1.Effectiveness of digital three-dimensional printing osteotomy guide plate assisted total knee arthroplasty in treatment of knee osteoarthritis patients with femoral internal implant.

OBJECTIVE:
To investigate the effectiveness of digital three-dimensional (3D) printing osteotomy guide plate assisted total knee arthroplasty (TKA) in treatment of knee osteoarthritis (KOA) patients with femoral internal implants.
METHODS:
The clinical data of 55 KOA patients who met the selection criteria between July 2021 and October 2023 were retrospectively analyzed. Among them, 26 cases combined with femoral implants were treated with digital 3D printing osteotomy guide plate assisted TKA (guide plate group), and 29 cases were treated with conventional TKA (control group). There was no significant difference in gender, age, body mass index, side, Kellgren-Lawrence classification, preoperative visual analogue scale (VAS) score, Hospital for Special Surgery (HSS) knee score, knee range of motion, and other baseline data between the two groups ( P>0.05). The operation time, intraoperative blood loss, incision length, postoperative first ambulation time, surgical complications; VAS score, knee HSS score, knee range of motion before operation, at 1 week and 3 months after operation, and at last follow-up; distal femoral lateral angle, proximal tibial medial angle, hip-knee-ankle angle and other imaging indicators at last follow-up were recorded and compared between the two groups.
RESULTS:
The operation time, incision length, intraoperative blood loss, and postoperative first ambulation time in the guide plate group were significantly lower than those in the control group ( P<0.05). In the control group, there were 1 case of incision rupture and bleeding and 1 case of lower limb intermuscular venous thrombosis, which was cured after symptomatic treatment. There was no complication such as neurovascular injury, incision infection, or knee prosthesis loosening in both groups. Patients in both groups were followed up 12-26 months, with an average of 16.25 months. The VAS score, HSS score, and knee range of motion improved at each time point after operation in both groups, and further improved with time after operation, the differences were significant ( P<0.05). The above indicators in the guide plate group were significantly better than those in the control group at 1 week and 3 months after operation ( P<0.05), and there was no significant difference between the two groups at last follow-up ( P>0.05). At last follow-up, the distal femoral lateral angle, the proximal tibial medial angle, and the hip-knee-ankle angle in the guide plate group were significantly better than those in the control group ( P<0.05).
CONCLUSION
The application of digital 3D printing osteotomy guide plate assisted TKA in the treatment of KOA patients with femoral implants can simplify the surgical procedures, overcome limitations of conventional osteotomy guides, reduce surgical trauma, achieve individualized and precise osteotomy, and effectively restore lower limb alignment and knee joint function.

2.Triangular Wave tACS Improves Working Memory Performance by Enhancing Brain Activity in the Early Stage of Encoding.

Working memory is an executive memory process that includes encoding, maintenance, and retrieval. These processes can be modulated by transcranial alternating current stimulation (tACS) with sinusoidal waves. However, little is known about the impact of the rate of current change on working memory. In this study, we aimed to investigate the effects of two types of tACS with different rates of current change on working memory performance and brain activity. We applied a randomized, single-blind design and divided 81 young participants who received triangular wave tACS, sinusoidal wave tACS, or sham stimulation into three groups. Participants performed n-back tasks, and electroencephalograms were recorded before, during, and after active or sham stimulation. Compared to the baseline, working memory performance (accuracy and response time) improved after stimulation under all stimulation conditions. According to drift-diffusion model analysis, triangular wave tACS significantly increased the efficiency of non-target information processing. In addition, compared with sham conditions, triangular wave tACS reduced alpha power oscillations in the occipital lobe throughout the encoding period, while sinusoidal wave tACS increased theta power in the central frontal region only during the later encoding period. The brain network connectivity results showed that triangular wave tACS improved the clustering coefficient, local efficiency, and node degree intensity in the early encoding stage, and these parameters were positively correlated with the non-target drift rate and decision starting point. Our findings on how tACS modulates working memory indicate that triangular wave tACS significantly enhances brain network connectivity during the early encoding stage, demonstrating an improvement in the efficiency of working memory processing. In contrast, sinusoidal wave tACS increased the theta power during the later encoding stage, suggesting its potential critical role in late-stage information processing. These findings provide valuable insights into the potential mechanisms by which tACS modulates working memory.

