1.Traditional Chinese Medicine Prevents and Treats Parkinson's Disease Through Mitochondrial Autophagy: A Review

Parkinson's disease (PD) is a neurological degenerative disease with a high clinical incidence, unclear etiology, and incurability. The main pathological features of PD are the loss of dopaminergic neurons in the midbrain substantia nigra and the formation of Lewy bodies. At present, the anti-parkinsonism drugs used in clinical practice have problems, such as decreasing efficacy and severe toxic side effects in the late stage of the disease. Mitochondrial autophagy is a self-regulation mode in which cells automatically remove damaged and aging mitochondria to maintain mitochondrial homeostasis. It is generally believed that the degree of mitochondrial autophagy is weakened in PD state, and this process mainly progresses through the ubiquitin-dependent pathway, non-ubiquitin-dependent pathway, and α-Synuclein-mediated mitochondrial autophagy pathway. Abnormal accumulation of damaged mitochondria will cause further damage to nerve cells and accelerate PD process. Therefore, restoring the mitochondrial autophagy balance, reducing the excessive accumulation of abnormal mitochondria, and reducing the damage of dopaminergic neurons in the substantia nigra are particularly important for the treatment of PD. Traditional Chinese medicine (TCM) has definite effect in the clinical prevention and treatment of PD, and studies have been carried out targeting the intervention of mitochondrial autophagy. A large number of studies have confirmed that single herbs and compound prescriptions of TCM can relieve nerve function defects and delay degenerative changes of nerve cells by restoring the balance of mitochondrial autophagy, thus playing a role in the treatment of PD. This paper systematically summarized the regulatory mechanisms of mitochondrial autophagy in the pathogenesis of PD and the influences of active ingredients from single herbs and compound prescriptions of TCM on mitochondrial autophagy. Furthermore, this paper explored the pathogenesis of PD and the basis of the therapeutic role of TCM through the intervention of mitochondrial autophagy, providing references for promoting the application of TCM in the prevention and treatment of PD.

In the context of the modernization of traditional Chinese medicine (TCM), the inheritance of the experiences of famous doctors faces significant challenges due to its complex nonlinear characteristics and dynamic evolution. There are still issues in the current inheritance system, such as the homogenization of talent cultivation models, lack of standardized mentoring practices, and monotonous evaluation method, which hinder the systematic inheritance and innovative development of famous doctors' experiences. Based on a systematic review of the current state of artificial intelligence (AI)-assisted inheritance of famous doctors' experiences, this study explores innovative pathways for deep integration of modern information technologies with famous doctors' experiences from key dimensions, including data authenticity assurance, interdisciplinary collaboration mechanisms, and the establishment of dynamic inheritance standards. It proposes a paradigm shift in the inheritance of TCM famous doctors' experiences in the AI era, aiming to build a new TCM inheritance system of "digital intelligence empowerment and cross-disciplinary innovation", providing theoretical support and practical pathways for the inheritance of famous doctors' experiences in TCM.
3.Differentiation and Treatment of Chemotherapy-Induced Nausea and Vomiting with Traditional Chinese Medicine Based on the Theory of "Yin Qi Descends,While All Yang Qi Ascends"

This paper discussed the differentiation and treatment of chemotherapy-induced nausea and vomiting (CINV) in traditional Chinese medicine, based on the theory of "yin qi descends, while all yang qi ascends” from Inner Canon of Yellow Emperor (《黄帝内经》). The core pathogenesis of CINV is attributed to chemotherapy-induced injury to the middle jiao (焦), resulting in the separation of yin and yang and loss of their mutual communication. Acute and explosive CINV is often characterized by "all yang qi ascends", with stomach yang constraint transforming into fire, and qi counterflow leading to vomiting, which can be treated by unblocking stomach yang, draining fire and scattering the counterflow, with Huoxiang Zhengqi Powder (藿香正气散) and medicinals of clearing function. For delayed cases, which are often due to "yin qi descends", and the accumulation of turbid yin obstructs and constrains the deficient yang, the treatment should focus on promoting qi flow to eliminate turbidity, unblocking and boosting yang qi, and Chengqi-series Formulas (承气类方) or Xiao Banxia Decoction (小半夏汤) can be considered. Refractory and anticipated cases usually involve stomach-kidney deficiency and zang-fu (脏腑) organs disharmony, making yin and yang difficult to restore balance, for which the treatment should focus on nourishing both the stomach and kidneys and harmonize the zang-fu organs, with formulas such as Fuzi Lizhong Decoction (附子理中汤) or Xiaoyao Powder (逍遥散) modified as appropriate.
4.Modern Expanded Application of Ancient Classic Formulae from the Perspective of Syndrome‑Formula Ontology Reconstruction

