1.Disseminated Histoplasmosis Presenting as Addisonian Crisis: A Diagnostic Mimic of Tuberculosis With Bilateral Adrenal Masses

Introduction:
Disseminated histoplasmosis is a rare but important cause
of adrenal insufficiency (AI), particularly in tuberculosis
(TB)-endemic regions, where it may mimic granulomatous
diseases. Adrenal involvement occurs in up to 80% of
disseminated cases, although overt AI is less common.
Reported cases described bilateral adrenal masses
mimicking malignancy or TB, even in immunocompetent
individuals. Addisonian crisis may be the initial
manifestation, especially when the diagnosis is delayed. In
TB-endemic settings, fungal infections are often overlooked,
leading to delayed diagnosis and inappropriate therapy.
Case:
We reported a case of a 68-year-old male with underlying
diabetes mellitus who presented with fever, cough,
dysphagia, and weight loss for 1 month. He was
empirically treated as smear-negative disseminated TB.
On day 2 of therapy, he developed hypotension (80/50
mmHg), hypoglycemia (3.9 mmol/L), hyponatremia
(Na 129 mmol/L), and hyperkalemia (K 5.1 mmol/L),
suggestive of adrenal crisis, and was started on intravenous
hydrocortisone. Serum cortisol prior to treatment was 61 nmol/L. Computed tomography (CT) imaging revealed
bilateral lipid-poor adrenal lesions (right: 3.6 × 2.2 × 4.9 cm,
Hounsfield Unit (HU) 36 and absolute washout 33%; left:
3.5 × 2.4 × 5.4 cm; HU 35 and absolute washout 17%),
raising suspicion of infectious or malignant etiologies.
Endoscopic ultrasound-guided biopsy demonstrated
necrotizing granulomatous inflammation with budding
fungal yeasts on Pituitary Apoplexy Score and GMS
staining, consistent with Histoplasma capsulatum. TB and
malignancy were excluded. He received amphotericin B for
14 days, followed by oral itraconazole for 1 year, and oral
hydrocortisone replacement. At 1-year follow-up, adrenal
lesions remained stable on CT images, and he continued
to require hydrocortisone replacement.
Conclusion
This case highlights the importance of considering
disseminated histoplasmosis as a differential diagnosis
of bilateral adrenal masses with AI, especially with poor
response to anti-TB therapy. Early tissue diagnosis is
essential, as imaging findings are non-specific. Prompt
recognition is critical to prevent life-threatening adrenal
crisis and improve clinical outcomes.

Hepatitis C virus (HCV) infection is known to influence the seminal and hormonal parameters of infected men. This study was performed to assess the effects of HCV clearance using direct-acting antiviral (DAA) agents on semen and hormonal parameters.
Methods:
A total of 50 patients with chronic HCV were enrolled, and conventional semen analysis was performed according to World Health Organization guidelines. Basal levels of total testosterone, free testosterone (FT), follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), prolactin, and sex hormone-binding globulin (SHBG) were assessed before and 3 months after treatment with DAAs.
Results:
Following DAA treatment, statistically significant increases were observed in sperm motility and the proportion of grade A sperm. Additionally, the percentage of abnormal forms was significantly decreased after treatment (p=0.000). However, no significant differences were observed in semen volume, concentration, or total sperm count. Sex hormone analysis of patients after DAA treatment revealed significant increases in FT, LH, and FSH levels, along with significant decreases in SHBG, prolactin, and E2 levels.
Conclusion
Following HCV clearance, we noted an improvement in sperm motility and an increase in the percentage of sperm with normal morphology. Treatment with DAAs was also associated with increased levels of FT and LH, along with decreased levels of SHBG, prolactin, and E2.