3.Clinical application of personalized osteotomy guide based on rapid 3D printing in knee arthroplasty

Objective To evaluate the clinical efficacy of a rapid 3D-printed patient-specific osteotomy guide in knee replacement surgery,and provide guidance for its widespread clinical application.Methods A total of 80 patients with end-stage knee osteoarthritis who had undergone Total Knee Arthroplasty(TKA)were selected and randomly divided into two groups.The first group comprised 40 cases(40 knees)that underwent traditional TKA,while the second group consisted of 40 patients(40 knees)in the 3D-printed osteotomy guide group.Various parameters,including surgery duration,intraoperative blood loss,time to ambulation after surgery,mechanical axis angle between femur and tibia post-surgery,Visual Analog Scale(VAS)score,Hospital for Special Surgery(HSS)knee score,Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score,range of motion(ROM)of the knee,and other indicators were analyzed separately for both groups.Statistical analyses were conducted.Results All the patients underwent surgery smoothly and were followed up for 3~12 months.In com-parison to the traditional osteotomy guide group,the 3D-printed osteotomy guide group demonstrated significantly shorter surgery duration(P<0.05),reduced intraoperative blood loss,earlier time to first ambulation after surgery(P<0.05),lower postoperative VAS score,and smaller mechanical axis angle between the femur and tibia after surgery(P<0.01).At 6 months post-surgery,both groups showed significantly improved KSS scores compared to preoperative values(P<0.01).The KSS score of the 3D-printed guide group was higher than that of the traditional surgical guide group at 6 months after surgery(P<0.05),but there was no statistical difference in KSS scores between the two groups(P>0.05).WOMAC scores for both groups decreased over time at 3 and 6 months post-surgery compared to preoperative scores(P<0.05).At these time points,WOMAC scores were lower in the 3D-printed guide group than in the traditional osteotomy guide group(P<0.05).One case of hematocele and infection occurred in the traditional osteotomy guide group;however,successful discharge was achieved following re-debridement while preserving the prosthesis.None of the surgical patients experienced complications such as neurovascular injury,deep vein thrombosis,prosthesis loosening or periprosthetic fractures.Conclusion Compared to conventional total knee arthroplasty(TKA)surgery,the utilization of a rapid 3D-printed osteotomy guide for knee replacement presents several advantages,including reduced surgical operation duration,minimal intraoperative blood loss,precise and expeditious osteotomies,accelerated postoperative recovery,and heightened patient satis-faction during medical consultations.Notably,its clinical efficacy surpasses that of traditional approaches.
4.Clinical management of thrombocytopenia in cirrhosis

Thrombocytopenia is one of the common complications of cirrhotic patients, which can induce an increasing bleeding risk and closely correlate with bleeding following invasive procedures. Consequently, how to respond to thrombocytopenia is crucial for improving the prognosis of patients with cirrhosis. This article reviews the main mechanisms of cirrhosis concurrent with thrombocytopenia, as well as the corresponding clinical management strategies.
5.Identification of banana ADA1 gene family members and their expression profiles under biotic and abiotic stresses.

The Spt-Ada-Gcn5-acetyltransferase (SAGA) is an ancillary transcription initiation complex which is highly conserved. The ADA1 (alteration/deficiency in activation 1, also called histone H2A functional interactor 1, HFI1) is a subunit in the core module of the SAGA protein complex. ADA1 plays an important role in plant growth and development as well as stress resistance. In this paper, we performed genome-wide identification of banana ADA1 gene family members based on banana genomic data, and analyzed the basic physicochemical properties, evolutionary relationships, selection pressure, promoter cis-acting elements, and its expression profiles under biotic and abiotic stresses. The results showed that there were 10, 6, and 7 family members in Musa acuminata, Musa balbisiana and Musa itinerans. The members were all unstable and hydrophilic proteins, and only contained the conservative SAGA-Tad1 domain. Both MaADA1 and MbADA1 have interactive relationship with Sgf11 (SAGA-associated factor 11) of core module in SAGA. Phylogenetic analysis revealed that banana ADA1 gene family members could be divided into 3 classes. The evolution of ADA1 gene family members was mostly influenced by purifying selection. There were large differences among the gene structure of banana ADA1 gene family members. ADA1 gene family members contained plenty of hormonal elements. MaADA1-1 may play a prominent role in the resistance of banana to cold stress, while MaADA1 may respond to the Panama disease of banana. In conclusion, this study suggested ADA1 gene family members are highly conserved in banana, and may respond to biotic and abiotic stress.

