1.Research on the current status and influencing factors of post ICU syndrome in severe patients

Objective To explore the current situation and influencing factors of post-intensive care syndrome(PICS)in critically ill patients a month after their transfer out,and to provide a basis for formulating individualized intervention measures.Methods By the convenience sampling method,550 patients who were transferred out of the ICU of a tertiary A hospital in Guizhou Province for a month from April to November 2024 were selected as the survey subjects.The general information of the patients was collected and sorted out.During their stay in the ICU,the Barthel Index,the Richards Campbell Sleep Scale,and the Perceived Social Support Scale were employed for evaluation.A month after the patients were transferred out of the ICU,the assessment was completed through telephone follow-up using the Chinese version of the Brain Care Monitoring Questionnaire for Healthy Aging.According to the assessment a month after the patients were transferred out,they were divided into a PICS group and a non-PICS group.Univariate analysis and binary Logistic regression analysis were completed based on the data of the 2 groups.Results A total of 550 questionnaires were distributed,and 442 valid questionnaires were ultimately collected,with a valid questionnaire collection rate of 80.3%.Among them,194 cases(43.9%)were high-risk patients with PICS.Binary logistic regression analysis showed that advanced age,pulmonary infection,total hospitalization time>15 days,and internal environment disorder were the risk factors for PICS in ICU patients(P<0.05).Conclusion The risk of PICS is relatively high in critically ill patients a month after the transfer.Medical staff need to pay attention to patients who are older,have pulmonary infections and internal environment disorders,and formulate personalized intervention measures for different disease types to improve the long-term health prognosis of patients.
2.Regression mixture analysis of the relationship between academic burnout and psychological resilience in high school students

Objective:To investigate the latent classes of academic burnout and its relationship with psycho-logical resilience in high school students.Methods:The Academic Burnout Scale for High School Students(ABS-HSS)and the Resilience Scale for Chinese Adolescents(RSCA)were used to investigate the current status of aca-demic burnout and the levels of psychological resilience among 786 high school students,respectively.The latent class analysis(LCA)was conducted to explore the latent classes of academic burnout among high school students,and this study further examined the relationship between academic burnout and psychological resilience by regres-sion mixture modeling(RMM).Results:There were three latent classes,including the low burnout group(69.9%),the low achievement group(9.8%)and the overwhelmed group(20.3%).The regression mixture anal-ysis further indicated that the high school students with lower psychological resilience experienced higher academic burnout(OR=0.22-0.56),and the students with difficulty in emotional control mostly felt overwhelmed in their learning(OR=0.23).Conclusion:These findings suggest that the academic burnout among high school students exhibits distinct categorical characteristics and is closely assoiated with psychological resilience.
3.Association between nocturnal blood pressure decline rate and the risk of chronic heart failure

Objective To explore the association between nocturnal blood pressure decline rate and chronic heart failure(CHF).Methods In this study,a total of 711 individuals who underwent 24-h ambulatory blood pressure monitoring were divided into control group(n=433)and CHF group(n=278).The characteristics of the study population and nocturnal blood pressure decline rate were compared between CHF group and control group.Restricted spline analysis was utilized to examine the linear or non-linear association between nocturnal blood pressure decline rate and the prevalence of CHF.The relationship between nocturnal blood pressure decline rate and CHF was further investigated based on multivariable Logistic regression model.Results Nocturnal systolic blood pressure decline rate was significantly different in CHF group and control group[(3.3±7.1)mmHg vs.(5.0+6.7)mmHg,P=0.001].No significant difference in nocturnal diastolic blood pressure was found between the two groups[(5.7±7.6)mmHg vs.(6.8±7.2)mmHg,P=0.061].Restricted spline analysis showed a significant linear association between nocturnal systolic blood pressure decline rate and CHF risk.After adjusting for age,sex,smoking,body mass index,24-hour systolic blood pressure,24-hour diastolic blood pressure,type 2 diabetes mellitus and hyperlipidemia,multivariate Logistic regression analysis revealed that nocturnal systolic blood pressure decline rate was negatively associated with the prevalence of CHF(OR=0.97,95%CI:0.95-0.99;P=0.010).However,no significant association was found between nocturnal diastolic blood pressure decline rate and CHF risk.Conclusion Nocturnal systolic blood pressure decline rate is associated with the prevalence of CHF.Individuals with decreased nocturnal systolic blood pressure decline have a significantly increased risk of CHF.
4.Inhibitory effect of phillyrin on myocardial fibrosis in diabetes mellitus rats by activating PI3K/AKT/eNOS signaling pathway

