1.Potential mechanism of Piper nigrum extract in improving depressive-like behaviors in chronic restraint stress mice

Objective Network pharmacology and molecular docking techniques were used to predict the potential mechanisms by which the active components of Piper nigrum(PN)regulate depressive-like behaviors in chronic restraint stress(CRS)mice.Methods The major chemical components and targets of PN were screened using the Traditional Chinese Medicine Systems Pharmacology database.Targets related to ferroptosis and depression were obtained from the Online Mendelian Inheritance in Man,GeneCards,and FerrDB databases.The intersecting targets were then subjected to Gene Ontology and Kyoto Encyclopedia of Genes and Gnomes(KEGG)pathway enrichment analyses,and molecular docking was performed to validate the binding capacities between the core targets and their corresponding active components.Finally,we established a CRS mouse model.Mice were treated with PN 75,150,and 300 mg/kg for 4 weeks,followed by behavioral assessments and reverse transcription-quantitative polymerase chain reaction(RT-qPCR)to verify the expression of core genes.Results Nine active components were screened from PN,corresponding to 27 targets,and 8377 targets related to depression and 547 targets associated with ferroptosis were screened from the databases.The intersection of these three sets resulted in 25 target genes.KEGG enrichment analysis revealed that these core targets were predominantly enriched in signaling pathways,including cholinergic synapses,serotonergic synapses,and neuroactive ligand-receptor interactions.Molecular docking result showed that the main active components of PN had strong binding affinities for the targets CHRM2,SLC6A4,PTGS2,and SLC6A2.Behavioral assessments demonstrated that PN significantly increased the sucrose preference index(P<0.01,P<0.001),reduced immobility time in the tail suspension and forced swimming tests(P<0.01,P<0.001),and enhanced exploratory behavior in the open field test(P<0.05.P<0.01,P<0.001).PN significantly reduced the serum levels of inflammation markers(P<0.05.P<0.01,P<0.001),as shown by enzyme-linked immunosorbent assay,and neurotransmitter analysis revealed that PN significantly increased the levels of serotonin and acetylcholine in the mouse hippocampus(P<0.05).RT-qPCR showed that PN demonstrated the mRNA expression of SLC6A4(P<0.05.P<0.01,P<0.001).Conclusions PN may improve depressive-like behavior in mice by modulating serotonin and acetylcholine levels,inhibiting inflammatory responses,participating in immune regulation,and exerting neuroprotective effects.
2.Clinical characteristics and oncological outcomes of patients with clear cell borderline ovarian tumor

Objective To explore the clinical and pathological characteristics and oncological outcomes of clear cell borderline ovarian tumor(CCBOT),and provide guidance for the diagnosis and treatment of this disease in the future.Methods A total of 23 CCBOT patients who underwent surgical treatment in our hospital from May 2011 to Aug 2022 were enrolled,and the clinical information of 19 CCBOT patients in the Ricotta's cohort was integrated.The clinical pathological characteristics and oncological outcomes of these 42 patients were retrospectively analyzed.Results This study enrolled 42 CCBOT patients,including 23 from our cohort and 19 from the Ricotta G's cohort.Compared with the Ricotta's cohort,the proportion of CCBOT patients with endometriosis in our cohort(34.8%)was significantly higher than that in the Ricotta's cohort(5.3%)(P<0.05).In our cohort,22 patients had information on intraoperative frozen section evaluation,and the accuracy of intraoperative frozen section evaluation was 50%(11/22).The other 50%(11/22)of patients were underestimated by intraoperative frozen section evaluation.The merged cohort indicated that the median age of the patients was 60 years old,occuring more common in postmenopausal women(66.7%).Most cases presented with pelvic masses(59.5%)and abdominal pain or distension(19.0%).This disease mostly involved unilateral ovaries(90.5%).Information on preoperative tumor markers was largely missed,making it difficult to draw an accurate evaluation for them.Ten cases(23.8%)underwent fertility sparing surgery,and 32 cases(76.2%)underwent radical surgery.The pathologic report indicated that all patients were stage I patients and 21.4%patients had endometriosis.Among these patients,32 patients underwent total hysterectomy,of which 10(31.3%)had concurrent endometrial lesions.The median follow-up time was 68 months,with a minimum follow-up of one month and a maximum follow-up of 231 months,and no recurrence or death was observed.Conclusion CCBOT patients usually had an good prognosis with a low recurrence rate.Fertility sparing surgery was safe and feasible,but attention should be paid to exclude concurrent endometrial lesions.Given the rarity of CCBOT,future multicenter prospective studies are needed to better elucidate the clinical pathological features and prognosis of patients with CCBOT.
3.Comparison of the application effects of bronchial blocker and double-lumen endotracheal tube in pediatric thoracoscopic lobectomy