As the core carrier of the inheritance and innovation of traditional Chinese medicine (TCM), the modern expanded application of classic formulae is an inevitable trend for TCM to adapt to the changes in disease spectrum and achieve academic development. However, several challenges remain, including the vague definition of syndrome-formula ontology between ancient and modern times, the insufficient adaptability of the evidence grading system, and the disconnection between theory and clinical practice, having severely restricted the precise application and standardized development of classic formulae. Based on the current status of the modern expanded application of classic formulae, and grounded in the core theory of formula-syndrome correspondence in TCM, this paper constructs a theoretical framework of "syndrome-formula ontology reconstruction". The framework systematically expounds its core connotations, theoretical foundations and practical logic, and further clarifies the reconstruction direction of the TCM-specific evidence grading system by taking correspondence between formula and syndrome as the core, constructing a multi-dimensional and integrated evaluation framework, and adhering to the orientation of clinical application. The ultimate goal is to form a theoretical paradigm characterized by "syndrome-formula ontology reconstruction-evidence grading reconstruction-precise clinical application", thereby providing theoretical support for the digital inheritance, evidence-based development, and modern application of classical prescriptions.
5.Pathogenesis and Treatment Strategies of Tumor Angiogenesis Based on the Theory "Latent Wind in Collaterals"

This article combined the pathogenic characteristics of "latent wind" with the theory of collateral diseases to clarify the pathological features of tumor blood vessels, including their active proliferation, high permeabi-lity, and promotion of metastasis. The theory framework of "latent wind in collaterals" as the tumor mechanism was proposed, which suggests that at the site of tumor lesions, the collaterals inherit the nature of latent wind to grow excessively, adopt an open and discharge nature to leak essence, and tumor toxins, characterized by their rapid movement and frequent changes, spread and metastasize, driving the progression of malignant tumors. Focusing on the fundamental pathogenesis of "latent wind in collaterals", specific clinical treatment principles and methods centered on treating wind are proposed, including regulating qi and dispelling wind, clearing heat and extinguishing wind, unblocking collaterals and expelling wind, and reinforcing healthy qi to calm wind, so as to provide references for enhancing the precision of traditional Chinese medicine in treating malignant tumors.
6.ResNet-Vision Transformer based MRI-endoscopy fusion model for predicting treatment response to neoadjuvant chemoradiotherapy in locally advanced rectal cancer: A multicenter study.

BACKGROUND:
Neoadjuvant chemoradiotherapy followed by radical surgery has been a common practice for patients with locally advanced rectal cancer, but the response rate varies among patients. This study aimed to develop a ResNet-Vision Transformer based magnetic resonance imaging (MRI)-endoscopy fusion model to precisely predict treatment response and provide personalized treatment.
METHODS:
In this multicenter study, 366 eligible patients who had undergone neoadjuvant chemoradiotherapy followed by radical surgery at eight Chinese tertiary hospitals between January 2017 and June 2024 were recruited, with 2928 pretreatment colonic endoscopic images and 366 pelvic MRI images. An MRI-endoscopy fusion model was constructed based on the ResNet backbone and Transformer network using pretreatment MRI and endoscopic images. Treatment response was defined as good response or non-good response based on the tumor regression grade. The Delong test and the Hanley-McNeil test were utilized to compare prediction performance among different models and different subgroups, respectively. The predictive performance of the MRI-endoscopy fusion model was comprehensively validated in the test sets and was further compared to that of the single-modal MRI model and single-modal endoscopy model.
RESULTS:
The MRI-endoscopy fusion model demonstrated favorable prediction performance. In the internal validation set, the area under the curve (AUC) and accuracy were 0.852 (95% confidence interval [CI]: 0.744-0.940) and 0.737 (95% CI: 0.712-0.844), respectively. Moreover, the AUC and accuracy reached 0.769 (95% CI: 0.678-0.861) and 0.729 (95% CI: 0.628-0.821), respectively, in the external test set. In addition, the MRI-endoscopy fusion model outperformed the single-modal MRI model (AUC: 0.692 [95% CI: 0.609-0.783], accuracy: 0.659 [95% CI: 0.565-0.775]) and the single-modal endoscopy model (AUC: 0.720 [95% CI: 0.617-0.823], accuracy: 0.713 [95% CI: 0.612-0.809]) in the external test set.
CONCLUSION
The MRI-endoscopy fusion model based on ResNet-Vision Transformer achieved favorable performance in predicting treatment response to neoadjuvant chemoradiotherapy and holds tremendous potential for enabling personalized treatment regimens for locally advanced rectal cancer patients.

7.Overall strategy for development and application of core outcome set of traditional Chinese medicine.

