1.Vasovagal syncope patients' experience of emotion management and program needs from the doctor-nurse-patient perspective: a phenomenological research

Objective:To explore the experience of emotion management and program needs of patients with vasovagal syncope (VVS) from the doctor-nurse-patient perspective.Methods:This study was a phenomenological study in qualitative research. Using purposive sampling method, four medical specialists, four nursing specialists, and 10 patients with VVS from the Syncope Center of Shanghai East Hospital, Tongji University were selected from March to May 2024 for semi-structured interviews. Colaizzi's phenomenological data analysis was used to analyze the data.Results:Theme 1 was the initial perception and experience of emotion management, in which the doctor-nurse perspective included two sub-themes of emotion management met the actual needs of clinical patients and the management program was too specialized and unknown, and the patient perspective included three sub-themes of differences in the ability of individuals to recognize their own emotional state, lack of knowledge related to emotion management, and lack of social support. Theme 2 was the need for emotion management programs, in which the doctor-nurse perspective involved two sub-themes of multidisciplinary cooperation, suitable for clinical operation and follow-up, and the combination of online and offline, and in-hospital and out-of-hospital linkage, and the patient perspective involved two sub-themes of the need for healthcare guidance, easy to operate, and the diversification of forms, personalization, and meeting the needs of different populations.Conclusions:The content of emotion management is relatively unknown to doctors, nurses and patients, and even more so to patients. VVS emotion management work is still in its infancy, and emotion management as a branch of rehabilitation needs to be emphasized for its development, and psychological assessment and intervention work should be incorporated into the routine of syncope rehabilitation.

Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disease affecting both upper and lower motor neurons in the brain and spinal cord. One important aspect of ALS pathogenesis is superoxide dismutase 1 (SOD1) mutant-mediated mitochondrial toxicity, leading to apoptosis in neurons. This study aimed to evaluate the neural protective synergistic effects of ginsenosides Rg1 (G-Rg1) and conditioned medium (CM) on a mutational SOD1 cell model, and to explore the underlying mechanisms. We found that the contents of nerve growth factor, glial cell line-derived neurotrophic factor, and brain-derived neurotrophic factor significantly increased in CM after human umbilical cord mesenchymal stem cells (hUCMSCs) were exposed to neuron differentiation reagents for seven days. CM or G-Rg1 decreased the apoptotic rate of SOD1G93A-NSC34 cells to a certain extent, but their combination brought about the least apoptosis, compared with CM or G-Rg1 alone. Further research showed that the anti-apoptotic protein Bcl-2 was upregulated in all the treatment groups. Proteins associated with mitochondrial apoptotic pathways, such as Bax, caspase 9 (Cas-9), and cytochrome c (Cyt c), were downregulated. Furthermore, CM or G-Rg1 also inhibited the activation of the nuclear factor-kappa B (NF-κB) signaling pathway by reducing the phosphorylation of p65 and IκBα. CM/G-Rg1 or their combination also reduced the apoptotic rate induced by betulinic acid (BetA), an agonist of the NF-κB signaling pathway. In summary, the combination of CM and G-Rg1 effectively reduced the apoptosis of SOD1G93A-NSC34 cells through suppressing the NF-κB/Bcl-2 signaling pathway (Fig. 1 is a graphical representation of the abstract).

5.Investigation and analysis on resources and radiation protection status of interventional radiology in Beijing

Objective:To ascertain the distribution of interventional radiodiagnosis and treatment resources and the current situation of radiological protection in Beijing, standardize the interventional radiodiagnosis and treatment, and promote the implementation of radiation protection regulatory measures.Methods:Various medical institutions at differetn levels that perform interventional radiodiagnosis and treatment in Beijing were identified as the survey objects. With special questionnaires designed, the investigation groups at two levels of municipality and district was established to investigate the basic situation of interventional radiology and occupational health monitoring by the end of 2020. The indexes and parameters were analyzed and evaluated under the relevant national regulations and standards.Results:By the end of 2020, there were 93 medical institutions performing interventional radiology in Beijing, together with 236 digital subtraction angiography machines (DSA) with higher than 800 mA. A total of 135 593 cases of interventional radiological surgical operation were performed. There were 40 hospitals annually performing more than 1 000 cases and 41 hospitals perfoming 10-1 000 cases. There were 3 539 interventional radiological workers, with 99.0% holding radiological worker certificates. The passing rates of occupational health examination, personal dose monitoring and radiation protection training were 96.9%, 99.5% and 95.8%, respectively. A total of 3 532 sets of protective articles were provided for the workers, of which 98.9% were equipped with split or integrated lead clothing, but 6.5% were not equipped with lead protective glasses and 54.9% were not equipped with lead protective gloves.Conclusions:The radiation protection management for the interventional radiodiagnosis and treatment is generally good. However, the regulatory mechanism should be further improved based on the current distribution of interventional radiodiagnosis and treatment resources, with focus on strengthening the occupational health examination, the radiation protection training, and the configuration and use of protective equipment.
6.Analysis of influencing factors on occupational pressure of medical staff in a third-level first-class hospital of traditional Chinese medicine in Tianjin

