1.Pharmacoeconomic evaluation of durvalumab combined with chemotherapy as first-line therapy for advanced biliary tract cancer

OBJECTIVE To assess the cost-effectiveness of durvalumab combined with chemotherapy as a first-line treatment for advanced biliary tract cancer from the perspective of the Chinese healthcare system. METHODS Using data from the TOPAZ-1 clinical trial, a three-state Markov model comprising progression-free survival (PFS), progressive disease (PD) and death was developed, with a cycle length of 21 days and a 10-year time horizon. Patients in the observation group received durvalumab in combination with gemcitabine and cisplatin, whereas those in the control group received placebo plus the same chemotherapy regimen. The evaluation indexes were quality-adjusted life year (QALY) and the incremental cost-effectiveness ratio (ICER). The willingness-to-pay (WTP) threshold was set at three times the 2024 Chinese per capita gross domestic product (GDP) (287 247 yuan/QALY). The sensitivity analyses, along with scenario analyses, were performed. RESULTS In the base-case analysis, the ICER of observation group compared to control group was 1 166 344.46 yuan/QALY, far exceeding the WTP threshold, indicating that the regimen was not cost-effective. One-way sensitivity analysis identified the PD state utility, discount rate, cost of durvalumab, and PFS state utility as the main drivers of ICER variation. Probabilistic sensitivity analysis showed that, at the above WTP threshold, the probability of the acceeptance of this regimen was 0, further supporting the robustness of the base-case findings. In the scenario analysis, inclusion of a patient assistance program reduced the ICER to 235 885.16 yuan/ QALY, below the above WTP threshold, suggesting cost-effectiveness under this assistance program. However, when applying a regional WTP threshold set at three times the per capita GDP (158 475 yuan/QALY) of Gansu Province (the province with the lowest GDP in China in 2024), the ICER remained above the threshold, indicating that the regimen was not cost-effective at the regional level. CONCLUSIONS At current pricing, durvalumab plus chemotherapy as a first-line treatment for advanced biliary tract cancer is not cost-effective in China. Although the introduction of a patient assistance program can substantially reduce the ICER and achieve cost-effectiveness at a WTP threshold set at three times the 2024 per capita GDP of China, due to limited affordability in low-income areas, the program remains not cost-effective.
2.Treatment of rheumatoid arthritis of wrist using Ilizarov wrist joint distraction technique: a case report.

OBJECTIVE:
To report the clinical experience of using Ilizarov wrist joint distraction technique in the treatment of a case of rheumatoid arthritis of the wrist.
METHODS:
In January 2019, a 49-year-old female patient with rheumatoid arthritis of the left wrist, complicated by ulnar impaction syndrome, was admitted for treatment. Preoperatively, the active range of motion of the left wrist was as follows: extension 0°-flexion 0°, pronation 65°-supination 35°, and grip strength of 4.0 kg. The visual analogue scale (VAS) score was 9, and the Cooney wrist function score was 15, indicating poor function. As conservative treatment failed to achieve symptom relief, Ilizarov wrist joint distraction surgery was performed. Postoperatively, joint distraction was applied at 2 mm increments on postoperative days 2 and 7, in 4 separate sessions.
RESULTS:
Postoperative X-ray film examination at 7 days revealed a distraction of 3.6 mm in the affected wrist joint compared to the contralateral side. The external fixator was removed 2.5 months postoperatively. At 22 months postoperatively, X-ray film and MRI examinations revealed that the joint space of the left wrist had returned to near-normal, with significant reduction in joint effusion and synovial proliferation. The active range of motion of the left wrist improved to extension 15°- flexion 30°, pronation 90°-supination 90°, with a grip strength of 18.0 kg. The wrist pain VAS score decreased to 0, and the Cooney wrist function score improved to 90, indicating excellent function. At 50 months postoperatively, follow-up X-ray film, MRI, and functional assessments showed the results similar to those at 22 months.
CONCLUSION
Ilizarov wrist joint distraction may be a viable treatment option for rheumatoid arthritis of the wrist.

