1.Epidemiological characteristics of respiratory human adenovirus infections in hospitalized children in Ningbo from 2019 to 2024

OBJECTIVE To investigate the epidemiological characteristics of respiratory human adenovirus(H AdV)infections in hospitalized children in Ningbo,and to provide data for formulating infection prevention and control strategies for HAdV.METHODS A total of 65 022 children hospitalized with respiratory infections at the Women and Children's Hospital of Ningbo University from Jul.2019 to Dec.2024 were selected as the study sub-jects.Multiple reverse transcription-polymerase chain reaction(RT-PCR)and capillary electrophoresis were used to detect 11 non-bacterial pathogens.Basic and clinical information of the children was collected for analysis.RESULTS A total of 65 022 specimens were tested,with 4 292 cases tested positive for HAdV positivity,yielding a positive rate of 6.60%.The lowest positive rate was observed in 2023(3.22%),while the highest was in 2024(13.97%).Compared to the years 2019-2023,the overall HAdV positive rate was high in 2024,peaking at 26.80%in Jun.,indicating an outbreak.The total HAdV positive rate was higher in boys(6.82%)than in girls(6.32%)(P=0.006).The highest HAdV positive rate was observed in the 2 to<6 age group(9.00%),while the lowest was in the 0 to<1 age group(2.33%).Among the HAdV-positive specimens,2 658 cases(61.93%)were single infections,and 1 634 cases(38.07%)were co-infections.The non-bacterial respiratory pathogens with the highest co-infection rates were human rhinovirus(34.09%),Mycoplasma pneumoniae(20.44%)and human parainfluenza virus(5.75%).CONCLUSIONS A n outbreak of HAdV infections is observed among hospitalized children in Ningbo in 2024.Higher positive rates are found in boys aged 2 to<6 years,with a certain proportion of co-infections.
2.Construction and validation of a dynamic nomogram prognostic model based on perineural invasion and lymphovascular tumor embolus for patients with gastric cancer after postoperative chemotherapy

Objective:To verify the prognostic value of perineural invasion and lymphovascular tumor embolus for patients with gastric cancer undergoing gastrectomy and postoperative chemotherapy, and establish a prognostic prediction nomogram model.Methods:According to 7∶3 radio, 781 gastric cancer patients were randomly divided into training cohort and internal validation cohort. One hundred fifty patients were utilized as the external validation cohort. Univariate and multivariate analysis were performed to evaluate the prognostic value of perineural invasion and lymphovascular tumor embolus, and construct the dynamic nomogram. The concordance index (C-index), net reclassification index and integrated discrimination improvement index, receiver operating characteristic curve, calibration curves and decision curve analysis were used to evaluate the nomogram.Results:Perineural invasion ( HR=1.486, 95% CI: 1.150-1.919, P<0.01) and lymphovascular tumor embolus ( HR=1.321, 95% CI: 1.030-1.693, P<0.05) were independent prognostic risk factors for patients with gastric cancer after gastrectomy and postoperative chemotherapy. C-index (training cohort: 0.734, internal validation cohort: 0.755, external validation cohort: 0.715), net reclassification index (training cohort: 0.228 for 3-year and 0.213 for 5-year OS prediction; internal validation cohort: 0.211 for 3-year and 0.279 for 5-year OS prediction; external validation cohort: 0.220 for 3-year and 0.440 for 5-year OS prediction) and integrated discrimination improvement index (training cohort: 0.051 for 3-year and 0.041 for 5-year OS prediction; internal validation cohort: 0.027 for 3-year and 0.036 for 5-year OS prediction; external validation cohort: 0.063 for 3-year and 0.153 for 5-year OS prediction) indicated that the nomogram performed better than the traditional TNM staging system ( P<0.05). Conclusions:Perineural invasion and lymphovascular tumor embolus are independent prognostic risk factors of gastric cancer patients after postoperative chemotherapy. The novel dynamic nomogram model based on perineural invasion and lymphovascular tumor embolus provides better assistance in evaluating prognosis of gastric cancer patients.
3.Leonurine improve necrotic apoptosis in rats with myocardial ischemia/reperfusion injury by regulating RIP1-RIP3-MLKL pathway