Objective:To explore the status and influencing factors of occupational pressure among medical staff in a third-level first-class hospital of traditional Chinese medicine in Tianjin, and to provide reference for formulating relevant policies.Methods:From September to October in 2019, doctors, nurses, pharmacists laboratory, radiology and management personnel were randomly selected as the research objects. A total of 191 questionnaires were distributed and recovered, and 189 valid questionnaires were recovered, with an effective recovery rate of 98.95%. The Scale for Occupational Stressor on Clinician was used for investigation. The influence of different characteristics on the occupational pressure of medical staff was analyzed.Results:The average total score of occupational pressure was (94.8±15.4) . There were significant differences in occupational pressure among different age groups ( P<0.05) . There were significant differences in total pressure score, organization management, occupational interest, workload, external environment, doctor-patient relationship and other dimensions ( P<0.05) . The average total score of occupational pressure of doctors was (101.7±13.3) , which was significantly higher than that of nurses, pharmacists, laboratory, rodiology and managers ( P<0.05) . Conclusion:The occupational pressure of doctors is relatively serious, and the occupational pressure should be alleviated from the external environment, doctor-patient relationship and workload.

8.Improvement Effects of Erythropoietin Derivative Helix B Surface Peptide on Epirubicin-induced Myocardial Injury in Rats and Its Mechanism Study

OBJECTIVE: To study the improvement effects and its mechanism of erythropoietin derivative helix B surface peptide (HBSP) on epirubicin-induced myocardial injury in rats. METHODS: SD rats were randomly divided into control group, model group, erythropoietin (EPO) group and HBSP group, with 10 rats in each group. Except for control group, other groups were given epirubicin 2.5 mg/kg intraperitoneally, once a week, for consecutive 6 weeks to induce myocardial injury model. EPO group and HBSP groups were given rhEPO 5 000 u/kg or HBSP 60 μg/kg intraperitoneally, 3 times a week (one day before medication, first day and third day after medication of epirubicin), for consecutive 6 weeks. At the beginning of the first administration, the rats were weighed at the 11th week. The cardiac function was measured by echocardiography [left ventricular end-diastolic diameter (LVIDd), ejection fraction (EF), fractional shortening (FS) and cardiac output(CO)]. The serum levels of troponin I (cTnI) and amino-terminal B-type natriuretic peptide (NT-proBNP) were determined by ELISA. mRNA expression of PI3K and Akt in myocardial tissue was detected by RT-PCR. The protein expression of p-PI3K and p-Akt in rat myocardium were detected by Western blot. RESULTS: During research period, two rats died in model group, one in EPO group and none in HBSP group. Compared with control group, body weight, the levels of EF, FS and CO were decreased significantly in model group, while the contents of LVIDd and cTnI, NT-proBNP were increased significantly; mRNA expression of PI3K and Akt as well as protein expression of p-PI3K and p-Akt were decreased significantly, above differences were statistical significant (P<0.05). Compared with model group, body weight, the levels of EF, FS and CO were increased significantly in EPO group and HBSP group, while the contents of LVIDd and cTnI, NT-proBNP were decreased significantly; mRNA expression of PI3K and Akt as well as protein expression of p-PI3K and p-Akt were increased significantly, above differences were statistical significant (P<0.05). Compared with EPO group, the contents of cTnI and NT-proBNP were decreased significantly in HBSP group, with statistical significance (P<0.05). CONCLUSIONS: HBSP can improve myocardial injury in rats as much as EPO, and has less toxity. Their mechanism may be related to the activation of PI3K/Akt signaling pathway.
9.Effects of Aerobic and Resistance Exercises on Morphology and Activity of Brown Adipose Tissues in Rats