Objective:To study the inhibitory effect of phillyrin(PN)on myocardial fibrosis in diabetes mellitus(DM)rats and its mechanism.Methods:Sixty SD rats were randomly assigned:control group,DM group,low-dose PN group(10 mg/kg),high-dose PN group(30 mg/kg)and LY294002 group(PN 30 mg/kg+PI3K inhibitor 100 mg/kg),with 12 rats in each group,DM model was constructed by streptozotocin.After 8 weeks of drug intervention,FBG was measured with a blood glucose meter,the LVFS,LVEF,LVPWT and E/A were measured by echocardiography;HE staining and Masson staining were used to changes of myocardium,and the CVF was measured;ELISA was used to detect the Vaspin,TNF-α,IL-1β and IL-6 in myocardium;Western blot was used to detect the expressions of α-SMA,FSP-1,CD31,COL1,PI3K,p-PI3K,AKT,p-AKT,eNOS and p-eNOS in myocardium.Results:Compared with the control group,the myocardial cells in DM group were swollen,gaps were enlarged,and collagen fibers were in-creased,the levels of FBG,CVF,Vaspin,TNF-α,IL-1β,IL-6,α-SMA,FSP-1 and COL1 were increased obviously,the levels of LVFS,LVEF,LVPWT,E/A,CD31,p-PI3K/PI3K,p-AKT/AKT,p-eNOS/eNOS were reduced obviously(P<0.05).Compared with the DM group,the swelling of myocardial cells,the normal gap,and the decrease of collagen fibers in the low-dose and high-dose PN groups were reduced,the levels of FBG,CVF,Vaspin,TNF-α,IL-1β,IL-6,α-SMA,FSP-1 and COL1 were decreased obviously,the LVFS,LVEF,LVPWT,E/A,CD31,levels of p-PI3K/PI3K,p-AKT/AKT,p-eNOS/eNOS were increased obviously(P<0.05).LY294002,AKT/eNOS pathway inhibitor,obviously weakened the relieving effect of PN on myocardial fibrosis in DM rats.Conclu-sion:PN may improve cardiac function,reduce myocardial fibrosis and inhibit endothelial mesenchymal transdifferentiation(EndMT)in DM rats by activating PI3K/AKT/eNOS pathway.
5.iKcr-RG:A Two-branching Strategy Based on ResNet and BiGRU to Predict Lysine Acylation Sites of Non-histone Proteins

Lyase acylation is a post-translational modification of proteins that plays a key role in cellular function,gene transcription and cellular metabolism.Meanwhile,lysine acylation is also involved in mul-tiple biological processes in life forms,and its abnormality may be associated with the occurrence and de-velopment of many diseases.Therefore,prediction of lysine acylation sites is important for the diagnosis and treatment of diseases.Although biomedical experiments can detect lysine acylation sites with high precision,they are costly and time-consuming.To address this problem,researchers have developed more convenient and efficient computational methods as an alternative to traditional biomedical experi-mental techniques.In this study,we developed a prediction model iKcr-RG based on a deep learning ap-proach,which employs a two-branching strategy using both ResNet and BiGRU to extract both local and global feature information from the original sequence encoding.To further improve the performance of the model,we innovatively designed a feature fusion method.After these optimizations,this study demon-strates stronger robustness and stability in unbalanced data.In the independent dataset results,specificity(Sp),sensitivity(Sn),accuracy(Acc)and Matthews correlation coefficient(MCC)were 0.8109,0.7902,0.7940,and 0.4978 respectively.The iKcr-RG model was better than existing prediction mod-els in predicting lysine acylation sites,and this study provides new ideas and methods for the application of deep learning in bioinformatics.
6.Impacts of remifentanil on the proliferation,migration,and invasion of liver cancer cells by regulating SPHK1/S1P/S1PR3 signaling pathway

Objective:To explore the impacts of remimazolam(REM)on the proliferation,migration,and invasion of hepatocellular carcinoma cells by regulating sphingosine kinase 1/sphingosine 1-phosphate/sphingosine 1-phosphate receptor 3(SPHK1/S1P/S1PR3)signaling pathway.Methods:Human hepatocellular carcinoma cell line HepG2 was treated with REM at concentrations of 0,20,40,80,160,and 320 μmol/L respectively.Cell survival rate was measured,and drug concentrations were screened.Based on the results of cell survival rate,the logarithmic phase cells were di-vided into five groups:Control group,low,medium,and high concentrations of remifentanil groups(REM-L,REM-M,REM-H groups;20,40 and 80 μmol/L),and high concentrations of remifentanil+SPHK1 activator group(REM-H+K6PC-5 group).MTT assay was used to detect the survival rate of HepG2 cells.Plate cloning experiment was performed to de-tect cell proliferation ability.Scratch experiment was performed to detect cell migration ability.Transwell chamber method was performed to detect cell invasion ability.Flow cytometry was used to detect cell apoptosis.Western blot was used to detect the SPHK1,S1P,S1PR3,Bax,and Bcl-2 proteins in cells.Results:For the Control group,the REM-L,REM-M,and REM-H groups showed that the number of HepG2 cell clones,scratch healing rate,invasion rate,Bcl-2,SPHK1,S1P,S1PR3,and N-cadherin proteins gradually decreased with increasing REM concentration,while the apoptosis rate and Bax,E-cadherin proteins showed an opposite trend(P<0.05).For the REM-H group,the REM-H+K6PC-5 group showed a clear increase in the number of HepG2 cell clones,scratch healing rate,invasion rate,Bcl-2,SPHK1,S1P,S1PR3,and N-cadherin proteins,and a clear decrease in the apoptosis rate and Bax,E-cadherin proteins(P<0.05).Conclusion:REM may inhibit the proliferation,migration,and invasion of hepatocellular carcinoma cells and promote cell apoptosis by suppressing SPHK1/S1P/S1PR3 signaling pathway.
7.Summary of the best evidence for nursing management of patent foramen ovale for cTEE detection