Objective:To investigate impact of leonurine(Leo)on necrotic apoptosis in rats with myocardial ischemia/reperfu-sion(MIR)injury by regulating receptor interacting protein 1(RIP1)-receptor interacting protein 3(RIP3)-mixed lineage kinase domain-like protein(MLKL)pathway.Methods:Fifteen rats were randomly selected as sham surgery group(Sham),left anterior descending coronary artery of remaining rats were ligated to construct MIR model,and randomly grouped into MIR group,Leo group(15 mg/kg),Compound 6i group(1 mg/kg RIP1-RIP3-MLKL pathway activator Compound 6i)and Leo+Compound 6i group(15 mg/kg Leo+1 mg/kg Compound 6i),with 15 rats in each group.MIR group and Sham group were injected with equal amounts of physiological saline.Cardiac function indicators were observed through ultrasound electrocardiogram;ELISA was applied to detect myocardial injury indicators and inflammatory factor levels;HE staining was applied to observe pathological damage of heart;TTC staining was applied to detect myocardial infarction area;TUNEL staining was applied to detect cell apoptosis;Western blot was applied to detect expres-sions of RIP1-RIP3-MLKL pathway proteins.Results:Myocardial cells in Sham group were intact and arranged in an orderly manner;arrangement of myocardial cells was disordered and swollen,myofibril contracted,and sarcolemma was destroyed in MIR group,ejec-tion fraction(EF)and cardiac output(CO)level were obviously lower than Sham group(P<0.05),Mb,cTnⅠ,CK-Mb contents,IL-6,IL-18 levels,myocardial infarction area,myocardial cell apoptosis rate,RIP1,RIP3 and MLKL protein levels were obviously increased(P<0.05);compared with MIR group,Leo group had improved cell arrangement disorder and edema,and obviously increased EF and CO levels(P<0.05),Mb,cTnⅠ,CK-Mb contents,IL-6,IL-18 levels,myocardial infarction area,myocardial cell apoptosis rate,RIP1,RIP3 and MLKL protein levels were obviously reduced(P<0.05),trend of Compound 6i group was opposite;myocardial tissue structure of Leo+Compound 6i group was similar to MIR group,and Compound 6i eliminated cardioprotective effect of Leo on MIR rats.Conclusion:Leo may alleviate myocardial necrosis apoptosis in MIR rats by down-regulating RIP1-RIP3-MLKL pathway,thus playing a therapeutic role in MIR.

7.Clinical application of serum microRNA-198 and microRNA-365 combined with gastroscopy in diagnosis of chronic atrophic gastritis

Objective To analyze the value of serum microRNA-198(miRNA-198)and miRNA-365 combined with gastroscopy in diagnosis of chronic atrophic gastritis(CAG).Methods 180 patients with CAG who were treated from January 2022 to January 2023 were regarded as the observation group,and 180 patients with chronic non-atrophic gastritis in the same period were studied as control group.According to the degree of atrophy,patients with CAG were grouped into 56 mild cases,84 moderate cases and 40 severe cases.The subjects were examined by gastroscopy,and the relative expression levels of miRNA-198 and miRNA-365 in serum were detected by real-time fluorescent quantitative polymerase chain reaction(qRT-PCR).The diagnostic value of serum miRNA-198 and miRNA-365 levels in patients with CAG was analyzed by receiver operator characteristic curve(ROC curve).The diagnostic value of serum miRNA-198 and miRNA-365 combined with gastroscopy for CAG was analyzed with four grid tables.Results Compared with the control group,the levels of serum miRNA-198 and miRNA-365 in patients with CAG in the observation group were obviously lower,with the aggravation of atrophy,the levels of serum miRNA-198 and miRNA-365 decreased in turn,the differences were statistically significant(P<0.05);the area under the curve(AUC)of serum miRNA-198 and miRNA-365 levels in the diagnosis of CAG was 0.898 and 0.945,respectively,the sensitivity,specificity and accuracy of combined gastroscopy in the diagnosis of CAG were 98.33%,81.11%and 89.72%,respectively,its diagnostic efficacy was higher than that of serum miRNA-198,miRNA-365 and gastroscopy alone.Conclusion The levels of serum miRNA-198 and miRNA-365 in patients with CAG are decreased,they have certain diagnostic value for CAG,their combination with gastroscopy can obviously improve the diagnostic value of CAG.
8.Epidemic characteristics of mycoplasma pneumoniae infection and drug-resistant gene mutations in hospitalized children aged 0-17 in Ningbo City from 2019 to 2023