5.Elevated Serum Galectin-1 and Galectin-3 Levels in Children With Specific Learning Disorder: A Case Control Study

Objective:
Specific learning disorder (SLD) is a neurodevelopmental disorder in which underlying pathogenesis and etiological factors are not fully understood. Neuroinflammatory response (measured with serum levels of galectin-1 and galectin-3), which is associated with learning and memory, may play an important role in the etiopathogenesis of SLD. Aim of the present study is to examine whether serum galectin-1 and galectin-3 levels are related to SLD.
Methods:
The current study consisted of 42 treatment-naive children with SLD and 42 control subjects. All of the subjects were assessed using semi-structured psychiatric examination to diagnose SLD and exclude attention-deficit hyperactivity disorder. Serum galectin-1 and galectin-3 levels were measured via venous blood samples.
Results:
The SLD and control group did not differ significantly in terms of age, sex, and body mass index (BMI). The SLD group had significantly higher serum levels of galectin-1 (8.78±2.97 vs. 7.40±2.03, p=0.019) and galectin-3 (1.86±0.93 vs. 1.32±0.69, p=0.003) than the control group when controlled for age, sex, and BMI.
Conclusion
Higher serum levels of galectin-1 and galectin-3 in children with SLD may indicate the role of neuroinflammatory response in the pathogenesis of SLD. Other mechanisms involving galectin-1 and galectin-3 related to learning may play a part in the etiology of SLD.
6.Knowledge, Attitude, and Behavior Levels of Postmenopausal Women about Osteoporosis

Background:
Osteoporosis can be delayed by providing accurate and adequate information to people at risk. Therefore, we aimed to determine the knowledge, attitude, and behavior levels of women in the postmenopausal period, which is the largest group at risk.
Methods:
The study was conducted in a tertiary Training and Research Hospital between 1 December 2018 and 1 May 2019 in 225 postmenopausal women who applied to the Family Medicine outpatient clinic and bone mineral density (BMD) outpatient clinics for BMD measurement or had previously had this measurement at least once. A questionnaire evaluating the knowledge, attitudes, and behavior levels related to osteoporosis was applied to all patients included in the study.
Results:
The mean age was 58.05±9.1 years. The median osteoporosis knowledge score was 7 out of 19 points. A total of 119 (52.9%) had low knowledge scores and 106 (47.1%) had higher knowledge scores. Of the individuals with high scores, 40 (37.7%) were smoking, 64 (60.4%) did not sunbathe, 89 (84%) did not consume the recommended daily amount of calcium, and 58 (54.7%) were not exercising in the recommended time. It was seen that those who were university graduates, who had previously learned about osteoporosis from a health professional, and who had a family history of osteoporosis had higher knowledge levels.
Conclusions
Even in postmenopausal women who are aware that they are in the risk group and that they should have BMD, their knowledge, attitude, and behavior levels on osteoporosis were found to be quite low.
7.Are there any further modalities for prediction of subclinical volume overload in advanced stages of chronic kidney disease?

Background:
Subclinical volume overload in chronic kidney disease (CKD) patient represents a debatable issue. Although many tools were used to detect volume overload in such patients, many non-specific results were due to presence of comorbidities. Bioimpedance spectroscopy is an objective fluid status assessment method, which is shown superior to classical methods in many studies. Combining some of these tools may improve their accuracy and specificity. Inferior vena cava collapsibility index (IVCCI) with brain natriuretic peptide (BNP) can be combined for more specific volume assessment. This study was performed to assess the usage of combined IVCCI and BNP levels in CKD patients to predict subclinical volume overload.
Methods:
One hundred and ten patients with CKD (stages 4 and 5) not on dialysis and having normal left ventricular systolic function were included in this study. Exclusion criteria were: (1) patients with other causes of raised BNP than volume overload and (2) patients on diuretics. A complete medical history was obtained, and thorough examination and laboratory tests were performed for all included patients. IVCCI and BNP serum levels were evaluated. The patients who exhibited an overhydration (OH)/extracellular water (ECW) ratio of >15% were considered to have volume overload.
Results:
Twenty-six patients (23.6%) had subclinical hypervolemia as diagnosed by OH/ECW ratio of >15%. IVCCI ≤ 38% had higher diagnostic performance than BNP ≥ 24 pg/mL. Combining both IVCCI ≤ 38% and BNP ≥ 24 pg/mL increased the specificity and positive predictive value for detection of subclinical hypervolemia.
Conclusion
Combined elevated BNP level and decreased IVCCI are more precise tools for subclinical volume overload detection in CKD patients.