6.Clinical effect of endoscopic sclerosing agent and tissue glue injection combined with ligation for the treatment of rectal ectopic varices bleeding

Objective To explore the clinical effect of endoscopic sclerosing agent and tissue glue injection combined with ligation in the treatment of rectal ectopic varices(EV)bleeding.Method A retrospective analysis was conducted on 60 patients with rectal EV bleeding who received endoscopic treatment from January 2017 to January 2022.According to the surgical method,the patients were divided into two groups,with 30 cases in the control group receiving endoscopic ligation treatment;30 cases in the observation group were treated with endoscopic scleroing agent and tissue glue injection combined with ligation.Follow up was conducted according to a cycle of 1,3,6 and 12 months after surgery,and the following indicators were compared between the two groups of patients:successful surgical hemostasis rate,incidence of postoperative complications,mortality rate,improvement of postoperative varicose vein diameter,and postoperative response rate.Result The success rate of surgical hemostasis in the observation group was 93.3%,significantly higher than 66.7%in the control group,the incidence of abdominal pain and fever in the observation group was 10.0%and 10.0%,respectively,significantly lower than 33.3%and 36.7%in the control group,there were 2 deaths in the observation group and 9 deaths in the control group,the mortality rate in the observation group was 6.7%,significantly lower than the 30.0%in the control group,the preoperative venous diameter of the observation group was(1.60±0.36)cm,and the postoperative diameter continued to decrease,at the 12th month after surgery,the diameter was(0.78±0.31)cm,while in the control group,the preoperative venous diameter was(1.52±0.26)cm,the postoperative diameter continued to increase,and at the 12th month after surgery,the diameter was(1.55±0.36)cm,the postoperative effective rate of the observation group was significantly better than that of the control group,the differences were statistically significant(P<0.05).Conclusion The combination of endoscopic scleroing agent and tissue glue injection combined with ligation for the treatment of rectal EV bleeding has a higher success rate and effective rate of surgical hemostasis,fewer postoperative complications,lower mortality rate during follow-up observation,and satisfactory clinical efficacy.It is worth further promoting and applying in clinical practice.
7.Research Progress on Epidural Electrical Stimulation for Motor Disorders after Spinal Cord Injury

In recent years,the incidence of spinal cord injury(SCI)has shown an increasing trend,and the resulting motor impairment cause significant harm to patients themselves and public health.Although the current clinical treatments for motor disorders after SCI are diverse,mainly focusing on improving dysfunction and improving patients' daily living ability through external stimulation,there is still a lack of radical solutions.Epidural electrical stimulation(EES)as an emerging treatment technology,has been shown in a number of international clinical studies on the SCI significant poten-tial of motor function recovery,has significant curative effect,convenient,widely applicable,adjustable advantages,the rehabilitation of movement disorders after SCI has a lot of big benefits,in the spinal cord injury rehabilitation has great prospect.However,most of the domestic research are limited to the neurogenic bladder caused by abnormal autonomic nerve function after SCI,and mostly combine traditional Chinese and western medicine rehabilitation treatment,the emerging therapy of EES,this paper aims to review the basic principle of EES,potential mechanism of action and its c lini-cal application progress in the treatment of movement disorders after SCI,in order to provide reference for the clinical ap-plication and scientific research of EES treatment of motor disorders after SCI.
8.Development of the Self-Stigma Scale for Drug Addicts