To analyze the infection and drug-resistant gene 23S rRNA mutations of mycoplasma pneumoniae (Mp) in hospitalized children aged 0-17 in Ningbo City from 2019 to 2023. Throat swabs were collected from hospitalized children with respiratory tract infections in Ningbo University Affiliated Women and Children′s Hospital from 2019 to 2023. They were subjected to real-time fluorescence quantitative polymerase chain reaction detection to analyze Mp infection and drug-resistant gene (23S rRNA) mutations. Intergroup comparisons were made by the Chi-square test or Fisher′s exact probability method. A total of 18 968 hospitalized children were included, with a total positive rate of 30.37% (5 760/18 968). The total positive rate of drug-resistant gene mutations was 82.45% (4 749/5 760). The positive rate of Mp in male children was 29.26%, which was lower than that in female children (31.67%, χ 2=12.948, P<0.001). The positive rate of Mp drug-resistant gene mutations in male children was 82.52%, which was higher than that in female children(82.37%, χ 2=0.021, P=0.885). The positive rates of Mp increased with age ( χ 2=1 722.21, P<0.001). The positive rates of Mp drug-resistant gene mutations also increased with age ( χ 2=13.152, P<0.001). In the four seasons, the total positive rate of Mp in summer and autumn was significantly higher than that in winter and spring ( χ 2=1 085.149, P<0.001). Among them, the Mp positive rates in the summer and autumn of 2019 were as high as 38.26% and 34.49%, while in the summer and autumn of 2020, the Mp positive rates were 2.55% and 1.65%, respectively, which were the lowest in previous years. In the summer and autumn of 2023, the Mp positive rates increased to 47.22% and 51.06%. There was no statistically significant difference in the detection rate of Mp drug-resistant gene mutations among the four seasons. In Conclusion, Mp infection was more prevalent in the summer and autumn in Ningbo city and females and children aged 7-17 were more susceptible. The epidemic of Mp infection in Ningbo occurred in the summer of 2019. After the COVID-19 pandemic in 2020, the positive rate of Mp rapidly decreased and later remained in a low incidence state. After the lifting of restrictive prevention and control measures in 2023, the Mp positive rate returned to an epidemic state. The positive rate of Mp drug-resistant gene (23S rRNA) mutations was relatively high.

10.Effects of pre-management led by specialist nurses in patients with minimally invasive lumbar fusion

Objective:To explore the effect of pre-management led by specialist nurses in minimally invasive lumbar fusion.Methods:From January to June 2022, 154 patients with degenerative lumbar diseases who were admitted to the Department of Spinal Orthopedics of Xinxiang Central Hospital for lumbar fusion were selected by convenience sampling. According to the method of random number table, the patients were divided into control group and observation group, with 77 cases each. The control group was given routine preoperative nursing, while the observation group carried out pre-management led by specialist nurses on the basis of the control group. The lumbar spine function (Macnab criteria) and Oswestry Disability Index (ODI) of the two groups were evaluated before and two weeks after the intervention, and the time of getting out of bed and the days of hospitalization after the operation were recorded and compared between the two groups.Results:The Macnab criteria of the two groups at two weeks after operation was higher than that before intervention, and the ODI score was lower than that before intervention, and the differences were statistically significant ( P<0.05) . Two weeks after operation, the Macnab criteria of the observation group was higher than that of the control group, and the ODI score was lower than that of the control group, with statistically significant differences ( P<0.05) . The time of getting out of bed and the days of hospitalization after operation in the observation group were shorter than those in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The pre-management led by specialist nurses can alleviate the postoperative low back pain of patients with minimally invasive lumbar fusion, promote the recovery of lumbar function, shorten the postoperative time of getting out of bed and hospital stay, and improve the postoperative rehabilitation of patients.
