1.Exploration on Medication Law of Professor Zeng Binfang in the Treatment of Hepatitis B Virus-Related Cirrhosis in Compensatory Stage Based on Data Mining

Objective To explore the medication law of Professor Zeng Binfang for the treatment of hepatitis B virus-related cirrhosis in compensatory stage through data mining technology.Methods Medical case data of patients with hepatitis B virus-related cirrhosis in compensatory stage treated by Professor Zeng Binfang in outpatient clinics and hospital wards from October 2018 to November 2023 were collected.Patients'basic information and diagnoses were systematically input into Excel 2021 to create a standardized medical case database.Ancient and Modern Medical Case Cloud Platform 2.3.9 was used to perform statistical analysis on drug frequency,properties,tastes and meridians,efficacy classification,and association rules,hierarchical clustering,and complex networks were conducted.Results Totally 555 prescriptions were included,involving 303 kinds of Chinese materia medica,and a total frequency of 8 276 times.38 kinds of high-frequency Chinese materia medica were identified.The main properties were neutral,warm and cold,the main tastes were sweet,bitter and pungent,the main meridians were spleen meridians,liver meridians,and the main efficacy was tonics,urine excretion to strain off dampness,heat-clearing drugs,and promoting blood circulation and reducing blood stasis drugs.Association rules and hierarchical clustering resulted in 15 drug pairs and 5 clustered formulas,respectively.The core TCM combinations constructed by complex network was Angelicae Sinensis Radix,vinegar Trionycis Carapax,Glycyrrhizae Radix et Rhizoma Praeparata cum Melle,Bupleuri Radix,Poria,Polyporus,Chuanxiaong Rhizoma,Scutellariae Radix,and Atractylodis Macrocephalae Rhizoma.Conclusion Professor Zeng Binfang advocates for a treatment protocol that"liver spleen coordination"and"Yi Gui homology",and applies the combination of"elimination"and"cleaning"methods to the"supplement"method during the stage of excessive pathogenic factors energy and deficiency of healthy qi in patients with hepatitis B virus-related cirrhosis in compensatory stage according to underlying pathogenic characteristics of damp-heat toxin retention with the blood syndrome and chronic deficiency with stasis.This combination of attacking and supplementing,as well as regulating cold and heat,can provide a certain reference for clinical treatment.
2.Analysis of the prevalence status and clinical characteristics of the hepatitis D virus in the Xinjiang region

Objective:To investigate the prevalence status and the clinical characteristics of hepatitis D virus (HDV) among patients chronically infected with hepatitis B virus (HBV) in the Xinjiang region.Methods:A cross-sectional study was conducted. Serum samples from 1 830 patients with chronic HBV infection who visited the Traditional Chinese Medicine Hospital of Xinjiang Uygur Autonomous Region from December 2022 to October 2023 were collected. All sera were tested for anti-HDV IgG and IgM. Sera positive for anti-HDV IgG or IgM were selected for HDV RNA detection. HDV RNA-positive sera were sequenced to determine the HDV genotype. Age, gender, HBV course, and anti-HBV treatment status were used as scoring items based on the propensity score matching (PSM) method. Chronic HBV patients with negative anti-HDV were matched in a ratio of 1∶1. The clinical characteristics of anti-HDV -positive-patients were analyzed. The t-test was used for comparison between groups of normally distributed continuous data. The Wilcoxon signed-rank test was used for comparison between groups of skewness distribution. The χ2 test was used for comparison between groups of enumeration data. Results:The positive detection rates of anti-HDV IgG, anti-HDV IgM, and HDV RNA in 1 830 cases with chronic HBV infection were 2.24% (41/1 830), 1.09% (20/1 830), and 1.69% (31/1 830), respectively. All HDV RNA-positive patients had HDV genotype 1. Two anti-HDV-positive patients had negative hepatitis B surface antigen (HBsAg). Gender, age, HBV course, and anti-HBV treatment status had no significant difference. The quantification of HBsAg, liver biochemical indexes (alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, and total bile acids), the proportion of patients with liver cirrhosis, and alpha-fetoprotein were significantly higher in the anti-HDV-positive group than in those in the anti-HDV-negative group ( P<0.05). Conclusion:The prevalence rate of HDV in chronic HBV-infected patients at a single center in the Xinjiang region was 2.24%, with the primary genotype being 1. Furthermore, overlap infection should be paid attention to because it might aggravate liver damage.

5.Expression of Notch1, Notch3 and Hes1 in gastrointestinal stromal tumors and their clinical significance

