ObjectiveTo evaluate the clinical efficacy of Huayu Tongluo Formula (化瘀通络方, HTF) in patients with mid-stage diabetic kidney disease of blood stasis syndrome and explore its potential mechanisms. MethodsA multi-center, randomized, double-blind, placebo-controlled clinical trial was conducted. Ninety patients of mid-stage diabetic kidney disease of blood stasis syndrome were divided into a control group of 46 cases and a treatment group of 44 cases. Both groups received conventional western medicine treatment, the treatment group additionally taking HTF, while the control group taking a placebo of the formula. The treatment was administered once daily for 24 weeks. The primary outcomes included 24-hour urine total protein (24 h-UTP), serum albumin (Alb), glycated hemoglobin (HbA1c), and serum creatinine (Scr). The secondary outcomes included changes in levels of endothelin-1 (ET-1), nitric oxide (NO), vascular endothelial growth factor (VEGF), and traditional Chinese medicine (TCM) syndrome scores before and after treatment. Clinical efficacy was evaluated based on TCM syndrome scores and overall disease outcomes. Adverse reactions and endpoint events were recorded. ResultsIn the treatment group after treatment, 24 h-UTP, ET-1, and VEGF levels significantly decreased (P<0.05), Alb and NO levels significantly increased (P<0.05); while the TCM syndrome scores for edema, lumbar pain, numbness of limbs, dark purple lips, dark purple tongue or purpura, and thin, rough pulse all significantly decreased (P<0.05). In the control group, no significant changes were observed in any of the indicators after treatment (P>0.05).Compared with the control group, the treatment group showed significant reductions in 24 h-UTP, ET-1, and VEGF levels, and increases in Alb and NO levels (P<0.05). The TCM syndrome scores for edema, lumbar pain, dark purple tongue or purpura, and thin, rough pulse were all lower in the treatment group than in the control group (P<0.05). The total effective rate of TCM syndrome in the treatment group was 59.09% (26/44), and the overall clinical effective rate was 45.45% (20/44). In the control group, these rates were 15.22% (7/46) and 8.7% (4/46), respectively, with the treatment group showing significantly better outcomes (P<0.05). A total of 7 adverse events occurred across both groups, with no significant difference (P>0.05). No endpoint events occurred during the study. ConclusionOn the basis of conventional treatment of Western medicine, HTF can further reduce urinary protein levels and improve clinical symptoms in patients with mid-stage diabetic kidney disease of blood stasis syndrome. The mechanism may be related to its effects on endothelial function.

Chinese Journal of Tissue Engineering Research 2025;29(2):302-311
BACKGROUND:At present, the pathogenesis of osteoarthritis is still unclear, and there is a lack of effective means to control the disease.Research on osteoarthritis is mostly concentrated in the field of immunity, and there are few studies in the field of oxidative stress. OBJECTIVE:To explore the roles of oxidative stress and immune infiltration in osteoarthritis and to predict related miRNAs and therapeutic agents. METHODS:The GSE55235 dataset(10 samples of osteoarthritis and 10 healthy control samples)and the GSE55457 dataset(10 samples of osteoarthritis and 10 healthy control samples)were obtained from the GEO database for merging to obtain their differentially expressed genes that were combined with oxidative stress genes to get the differentially expressed genes of oxidative stress. The differentially expressed genes of oxidative stress were analyzed for KEGG and GO enrichment, and the osteoarthritis pathways and biological processes were evaluated using GSEA enrichment analysis. The protein-protein interaction network was constructed using the STRING online website and Cytoscape software, and the Degree algorithm was run to get the key genes. The GSE1919 dataset was obtained from the GEO database as a validation dataset, and the key genes were analyzed by variance analysis and receiver operating characteristic curve analysis to get the core genes. In addition, immune infiltration was evaluated by CIBERSORT and the correlation between core genes and immune cells was explored.miRNA prediction of core genes was performed using TargetScan and target drugs were predicted using the DSigDB database. RESULTS AND CONCLUSION:Sixty-five differentially expressed genes and five core genes(IL1B,CXCL8,MYC,NFKBIA,JUN)associated with oxidative stress were identified.Enrichment analysis showed that differentially expressed genes associated with oxidative stress were concentrated in the pathways of oxidative stress,interleukin-17,osteoclast differentiation, fluid shear stress and atherosclerosis. The area under the receiver operating characteristic curve for the five core genes exceeded 0.85,indicating their excellent specificity and sensitivity in diagnosing bone and joint conditions,as well as their close association with immune cells. The predicted miRNA was has-miR-3937,and the therapeutic small-molecule drugs were metformin, ionomycin and celecoxib. To conclude, oxidative stress and immune infiltration exist in osteoarthritis, and immune infiltration is involved in activating oxidative stress. The core genes and predicted miRNAs can be used as novel markers for the diagnosis of osteoarthritis, and small molecule drugs are predicted to treat osteoarthritis.

