BACKGROUND:Currently, numerous experiments delve into the intricate anatomy and biomechanical behavior of distinct segments of the supraspinatus muscle. However, the impact of shoulder joint stress resulting from damage to various regions of this muscle remains a scarcely explored domain.Understanding the repercussions of supraspinatus muscle injuries across different regions on the stress distribution and magnitude of articular cartilage and the glenoid is crucial for providing some theoretical support for clinical diagnosis and treatment. OBJECTIVE:To ascertain the maximum stress values by simulating different degrees of supraspinatus muscle rupture on the humeral cartilage surface, glenoid lip,and glenoid cartilage joint surface using three-dimensional finite element software. METHODS:Normal and healthy shoulder joint CT or MRI scans were processed through Mimics and Geomagic to extract molds.Subsequently, models were constructed via Solidworks.Varying degrees of supraspinatus muscle damage were simulated for each model to mimic fractures in different regions. Finally,Ansys, mechanical software, was employed for three-dimensional finite element biomechanical analysis, calculating stress values for the humeral cartilage surface, glenoid lip,and glenoid cartilage joint surface. RESULTS AND CONCLUSION:(1)With worsening degrees of supraspinatus muscle injury, the stress on the shoulder joint cartilage surface and glenoid lip escalated.(2)Among various regions, the anterior part of the supraspinatus muscle exhibited paramount significance.(3)While supraspinatus muscle fractures of differing degrees impacted the magnitude of cartilage stress on the glenoid labial surface, the stress distribution remained constant.(4)It is indicated that during the initial stages of horizontal abduction of the shoulder joint, the anterior region assumes a pivotal role, followed by the posterior deep region.Injury to the anterior part of the supraspinatus muscle leads to a significant surge in stress within the shoulder joint's soft tissue, potentially causing damage to the top of the glenoid lip and the anterior part of the glenoid cartilage.

ObjectiveTo understand the surveillance of influenza-like illness (ILI) and influenza vaccination status in Jing’an District, Shanghai, and to provide a basis for optimizing influenza prevention and control strategies. MethodsThe sentinel surveillance data for ILI and virological surveillance data of influenza viruses in Jing’an District were collected from the Chinese influenza surveillance information system, and data for influenza vaccination were collected from Shanghai immunization information system from September 2017 to August 2023. Epidemiological characteristics of ILI, influenza etiology, and the temporal and population distributions of influenza vaccination were analyzed using descriptive epidemiological methods. ResultsILI as a percentage of total visit surveillance units (ILI%) reported by sentinel hospital was increased in Jing’an District of Shanghai from September 2017 to August 2023 (F=18.841, P=0.012). The peak of the influenza cases mainly appeared in winter-spring, but there were two peaks in winter-spring and summer from September 2019 to August 2020, from September 2020 to August 2021, and from September 2021 to August 2022. In particular, there were two peaks in winter-spring from September 2022 to August 2023, with a rebound during the descending process. The average positive rate of ILI was 21.64% (2 421/11 189) during the 6 years. There was a peak in winter-spring during every year with the exception of the period from September 2020 to August 2021. The dominant strains were B/Yamagata and A/H1N1 in winter-spring from September 2017 to August 2018. The dominant strain was A/H1N1 in winter-spring from September 2018 to August 2019 and from September 2022 to August 2023. The dominant strain was B/Victoria in winter-spring from September 2019 to August 2020 and from September 2021 to August 2022. Different subtype strains occurred alternately, and the dominant strains were A/H1N1 and A/H3N2 in recent years. The influenza vaccination coverage was 2.94% from September 2017 to August 2023, and the vaccination coverage was highest in young children. The vaccination coverage for females was higher than that for males (χ2=546.963, P<0.001), and the vaccination coverage for registered residents was higher compared to that for migrants (χ2=123.141, P<0.001). ConclusionILI% exhibits an upward trend in Jing’an District of Shanghai, and the dominant strain is A subtype. The influenza vaccination coverage is still low, which is insufficient to have an impact on the spread of influenza. It is recommended that the surveillance of ILI and variations of influenza virus strains should be improved continuously, and effective steps should be taken to promote influenza vaccination.

This study investigated the expression of the soluble nontoxic mutant CRM197 of diphtheria toxin,to prepare a safe and effective recombinant CRM197 protein.Codon-optimized CRM197 gene sequences were cloned into the pCold Ⅱ and pET-28a(+)prokaryotic expression vectors. The successfully cloned recombinant plasmids were screened and transformed into Escherichia coli BL21(DE3)competent cells. After induction of expression, the protein expression type was determined through SDS-PAGE analysis. The conditions for expression of the solublerecombinant protein were then optimized. The recombinant CRM197 protein was purified through two rounds of Ni-NTA affinity chromatography. The purified protein was used to immunize mice, and antibody levels in the se-rum were measured with ELISA.The codon adaptation index(CAI)of the optimized sequence increased from 0.72 to 0.93.The recom-binant plasmids pET-28a(+)-CRM197 and pCold Ⅱ-CRM197 were successfully constructed,as confirmed through colony PCR and double digestion.Expression analysis revealed that pET-28a(+)-CRM197 was expressed primarily as inclusion bodies, whereas pColdⅡ-CRM197 was expressed predominantly in soluble form. The conditions for soluble protein expression via pCold Ⅱ-CRM197 were optimized. When the inoculum was 3%and the IPTG concentration was 0.4 mmol/L,induction at 20 ℃ for 24 h significantly increased the expression of the soluble target protein. The pCold Ⅱ-CRM197 recombinant protein was purified from the supernatant with Ni-NTA affinity chromatography, thus resulting in a target protein with a purity greater than 98%.ELISA after three rounds of immuniza-tion indicated that the levels of IgG,IgM,IgG1,and IgG2a antibodies in the serum in immunized mice were significantly higher than those in the control group. In summary, the CRM197 recombinant protein was successfully prepared with the pCold Ⅱvector and exhib-ited high soluble expression, high purity, and favorable immunogenicity.

