023. They were 1 male aged 49 years old and 3 females aged 39, 44, and 59 years old. Their time from injury to surgery was 3, 4, 9, and 12 days, respectively. All patients underwent arthroscopic suturing of the PCL femoral avulsion ligament junction. The sutures were pulled towards the medial femoral condyle through a bone tunnel created in the area of the medial femoral condyle ligament footprint and fixated with the Versalok anchor. Magnetic resonance imaging (MRI) was performed at 6 months postoperatively to assess union of the avulsion fracture and PCL continuity. The postoperative active knee flexion and extension, ligament tension, knee stability and knee functional recovery were recorded at the final follow-up.Results:One female patient sprained her knee while walking 6 months after surgery and then gradually developed knee pain and instability symptoms with a positive posterior drawer test (grade Ⅲ). She underwent PCL ligament reconstruction 7 months after surgery so that her follow-up was terminated. The remaining 3 patients were followed for 6, 12 and 13 months, respectively. Their active knee extension and flexion activities were comparable to those on the unaffected side and within the normal range at the last follow-up. Two patients had a negative posterior drawer test and 1 patient a positive posterior drawer test (grade I) but no symptoms of knee instability.Conclusion:Arthroscopic suture and bone tunnel Versalok anchor fixation technique in the treatment of acute femoral PCL avulsion fractures in adults is feasible and effective.
7.Arthroscopic suture half-tie and Versalok anchor fixation for avulsion fracture of the tibial insertion of anterior cruciate ligament in adults

Objective:To investigate the early clinical efficacy of arthroscopic suture half-tie combined with Versalok anchor fixation in the treatment of avulsion fracture of the tibial insertion of anterior cruciate ligament (ACL) in adults.Methods:A retrospective study was conducted to analyze the clinical data of the 26 adult patients with avulsion fracture of the ACL tibial insertion who had been treated by arthroscopic suture half-tie combined with Versalok anchor fixation between June 2020 and December 2022 at Sports Medicine Center, Honghui Hospital, Xi'an Jiaotong University. This cohort consisted of 12 males and 14 females with an age of (26.6±7.5) years. According to the Meyers-McKeever classification, there were 9 cases of type Ⅱ, 12 cases of type Ⅲ, and 3 cases of type Ⅳ. The time from injury to surgery was (9.3±6.8) days. Follow-up evaluations were conducted at postoperative 3 days, 3 months, and 12 months, including imaging assessments for fracture healing, clinical examinations of knee flexion and extension, anterior drawer and Lachman tests to assess the ACL tension. At the last follow-up, KT-2000 test was performed to quantify ACL laxity, and visual analogue scale (VAS) and Lysholm knee score were used to assess postoperative pain and functional recovery.Results:Primary wound healing was achieved in all the 26 patients. One patient required ACL reconstruction due to laxity and mild extension limitation at postoperative 6 months. The other 25 patients were followed for (15.4±3.2) months. At postoperative 12 months, X-rays showed good bone healing at the fracture site of the tibial intercondylar eminence, and both the Lachman and anterior drawer tests were negative. At postoperative 3 months and the last follow-up, the active knee extension and flexion were 128.8° (121.2°, 138.3°) and 52.7° (38.2°, 63.2°), respectively, significantly better than the preoperative value [52.7° (38.2°, 63.2°)] ( P<0.05), and the KT-2000 test showed that the disparity between the affected and healthy sides were 2.5 (1.2, 4.2) mm and 1.2 (0.8, 3.4) mm, respectively, significantly smaller than the preoperative value [7.1 (5.2, 11.2) mm] ( P<0.05). At postoperative 3 and 12 months, the VAS pain scores were significantly decreased and the knee Lysholm scores were significantly improved compared with the preoperative values ( P<0.05). Conclusion:In the treatment of avulsion fracture of the ACL tibial insertion in adults, arthroscopic suture half-tie combined with Versalok anchor fixation is a minimally invasive, reliable, and straightforward technique, leading to excellent early clinical outcomes.

The phosphatidylinositol 3-kinase/protein kinase B/mammalian target of rapamycin(PI3K/Akt/mTOR)signaling pathway plays a crucial role in the regulation of renal fibrosis by participating in inflammatory response,oxidative stress and autophagy.Paeoniflorin exhibits remarkable efficacy in treating myocardial and liver fibrosis.This article provides a comprehensive review on the research progress of paeoniflora in preventing and treating renal fibrosis through modulation of the PI3K/Akt/mTOR signaling pathway,offering novel insights for traditional Chinese medicine-based approaches to prevent and treat renal fibrosis.
9.Thermosensitive antibacterial hydrogel for treatment of infected bone defects

