ASSOCIATION BETWEEN MEDICATION ADHERENCE AND BLOOD PRESSURE CONTROL AND FACTORS ASSOCIATED WITH ANTIHYPERTENSIVE MEDICATION ADHERENCE IN THE MELAKA TENGAH DISTRICT: A CROSS-SECTIONAL SURVEY
Introduction: The burden of cardiovascular disease and early morbidity and mortality is exacerbated by hypertension. According to the 2019 National Health and Morbidity Survey, 30% of adults in Malaysia aged over 18 years had hypertension. Our study aimed to investigate the association between medication adherence and blood pressure control and the factors associated with antihypertensive medication adherence.
Methods: This cross-sectional study was conducted among adult patients with hypertension who had been under treatment at public primary health clinics in the Melaka Tengah District. A self-administered questionnaire was used, and blood pressure was measured. Descriptive and multivariate logistic regression analyses were conducted using the Statistical Package for the Social Sciences (version 28).
Results: A total of 1531 patients were recruited in this study. Among them, 74.1% had good antihypertensive medication adherence, and 51.4% had their blood pressure controlled. Medication adherence was significantly associated with blood pressure control (P<0.005). The multivariate analysis showed that the determinants for antihypertensive medication non-adherence were Malay ethnicity, secondary education, farther distance from the clinic, experience of side effects of antihypertensive medications, concern about long-term side effects and usage of alternative medicine (P<0.05). Taking multiple antihypertensive medications was the only factor associated with uncontrolled blood pressure (P<0.05).
Conclusion: Addressing issues on medication adherence is important to ensure blood pressure control. The factors associated with non-adherence should be closely monitored to improve blood pressure control and prevent adverse health outcomes. Single-pill combination antihypertensive medications are encouraged to reduce pill burden and improve blood pressure control.
2.Safety Culture in Handling Radioactive Materials for Radiation Practitioners: A Review

Safety culture refers to how safety is addressed and communicated in the workplace. It
encompasses the attitudes, beliefs, perceptions, and values of all employees in an organization
about safety. A good safety culture can be promoted by management through a commitment to
safety, realistic practices for handling hazards, continuous organizational learning and concern
for hazards shared across the workforce. The objective of this paper is to reviews the safety
culture in handling radioactive sources. The radioactive substances used should comply with the
following characteristics where radiotoxicity must be as low as possible, short-living isotopes
are preferred to long-living ones and the amounts used must be kept to a minimum. Therefore,
the As Low As Reasonably Achievable (ALARA) principle was applied that based on the
minimization of radiation doses and limiting the release of radioactive materials into the
environment by employing all reasonable methods. Besides that, the ALARA principle is an
integral part of all activities that involve the use of radiation or radioactive materials and can
help prevent unnecessary exposure as well as overexposure. The three major ALARA principles
to assist with maintaining doses are time, distance and shielding. It takes a whole team effort to
successfully implement the ALARA in safety culture while doing routine elements of working
in handling radioactive materials.
3.HLA-B*15:02 screening in epileptic patients using a high resolution melting-real time PCR (HRM-QPCR) method

