Lipid storage myopathy (LSM) is a lipid metabolic disorder characterized by excessive lipid droplet accumulation in muscle fibers. Classic multiple Acyl-CoA dehydrogenase deficiency (MADD), also known as glutaric aciduria type Ⅱ, is a disease with various clinical manifestations caused by mutations in electron transfer flavoprotein (ETF) and ETF-ubiquinone oxidoreductase. In recent years, a large amount of evidence has shown that classic late-onset MADD caused by mutations in the electron transfer flavoprotein dehydrogenase gene is the main cause of LSM. Besides classic MADD, many other diseases with similar changes in blood acylcarnitines and urinary organic acids can also cause LSM, and such diseases are call MADD-like disorders or MADD spectrum. This article reviews the clinical, pathological, biochemical, and molecular features of LSM with various etiologies and the latest advances in treatment, with a focus on the latest findings associated with MADD spectrum.

6.Neuroimaging Mechanism of The Modified Electro-convulsive Therapy on The Anti-depressive Effects and Cognitive Impairment

Modified electro-convulsive therapy (MECT) is one of the most potent treatments for major depressive disorder (MDD). However, it remains a second-line option due to significant side effects, such as transient memory loss. The relationship between therapeutic efficacy and cognitive impairment warrants further investigation to develop improved treatment regimens. In this review, we examine recent evidence from magnetic resonance imaging (MRI) studies aiming to identify structural and functional brain changes specifically associated with both the antidepressant effects and the amnesic outcomes of MECT. MECT induces widespread alterations across multiple brain systems. Increases in gray matter volume (GMV) have been observed in the prefrontal, temporal, and parietal cortices, as well as in subcortical regions such as the hippocampus (HP), amygdala, and striatum. Strengthening of myelination has also been reported along the dorsolateral prefrontal-limbic pathways. Functional changes include increased spontaneous neural activity in prefrontal areas, reorganization of intrinsic connectivity within the default mode network (DMN), and altered functional connectivity (FC) among the DMN, salience network (SN), and central executive network (CEN). Correlational studies have identified structural and functional alterations linked to antidepressant efficacy, including right hippocampal volume enlargement, prefrontal cortical thickening, reduced iron deposition in the striatum, decreased FC within certain DMN nodes, and enhanced effective connectivity from the dorsolateral prefrontal cortex (DLPFC) to the right angular gyrus. In contrast, the amnesic effects have been associated with increased volumes in the left hippocampus and bilateral dentate gyrus; enhanced FC in the left angular gyrus and left posterior cingulate cortex (PCC); increased FC between the right ventral anterior insula and DLPFC; and reduced FC in the left thalamus and bilateral precuneus. Changes in the hippocampus appear to correlate with both antidepressant efficacy and memory impairment. Clinical studies have found no significant correlation between the severity of memory impairment and the reduction in depressive symptoms, suggesting that the therapeutic and adverse effects may arise from distinct regional or subregional mechanisms. Supporting this hypothesis, recent findings show that increased right hippocampal volume is significantly associated with reduced depression scores, whereas increased volume in the left dentate gyrus correlates with declines in delayed recall performance. Additionally, enhanced connectivity between the anterior hippocampus and middle occipital gyrus (MOG) has been linked to mood improvement, while decreased FC between the mid-hippocampus and angular gyrus has been associated with impairments in memory integration. In conclusion, current evidence suggests that the antidepressant and memory-impairing effects of MECT may localize to distinct hippocampal subregions. These effects likely result from differential modulation of local neural activity and functional connectivity, leading to divergent behavioral outcomes. Given that both effects may originate in deep and spatially constrained structures such as the hippocampus, small-sample studies and conventional methodologies may fail to differentiate them effectively. Future research should employ large-scale, longitudinal designs utilizing high-field MRI and multimodal neuroimaging to characterize MECT-induced structure-function coupling in the hippocampus and its integration at the network level. Additionally, multiscale analyses spanning molecular, circuit, and network dimensions would be beneficial.
7.Alvianolic acid B exerts a protective effect on random skin flaps through autophagy mediated by activation of TFE3

Aim To observe the role of salvianolic acid B(Sal B)in enhancing the survival of random skin flaps and to preliminarily explore its potential mecha-nisms.Methods The appearance,degree of edema,color and hair condition of the skin flap were evaluated seven days after operation.The vascular network and blood flow of random flaps were measured by laser Doppler flow measurement.HE staining was used to detect the growth of microvessels in random flaps.The expressions of VEGF and CD34 were detected by im-munohistochemistry,the expressions of RIPK1,2 and LC3 Ⅱ were detected by immunofluorescence,and the effects of autophagy related proteins and signaling path-ways were detected by Western blot.Results The ex-perimental results showed that Sal B induced autophagy in the random skin flaps,promoted angiogenesis,and reduced oxidative stress and necrotic apoptosis,signifi-cantly increasing the survival rate of the flaps.Immu-nohistochemistry,immunofluorescence staining,and Western blot confirmed that Sal B induced autophagy in the random skin flaps by activating TFE3 protein.Conclusion Sal B can promote autophagy in cells of random skin flaps and reduce their necrotic apoptosis by activating TFE3 protein.
8.Clinical effect of allogeneic peroneal bone marrow support combined with plate fixation for the treatment of Neer type Ⅳ proximal humeral fractures

