BACKGROUND:
The mechanisms distinguishing metabolically healthy from unhealthy phenotypes within the same BMI categories remain unclear. This study aimed to investigate the associations between regional fat distribution and metabolically unhealthy phenotypes in Chinese adults across different BMI categories.
METHODS:
This cross-sectional study involving 11833 Chinese adults aged 20 years and older. Covariance analysis, adjusted for age, compared the percentage of regional fat (trunk, leg, or arm fat divided by whole-body fat) between metabolically healthy and unhealthy participants. Trends in regional fat percentage with the number of metabolic abnormalities were assessed by the Jonckheere-Terpstra test. Odds ratios (ORs) and their 95% confidence intervals (CIs) were estimated by logistic regression models. All analyses were performed separately by sex.
RESULTS:
In non-obese individuals, metabolically unhealthy participants exhibited higher percent trunk fat and lower percent leg fat compared to healthy participants. Additionally, percent trunk fat increased and percent leg fat decreased with the number of metabolic abnormalities. After adjustment for demographic and lifestyle factors, as well as BMI, higher percent trunk fat was associated with increased odds of being metabolically unhealthy [highest vs. lowest quartile: ORs (95%CI) of 1.64 (1.35, 2.00) for men and 2.00 (1.63, 2.46) for women]. Conversely, compared with the lowest quartile, the ORs (95%CI) of metabolically unhealthy phenotype in the highest quartile for percent arm and leg fat were 0.64 (0.53, 0.78) and 0.60 (0.49, 0.74) for men, and 0.72 (0.56, 0.93) and 0.46 (0.36, 0.59) for women, respectively. Significant interactions between BMI and percentage of trunk and leg fat were observed in both sexes, with stronger associations found in individuals with normal weight and overweight.
CONCLUSIONS
Trunk fat is associated with a higher risk of metabolically unhealthy phenotype, while leg and arm fat are protective factors. Regional fat distribution assessments are crucial for identifying metabolically unhealthy phenotypes, particularly in non-obese individuals.

Journal of China Pharmaceutical University 2025;56(6):689-699
The development of antiviral drugs targeting SARS-CoV-2 remains a major strategy for the prevention and treatment of COVID-19 infection. Current anti-SARS-CoV-2 agents mainly act on key viral protein targets involved in the viral life cycle, such as the main protease (Mpro), papain-like protease (PLpro), and RNA-dependent RNA polymerase (RdRp). However, these drugs are insufficient to address the challenges posed by the rapid mutation rate and drug resistance of SARS-CoV-2. In recent years, viral RNA has demonstrated to be a highly promising therapeutic target. Small-molecule drugs that target viral RNA possess distinct advantages, featuring novel mechanisms of action and the potential for broad-spectrum antiviral activity as well as the ability to overcome drug resistance. Such agents provide a new avenue for combating coronavirus threats. This review systematically summarizes recent advances in the development of small-molecule inhibitors targeting conserved SARS-CoV-2 RNA structures, including the frameshift stimulation element (FSE), stem-loops (SL), G-quadruplexes (G-4), and RNA-targeting chimeras (RNATAC), aiming to provide some insight to guide future.
3.Undifferentiated liver cancer diagnosed by endoscopic ultrasound-guided fine needle aspiration:a case report and literature review
Academic Journal of Naval Medical University 2025;46(10):1371-1376
Objective To explore the histological and immunohistochemical characteristics of undifferentiated liver cancer in small tissue biopsy specimens. Methods Endoscopic ultrasound-guided fine needle aspiration(EUS-FNA)was used to obtain liver tissue strips and cell samples. The tissue and cell morphology of a case of liver undifferentiated carcinoma were analyzed by hematoxylin-eosin staining, immunohistochemical staining, and cell smear techniques. The related literatures of undifferentiated liver cancer were retrieved from PubMed and Wanfang databases to summarize the diagnostic criteria of undifferentiated liver cancer. Results Histologically, this case of undifferentiated liver cancer presented as a patchy arrangement of oval or spindle shaped tumor cells, with large, deeply stained, and distinctly atypical anaplastic cells visible in between.Localized nuclear fission images could be observed. The cytological manifestation was nuclear atypical cells, with less cytoplasm in atypical cells, and some cells had red staining of cytoplasm resembling squamous cell carcinoma cytoplasm. The immunohistochemical results showed epithelial membrane antigen(EMA)(+),cytokeratin(CK)18(+),glutamine synthetase(GS)(-),carbohydrate antigen 19-9(CA19-9)(-),vimentin(-),CK7(-),CK20(-),alpha fetoprotein(AFP)(-),glypican 3(Gpc-3)(-),hepatocellular paraffin antigen-1(Hep-1)(-),and chromogranin A(CgA)(-).The results of literature review showed that undifferentiated liver cancer was more common in middle-aged and elderly men,and generally appeared as isolated circular nodules. In 8 out of 9 cases, tumor cells were observed to grow diffusely under the microscope, and no typical glandular or beam like growth patterns of liver tumors were observed. The malignant degree of tumor cells was high, with 5 out of 9 cases showing significantly atypical interstitial cells.Immunohistochemical features included positive expression of epithelial derived markers(such as pan cytokeratin[CKpan],EMA,and CK18)and negative expression of liver derived markers(AFP,Hep-1,Gpc-3,and arginase-1),which might be accompanied by positive expression of neuroendocrine markers. Conclusion The histological morphology of undifferentiated liver cancer is characterized by diffuse distribution of anaplastic cells, without any glandular or cord like arrangement. The tumor often expresses epithelial derived markers, while liver derived markers show negative expression.