Objective To compare the application effects of bronchial blocker(BB)and double-lumen endotracheal tube(DLT)in pediatric thoracoscopic lobectomy.Methods A total of 80 pediatric patients who underwent thoracoscopic lobectomy at Xuzhou Children's Hospital from February 2020 to February 2023 were randomly divided into the DLT group(n=40)and the BB group(n=40)by envelope drawing.The DLT group received treatment with double-lumen endotracheal tubes,while the BB group was treated with bronchial blockers.Perioperative indicators,mean arterial pressure(MAP),heart rate(HR)at different times,airway pressure,pre-and post-operative recovery quality scores(QoR-15),intraoperative lung collapse(LCS)scores,modified Aldrete recovery scores,and adverse reactions were compared between the two groups.Results The comparison of single-lung ventilation time,intubation positioning time,and anesthesia time between the two groups showed no statistically significant differences(P>0.05);however,the operation time of the BB group was(68.37±6.28)minutes and the hospital stay was(4.56±0.35)days,while those of the DLT group were(76.87±8.34)minutes and(5.97±0.70)days,respectively.There was a statistically significant difference between the two groups(P<0.05).There were no significant differences in the mean arterial pressure(MAP)and heart rate(HR)before induction(T0)between the two groups(P>0.05).The MAP of the BB group immediately after intubation(T1),2 minutes after intubation(T2),immediately after extubation(T3),and 2 minutes after extubation(T4)was(91.53±8.28)mmHg,(89.58±8.72)mmHg,(89.33±8.97)mmHg,and(95.40±9.86)mmHg,respectively.The HRS were(72.74±6.35)times/minute,(72.83±6.82)times/minute,(71.19±6.57)times/minute,and(73.63±7.46)times/minute respectively.The MAP in the DLT group was(83.88±7.29)mmHg,(83.40±7.66)mmHg,(85.17±7.91)mmHg,and(89.42±9.15)mmHg,respectively.HR was(66.68±6.17)times/minute,(64.27±6.19)times/minute,(65.97±6.28)times/minute,and(68.80±6.27)times/minute,respectively.There was a statistically significant difference between the two groups(P<0.05).The airway pressures of the BB group at 5 minutes of bilateral lung ventilation(T5),5 minutes of unilateral lung ventilation(T6),and 5 minutes of recovery of bilateral lung ventilation(T7)were(14.36±2.46)cmH2O,(17.34±2.75)cmH2O,and(15.30±2.32)emH2O,respectively.The values in the DLT group were(17.77±2.85)cmH2O,(21.35±3.17)cmH2O,and(18.49±2.99)cmH2O,respectively.There was a statistically significant difference between the two groups(P<0.05).There was no statistically significant difference in QOR-15 score and modified Aldrete recovery score one day before the operation between the two groups(P>0.05).One day after surgery,the QOR-15 score of the BB group was(119.33±12.20)points,and the intraoperative LCS score was(7.70±0.61)points,while those of the DLT group were(107.60±10.10)and(6.31±0.57)points,respectively.There was a statistically significant difference between the two groups(P<0.05).The incidence of adverse reactions was 10.0%in the BB group and 27.5%in the DLT group.There was a statistically significant difference between the two groups(P<0.05).Conclusion The application of bronchial blockers in pediatric thoracoscopic lobectomy shows better results compared to double-lumen endotracheal tubes.
4.Perioperative anesthesia management of infants undergoing endoscopic endonasal resection of craniopharyngioma