BACKGROUND:The mechanisms underlying the occurrence of pressure injuries are complex, and it is not entirely clear which factors play a central role in the development of pressure injuries and how these factors operate. OBJECTIVE:To investigate the relationship between Piezo-type mechanosensitive ion channel component 1(Piezo1)and the occurrence of pressure injuries. METHODS:(1)Cellular experiment:Human immortalized keratinocytes(HaCaT)were treated with Yoda1,a Piezo1 agonist,at different concentrations.Cell viability, calcium ion influx,Piezo1,and apoptosis-related protein expression were detected.(2)Animal experiment:Twelve Sprague-Dawley rats were randomly divided into a control group and three experimental groups, with three rats in each group. The control group was not subjected to pressure, while in the three experimental groups, magnets with a thickness of 1,2,and 3 mm were used to press on both sides of the rats'back for 1 hour, respectively,to establish the animal models of pressure injuries. After modeling, all traumatic tissues were excised and subjected to hematoxylin-eosin,Masson,immunofluorescence staining and western blot assay. RESULTS AND CONCLUSION:Cellular experiments:The results of live/dead cell staining showed that HaCaT cell apoptosis increased with the increase of Yoda1 concentration(0,2.5,5,and 10 μmol/L),and calcium ion influx increased with the increase of Yoda1 concentration(0,5,and 10 μmol/L),as well as with the prolongation of treatment time.Western blot assay results showed an increase in the expression of BAX,TG2,and PIEZO1 and a decrease in the expression of the expression of Bcl-2 protein in HaCaT cells in 5 and 10 μmol/L Yoda1 groups compared with the control group(0 μmol/L Yoda1).Animal experiments:The results of hematoxylin-eosin and Masson staining showed that the skin structure of the three experimental groups was damaged at the compression site, there was subcutaneous fat liquefaction and necrosis, and collagen was sparse and disorganized, and damage to the skin structure at the compression site was aggravated with the increase of magnet thickness.Immunofluorescence staining and western blot results showed that compared with the control group, the expression of BAX,TG2,Yap1 and PIEZO1 proteins was elevated, and the expression of Bcl-2 proteins was lowered in the three experimental groups. Moreover, the expression of related proteins showed more significant changes with the increase of magnet thickness(pressure). To conclude, skin compression activates PIEZO1,leading to a significant influx of calcium ions.As the pressure increases, this ultimately results in cell apoptosis due to calcium overload.

BACKGROUND:In recent years, deep learning technologies have been increasingly applied in the field of oral medicine, enhancing the efficiency and accuracy of oral imaging analysis and promoting the rapid development of intelligent oral medicine. OBJECTIVE:To elaborate the current research status, advantages, and limitations of deep learning based on oral imaging in the diagnosis and treatment decision-making of oral diseases,as well as future prospects, exploring new directions for the transformation of oral medicine under the backdrop of deep learning technology. METHODS:PubMed was searched for literature related to deep learning in oral medical imaging published from January 2017 to January 2024 with the search terms"deep learning, artificial intelligence, stomatology, oral medical imaging."According to the inclusion criteria,80 papers were finally included for review. RESULTS AND CONCLUSION:(1)Classic deep learning models include artificial neural networks, convolutional neural networks, recurrent neural networks, and generative adversarial networks.Scholars have used these models in competitive or cooperative forms to achieve more efficient interpretation of oral medical images.(2)In the field of oral medicine, the diagnosis of diseases and the formulation of treatment plans largely depend on the interpretation of medical imaging data.Deep learning technology, with its strong image processing capabilities, aids in the diagnosis of diseases such as dental caries, periapical periodontitis, vertical root fractures, periodontal disease, and jaw cysts,as well as preoperative assessments for procedures such as third molar extraction and cervical lymph node dissection, helping clinicians improve the accuracy and efficiency of decision-making.(3)Although deep learning is promising as an important auxiliary tool for the diagnosis and treatment of oral diseases,it still has certain limitations in model technology, safety ethics, and legal regulation.Future research should focus on demonstrating the scalability, robustness, and clinical practicality of deep learning, and finding the best way to integrate automated deep learning decision support systems into routine clinical workflows.