ObjectiveTo investigate the differences in abnormal liver biochemical parameters in pregnant patients during the epidemic or non-epidemic period of coronavirus disease 2019 (COVID-19). MethodsA retrospective analysis was performed for 539 pregnant women who were discharged from Department of Obstetrics, Peking University First Hospital, from October 2017 to March 2022 and had at least one abnormal liver biochemical parameter among alanine aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyl transpeptidase (GGT), total bilirubin (TBil), and total bile acid. The patients in the epidemic period of COVID-19 and those in the non-epidemic period of COVID-19 were compared in terms of etiology, coagulation parameters, aminotransferases, bile acid, and renal function. The independent samples t-test was used for comparison of continuous data between two groups, and the chi-square test was used for comparison of categorical data between two groups. ResultsAmong the patients discharged from Department of Obstetrics during the non-epidemic period of COVID-19, 262 had abnormal liver biochemical parameters, accounting for 1.46%, while 277 patients had abnormal liver biochemical parameters during the epidemic period of COVID-19, accounting for 1.73% among the patients discharged from Department of Obstetrics during the same period of time, and there was a significant difference between these two groups (χ2=3.947, P=0.047). The etiological analysis of the patients with abnormal liver biochemical parameters during the two periods showed that there was no difference in the proportion of patients with four pregnancy-specific liver diseases (hyperemesis gravidarum, preeclampsia and eclampsia, intrahepatic cholestasis of pregnancy, and acute fatty liver of pregnancy). As for the patients with abnormal liver biochemical parameters in pregnancy, there was no significant difference in the proportion of patients with normal creatinine and stimated glomerular filtration rate (eGFR) between the epidemic period and the non-epidemic period (86.78% vs 87.90%, χ2=0.141, P=0.708). The patients with ALT≥5×upper limit of normal accounted for 7.94% in the epidemic period of COVID-19 and 9.54% in the non-epidemic period (χ2=0.433, P=0.511), and the patients with severe cholestasis accounted for 7.75% in the epidemic period of COVID-19 and 9.27% in the non-epidemic period (χ2=0.392, P=0.531). The proportion of patients with obstetric bleeding during the epidemic period of COVID-19 was significantly lower than that during the non-epidemic period (14.61% vs 24.19%, χ2=489.334, P<0.001). ConclusionThere is no difference in the proportion of patients with pregnancy-specific liver diseases among the patients with abnormal liver biochemical parameters in pregnancy between the epidemic period and the non-epidemic period of COVID-19, and there is no change in the proportion of patients with normal creatinine and eGFR among these patients in the epidemic period of COVID-19.

The Spt-Ada-Gcn5-acetyltransferase (SAGA) is an ancillary transcription initiation complex which is highly conserved. The ADA1 (alteration/deficiency in activation 1, also called histone H2A functional interactor 1, HFI1) is a subunit in the core module of the SAGA protein complex. ADA1 plays an important role in plant growth and development as well as stress resistance. In this paper, we performed genome-wide identification of banana ADA1 gene family members based on banana genomic data, and analyzed the basic physicochemical properties, evolutionary relationships, selection pressure, promoter cis-acting elements, and its expression profiles under biotic and abiotic stresses. The results showed that there were 10, 6, and 7 family members in Musa acuminata, Musa balbisiana and Musa itinerans. The members were all unstable and hydrophilic proteins, and only contained the conservative SAGA-Tad1 domain. Both MaADA1 and MbADA1 have interactive relationship with Sgf11 (SAGA-associated factor 11) of core module in SAGA. Phylogenetic analysis revealed that banana ADA1 gene family members could be divided into 3 classes. The evolution of ADA1 gene family members was mostly influenced by purifying selection. There were large differences among the gene structure of banana ADA1 gene family members. ADA1 gene family members contained plenty of hormonal elements. MaADA1-1 may play a prominent role in the resistance of banana to cold stress, while MaADA1 may respond to the Panama disease of banana. In conclusion, this study suggested ADA1 gene family members are highly conserved in banana, and may respond to biotic and abiotic stress.