Objective:To summarize the key points of perioperative anesthesia management of infants undergoing endoscopic endonasal resection of craniopharyngioma.Methods:The medical records of infants, aged 1-3 yr, undergoing endoscopic endonasal resection of craniopharyngioma at Beijing Tiantan Hospital affiliated to Capital Medical University from January 1, 2023 to December 31, 2023, were retrospectively analyzed. The demographic and perioperative information was collected.Results:Six pediatric patients were included, with 5 males and 1 female, with height of (103±6) cm and weight of (16±3) kg. Five cases had abnormal anterior pituitary hormones before surgery. All the children with abnormal hormone levels were given hormone supplementation during surgery, except for those with elevated free thyroxine. None of the 6 children showed significant electrolyte abnormalities or diabetes insipidus. Five children received blood transfusion treatment, with an allogeneic red blood cell volume of (114±79) ml. Among them, 4 children received allogeneic plasma transfusion, with a volume of (75±69) ml. All the 6 pediatric patients were conscious with tracheal tubes removed after surgery. Three pediatric patients had hyponatremia, and 2 pediatric patients still had hypothyroidism. All the pediatric patients did not experience postoperative diabetes insipidus, cerebrospinal fluid rhinorrhea or epileptic seizures.Conclusions:Adequate preoperative assessments should be conducted, and detection of anterior pituitary hormone levels should be improved, with hormone replacement therapy administered when necessary in the perioperative anesthesia management for endoscopic endonasal resection of craniopharyngioma in infants. Vital signs should be closely monitored, and prompt treatment should be administered when diabetes insipidus and electrolyte disorders are found during surgery. Attention should be given to airway management to avoid airway spasms after surgery.

6.Research progress of anti-leucine-rich glioma-inactivated 1 protein antibody-associated encephalitis

Anti-leucine-rich glioma-inactivated 1 protein (LGI1) antibody-associated encephalitis is an autoimmune encephalitis mediated by LGI1 antibodies, which can occur in both adults and children. Its common clinical manifestations include epileptic seizures, cognitive and psychiatric disorders; rare symptoms include sleep disorders and autonomic disorders; and its characteristic manifestations are faciobrachial dystonic seizures and refractory hyponatremia. Since anti-LGI1 antibody-associated encephalitis is relatively rare in clinical practice, this article reviews the disease in terms of etiology and pathogenesis, clinical manifestations, auxiliary examinations, diagnosis and differential diagnosis, treatment, recurrence and prognosis. It aims to improve clinicians′ understanding of this disease, provide references for its early diagnosis and treatment, and thereby improve patients′ prognosis.

8.Skin pharmacokinetics of inositol nicotinate in heparin sodium inositol nicotinate cream

Objective To establish an HPLC method to determine the concentration of inositol nicotinate(IN) in rat skin, and study the pharmacokinetic characteristics of IN after transdermal administration of heparin sodium inositol nicotinate cream in rats. Methods HPLC method was used to establish a simple and rapid analytical method for the determination of IN concentration in the skin of rats at different time points after administration. The established method was used to study the pharmacokinetics of IN after transdermal administration of heparin sodium inositol nicotinate cream in rats, and the pharmacokinetic parameters were fitted with DAS software. Results The linearity of the analytical method was good in the concentration range of 0.25-20 μg/ml, the quantitative limit was 0.25 μg/ml, and the average recovery rate was 96.18%. The pharmacokinetic parameters of IN after transdermal administration of heparin sodium inositol nicotinate cream in rats were as follows: t1/2 was (4.555±2.054) h, Tmax was (6±0)h, Cmax was (16.929±2.153)mg/L, AUC0−t was (150.665±16.568) mg·h /L ,AUC0−∞ was (161.074±23.917) mg·h /L, MRT(0−t) was (9.044±0.618)h, MRT(0−∞) was (10.444±1.91) h, CLz/F was (0.19±0.03) L/(h·kg), and Vz/F was (1.19±0.437) L/(h·kg). Conclusion IN could quickly penetrate the skin and accumulate in the skin for a long time, which was beneficial to the pharmacological action of drugs on the lesion site for a long time. The method is simple, rapid, specific and reproducible, which could be successfully applied to the pharmacokinetic study of IN after transdermal administration in rats.
9.Study on the efficacy and mechanism of Tongbianling capsule in the treatment of constipation

