1.Genetic detection for hereditary cancer syndrome among general population

Objective To examine the significance of susceptible gene detection for hereditary cancer syndrome (HCS) among general population. Methods A total of 2 928 individuals undergoing routine health examinations in Healthcare Center of Zhongshan Hospital, Fudan University, from September 2021 to April 2024 were enrolled retrospectively. Next generation sequencing was employed to identify susceptible genes for HCS. American College of Medical Genetics and Genomics (ACMG) guideline was used to analyze the pathogenicity of variants. Clinical data, imagings, follow-up data were also collected. Results The overall mutation rate of HCS panel was 3.59% (105/2 928), with 0.61% (18/2 928) for MutY DNA glycosylase (MUTYH), 0.27% (8/2 928) for breast cancer susceptibility gene 1/2 (BRCA1/2) and 0.23% (7/2 928) for mismatch repair (MMR) genes. Conclusions Healthy individuals carrying tumor susceptible genes usually lack the relevant clinical phenotypes. Whether comprehensive testing needs to be carried out among healthy people remains to be further explored.
2.ApoB, ApoB/LDL-C, and non-HDL-C level changes at a hospital in Shanghai from 2014 to 2024: data analysis of a large sample with over 1.6 million cases

Objective:To analyze the variation trends in the levels of apolipoprotein B (ApoB), the ratio of apolipoprotein B to low-density lipoprotein cholesterol (ApoB/LDL-C), and non-high density lipoprotein cholesterol (non-HDL-C) among patients and individuals undergoing physical examinations at Zhongshan Hospital, Fudan University from 2014 to 2024.Methods:Data on ApoB, LDL-C, and nonHDL-C levels were collected from individuals (the overall population) who visited or underwent physical examinations at our hospital from January 1, 2014 to December 31, 2024, as well as from individuals (the fixed population) who were tested annually over the 11-year period. The Mann-Kendall test combined with Sen slope estimation was used to analyze the trends. The abnormal rates of ApoB and non-HDL-C in both the overall and fixed populations, as well as the changes in the first and last test results over the 11 years in the fixed population, were also analyzed.Results:The overall population totaled 1 679 440 cases, aged 57 (45, 67) years, and 718 738 cases (42.8%) were female, of which 1 250 234 cases (74.4%) were in the patient population and 429 206 cases (25.6%) were in the health check population. The fixed population of 1 560 cases was 56 (45, 65) years old and 655 cases (42.0%) were female, of which 1 044 cases (66.9%) were in the patient population and 516 cases (33.1%) in the health check population. Between 2014 and 2024, the overall population ApoB decreased from 0.842 g/L to 0.822 g/L (Sen slope -0.001 g·L -1·year -1, P=0.01), the abnormality rate decreased from 24.70% to 22.03% (Sen slope -0.50%/year, P=0.06), and 69.4% of the fixed population showed no change in the subgroups of initial and final tests. ApoB/LDL-C increased from 0.824 to 0.868 (Sen slope -0.003/year, P=0.72). Non-HDL-C decreased from 3.46 mmol/L to 3.32 mmol/L (Sen slope 0.018 mmol·L -1·year -1, P=0.35), with a statistically significant upward trend since 2019 (Sen slope 0.037 mmol·L -1·year -1, P=0.008), and the anomaly rate decreased from 24.3% to 22.7% (Sen slope 0.55%/year, P=0.64), with a statistically significant upward trend since 2019 (Sen slope 1.53%/year, P=0.014), and no change in the grouping of the first and last tests in 75.0% of the fixed population. The trends in the above items were consistent between the overall population and the fixed population. 20-<40 year olds had the most significant upward trend in non-HDL-C along with the least significant downward trend in ApoB/LDL-C and ApoB. Conclusion:ApoB in the overall and fixed populations from 2014-2024 in a Shanghai hospital showed a decreasing trend, and non-HDL-C and abnormality rates showed an increasing trend from 2019 onwards.
3.Clinical significance of germline multi-gene panel testing in patients with endometrial cancer

Objective:To analyze the relationship between clinicopathological features and germline mismatch repair (MMR) gene variants in endometrial cancer patients and to evaluate the clinical utility of germline multi-gene panel testing.Methods:This single-center, retrospective case series study included 100 endometrial cancer patients treated in Zhongshan Hospital, Fudan University between July 2022 and February 2024. We collected clinicopathological data and tumor molecular testing results. 61 cancer susceptibility genes were tested using next-generation sequencing, and the associations between the detection rate of germline variants and the clinicopathological characteristics in endometrial cancer patients were explored.Results:Among 100 patients, 28% (28/100) were found to have pathogenic variants in cancer susceptibility genes, of which 20 patients carried germline MMR gene variants and the remaining 8 patients carried variants in other cancer susceptibility genes. Of the 20 patients diagnosed with Lynch syndrome, only 40% (8/20) met the Chinese family history criteria for Lynch syndrome. Among 53 patients with intact MMR protein expression, 1 patient was identified with a germline MMR gene variant. In the 14 Lynch syndrome patients with confirmed microsatellite status, 5/14 of those showed low microsatellite instability or microsatellite stability. Germline multi-gene panel testing in all endometrial cancer patients additionally identified 1 Lynch syndrome patient and 8 patients with non-Lynch hereditary cancers.Conclusion:Current clinical screening criteria may miss some endometrial cancer patients with Lynch syndrome. Compared with traditional screening pattern, germline multi-gene panel testing not only improves the detection rate of Lynch syndrome in endometrial cancer patients but also identifies other hereditary cancer predispositions.
4.Value of MALDI-TOF MS direct-on-target growth assay in diagnosis of carbapenem-resistant Enterobacteriaceae

