Patent foramen ovale(PFO)is a common congenital heart defect that has been linked to various conditions, including cryptogenic stroke, migraine with aura, and decompression sickness. With the rapid advancement of interventional cardiology, interventional treatment has become the preferred approach for PFO patients.Conventional PFO closure procedures predominantly use metallic disc occluders, which, despite their excellent surgical outcomes, come with unavoidable device-related complications.Consequently, there is an urgent need for a percutaneous PFO closure strategy that does not require the permanent implantation of an occluder, aligning with the"intervention without implantation, implantation without residue"green philosophy to address the limitations of traditional PFO occluders.Transcatheter PFO suturing represents a technique that better conforms to the anatomical and physiological requirements of PFO closure, capable of overcoming many of the device-related complications associated with conventional PFO closure, offering good safety and efficacy. This paper reviews the research advancements in transcatheter PFO suturing, aiming to provide novel perspectives for the clinical management of these conditions.

To investigate the phenotype and genomic characterization of a mucoid-type Salmonella Saintpaul ST50 isolate from a urinary tract infection patient, promoting clinical diagnosis and treatment for urinary tract infections caused by Salmo-nella spp.Culture-based quantitative counts of midstream urine sample from the patient were conducted, and further biochemi-cal identification, mass spectrometry detection, serum agglutination test and antimicrobial susceptibility test(AST)were con-ducted on Salmonella isolate(2024JD5).Whole-genome sequencing(WGS)was performed on isolate 2024JD5 to predict sero-type,multilocus sequence type(MLST),resistance genes, and virulence genes. Two smooth-type of Salmonella Saintpaul ST50 were selected as comparative genomic reference strains from the Chinese local Salmonella genome database. The literature reviews of global Salmonella serotype of urinary tract infection were summarized.Specific serum agglutination confir-mation of isolate 2024JD5 failed due to characterization of the mucus type. The strain 2024JD5 was predicted as Salmonella Saintpaul(4,5,12:e,h:1,2)ST50 using WGS,and was resistant to ciprofloxacin, nalidixic acid, chloramphenicol and tetracy-cline with carrying aminoglycoside resistance genes aac(6')-Ⅰaa and aph(3)-Ⅱa,chloramphenicol resistance gene floR,tetra-cycline resistance gene tet, quinolone resistance gene qnrS1,and S83Y substitution in the gyrA gene was found in the quinolo-ne resistance determination region(QRDR).In addition, the strain 2024JD4 carried six types of non-plasmid-based mobile ge-netic elements and 144 virulence genes, including 71 secretion transporter genes and 58 fimbriae adhesion genes, respectively.Four types of fimbriae regulatory genes(csgB,csgC,fimW,fimY)were absent in comparison with smooth-type Salmonella Saintpaul. The literature reviews showed Salmonella Saintpaul was currently a rare Salmonella serotype in cases of urinary tract infections worldwide.Salmonella Saintpaul ST50 with mucoid-type is the pathogen of urinary tract infection with multi-drug resistant phenotypic and genotypic characteristics, and the high mucoid expression may be related to the compensatory mechanism of fimbriae regulatory genes absence in urinary tract colonization and adaptation.WGS combined with the Chinese local Salmonella genome database can effectively solve the diagnosis and biosafety assessments of rare Salmonella phenotypes.

This study draws on embedding theory and incentive mechanism theory to construct a multi-dimensional framework for analyzing the integration of county-level health insurance into primary healthcare.Using Qianjiang District,Chongqing,as a case study,it analyses how horizontally bundled funding and service transformation embed insurance into local health governance. The study finds that health insurance is embedded into primary governance through a systemic process driven by values, shaped by social structures, and supported by institutions, involving the reconfiguration of resources, relationships, and interests.Establishing primary care groups as budget units improves alignment between payment and service needs;linking budget limits, surplus incentives, and performance feedback strengthens provider motivation;and coordinated inter-agency mechanisms facilitate integration of fund management and service delivery.

Chinese Journal of Biochemistry and Molecular Biology 2025;41(4):597-606
Paclitaxel(PTX)is a first-line chemotherapy drug for breast cancer, but its resistance issues significantly impact clinical treatment efficacy.Fucosylation, especially core fucosylation,is closely related to tumor chemoresistance, resulting in poor chemotherapy responses and poor prognosis in patients. In this study,we investigated the effect and mechanism of the fucosylation inhibitor 2-fluorofucose(2-F-Fuc)on the chemosensitivity of paclitaxel-resistant breast cancer MCF-7/PTX cells. The drug resistanceindex(RI)of MCF-7/PTX cells was 8.49 by MTT assays.Western blotting,real-time PCR,enzyme-linked immu-nosorbent assay(ELISA)and Lens Culinaris Agglutinin(LCA)lectin imprinting showed that compared with MCF-7 cells, the expression of FUT8,MDR1and core fucosylation in MCF-7/PTX cells was high.Western blotting showed that 2-F-Fuc had a significant inhibitory effect on the growth of MCF-7/PTX cells, and the expression levels of FUT8 and MDR1 were significantly down-regulated after 2-F-Fuc treatment, and the down-regulation was more pronounced in the PTX and 2-F-Fuc combination group(P<0.05).Compared to the control, expression of PCNA in MCF-7/PTX cells in the PTX and the 2-F-Fuc group were down-regulated,and the apoptosis-related proteins, such as cleaved caspase-3 and Bax/Bcl-2 were in-creased.The level of p-PI3K and p-AKT were down-regulated,and the changes in the combination of 2-F-Fuc and PTX were more robust(P<0.05).The above results showed that the core fucosylation level of MCF-7/PTX cells was significantly increased, and 2-F-Fuc could reduce the core fucosylation level of MCF-7/PTX cells by inhibiting the expression of FUT8,and enhance the sensitivity of drug-resistant cells to PTX,which may correlate with the downregulation of PI3K/AKT signaling pathway proteins.