With the holistic concept of traditional Chinese medicine(TCM)as the foundation,Professor ZHOU Yunfeng proposes the three-part Tuina(Chinese therapeutic massage)manipulation to treat insomnia by"taking the head and brain as the essential and simultaneously regulating the abdomen and back".Taking the theories of Ying-nutrient and Wei-defensive Qi,Zang-Fu organs, and meridians as evidence, this method primarily regulates the mind and concurrently modulates Zang-Fu organs. The three-part Tuina manipulation regulates the spirit by grasping the five meridians(Governor Vessel,Bladder Meridian of Foot Taiyang, and Gallbladder Meridian of Foot Shaoyang)and stimulating points such as Baihui(GV20),Yintang(GV29),Taiyang(EX-HN5),and Shenting(GV24);it regulates the abdomen by selecting Zhongwan(CV12),Shenque(CV8),Qihai(CV6),and Guanyuan(CV4),which means to calm the mind by regulating spleen-stomach Qi activities;it treats the back by selecting Jianjing(GB21),Xinshu(BL15),Pishu(BL20),Weishu(BL21),and Shenshu(BL23),which means to calm the mind by regulating Qi activities of the whole body.Mind regulation runs through the whole process of this manipulation, which combines points from three body regions to produce a synergistic effect, concurrently treating the three body parts,i.e.,the head, abdomen, and back,to mitigate the clinical symptoms and improve sleep quality in insomniacs.

Aim To investigate the association of triglyceride-glucose(TyG)index with microvascular obstruction(MVO)after percutaneous coronary intervention(PCI)in patients with ST-segment elevated myocardial infarction(STEMI).Methods Individual patient-data were pooled from 310 patients with STEMI underwent emergency PCI in Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School from June 2018 to June 2021 for a prospective analysis.A week following the operation, cardiac magnetic resonance imaging was used to evaluate the MVO region and the patients were divided into two groups based on whether MVO occured after PCI:the MVO group(n=183)and the non-MVO group(n=127).The clinical data of the two groups were compared, and the linear relationship between TyG index and MVO was described using restricted cubic splines(RCS).Univariate and multivariate Logistic regression analysis were used to correct for confounding factors and identify independent risk factors for MVO occurrence. Results After adjustment for confounding factors,TyG index was an independent risk factor for MVO after emergency PCI in STEMI patients, and every 1-unit increased in TyG index, the risk of MVO increased by 1.24 times(OR=2.24,95%CI:1.07~4.71,P=0.033).The RCS curve analysis results showed that there was a linear re-lationship between the TyG index and the occurrence of MVO after emergency PCI(non-linear correlation test P=0.47).When the TyG index was greater than 9.5,the risk of MVO after emergency PCI significantly increased. Conclusion An increased TyG index is postively associated with the incidence of MVO in STEMI patients who have undergone PCI,and has clinical significantce for early prevention and risk stratification of MVO in STEMI patients.

Chinese Journal of Sports Medicine 2025;44(7):521-529
Objective To explore the influence of different surgical methods on the stress of knee me-niscus and patellofemoral joint surface of simulated episodic patellar dislocation using three-dimension-al finite element software under different flexion conditions. Methods The knee joint CT and MRI of pa-tients with episodic patellar dislocation were acquired and imported into Mimics and Geomagic to ex-tract the model, followed by the 0°,30°,60°,90° and 120° buckling construction using the Soild-works software. For each model, medial patellofemoral ligament reconstruction, medial retinaculum pli-cation,lateral retinaculum release, medial patellofemoral and patellotibial ligament reconstruction,as well as medial patellofemoral ligament reconstruction and lateral retinaculum release were conducted re-spectively.Finally,they were imported into the mechanical software Ansys for biomechanical analysis, and the stress results of knee meniscus and patellofemoral joint surface were output. Results The medi-al patellofemoral and patellotibial ligament reconstruction had the smallest meniscus peak stress com-pared with other methods. Moreover,it also exerted the smaller peak stress on the patellofemoral joint surface, along with the medial patellofemoral ligament reconstruction and the lateral retinaculum re-lease.Conclusion The medial patellofemoral and patellotibial ligament reconstruction can provide good patellofemoral contact stress and meniscus stress, effectively restore the stability of the knee joint.How-ever,medial patellofemoral ligament reconstruction and lateral retinaculum release can also provide bet-ter patellofemoral joint stress, but the meniscus stress is slightly higher,so we should be alert to the complications related to the meniscus after surgery.