Objective:To develop the Self-Stigma Scale for Drug Addicts(SSSDA),and test its validity and reliability.Methods:On the basis of literature analysis,open questionnaire survey,semi-structured interview and ex-pert consultation,the theoretical structure of the questionnaire was developed,and 943 drug addicts were test-ed.Sample 1(n=483)was used for item analysis and exploratory factor analysis,and sample 2(n=460)was used for confirmatory factor analysis,criterion related validity and internal consistency reliability analysis.Sixty-four drug addicts were retested 4 weeks later for test-retest reliability test.The criterion related validity was tested with the Drug Stereotype Threat Scale.Results:The scale consisted of 6 dimensions and 31 items,including self-negative cognition,stereotype identity,confidentiality,social avoidance,stigma experience,and stigma experience in the process of detoxification(factor loadings were from 0.41 to 0.81),which explained 64.09%of the total vari-ance.The 6-factor structure model fitted the data well(x2/df=2.82,RMSEA=0.06,CFI=0.92,GFI=0.85,TLI=0.91).The total scores and factor scores of the SSSDA were positively correlated with the DSTS scores(ICC=0.10-0.22,Ps<0.05).The Cronbach α coefficients for the total scale and each dimension were between 0.80 and 0.95,and the test-retest reliability coefficients(ICC)were between 0.82 and 0.94.Conclusion:The Self-stigma Scale for Drug Addicts(SSSDA)initially developed in this study has satisfactory reliability and validity.
9.Study on peel test of wearable electronic products based on peel force tester with full-angle

Objective:To develop adjustable peel force tester with peel angle from 0° to 180°,so as to realize the test for the performance of interface adhesion of wearable electronic products under multi angles.Methods:The rotating platform of angle adjustment component was used to realize free adjustment of peel angle from 0° to 180°.A method of peel test procedure based on the peel force tester with full-angle was established,and the comparative experiment between that and tensile testing machine was conducted.The peel tests of various kinds of wearable electronic products were conducted under peel angle of 45°,90° and 180°.Results:The test results of the peel force tester with full-angle were consistent with those of the tensile testing machine,which function opposed reliability.The peel test results of more than 10 wearable electronic products indicated that the peel force tester with full-angle could effectively determinate the performance of interface adhesion.Conclusion:The peel force tester with full-angle can effectively appear the performance of the interface adhesion of wearable electronic products,which provides basis for establishing the standard of peel test of wearable electronic products.
10.Effect of antithrombotic therapy selection on stroke recurrence based on drug gene polymorphism and thromboelastography

AIM:Clopidogrel and aspirin are com-monly used drugs for the secondary prevention of cerebrovascular disease.Due to drug resistance,their preventive effect is often affected.This article explores the clinical value of clopidogrel and aspirin pharmacogenetic genetic testing in the secondary prevention of ischemic stroke.METHODS:220 pa-tients with mild ischemic stroke or TIA admitted to our hospital from 2021.7 to 2022.9 were included and randomly divided into individualized treatment group and clopidogrel conventional treatment group(control group).The patients were followed up for one year to observe stroke recurrence and hemorrhagic events.RESULTS:(1)Compared with the control group,the recurrence rate of ischemic stroke in the individualized treatment group after 1-year follow-up was slightly lower(5.82%vs.7.92%,P>0.05),the risk of cerebral hemorrhage was simi-lar,but the risk of other occurrences was increased(6.79%vs.0.99%,P<0.05).(2)COX regression analy-sis showed that ESRS(HR 2.576,95%CI 1.226-5.413,P=0.013)and history of hypertension(HR 5.517,95%CI 1.624-18.737,P=0.006)were associated with recurrence of ischemic stroke,independent of anti-thrombotic regimen(HR 0.918,95%CI 0.291-2.894,P=0.883).CONCLUSION:Aspirin GPIBA,PTGS1,and ITGB3 gene polymorphisms have limited signifi-cance in guiding antiplatelet medication.Selecting aspirin maintenance therapy for clopidogrel CYP2C19*2*3 allele carriers cannot significantly re-duce the risk of recurrence of minor ischemic stroke and may increase other bleeding risks.COX regression analysis shows that ESRS and history of hypertension are independent risk factors for stroke recurrence.