BACKGROUND:The traditional method for treating infected bone defects is thorough debridement,filling with antibiotic-impregnated bone cement,and then autologous bone transplantation.However,there are problems such as insufficient antibiotic concentration and drug resistance,and limited bone mass.Therefore,it is of great clinical significance to seek dual-functional biomaterials that can locally release antibiotics at the site of infection and promote bone repair.OBJECTIVE:To design an injectable hydrogel composed of hyaluronic acid and oxidized dextran as a local delivery system for vancomycin to treat infection and promote bone regeneration.METHODS:(1)Vancomycin-loaded hyaluronic acid/oxidized dextran hydrogels were prepared to characterize the morphology,mechanical properties,and in vitro drug release of the hydrogels.(2)Hyaluronic acid/oxidized dextran hydrogels and vancomycin-loaded hyaluronic acid/oxidized dextran hydrogels were co-cultured with rabbit bone marrow mesenchymal stem cells.Cell activity and proliferation were detected by live-dead staining and CCK-8 assay.After osteogenic induction,alkaline phosphatase staining,alizarin red staining,RUNX2 immunofluorescence staining,and RT-qPCR detection(osteocalcin and bone morphogenetic protein 2 mRNA expression)were performed to evaluate the osteogenic differentiation of rabbit bone marrow mesenchymal stem cells.(3)The above two hydrogels were co-cultured with Escherichia coli(or Staphylococcus aureus)to detect the antibacterial ability of the hydrogels.(4)Thirty rabbits were selected to establish a 1.5 cm infected bone defect model in the middle of the left radius.Two weeks after modeling,they were randomly divided into three intervention groups:the blank group(n=10)did not receive any treatment;the control group(n=10)was injected with hyaluronic acid/oxidized dextran hydrogel at the bone defect site;the experimental group(n=10)was injected with vancomycin-loaded hyaluronic acid/oxidized dextran hydrogel at the bone defect site.12 weeks after injection,the samples were collected for Micro-CT scanning,tissue morphology observation,RT-qPCR detection(tumor necrosis factor α,interleukin 6 mRNA expression),and immunohistochemical staining.RESULTS AND CONCLUSION:(1)Vancomycin-loaded hyaluronic acid/oxidized dextran hydrogel had a good porous structure with a pore size between 100-300 μm,and had good mechanical properties and in vitro drug sustained release performance.(2)Live-dead staining and CCK-8 assay results confirmed that vancomycin-loaded hyaluronic acid/oxidized dextran hydrogel had good biocompatibility.Alkaline phosphatase staining,alizarin red staining,RUNX2 immunofluorescence staining,and RT-qPCR test results showed that both hydrogels could promote osteogenic differentiation of rabbit bone marrow mesenchymal stem cells.(3)Compared with hyaluronic acid/oxidized dextran hydrogel,vancomycin-loaded hyaluronic acid/oxidized dextran hydrogel could significantly inhibit the growth of Escherichia coli and Staphylococcus aureus.(4)Micro-CT scanning results showed that the bone volume fraction and bone density of new bone in the bone defect area of the experimental group were higher than those of the blank group and the control group(P<0.05).The bone tissue morphology observation results showed that the experimental group had better repair effect of bone defects compared with the blank group and control group.The expression of tumor necrosis factor α and interleukin 6 mRNA in the bone defect site in the experimental group was lower than that in the blank group and control group(P<0.05).Immunohistochemical staining showed that the protein expressions of osteocalcin and RUNX2 at the site of bone defects in the experimental group and control group were higher than those in the blank group(P<0.05).These findings indicate that vancomycin-loaded hyaluronic acid/oxidized dextran hydrogel can effectively promote bone regeneration under infection.

Paraplegia caused by spinal cord injury is a serious neurological complication, for which surgery is currently the main treatment method. Due to different surgical approaches, patients are usually expected to maintain a passive prone position for a long time or switch between the supine and prone positions. Affected by multiple factors such as neurogenic sensory disorders, pathological changes in muscle tone and operative duration, the risk of intraoperative acquired pressure injury (IAPI) is significantly increased. Current clinical prevention strategies for IAPI in these patients predominantly focus on localized pressure relief during positioning, lacking systematic, standardized comprehensive prevention protocols or evidence-based guidelines. To address it, Department of Nursing, Orthopedics Branch, China International Exchange and Promotive Association for Medical and Health Care, Spinal Trauma Professional Committee, Orthopedics Branch, Chinese Medical Doctor Association, Nursing Group of Spine and Spinal Cord Professional Committee of Chinese Association of Rehabilitation Medicine organized experts in relevant fields to formulate Guideline for the prevention of intraoperative acquired pressure injury in paraplegic patients with spinal cord injury ( version 2025), based on evidence-based medical evidence and latest research results and clinical practice at home and abroad. Eleven recommendations were put forward from the aspects of preoperative risk assessment, intraoperative prevention strategies, postoperative handover and monitoring, and supportive mechanisms for IAPI prevention, aiming to standardize the prevention measures and management strategies of IAPI in paraplegic patients with spinal cord injury and accelerate the recovery of patients and improve the therapeutic effect.