Objective To investigate the expression of Notch1,Notch3 and Hes1 in gastrointestinal stromal tumors(GIST) and their clinical significance.Methods Quantitative real-time polymerase chain-reaction(Q-PCR) and Western blot were applied to detect the mRNA and the expression of Notch1,Notch3 and Hes1 in 135 matched GIST specimens and adjacent tissues.Meanwhile,the expression of Notch1,Notch3 and Hes1 was detected by immunohistochemistry,and the relationship between their expression and clinicopathological factors in GIST patients was analyzed.In addition,a total of 40 wild type mice(WT) and Notch1 knockout mice(KO) was divided into WT group,KO group,WT+ GIST group and KO+GIST group,and the expression of Notch1,Notch3 and Hes1 in each group was detected.Results Compared with adjacent tissues,the mRNA andthe expression of Notch1,Notch3 and Hes1 were up-regulated in GIST tissues(P<0.05).The positive rates of Notch1,Notch3 and Hes1 in the GIST specimens (59.26 %,65.19 % and 62.22 %) were higher than those in the adjacent tissues(17.780%,22.22 % and 17.78 %),and the difference was statistically significant(P<0.05).Statistical analysis showed that the expression of Notch1 was significantly correlated with the NIH grade of GIST(x2 =8.532,P=0.002);the expression of Notch3 was significantly related with tumor metastasis of GIST (x2 =7.532,P=0.003);the expression of Hes1 was significantly associated with the tumor size of GIST(x2 =6.781,P=0.012).The expression of Notch1,Notch3 and Hes1 was higher in WT+GIST group compared to the expression found in WT group(all P<0.05).There were no significant differences in the expression of Notch1,Notch3 and Hes1 between WT+ GIST group and KO+GIST group.The expression of Notch1,Notch3 and Hes1 was lower in KO+ GIST group compared to the expression found in WT+GIST group(all P<0.05).Conclusion The expression of Notch1,Notch3 and Hes1 related to Notch signaling pathway is elevated in GIST tissues,and the activation of Notch signaling pathway may play an important role in the occurrence and progression of GIST.
6.The application value in diagnosis of chronic atrophic gastritis using the magnifying endoscopy and narrow-band imaging

Objective To explore the application value in diagnosis of chronic atrophic gastritis using the magnifying endoscopy and narrow-band imaging.Methods A total of 98 cases of chronic atrophic gastritis patients was selected,and carried out the ordinary white light endoscopy,magnifying endoscopy and narrow-band imaging(ME-NBI) and pathological examination in file,followed by comparison of the results.Results The sensitivity and specificity of chronic atrophic gastritis by magnifying endoscopy and narrowband imaging mode were 74.07% and 82.35%,respectively;which were significantly higher than that obtained by conventional endoscopy,which were 48.15% and 64.71% (P <0.05).In magnifying endoscopy and narrow-band imaging endoscopy group,60 patients were positive,which intestinal metaplasia accounted for 35%,while in conventional endoscopy group,39 patients were positive which was only 17.95% (P < 0.05).Conclusions Magnifying endoscopy and narrow-band imaging can improve the diagnostic accuracy of chronic atrophic gastritis.
7.Application of reversed perforator-plus sural neurofasciocutaneous flap in lower leg

Objective To report the operative technique and clinical results of reversed perforator-plus sural neurofasciocutaneous flap in lower leg. Methods Perforator-plus sural neurofasciocutaneous flaps, which designed along the axis of the sural nerve and based on the dual-pedicle of fasciocutaneous and per-oneal perforator, were used to reconstruct soft-tissue defect in lower extremities. Results Twenfy-four flaps, ranged from 25 cm×12 cm - 8 cm ×7 cm in size, survived completely without venous congestion and distal ischemia and necrosis. The color and texture of the flaps were good. The appearance and functional re-suits were satisfactory with following up for 6 to 12 months. Conclusion The modified technique in pediclehave minimized the complication, enlarged the size of the flap and improved the survive rates. It is a goodmethod in repairing large soft-tissue defects of extremities.
8.Clinical experiences of the reversed superficial peroneal neurocutaneous island flap based on the descending branch of the distal perforating branch of the pcroneal artery

Objective To report the operative technique and clinical experiences of the modified reversed superficial peroneal neurocutaneous island flaps for reconstruction of the ankle and foot.Methods According to the previous anatomical studies and our clinical experiences,we devised the reversed superficial peroneal neuroeutaneous island flap based on the descending branch of the distal perforator of the peronealrtery and its venae comitantes,and covered the soft defect of the ankle and foot with it.Results Twenty-one of the 23 flaps survived completely without complications,while the other two occurred marginal necrosis.The maximum surface of the flap in our series was 12 cm×13 cm.and the minimum one was 5 cm×4 cm.The length of the pedicle ranged from 5 cm to 10 cm.The texture of the flaps was good,while the cosmetic and function of them were evaluated as acceptable in all cased after 6 to 21 months follow-up.Conclusion The reversod superficial peroneal neurocutaneous island flaps is a versatile,reliable procedure useful in reconstruction of the ankle and foot.
9.Clinical features and risk factors of nonalcoholic fatty liver disease in Xinjiang area

Objective To investigate the clinical features and related risk factors of nonalcoholic fatty liver disease(NAFLD)in Xinjiang area for early diagnosis and prevention.Methods Retrospective case-control study was done to explore the clinical features and related risk factors of NAFLD with Logistic regression analysis.Results Levels of serum ALT increased significantly in the patients with NAFLD and were often more than 2-fold of the normal limit;serum AST was only increased mildly.Obesity,type 2 diabetes mellitus,heart and brain blood vessel disease and hyperlipidemia were syndromes of NAFLD.BMI、DBP、high lipid food、serum TG、serum ApoB were negatively correlated with the occurrence of NAFLD,while serum HDL was positively correlated with the occurance of NAFLD.Conclusion BMI、DBP、high lipid food、serum TG and serum ApoB may be independent risk factors of NAFLD.To prevent NAFLD and its syndromes,periodical physical examination,changing bad life styles and controlling obesity are most important.