Objective To explore clinical effect of allogeneic peroneal bone marrow support combined with plate internal fixation in treating Neer type Ⅳ proximal humeral fractures.Methods From December 2017 to December 2020,12 patients with Neer type Ⅳ proximal humeral fractures were treated with allogeneic peroneal bone marrow support combined with plate internal fixation,including 7 males and 5 females,aged from 56 to 78 years old;the time from injury to operation ranged from 1 to7 days.Operative time,fracture healing time and complications during follow-up were observed,and clinical efficacy was e-valuated by Constant-Murley score at the latest follow-up.Results All patients were obtained follow up for 20 to 29 months.All patients got bone healing and incisicons were healed at stage Ⅰ,operative time ranged from 95 to 138 min,blood loss ranged from 210 to 275 ml,fracture healing time ranged from 14 to 18 weeks.Two patients occurred postoperative shoulder stiffness and recovered after 2 weeks of passive exercise.There were no complications such as infection,poor wound healing,and fail-ure(fracture and loosening)of internal fixators occurred.Constant-Murley shoulder function score ranged from 69 to 89 at the latest follow up,2 patients got excellent results,9 good and 1 fair.Conclusion The application of allogeneic fibular bone mar-row placement could provide effective support for medial humerus,which is conducive to assisting reduction of fracture end,re-ducing occurrence of internal fixation failure caused by collapse of humerus head and screw perforation,and significantly im-proving function of shoulder joint.
9.Predictive value of serum sFlt-1 and LTB4 for cerebral vasospasm after interventional embolization of intracranial aneurysms

Objective To explore the predictive value of soluble fms-like tyrosine kinase-1(sFlt-1)and leukotriene B4(LTB4)in patients with intracranial aneurysms for cerebral vasospasm(CVS)after interventional embolization.Methods A total of 98 patients with intracranial aneurysms admitted to our hospital from January 2019 to September 2023 were regarded as the observation group,and were divided into the CVS group(32 cases)and the non CVS group(66 cases)according to whether CVS occurred or not within 3 to 5 days after surgery;102 healthy examinees in our hospital were selected as the control group.Enzyme-linked immunosorbent assay was used to detect serum levels of sFlt-1 and LTB4;the influencing factors for CVS after interventional embolization of intracranial aneurysms were analyzed by Logistic regression analysis;the predictive value of serum sFlt-1 and LTB4 levels for the occurrence of CVS after interventional embolization of intracranial aneurysms was analyzed by receiver operating characteristic(ROC)curve.Results The serum levels of sFlt-1 and LTB4 of patients in the observation group were obviously higher than those in the control group,and the differences were statistically significant(P<0.05).The serum levels of sFlt-1 and LTB4,and the proportions of patients with postoperative blood pressure fluctuation range≥30 mmHg and Hunt-Hess grade Ⅲ in the CVS group were obviously higher than those in the non CVS group,and the differences were statistically significant(P<0.05).SFlt-1(OR:2.985;95%CI:1.684 to 5.291)and LTB4(OR:2.868;95%CI:1.581 to 5.204)were the independent risk factors for CVS after interventional embolization of intracranial aneurysms(P<0.05).The area under the curve(AUC)of sFlt-1 and LTB4 alone and in combination for predicting the occurrence of CVS after interventional embolization of intracranial aneurysms were 0.839,0.825,and 0.915,respectively,with sensitivity of 84.44%,87.59%,and 81.36%,and specificity of 74.26%,75.87%,and 90.98%,respectively.The AUC of the combination of the two was higher than those of sFlt-1 and LTB4 alone,and the differences were statistically significant(Z=2.150,2.546,P<0.05).Conclusion The serum levels of sFlt-1 and LTB4 in patients with CVS after interventional embolization of intracranial aneurysms are increased,and the combination of the two can serve as the important indicators for predicting CVS.

The call of the times to"promote education informatization towards a new journey"has posed new challenges to edu-cation,and the education industry should be committed to modernizing education through education informatization.In recent years,online teaching has become the norm.In this context,our teaching team introduced short video teaching into teaching innovation to spread Animal Immunology knowledge.Animation and ideological and political elements were integrated into each video,making knowledge points from"abstract"to"concrete",which stimulated students'interest in learning and cultivate the quality of rigorous scholarship and moral cultivation.The short video comment area has become a medium for interaction between teachers and students.The real-time feedback of the data platform provides a reference for the optimization and design of teaching content.The short video teaching practice effectively solves the"pain points"in traditional teaching and makes the obscure animal immunology knowledge more understandable.Students have increased their interest in learning,using fragmented time to actively acquire knowledge,achieving the effect of improving the teaching quality of animal immunology through online teaching resources.
