1.Standards for the Application of Hemodynamic Monitoring Technology in Critical Care

With the rapid advancement of hemodynamic indices and monitoring technologies, their classification methods and application processes have become increasingly complex. Currently, no unified standard hasbeen established, making it difficult to fully meet the clinical requirements for hemodynamic management. To assist in hemodynamic monitoring assessment and therapeutic decision-making in critically ill patients, the Critical Hemodynamic Therapy Collaborative Group, in conjunction with the Critical Ultrasound Study Group, has jointly developed the Standard for the Application of Hemodynamic Monitoring Techniques in Critical Care. The first part of this standard systematically categorizes hemodynamic indicators into flow indicators, pressure and its derivative indicators, and tissue perfusion indicators, while elaborating on the clinical application of each. The second part establishes a standardized clinical implementation pathway for hemodynamic monitoring. It proposes a tiered monitoring strategy-comprising basic, advanced, indication-specific, and special scenario monitoring-tailored to different clinical settings. It emphasizes the central role of critical care ultrasound across all levels of monitoring and establishes hemodynamic assessment standards for organs such as the brain, kidneys, and gastrointestinal tract. This standard aims to provide a unified framework for clinical practice, teaching, training, and research in critical care medicine, thereby promoting standardized development within the discipline.
2.Influence of Oatp4c1-P-gp transmembrane transport system in the kidneys of obese mice on the pharmacokinetics of digoxin

Aim To investigate the expression of or-ganic anion transporting polypeptide 4C1(Oatp4c1)-P-glycoprotein(P-gp)in the kidneys of obese mice in-duced by high-fat diet(HFD),and its impact on the pharmacokinetic changes of digoxin.Methods C57BL/6 mice were randomly divided into the Chow group and the HFD group.Body weight and blood glu-cose were recorded weekly.After successful model es-tablishment,digoxin was intraperitoneally injected,and blood was collected at different time points.Part of the blood samples was used for LC-MS/MS detection,and the other part was used for the detection of other bio-chemical indicators.After 16 weeks,the organs were removed and weighed.HE and immunohistochemical staining was used to observe the renal pathology and the expression of Villin,a marker of proximal tubules.Western blot and qPCR were combined to detect the expression of Villin,Oatp4c1 and P-gp.Results In the HFD group,body weight and blood glucose in-creased significantly.The blood concentration of digox-in rose,the area under the curve increased,and the half-life was prolonged.The proximal tubular epithelial cells shed,and the protein expression of Villin,Oatp4c1 and P-gp decreased significantly.Conclu-sions The down-regulation of Oatp4c1-P-gp expres-sion in the kidneys of HFD mice leads to an increase in the blood concentration of digoxin and a decrease in re-nal clearance.
3.The value of Th17/Treg imbalance in the evaluation of intravenous immunoglobulin resistance in children with Kawasaki disease and Kobayashi score ≤4