Object To study the efficacy and potential mechanism of Tongbianling capsule in constipation. Methods The effects of Tongbianling capsule on intestinal motility in normal mice and carbon powder propulsion rate in small intestine of constipation model mice after were observed administration. The potential targets and key pathways of Tongbianling capsule in treating constipation were identified through network pharmacology. To verify the mechanism, the expression of p-PI3K/PI3K, p-AKT/AKT and CASP3 proteins in mouse colon tissue was detected by the western blot. Results The time for mice to excrete the first black stool was shortened and the number of fecal particles was increased in Tongbianling capsule administration group, and the carbon powder propulsion rate of mice in each Tongbianling capsule administration group was increased. The results of network pharmacology showed that treatment of constipation by Tongbianling capsule may be related to signaling pathways such as PI3K-Akt signaling pathway and 5-HT. The protein expression of p-PI3K/PI3K, p-AKT/AKT, and CASP3 in mouse colon tissue could be significantly downregulated in administration group. Conclusion Tongbianling capsule could effectively promote intestinal peristalsis in mice, increase the frequency of defecation, and effectively treat constipation. The mechanism of its action may be related to the direct or indirect regulation of intestinal motility by the PI3K-Akt signaling pathway.
10.Clinical study on the treatment of chronic atrophic gastritis with spleen and stomach weakness syndrome by Piwei Peiyuan Pill combined with moxibustion

Objective:
To determine the clinical efficacy and mechanism of Piwei Peiyuan Pill (PPP) combined with moxibustion for treating patients with chronic atrophic gastritis (CAG) with spleen and stomach weakness syndrome.
Methods:
Ninety-six CAG patients with spleen and stomach weakness syndrome who met the inclusion and exclusion criteria were enrolled at the Department of Spleen and Stomach Diseases of the Second Affiliated Hospital of Anhui University of Chinese Medicine from June 2022 to December 2023. The patients were randomly divided into a control, a Chinese medicine, and a combined group using a random number table method, with 32 cases in each group (two cases per group were excluded). The control group was treated with rabeprazole combined with folic acid tablets (both thrice daily), the Chinese medicine group was treated with PPP (8 g, thrice daily), and the combined group was treated with moxa stick moxibustion (once daily) on the basis of the Chinese medicine group for 12 consecutive weeks. Gastric mucosa atrophy in the three groups was observed before and after treatment. The gastric mucosal pathological score was evaluated. The Patient Reported Outcome (PRO) scale was used to evaluate the patients′ physical and mental health status and quality of life.An enzyme-linked immunosorbent assay was used to detect serum tumor necrosis factor (TNF)-α, interleukin (IL)-1β, IL-4, IL-10, IL-37, and transforming growth factor (TGF)-β levels in each group. Real-time fluorescence PCR was used to detect the relative expression levels of signal transducer and activator of transcription 3 (STAT3) and mammalian target of rapamycin (mTOR) mRNA in each group. Western blotting was used to detect the relative expression levels of proteins related to the STAT3/mTOR signaling pathway, and the adverse drug reactions and events were recorded and compared.
Results:
There was no statistical difference in age, gender, disease duration, family history of gastrointestinal tumors, alcohol consumption history, and body mass index among the three groups of patients.The total therapeutic efficacy rates of the control, Chinese medicine, and combined groups in treating gastric mucosal atrophy were 66.67% (20/30), 86.67% (26/30), and 90.00% (27/30), respectively (P<0.05). Compared to before treatment, the pathological and PRO scale scores of gastric mucosa in each group decreased after treatment, and TNF-α, IL-1β, IL-37, and TGF-β levels decreased. The relative STAT3 and mTOR mRNA expression levels, as well as the relative STAT3, p-STAT3, mTOR, and p-mTOR protein expression levels decreased (P<0.05), whereas the IL-4 and IL-10 levels increased (P<0.05). After treatment, compared to the control group, the pathological score of gastric mucosa, PRO scale score, TNF-α, IL-1β, IL-37, TGF-β content, relative STAT3 and mTOR mRNA expression levels, and relative STAT3, p-STAT3, mTOR, and p-mTOR protein expression levels in the Chinese medicine and combined groups after treatment were reduced (P<0.05), whereas the IL-4 and IL-10 levels increased (P<0.05). After treatment, compared to the Chinese medicine group, the combined group showed a decrease in relative STAT3, mTOR mRNA expression levels, and STAT3, p-STAT3, mTOR, and p-mTOR protein expression levels (P<0.05).
Conclusion
The combination of PPP and moxibustion may regulate the inflammatory mechanism of the body by inhibiting the abnormal activation of the STAT3/mTOR signaling pathway, upregulating related anti-inflammatory factor levels, downregulating pro-inflammatory factor expression, and increasing related repair factor expression, thereby promoting the recovery of atrophic gastric mucosa, reducing discomfort symptoms, and improving the physical and mental state of CAG patients with spleen and stomach weakness syndrome.