Objective:To retrieve, screen, evaluate and integrate the relevant evidence of nursing management for patent foramen ovale detected by contrast transesophageal echocardiography (cTEE) to provide a basis for clinical practice.Methods:A systematic search was conducted in relevant Chinese and English databases, guideline websites, and professional association websites such as China National Knowledge Infrastructure, Wanfang database, PubMed, and MedLink, etc. Relevant literatures on the detection of patent foramen ovale by cTEE were included, including guidelines, expert conconsensus, clinical decisions, evidence summaries, and systematic reviews. The search period was from the establishment of the database to July 31, 2024. Two evidence-based nursing researchers evaluated the quality of the literature and extracted relevant evidence in combination with clinical situations.Results:A total of 15 literatures were included. Among them, there were 3 guidelines, 6 expert consensuses, 3 clinical decisions, 2 quasi-experiments, and 1 systematic review. Thirty pieces of evidence were summarized from five aspects: assessment, education and publicity, preparation before examination, detection during examination, care after examination.Conclusions:The best evidence for the nursing management of patent foramen ovale detected by cTEE is of high quality and strong authority, which can provide evidence-based basis for standardizing clinical practice and accurately and efficiently detecting patent foramen ovale.
8.Current status and influencing factors of decision anxiety in guardians of children with acute appendicitis under ERAT background

Objective:To investigate the current status and influencing factors of decision anxiety in guardians of children with acute appendicitis under Endoscopic retrograde appendicitis therapy (ERAT) background, and to provide reference for the development of targeted intervention programs for decision anxiety.Methods:Convenient sampling method was used to select 254 guardians of children with acute appendicitis treated in the Department of Pediatrics, Second Affiliated Hospital of PLA Air Force Medical University from February 2023 to April 2024. A cross-sectional survey was conducted using the General Data Questionnaire, the State-Trait Anxiety Inventory Form Y-State anxiety subscale and the Preparation for Decision Making Scale. Multiple linear regression was used to analyze the influencing factors in guardians of children with acute appendicitis.Results:Among the guardians of 254 children with acute appendicitis, 156 were males and 98 were females, aged (37.44 ± 3.63) years old. The decision anxiety score of guardians of children with acute appendicitis was (52.49 ± 6.54). The results of multiple linear regression showed that age of children, gender of guardian, education level of guardian, per capita monthly income of family, decision making tendency and decision preparation were the main influencing factors of decision anxiety in guardians of children with acute appendicitis ( t values were -7.07-3.58, all P<0.05), which could explain 64.4% of the total variation. Conclusions:The decision anxiety in guardians of children with acute appendicitis is at a high level. Medical staff should provide targeted decision guidance, improve decision assistance programs and provide more effective decision support for guardians of children with acute appendicitis.

10.Analysis on the epidemic and genetic characteristics of varicella in Fengtai District, Beijing City, 2024-2025

This study analyzed the epidemiological characteristics and strain genotypic distribution of varicella in Fengtai District, Beijing, utilizing population-based surveillance data from March 2024 to February 2025 combined with laboratory nucleic acid detection and genotyping.We reported 522 varicella cases with male predominance (282 cases, 54.41%) and a majority aged >15 years (358 cases, 68.58%).A bimodal incidence pattern peaked in May and November, while 161 breakthrough infections (30.8%) occurred predominantly among students aged 6-20 years.The multivariate logistic regression model analysis showed that, compared to the 0-5 years age group, individuals aged 6-14 years ( OR: 2.729, 95% CI: 1.083-6.88), 15-20 years ( OR: 2.495, 95% CI: 1.158-5.378), and>20 years ( OR: 5.382, 95% CI: 2.478-11.689) exhibited progressively higher odds of oderate-to-severe rash; females demonstrated a lower risk compared to males ( OR: 0.485, 95% CI: 0.286-0.822); regarding vaccination status, recipients of one vaccine dose ( OR: 0.301, 95% CI: 0.161-0.564) and two doses ( OR: 0.203, 95% CI: 0.086-0.48) showed significantly reduced risks relative to unvaccinated individuals. Genotyping of 174 specimens identified 161 VZV-positive samples, with successful ORF22/38/62 sequencing in 142 samples confirming Clade 2 predominance (141 strains, 99.3%) and one Clade 5 strain; local isolates exhibited high vaccine-strain homology (ORF22 nucleotide:99.5%-100%, amino acid:99.3%-100%) with mutation sites partially overlapping other Chinese regions.