3.Experimental validation of the accuracy of reported dose-length product values in different CT scanner models

Objective To evaluate the accuracy of dose-length product (DLP) values in CT dose reports by comparing them with the DLP values measured using a pencil-type ionization chamber. Methods Experiments were conducted using four CT scanners with different detector row numbers from two manufacturers (A and B), along with a head phantom and a pencil-type ionization chamber. Other scanning parameters were fixed, while pairwise combinations of kV and mAs were tested. The DLP values were measured under various scanning conditions using the pencil-type ionization chamber (DLPmeasured) and the corresponding DLP values in CT dose reports were recorded (DLPreported). All data were subjected to statistical analysis. Results Differences were observed between DLPreported and DLPmeasured values. The smallest mean absolute value of ΔDLP was approximately 2.526 mGy·cm observed on the 40-row CT scanner of manufacturer A. There was a difference between the ΔDLPs of the four CT scanners. Both DLPreported and DLPmeasured exhibited a linear relationship with mAs. Conclusion The percentage errors between DLPreported and DLPmeasured values for all four CT scanners were within the national standard tolerance of 15%. DLP values in CT dose reports can serve as a reference for assessing patient radiation dose during scanning. However, for radiation dose-related research, data measured using dosimetric instruments such as pencil-type ionization chamber are recommended.
4.Application of Yttrium-90 microsphere selective internal radiation therapy in downstaging and conversion of hepatocellular carcinoma: a case report

This case report describes a 68-year-old male patient diagnosed with primary hepatocellular carcinoma (HCC). After receiving Yttrium-90 microsphere selective internal radiation therapy (90Y-SIRT), the tumor significantly reduced in size, and tumor markers alpha fetoprotein (AFP) and abnormal prothrombin (PIVKA-Ⅱ) decreased. Postoperative pathological results showed minimal residual tumor cells, indicating that 90Y-SIRT has good efficacy and safety in downstaging and conversion of HCC, thereby facilitating subsequent surgical resection.
5.Ultrasound-guided percutaneous catheterization and drainage combined with polidocanol sclerosis therapy in treatment of thyroid cysts

Objective:To investigate the therapeutic effectiveness of ultrasound-guided percutaneous catheterization for continuous negative pressure drainage combined with polidocanol in treating large thyroid cysts.Method:Clinical data of 38 patients with large thyroid cysts who were treated consecutively with catheter drainage combined with polidocanol sclerotherapy by the same doctor at Beijing Tsinghua Changgung Hospital from Jan 2021 to May 2024 were retrospectively analyzed. The effectiveness and safety were statistically evaluated, and the relationship between drainage volume and cyst volume was analyzed.Results:Among the 38 patients with thyroid cysts who completed the treatment, the median follow-up was 9 months (range: 3-24 months). The effectiveness rate was 92% (35/38), of which 32 cases (84%) met the cure standard. The maximum diameter of the cysts before treatment was (4.8±1.0) cm, and the maximum diameter of the residual nodules after treatment was (1.5±1.1) cm, the difference was statistically significant ( t=17.389, P<0.01). The amount of drainage exudate is related to the volume of the cyst and the maximum diameter before treatment ( t=-3.149, P=0.003; t=-3.057, P<0.005). 19% of patients showed transient low fever after the injection of polidocanol, with no other complications. Conclusion:For large thyroid cysts, ultrasound-guided percutaneous catheterization for continuous negative pressure drainage combined with polidocanol sclerotherapy is a safe and effective method.

Bidirectional communication between the heart and brain has been well documented,and their respective electrophysiological activities are not independent but rather exhibit a measurable degree of coupling.The traditional Chinese medicine(TCM)doctrine of"heart storing the spirit"posits that the heart governs all vital activities and that the heart and brain are functionally interdependent and mutually reinforcing.This paper systematically reviews current methodological approaches for investigating cardio-electroencephalographic coupling,summarizes empirical findings that elucidate human physiological and pathological mechanisms through this coupling framework,and integrates TCM-based perspectives on heart-brain interactions.Finally,prospects for applying emerging cardio-electroencephalographic techniques is outline to advance research on the"heart storing the spirit"theory.
7.Relationship Between Cognitive Function and Cardio-Electroencephalographic Coupling in Coronary Heart Disease Patients with Heart-Qi Deficiency Pattern