Background: The HLA-B*15:02 polymorphism in epileptic patients is known to be associated with carbamazepine-induced Stevens-Johnson syndrome (SJS). The prevalence of HLA-B*15:02 polymorphism seemed to be ethnic-specific with a higher frequency of HLA-B*15:02 in Asian compared to the Europeans. This study was performed to determine the frequency of the HLA-B*15:02 polymorphism in epileptic patients at the Chancellor Tuanku Muhriz Hospital-UKM Medical Centre (HCTM-UKMMC) using high resolution melting-real time PCR (HRM-QPCR) method. Methods: We performed a fast and effective in-house high resolution melting-real time polymerase chain reaction method and compared it with the conventional multiplex-PCR method. The specificity and sensitivity of each test were also determined using DNA from saliva. Results: Using the conventional multiplex-PCR approach for screening, 25 out of 64 (39.1%) epileptic patients were positive for HLA-B*15:02. However, using the HRM-QPCR technique, 24/64 (37.5%) of the patients were positive. The one patient who tested positive by the multiplex-PCR but negative using the HRM-QPCR turned out to be negative by DNA sequencing. The HRM-QPCR and DNA sequencing showed 100% sensitivity and specificity. The multiplex-PCR showed 100% sensitivity and 98.4% specificity compared to both HRM-QPCR and DNA sequencing. The HRM-QPCR is also more cost-effective (<$16.40 USD/test) and less time-consuming when compared to the multiplex-PCR ($25.15 USD/test).Conclusion: Our result suggested that multiplex PCR, HRM-QPCR and Sanger sequencing can be used for detection of HLA-B*15:02. However, a qualitative method such as multiplex PCR should be confirmed with other quantitative methods such as HRM-QPCR and Sanger sequencing.
4.Co-inheritance of compound heterozygous Hb Constant Spring and a single –α3.7 gene deletion with heterozygous δβ thalassaemia: A diagnostic challenge

Haemoglobin Constant Spring (Hb CS) mutation and single gene deletions are common underlying
genetic abnormalities for alpha thalassaemias. Co-inheritance of deletional and non-deletional alpha
(α) thalassaemias may result in various thalassaemia syndromes. Concomitant co-inheritance with
beta (β) and delta (δ) gene abnormalities would result in improved clinical phenotype. We report here
a 33-year-old male patient who was admitted with dengue haemorrhagic fever, with a background
history of Grave’s disease, incidentally noted to have mild hypochromic microcytic red cell indices.
Physical examination revealed no thalassaemic features or hepatosplenomegaly. His full blood
picture showed hypochromic microcytic red cells with normal haemoglobin (Hb) level. Quantitation
of Hb using high performance liquid chromatography (HPLC) and capillary electrophoresis (CE)
revealed raised Hb F, normal Hb A2 and Hb A levels. There was also small peak of Hb CS noted in
CE. H inclusions was negative. Kleihauer test was positive with heterocellular distribution of Hb
F among the red cells. DNA analysis for α globin gene mutations showed a single -α-3.7 deletion
and Hb CS mutation. These fi ndings were suggestive of compound heterozygosity of Hb CS and
a single -α-3.7 deletion with a concomitant heterozygous δβ thalassaemia. Co-inheritance of Hb
CS and a single -α-3.7 deletion is expected to result at the very least in a clinical phenotype similar
to that of two alpha genes deletion. However we demonstrate here a phenotypic modifi cation of α
thalassemia presumptively as a result of co-inheritance with δβ chain abnormality as suggested by
the high Hb F level.
5.Optimal protein extraction methods from diverse sample types for protein profiling by using Two-Dimensional Electrophoresis (2DE)

There is a great diversity of protein samples types and origins, therefore the
optimal procedure for each sample type must be determined empirically. In order to obtain a
reproducible and complete sample presentation which view as many proteins as possible on
the desired 2DE gel, it is critical to perform additional sample preparation steps to improve
the quality of the final results, yet without selectively losing the proteins. To address this, we
developed a general method that is suitable for diverse sample types based on phenolchloroform
extraction method (represented by TRI reagent). This method was found to yield
good results when used to analyze human breast cancer cell line (MCF-7), Vibrio cholerae,
Cryptocaryon irritans cyst and liver abscess fat tissue. These types represent cell line,
bacteria, parasite cyst and pus respectively. For each type of samples, several attempts were
made to methodically compare protein isolation methods using TRI-reagent Kit, EasyBlue Kit,
PRO-PREPTM Protein Extraction Solution and lysis buffer. The most useful protocol allows the
extraction and separation of a wide diversity of protein samples that is reproducible among
repeated experiments. Our results demonstrated that the modified TRI-reagent Kit had the
highest protein yield as well as the greatest number of total proteins spots count for all type
of samples. Distinctive differences in spot patterns were also observed in the 2DE gel of
different extraction methods used for each type of sample.