Objective To compare the effect of aerobic and resistance exercise on brown adipose tissues in rats.Methods Sprague-Dawley rats were randomly assigned to a control(C)group,an aerobic exercise (T) group and a resistance exercise(L)group.The rats of group T performed treadmill exercise 5 days per week for 8 weeks,while those of group L climbed vertical ladders with progressively increased weights once in 3 days for 8 weeks.All rats were weighed before and after the intervention.Then the bilateral interscapular brown adipose tissue was isolated and weighed at 48h after the last exercise.The hematoxylin-eosin staining was used to observe the morphology of adipocytes,and the quantitative real-time PCR and Western blotting were employed to detect the mRNA and protein expression of browning genes [PR-domain-containing 16 (PRDM 16),PPAR gamma coactivator-1α (PGC-1 α) and uncoupling protein 1(UCP1)] respectively.Results After the intervention,the increment of weight of group T and group L were only 52% and 70% of group C(P<0.01 and P<0.05).The weight of BAT in both group T and L was lower than group C,but without significance(P>0.05).The average size of lipid droplets in group T decreased significantly(P<0.01) and slightly in group L(P>0.05) compared with group C.The PRDM16,PGC-1α and UCP1 mRNA expression of group T was 1.78(P<0.01),2.2(P< 0.01) and 1.3(P>0.05) times of group C,while that of group L was 1.30(P>0.05),1.25(P>0.05) and 1.21(P>0.05) times of group C.The PRDM16,PGC-1α and UCP1 protein level in group T was 1.46-fold(P<0.05),1.56-fold(P>0.05)and 1.20-fold(P>0.05),while that of group L was 1.08-fold(P>0.05),0.94-fold(P>0.05) and 1.20-fold(P>0.05) of group C.Conclusion Eight weeks of aerobic exercise can significantly make the lipid droplets of adipocyte smaller,increase the differentiation and metabolic activity of BAT,and weakly stimulate BAT thermogenesis.However,8 weeks of resistance exercise has no significant effect on BAT.
10.A comparative study of deglutition function of amyotrophic lateral sclerosis patients with or without diabetes mellitus type Ⅱ

Objective To explore the association between diabetes mellitus type Ⅱ (DM) and deglutition function of amyotrophic lateral sclerosis (ALS) patients.Methods Seventy-five ALS patients older than 45 years and admitted to our hospital from August 2008 to November 2015 were selected into this study;67 of them were without DM and 8 were with DM.Water swallow test,amyotrophic lateral sclerosis severity scale-swallow (s-ALSSS),amyotrophic lateral sclerosis functional rating scale (ALSFR-R) and videofluoroscopic swallow study (VFS) were performed to evaluate the deglutition functions of these ALS patients with or without DM,and the results of the two groups were compared.Results (1) Patients with DM had significantly higher s-ALSSS scores,ingurgitation part in ALSFRS-R scores and in parts of VFS scores,such as transportation to pharyngeal,pharyngeal transit,flow into pharyngeal before reflexion,epiglottic vallecula residue,and piriform sinus residue than patients without DM (8.88±1.34 vs.7.54±1.47,3.50±0.54 vs.2.96±0.77,2.88±0.35 vs.2.16±0.69,2.75±0.46 vs.2.09±0.69,2.88±0.35 vs.2.42±0.56,2.88±0.35 vs.2.39±0.58,P<0.05).(2) Scores of Kubota drinking test,ALSFRS-R,VGF (including oral phase,pharyngeal phase and aspiration degree) in patients without DM were 2.15±1.12,7.18±1.41(1.78±0.69,1.69±0.60 and 3.72±0.65),and those in patients with DM were 1.88±1.34,8.00±0.93(2.13±0.64,2.00±0.53 and 4.00±0.00);no significant differences were noted between the two groups (P>0.05),but there was a trend showing that DM patients had higher scores in these evaluations.Conclusion As compared with ALS patients without DM,ALS patients with DM get more mildly impaired deglutition function.