Objective:To investigate the value of T helper 17 cells(Th17)/regulatory T cells(Treg)imbalance in the evaluation of intravenous immunoglobulin(IVIG)resistance in children with Kawasaki disease and Kobayashi score≤4.Methods:A total of 78 children with Kawasaki disease and Kobayashi score ≤ 4 admitted to Hunan Children's Hospital from January 2020 to December 2023 were prospectively selected as the study subjects,all of whom received IVIG treatment.In the acute phase,the proportion of Th17 cells and Treg cells was detected.Children were divided into IVIG sensitive group and IVIG resistance group based on their responsiveness to IVIG treatment.Baseline data of children with different IVIG treatment responsiveness,acute Th17 cell inflammatory factors [interleukin(IL)-17,IL-21,tumor necrosis factor-α(TNF-α)],Treg cell inflammatory factors [IL-10,IL-35,transforming growth factor-β(TGF-β)] levels,and Th17/Treg values were compared.The correlation between Th17/Treg values and IVIG resistance in children with Kawasaki disease was analyzed using a restricted cubic spline model(RCS).According to the threshold of correlation between Th17/Treg values obtained from RCS analysis and drug resistance in children,Th17/Treg was grouped,with a focus on analyzing the predictive value and clinical benefits of Th17/Treg values for IVIG resistance in children with Kawasaki disease.Results:Among the 78 children with Kawasaki disease,16 were resistant to IVIG treatment,accounting for 20.51%.The levels of C-reactive protein(CRP),IL-17,and Th17/Treg in the acute phase of children in the IVIG resistance group were higher than those in the IVIG sensitive group,while the levels of IL-10 were lower than those in the IVIG sensitive group( P<0.05).RCS analysis showed that there was a non-linear dose-response relationship between IVIG resistance and acute Th17/Treg values in children with Kawasaki disease( P<0.05).When the acute Th17/Treg value was greater than 1.05,the risk of IVIG resistance in children with Kawasaki disease increased with the increase in indicator levels.The levels of CRP and IL-17 in the acute phase of children with Th17/Treg>1.05 were higher than those in the Th17/Treg < 1.05 group,while IL-10 levels were lower than those in the Th17/Treg<1.05 group.The proportion of children resistant to IVIG treatment was higher than that in the Th17/Treg<1.05 group( P<0.05).Multivariate Logistic regression analysis showed that CRP,IL-17,IL-10,and Th17/Treg were the influencing factors of IVIG resistance in children with Kawasaki disease( P<0.05).It was found through a nomogram that the C-index of the acute phase Th17/Treg values and their secretion of inflammatory factors in children with Kawasaki disease and Kobayashi score ≤ 4,as well as other major indicators,predicted the risk of IVIG resistance.The C-index was 0.975(95% CI 0.944-1.000),indicating good discrimination.When drawing the decision curve,it was found that compared to using each indicator separately,the Th17/Treg value and its secreted inflammatory factors in the acute phase assisted other major indicators in drawing the decision curve with a higher net benefit rate,with a maximum net benefit rate of 0.205. Conclusion:IVIG resistance in children with Kawasaki disease and Kobayashi score≤4 is related to Th17/Treg imbalance.When the Th17/Treg value in the acute phase of the disease is greater than 1.05,the risk of IVIG resistance is higher.The inflammatory factors IL-17 and IL-10 secreted by the two can assist other known indicators related to IVIG resistance in Kawasaki disease patients,improving the accuracy of predicting resistance risk.
4.Evaluation of pediatric adenoid hypertrophy based on CBCT and lateral cephalograms with multiple regression analysis

Objective:To investigate the correlations and regression models among adenoid-nasopharynx measurement indices (the linear ratio A/N, the two-dimensional area ratio 2D_A/N, and the three-dimensional volume ratio 3D_A/N), to evaluate the feasibility of predicting three-dimensional parameters from two-dimensional measurements.Methods:This cross-sectional study analysed 4 307 CBCT scans and lateral nasopharyngeal radiographs from 4-12-year-old children (July 2023-January 2025). Children with acute infection, major systemic disease, craniofacial anomalies, prior maxillofacial surgery, poor-quality images, or no consent were excluded. 3D_A/N was defined as adenoid volume/(adenoid+airway volume); 2D_A/N as adenoid area/total area; and linear A/N as adenoid thickness/nasopharyngeal thickness. Pearson correlation and multiple linear regression were used to quantify 2D-to-3D agreement, and independent-sample t-tests were employed to compare age, sex and seasonal subgroups. Results:(1) Significant sex-related differences in adenoid hypertrophy were detected at ages 5, 8, 9 and 11 years. In the 5-year-old group, females had significantly higher 3D_A/N and 2D_A/N values than males (3D_A/N: 0.76±0.10 vs. 0.74±0.10, t=-1.99, P=0.047; 2D_A/N: 0.66±0.10 vs. 0.64±0.09, t=-2.71, P=0.007). In the 8-year-old group, males exhibited significantly higher A/N and 3D_A/N values than females (A/N: 0.64±0.06 vs. 0.63±0.06, t=4.49, P<0.001; 3D_A/N: 0.66±0.12 vs. 0.64±0.12, t=2.00, P=0.046). In the 9-year-old group, males showed significantly higher 3D_A/N values than females (0.65±0.11 vs. 0.63±0.12, t=2.46, P=0.014). In the 11-year-old group, males had significantly higher A/N values than females (0.62±0.06 vs. 0.59±0.05, t=4.79, P<0.001). (2) The linear A/N ratio correlated moderately with 3D_A/N ( r=0.82), whereas, 2D_A/N showed an excellent correlation ( r=0.97); multiple regression confirmed 2D_A/N as the principal predictor of 3D_A/N. (3) Seasonal analysis revealed markedly greater adenoid hypertrophy in winter than in summer for children aged 4-9 years, while, no seasonal difference was found in the 10-12 year group; overall, winter hypertrophy was most pronounced in younger children. Conclusion:2D_A/N demonstrates a marked advantage in predicting 3D_A/N. When combined with A/N and age factors, it can effectively model changes in the 3D volume ratio of the adenoid, underscoring the clinical value of lateral cephalometric measurements for evaluating adenoid hypertrophy in children. Moreover, adenoid hypertrophy is influenced by age, sex and seasonal factors.
5.Correlation between SⅡ and early neurological deterioration in patients with branch atheromatous disease