Objective To explore their inter-relationship,cognitive performance and cardio-electroencephalographic coupling in coronary heart disease(CHD)patients with heart-qi deficiency pattern were characterized.Methods Thirty CHD patients who met the diagnostic criteria for heart-qi deficiency were enrolled.Thirty healthy volunteers without heart-qi deficiency or CHD served as controls.Cognitive function was assessed with the repeatable battery for the assessment of neuropsychological status(RBANS).Simultaneous resting-state electrocardiogram and electroencephalogram were recorded,and the strength of heart-brain coupling was quantified by calculating the maximal information coefficient(MIC)between heart-rate variability(HRV)and EEG signals.Results Compared with controls,heart-qi deficiency CHD patients showed lower RBANS total scores and reduced performance in immediate memory,visuospatial/constructional ability,and attention.Significant between-group differences in HRV-EEG MIC values were observed at several channels(P<0.05).In patients,the MIC values for HRV-Beta(channel FC1)and HRV-Delta(channel F2)were positively correlated with RBANS total score(P<0.05).Conclusion CHD patients with heart-qi deficiency exhibit impaired cognition and altered cardio-electroencephalographic coupling.These findings suggest that heart-brain interactions may contribute to the pathophysiological mechanisms underlying cognitive decline in this population.
8.Effects of LINC01915 on proliferation,migration,and invasion of human colorectal cancer cells and its mechanism

Objective To observe the effects of LINC01915 on the proliferation,migration and invasion of human colorectal cancer cells,and to explore its possible regulatory mechanism.Methods The colorectal cancer cell lines of SW620,SW480,LOVO,HCT116 and the normal colorectal cell line of NCM460 were selected,and the expression of LINC01915 was detected by RT-qPCR.HCT116 cell line with the highest expression of LINC01915 was taken and divided into the upregulation group,the downregulation group,the upregulation control group,the downregulation control group and the blank group.The transfection efficiency of each group was detected by fluorescence microscope;RT-qPCR was used to detect the expression of LINC01915 in each group;methyl thiazolyl tetrazolium(MTT)assay was used to detect the proliferative activity in each group;scratch wound healing assay and Transwell assay were used to detect the migration and invasion activities in each group;RT-qPCR was used to detect the expression of miR-92a-3p and mRNA expression of large tumor suppressor homolog 2(LATS2),transcription factor 21(TCF21)and Kruppel like factor 4(KLF4);and Western blot was used to detect the expression of LATS2,TCF21,and KLF4 proteins.Dual-fluorescein reporter assay was used to verify the targeting relationship between LINC01915 and miR-92a-3p.Results The expression of LINC01915 in various human colorectal cancer cell lines were lower than that in the NCM460 cell(P<0.05),and the highest LINC01915 expression in human colorectal cancer cell lines was observed in HCT116(P<0.05).The transfection efficiency of cells in each transfection group was high.Compared with the blank group and the upregulation control group,the expression of LINC01915,and mRNA and protein expression of LATS2,TCF21,and KLF4 in the upregulation group increased(P<0.05),and the A value,scratch healing rate,number of invasive cells and miR-92a-3p expression decreased(P<0.05).Compared with the blank group and the downregulation control group,the expression of LINC01915,and mRNA and protein expression of LATS2,TCF21,and KLF4 in the downregulation group decreased(P<0.05),and the A value,scratch healing rate,number of invasive cells and miR-92a-3p expression increased(P<0.05).LINC01915 had binding sites with miR-92a-3p,and compared with the miR-NC group,the miR-92a-3p mimics group showed a decrease in the luciferase activity of WT-LINC01915(P<0.05).Conclusion The expression of LINC01915 in the human colorectal cancer cell lines decreases,and upregulation of LINC01915 expression can inhibit cell proliferation,migration,and invasion,which may be related to the up-regulation of the expression of LATS2,TCF21 and KLF4 by inhibiting miR-92a-3p.