OBJECTIVE To evaluate the safety of fondaparinux in pregnancy and provide reference for its rational clinical application. METHODS A search was conducted in databases including CNKI, Wanfang, PubMed, Embase, and Elsevier (the search time was from the construction of the database to December 17, 2024) to collect case report literature on fondaparinux use during pregnancy. Patient demographic information, fondaparinux use during pregnancy, concomitant medications, clinical manifestations, and treatment details were extracted for descriptive statistical analysis. RESULTS A total of 17 case reports regarding the use of fondaparinux during pregnancy were collected, involving 42 patients from 11 countries and 47 pregnancy records. Among these, 20 cases involved the use of fondaparinux for the prevention of pregnancy-related venous thromboembolism (VTE), while 27 cases were fondaparinux treatment due to related conditions. A total of 29 occurrences of the patients were treated with fondaparinux due to a (family) history of VTE. Nine occurrences of complicated pregnancies were reported, and 35 patients had records of comorbidities or relevant medical histories. The adverse events that occurred during pregnancy with the use of fondaparinux include postpartum hemorrhage (7 cases) and excessive anticoagulation caused by inappropriate dosage (1 case). Among the 7 cases of postpartum hemorrhage, 3 cases had a blood loss of no less than 1 000 mL (including 2 cases with uterine atony), 3 cases had a drug discontinuation time of ≤12 h. CONCLUSIONS Based on the existing literature, the safety of fondaparinux during pregnancy is generally manageable, with the main adverse event being postpartum hemorrhage. The dosage, interval between discontinuation,comorbidities/medical history, and concomitant medications of fondaparinux may be the main causes of its adverse events.

Gene regulation relies on the precise binding of transcription factors (TFs) at regulatory elements, but simultaneously detecting hundreds of TFs on chromatin is challenging. We developed cFOOT-seq, a cytosine deaminase-based TF footprinting assay, for high-resolution, quantitative genome-wide assessment of TF binding in both open and closed chromatin regions, even with small cell numbers. By utilizing the dsDNA deaminase SsdAtox, cFOOT-seq converts accessible cytosines to uracil while preserving genomic integrity, making it compatible with techniques like ATAC-seq for sensitive and cost-effective detection of TF occupancy at the single-molecule and single-cell level. Our approach enables the delineation of TF footprints, quantification of occupancy, and examination of chromatin influences on TF binding. Notably, cFOOT-seq, combined with FootTrack analysis, enables de novo prediction of TF binding sites and tracking of TF occupancy dynamics. We demonstrate its application in capturing cell type-specific TFs, analyzing TF dynamics during reprogramming, and revealing TF dependencies on chromatin remodelers. Overall, cFOOT-seq represents a robust approach for investigating the genome-wide dynamics of TF occupancy and elucidating the cis-regulatory architecture underlying gene regulation.

9.Acetylation modification and chronic obstructive pulmonary disease
Basic & Clinical Medicine 2025;45(4):536-541
The main features of chronic obstructive pulmonary disease include chronic airway inflammation,persis-tent airflow limitation, progressive dyspnea, and impaired lung function.Acetylation is one of the most common post-translational modifications of proteins, which plays an important role in the pathogenesis of chronic obstructive pulmonary disease, such as oxidative stress, inflammatory reaction, and airway remodeling,by regulating gene ex-pression,cell signaling, and protein function. Therefore, exploring the regulatory mechanism of acetylating modifi-cation in chronic obstructive pulmonary disease can not only deeply understand the pathophysiological mechanism of the disease, but also provide a theoretical basis for the development of new treatment strategies.