Objective To explore the relationship between systemic immune-inflammation index(SⅡ)at admission and occurrence of early neurological deterioration(END)in patients with branch atheromatous disease(BAD).Methods A retrospective analysis was performed on 326 BAD patients admitted in Department of Neurology of Handan Central Hospital from October 2021 to February 2024.Based on occurrence of END or not,they were divided into END group(97 cases)and non-END group(229 cases).Clinical data of the patients were collected,and multivari-ate logistic regression analysis was used to identify the END risk variables in BAD patients.ROC curve was plotted to evaluate the value of NIHSS score,hs-CRP and SⅡ in predicting the inci-dence of END in the patients.Results Significantly advanced age,higher NIHSS score at admis-sion,and elevated hs-CRP level,neutrophil count and SⅡ,but lower platelet and lymphocyte counts were observed in the END group than the non-END group(P<0.05,P<0.01).Multi vari-ate logistic regression analysis indicated that NIHSS score at admission(OR=1.134,95%CI:1.050-1.226,P=0.001),hs-CRP(OR=1.131,95%CI:1.024-1.249,P=0.015),and SⅡ(OR=1.001,95%CI:1.001-1.002,P=0.003)were independent risk factors for END in BAD patients.The AUC value of SⅡ in the prediction of END was 0.660,which was significantly higher than that of NIHSS score and hs-CRP in BAD patients(P<0.05).Conclusion SⅡ is an independent risk factor for END in BAD patients,and SⅡ at admission has a certain predictive value for the oc-currence of END in these patients.

8.Exploration on the relationship between the cross-sectional area of paravertebral muscles and spino-pelvic sagittal imbalance in patients with lumbar spondylolisthesis based on MRI-T2WI

Objective To investigate the relationship between the cross-sectional area of paravertebral muscles and spino-pelvic sagittal imbalance in patients with lumbar spondylolisthesis based on MRI-T2WI.Methods A retrospective analysis was conducted on the imaging data of 38 patients with lumbar spondylolisthesis(spondylolisthesis group)and the medical records of 38 patients with lumbar degeneration but without L4-L5 segments lumbar spondylolisthesis(non-spondylolisthesis group).The Annotate software was used to measure the cross-sectional areas of psoas major and erector spinae at the L4-L5 segments on the MRI-T2WI sequences in the included cases.Spino-pelvic sagittal parameters[pelvic incidence(PI),pelvic tilt(PT),sacral slope(SS),lumbar lordosis(LL)]were measured based on the lateral lumbar X-ray images.The cross-sectional areas of psoas major and erector spinae and changes in spino-pelvic sagittal parameters were compared between the two groups.According to the degree of lumbar spondylolisthesis,patients in spondylolisthesis group were divided into different subgroups(group Ⅰ and group Ⅱ).The cross-sectional areas of psoas major and erector spinae,and changes in spino-pelvic sagittal parameters were compared among patients in the subgroups.The correlation between the cross-sectional areas of psoas major and erector spinae and changes in spino-pelvic sagittal parameters were analyzed by Pearson correlation analysis.Results The cross-sectional areas of psoas major and erector spinae at the L4-L5 segments were lower in the spondylolisthesis group than in the non-spondylolisthesis group(P<0.001).In the non-spondylolisthesis group,structures of lumbar spines and its surrounding tissues were clear,the segments were continuous,there was no lumbar spondylolisthesis but there was symmetrical distribution of muscles in bilateral spines.In the spondylolisthesis group,there was spondylolisthesis and dislocation at the L4-L5 segments,a small step-like change at the posterior vertebral margin and asymmetrical distribution of muscles in bilateral spines.The PI,PT,SS and LL were higher in the spondylolisthesis group than in the non-spondylolisthesis group(P<0.001).The cross-sectional areas of psoas major and erector spinae were lower in the group Ⅱ lumbar spondylolisthesis than in the group Ⅰ(P<0.05),and the PI,PT,SS and LL were higher than in the group Ⅰ(P<0.05).The cross-sectional areas of psoas major and erector spinae in patients with lumbar spondylolisthesis were negatively correlated with PI,PT,SS and LL(P<0.05).Conclusion MRI-T2WI can be used to observe changes in the cross-sectional area of paravertebral muscles in patients with lumbar spondylolisthesis,and these changes are correlated with spino-pelvic sagittal imbalance.
9.Analysis of lipid metabolism gene mutations and pathogenicity in patients with hypertriglyceridemia-associated acute pancreatitis