9.Evaluation of beak and feather disease virus, avian polyomavirus and avian papillomavirus of captives psittacine birds in Seri Kembangan, Selangor, Malaysia

Aims:
Psittacine birds such as parrots, macaws, cockatoos, lovebirds and parakeets, are widely reared as household
pets or at aviary due to their attractive features. However, the status of virus-causing diseases of psittacine species in
Malaysia is fairly under-documented. Therefore, this study was aimed to detect the presence of three common avian
viruses that infect psittacine birds, i.e. beak and feather disease virus (BFDV), avian polyomavirus and avian
papillomavirus.
Methodology and results:
Faecal samples from twelve asymptomatic captive psittacine birds of different species were
collected from an undisclosed animal garden in Serdang, Selangor, Malaysia. Briefly, the sample was homogenised and
resuspended with SM buffer with the ratio 1:1 (weight of sample/g: volume of SM buffer/mL) before centrifugation at
1,000 × g for 20 min. The supernatant was collected and filtered before subjected to genomic DNA extraction using a
commercialised kit. Polymerase chain reaction (PCR) technique was used to screen the V1, VP1 and L1 genes of beak
and feather disease virus (BFDV), avian polyomavirus and avian papillomavirus, respectively. Findings revealed that the
samples were negative for BFDV and avian polyomavirus. However, positive results of 1.5 kbp PCR amplicon were
detected for avian papillomavirus in four out of the 12 samples (33.33%), which was from the white-crested cockatoo,
African grey parrot, yellow-collared macaw and Senegal parrot. Sequence analysis of the L1 gene from the Senegal
parrot Poicephalus senegalus revealed 93% identity to a reference Psittacus erithacus timneh avian papillomavirus.
Conclusion, significance and impact of study
This study added to the limited prevalence data of three important
avian viruses which infect captive psittacines in Seri Kembangan, Selangor, Malaysia. Avian papillomavirus, but not
BFDV and avian polyomavirus, was detected in the collected captive psittacine birds. Therefore, a routine screening can
be performed to monitor the health status of birds despite their asymptomatic manifestation, in order to prevent possible
virus transmission.

10.Detection and distribution of putative pathogenicity-associated genes among serologically important Leptospira strains and post-flood environmental isolates in Malaysia

Aims: Leptospirosis is an infectious disease that is endemic to many tropical regions. Large epidemics usually happen
after heavy rainfall and flooding. This potentially fatal zoonosis is caused by pathogenic bacteria belonging to the genus
Leptospira. Leptospirosis can be diagnosed using specific biomarkers such as target genes and virulence indicators that
are well preserved across various Leptospira spp., including those that are prevalent in clinical samples and in the
environment. To date, several pathogenicity-determinant genes, including lipL32 and lipL41, have been described and
used for diagnosing leptospirosis. However, prevalence of these genes in leptospiral strains is unclear.
Methodology and results: In the present study, we assessed the distribution of eight pathogenicity-determinant genes
in reference Leptospira strains and environmental isolates in Malaysia, by polymerase chain reaction (PCR). We found
that only lipL32 and ligB were consistently expressed in all pathogenic Leptospira strains compared with the other tested
genes. Moreover, our results suggested that the use of lipL41, lipL21, ompL1, lfb1, ligA, and ligC as biomarkers could
incorrectly misdetect pathogenic Leptospira strains present in the environment.
Conclusion: Thus, our results suggest that the pathogenicity-determinant genes lipL32 and ligB can be used as
biomarkers for detection pathogenic Leptospira