OBJECTIVE To evaluate the value of matrix-assisted laser desorption ionization time-of-flight-mass spectrometry(MALDI-TOF MS)in direct identification of carbapenem-resistant Escherichia coli and Klebsiella pneumoniae positive for blood culture.METHODS The blood culture bottles that were positive for E.coli or K.pneumoniae were collected from the patients with bloodstream infection who were treated in Zhongshan Hospi-tal,Fudan University from Jul.2021 to Jun.2023.The isolates were collected by using a gel-contact clotting tube,then the tested strains were respectively mixed with 4 μg/ml of imipenem,4 μg/ml of meropenem and 2 μg/ml of ertapenem for coculture and were incubated at 35 ℃ for 4 and 5 hours,finally,the strains were i-dentified by using the mass spectrum.The minimum inhibitory concentrations(MICs)of the three types of drugs were tested by microbroth dilution method and were set as the golden standards for the test.RESULTS Totally 31 strains of E.coli and 28 strains of K.pneumoniae that were positive in blood culture bottles were collected,both of the effective rates of controlled growth of the E.coli strains were 100.00%after the incuba-tion for 4 and 5 hours,and the effective rates of controlled growth of the K.pneumoniae strains were 9 6.43%and 100.00%after the incubation for 4 and hours,respectively.The sensitivity,specificity,positive predictive value and negative predictive value of the K.pneumoniae strains against the three types of drugs were 100.00%after the incubation for 5 hours.The specificity and positive predictive value of the E.coli strains against imipenem,meropenem and ertapenem were 100.00%after the incubation for 5 hours;the sensitivities were 73.58%,78.93%and 78.93%,respectively;the negative predictive values were 70.60%,75.00%and 75.00%,respectively.CONCLUSIONS MALDI-TOF MS is rapid and accurate for direct identification of the carbapenem-resistant E.coli and K.pneumoniae strains positive in blood culture bottles,and the accuracy reaches at 100.00%for the test of drug-resistant K.pneumoniae strains after the incubation for 5 hours.The method may provide evidence for clinical treatment of bloodstream infections induced by carbapenem-resistant Enterobacteriaceae.
5.Research on the application of PDCA cycle in the management of low-value medical consumables

Objective To explore the application value of the Plan-Do-Check-Act(PDCA)cycle in the refined whole-process management of low-value medical consumables.Methods Using the PDCA cycle as a framework,the Plan phase in-volved analyzing the causes of traceability difficulties for low-value medical consumables.The Do phase implemented multiple im-provement measures,including establishing standardized management systems,strengthening personnel supervision,creating a split-item tertiary inventory,and introducing the Unique Device Identification(UDI)system.The Check phase evaluated man-agement effectiveness by comparing data from June to December 2023 and June to December 2024.The Act phase refined institu-tional norms and operational processes through a closed-loop feedback mechanism.Results After PDCA management,the appli-cation time for low-value medical consumables[(6.16±1.47)min]and the collection time[(15.08±1.89)min]were signif-icantly shorter than before implementation(P<0.001).The acceptance qualification rate increased from 92.96%to 99.11%.The application of the split-item tertiary inventory ensured synchronization between inventory deduction and billing data.Conclu-sion The PDCA cycle improves the traceability of low-value medical consumables through systematic quality improvement.Its dynamic feedback mechanism supports continuous improvement,providing an effective pathway for hospitals to achieve refined management and cost control.
6.Assessment of the Feasibility of Bloodstream Infection Diagnosis with A 4-Day Incubation Time Setting in the BacTALERT VIRTUO Blood Culture System

Objective To evaluate the impact of setting a 4-day incubation time on the detection performance of bloodstream infection(BSI)pathogens by the BacTALERT VIRTUO blood culture system(VIRTUO).Methods Retrospectively collecting blood culture(BC)results from Zhongshan Hospital,Fudan University,from June 2021 to December 2023,and comparing the time-to-detection for clinically significant microorganisms.Results A total of 87 052 BC bottles were collected,of which 7 167(8.23%)were positive.95%of aerobic and anaerobic bottles were reported positive at 65.67 h and 91.2 h respectively.The average(median)time-to-detection for Staphylococcus aureus(S.aureus)was 21.76(16.08)h,Klebsiella pneumoniae(KP)was 15.32(11.65)h,Escherichia coli(E.coli)was 16.14(10.87)h,and Candida species was 35.2(28.5)h.Only 213 bottles(0.24%)were reported positive after 4 days of incubation;among them,88 bottles(41.31%)cultured Propionibacterium acnes(P.acnse).Medical chart reviews for BC reported positive after 96 h(4 days)showed no clinical significance.Simulated growth experiments were conducted on 11 fastidious bacteria,and the median detection time was only 10.8 h,indicating that an incubation time of 4 days is sufficient for fastidious bacteria.Conclusion A 4-day incubation is adequate for VIRTUO system.Reporting 1 day earlier to the doctors can reduce the positive rate of contaminants and the use of antimicrobial agents.