Objective:To investigate lipid metabolism gene mutations and pathogenicity of hypertriglyceridemia acute pancreatitis (HTG-AP) patients.Methods:Clinical data of 495 HTG-AP patients admitted from June 2018 to June 2020 in the center for severe acute pancreatitis of Eastern Theater General Hospital were retrospectively analyzed. Whole-exome sequencing and mutation verification were performed by next-generation sequencing technology and Sanger sequencing. The pathogenicity of gene mutation was analyzed by population mutation ratio, pathogenicity prediction software, conservation scoring software, protein structure prediction, and in vitro experiments. Results:The mutation ratio of lipid metabolism-related genes, namely LPL, APOA5, LMF1, GPIHBP1, and APOC2, were 14.81%, 55.78%, 43.61%, 1.62%, and 0.61%, respectively. Among them, 44 heterozygous mutations in LPL gene were detected including 36 missense mutations, 5 nonsense mutations and 3 frameshift mutations, which were all rarely carried in single patient. Six HTG-AP patients carried the LPL gene heterozygous mutation c.835C>G (p.Leu279Val). The mean level of serum triglyceride at the onset of HTG-AP was 27.4 mmol/L. All of them had a history of recurrent HTG-AP, and most of them had severe acute pancreatitis. The serum LPL concentration and activity were lower than the normal level. The pathogenicity analysis results suggested that the LPL p.Leu279Val was a rare, highly possible pathogenic and highly conserved gene mutation. The in vitro results showed that the LPL p.Leu279Val could significantly reduce the synthesis and secretion ability of LPL as well as its enzymatic activity. Conclusions:The mutation ratio of lipid metabolism-related genes, including LPL, APOA5, LMF1, GPIHBP1, and APOC2, are relatively high in the HTG-AP patients. The LPL p.Leu279Val is a rare and highly possible pathogenic gene mutation, which may lead to recurrent episodes of HTG-AP.
10.The predictive value of 18F-PSMA-1007 PET/CT for positive surgical margins after radical prostatectomy

Objective The application of 18F-PSMA-1007 PET/CT in the precise diagnosis and treatment of prostate cancer(PCa)is gradually increasing.This study aimed to explore the predictive value of 18F-PSMA-1007 PET/CT for positive surgical margins after radical prostatectomy(RP).Methods A total of 173 PCa patients who were pathologically confirmed and underwent RP in our hospital from July 2020 to August 2023 were retrospectively recruited.All patients underwent 18F-PSMA-1007 PET/CT whole-body imaging before surgery.According to the postoperative pathological results,they were divided into a negative surgical margin group with 83 cases and a positive surgical margin group with 90 cases.The differences in age,SUVmax,SUVmean,preoperative total prostate specific antigen(tPSA),prostate specific antigen density(PSAD),prostate volume,Gleason score,and clinical T staging between the groups were compared,and the risk factors were further analyzed by Logistic regression.Results The preoperative tPSA(Z=-3.252,P=0.001),SUVmax(Z=2.531,P=0.011),and clinical T staging(P=0.018)were all lower in the negative surgical margin group than in the positive surgical margin group.PSAD(OR=3.492,95%CI:1.095-11.133,P=0.035),SUVmax(OR=1.036,95%CI:1.003-1.070,P=0.034),and clinical T staging(OR=3.364,95%CI:1.117-10.133,P=0.045)were independent risk factors for positive surgical margins.Conclusion 18F-PSMA-1007 PET/CT has certain predictive value for positive surgical margins after RP in PCa patients.High PSAD,high SUVmax and high clinical T stage were independent risk factors for predicting positive margin after PR surgery,thus providing some reference for the clinical treatment strategy of PCa patients.