The scientific and standardized evaluation of clinical efficacy of traditional Chinese medicine(TCM) is the core requirement for promoting the high-quality development of TCM. Building a recognized evaluation outcome system that conforms to the clinical efficacy characteristics of TCM is a key fundamental issue in the production and transformation of clinical evidence in TCM. In response to the heterogeneity of evaluation outcomes and core issues such as "western law in the middle", the research on the core outcome set of TCM(COS-TCM) has undergone more than ten years of exploration and practice. Its methodological system has continued to deepen under the coordinated development of theoretical basis, technical methods, platform support, and talent team, achieving an important leap from early introduction to standardized system construction and entering a new stage of systematic development. However, the overall research scale, quality, and the translation and application of research results in COS-TCM are still insufficient. In response to the opportunities and challenges of the new development stage, this article systematically reviews the development history and research status of COS-TCM, clarifies the basic principles of "international standards + TCM characteristics" and the key tasks of "selection, improvement, and creation", and proposes a three-step development path of "exploration and research, standard development, and regulatory transformation" to promote the standardization, systematization, and scientific development of related research. To ensure the effective implementation of research results, key promotion strategies such as upgrading research platforms, strengthening support systems, and optimizing collaborative mechanisms have been planned to drive COS-TCM to better serve clinical research, evidence translation, and new drug review.

8.Electrical stimulation based on triboelectric nanogenerator promotes osteogenesis of MC3T3-E1 cells on titanium surfaces.

This paper aims to explore the effect of electrical stimulation of triboelectric nanogenerators (TENGs) on the osteogenic and other biological behaviors of mouse embryonic osteoblast precursor cells (MC3T3-E1 cells) on titanium surfaces. First, an origami-type TENG was fabricated, and its electrical output performance was tested. The optimal current of the generator and the feasibility of the experiment were verified by the CCK-8 assay and scratch assay. At the optimal current, the osteogenic conditions of the cells in each group were determined by quantitative analysis of the total protein content, alkaline phosphatase (ALP) activity, and alizarin red staining (ARS) on the titanium surface. Finally, the adhesion and spreading of cells on the titanium surface after electrical stimulation were observed. The results showed that the TENG had good electrical output performance, with an open-circuit voltage of 65 V and a short-circuit current of 42 μA. Compared with the rest of the current, a current strength of 30 μA significantly improved cell proliferation and migration, osteogenesis, and adhesion and spreading capabilities. The above results confirm the safety and operability of TENG in biomedical applications, laying the foundation for future TENG applications in reducing the time of bone integration around titanium implants after surgery.

9.Can greenspace modify the combined effects of multiple air pollutants on pulmonary tuberculosis treatment outcomes? An empirical study conducted in Zhejiang Province, China.

BACKGROUND:
Evidence on the combined effects of air pollutants and greenspace exposure on pulmonary tuberculosis (PTB) treatment is limited, particularly in developing countries with high levels of air pollution.
OBJECTIVE:
We aimed to examine the individual and combined effects of long-term exposure to air pollutants on PTB treatment outcomes while also investigating the potential modifying effect of greenspace.
METHODS:
This population-based study included 82,784 PTB cases notified in Zhejiang Province, China, from 2015 to 2019. The 24-month average concentrations of particulate matter with an aerodynamic diameter ≤2.5 µm (PM2.5), ozone (O3), nitrogen dioxide (NO2), and sulfur dioxide (SO2) before PTB diagnosis were estimated using a dataset derived from satellite-based machine learning models and monitoring stations. Greenspace exposure was assessed using the annual China Land Cover Dataset. We conducted analyses using time-varying Cox proportional hazards models and cumulative risk indices.
RESULTS:
In individual effect models, each 10 µg/m3 increase in PM2.5, NO2, O3, and SO2 concentrations was associated with hazard ratios for PTB treatment success of 0.95 (95% confidence interval (CI): 0.93-0.97), 0.92 (95% CI: 0.91-0.94), 0.98 (95% CI: 0.97-0.99), and 1.52 (95% CI: 1.49-1.56), respectively. In combined effect models, long-term exposure to the combination of air pollutants was negatively associated with PTB treatment success, with a joint hazard ratio (JHR) of 0.79 (95% CI: 0.63-0.96). Among the pollutants examined, O3 contributed the most to the increased risks, followed by PM2.5 and NO2. Additionally, areas with moderate levels of greenspace showed a reduced risk (JHR = 0.81, 95% CI: 0.62-0.98) compared with the estimate from the third quantile model (JHR = 0.68, 95% CI: 0.52-0.83).
CONCLUSIONS
Combined air pollutants significantly impede successful PTB treatment outcomes, with O3 and PM2.5 accounting for nearly 75% of this detrimental effect. Moderate levels of greenspace can mitigate the adverse effects associated with combined air pollutants, leading to improved treatment success for patients with PTB.

Jansen-de Vries syndrome, also known as intellectual developmental disorder with gastrointestinal difficulties and high pain threshold, is a rare autosomal dominant disorder characterized by multisystem involvement. This article reports the case of a young child who presented with global developmental delay, gastrointestinal dysfunction, intellectual disability, and short stature. Distinct facial features included a broad forehead, low nasal bridge, thin upper lip, and widely spaced and misaligned teeth. Additional phenotypic findings involved small hands and feet, as well as digital anomalies.
