Objective To investigate the effects of sufentanil on proliferation,invasion,migration,and apoptosis of thyroid cancer cells,and analyze whether its mechanism is related to the AMP-activated protein kinase(AMPK)-silent information regulator 1(SIRT1)-peroxisome proliferat or activator receptor gamma coactivator 1α(PGC-1α)signaling pathway.Methods The human thyroid cancer cell line(TPC-1)was separated into a control group,L,M,H-sufentanil groups,and H-sufentanil+AICAR group.TPC-1 cell activity,apoptosis,invasion,migration,and expression of proteins were detected using CCK-8 assay,flow cytometry,Transwell assay,scratch assay,and Western blot,respectively.Results Compared with the control group,the absorbance value,invasion number,scratch healing rate,cleaved-Caspase-3,programmed cell death 1 ligand 1,B lymphoblastoma-2,p-AMPK/AMPK,SIRT1,and PGC-1αprotein levels in L,M,and H-sufentanil groups were reduced;the apoptosis rate and Bax protein level were increased(P＜0.05).AICAR treatment reversed the effect of H-sufentanil on the above indicators(P＜0.05).Conclusion Sufentanil may inhibit the AMPK-SIRT1-PGC-1α signaling pathway,thereby inhibiting the proliferation and invasion migration of thyroid cancer cells,and promoting cell apoptosis.

Objective:To explore the effect of esketamine on lung ischemia-reperfusion oxidative stress markers and pulmonary complications in patients undergoing radical surgery for lung cancer.Methods:A prospective randomized-controlled study was conducted. A total of 60 patients undergoing radical lung cancer surgery in Cangzhou People's Hospital from October 2023 to April 2024 were selected as the research objects. The patients were randomly divided into group E (30 cases) and group C (30 cases) according to the envelope drawing method. The patients in group E received an intravenous injection of esketamine at 0.5 mg/kg before skin incision, followed by continuous intravenous infusion until 30 minutes before the end of surgery. The patients in group C was administered an equivalent dose of 0.9% NaCl solution. The lung function, inflammatory factors, oxidative stress markers, and the incidence of complications were compared between the 2 groups at different time intervals [after anesthesia induction (T 1), 60 minutes after single-lung ventilation (T 2), 30 minutes after resuming bilateral lung ventilation (T 3), and 60 minutes after resuming bilateral lung ventilation (T 4)]. Results:The age of patients in both groups was (62±8) years. At T 2, T 3, and T 4, the oxygenation index (OI) of group E [(413±57), (387±52), (364±48) mmHg (1 mmHg = 0.133 kPa), respectively] was higher than that of group C [(377±52), (359±47), (333±42) mmHg, respectively]. The intrapulmonary shunt rate of group E [(26.9±3.3)%, (24.6±3.1)%, (13.3±2.2)%, respectively] was lower than that of group C[(33.5±4.3)%, (28.4±3.7)%, (16.2±2.7)%, respectively], and the differences were statistically significant ( t = 2.56, 2.16, 2.66, 6.29, 4.34, 4.32, respectively); the differences in the above indictors at T 1 between the 2 groups were not statistically significant (all P > 0.05), and there were statistically significant differences in the above indictors at T 1, T 2, T 3, T 4 between the 2 groups (all P < 0.001). At T 4, the levels of serum tumor necrosis factor-alpha, interleukin-6, and interleukin-8 in group E [(93±11) ng/ml, (14.9±1.7) pg/ml, (11.4±2.2) pg/ml, respectively] were lower than those in group C [(136±15) ng/ml, (19.9±2.8) pg/ml, (15.6±4.6) pg/ml, respectively], and the differences were statistically significant ( t = 12.89, 8.40, 4.61, all P < 0.001). At T 1, the differences in the levels of inflammatory factors of both groups were not statistically significant (all P > 0.05); and the levels of inflammatory factors at T 4 were lower than those at T 1 of both groups, and the differences were statistically significantly (all P < 0.001). At T 4, the level of superoxide dismutase in group E [(66±6) U/ml] was higher than that in group C [(56±5) U/ml]. The levels of myeloperoxidase, malondialdehyde and catalase in group E [(3.0±0.4) U/g, (4.01±0.26) mmol/ml, (44±5) U/ml, respectively] were lower than those in group C [(5.7±0.7) U/g, (4.91±0.52) mmol/ml, (56±6) U/ml, respectively],and the differences were statistically significant ( t = 6.43, 18.83, 8.48, 8.74, all P < 0.001). There were no statistically significant differences in the levels of oxidative stress indicators between the 2 groups at T 1, while there were statistically significant differences in the levels of oxidative stress indicators between the 2 groups at T 1 and T 4 (all P < 0.001). The incidence of complications in group E [6.7% (2/30)] was lower than that in group C [26.7% (8/30)], and the differences were statistically significant ( χ2 = 4.32, P = 0.038). Conclusions:Esketamine can alleviate lung ischemia-reperfusion injury, improve postoperative oxygenation function, and reduce the incidence of complications.

Objective To determine the effects of delayed umbilical cord clamping on the postpartum hemorrhage,instant and long-term newborn anemia,newborn jaundice.Methods In total,303 infants were selected during October 2016 to June 2017 in three hospitals in Beijing.They were randomly allocated into two groups receiving instant clamping of umbilical cord (less than 60s after delivery,n=158) and delayed clamping of umbilical cord(after cord pulsation ceased,n=145).Relevant indicators of maternal and neonatal outcomes are compared.Results There were significant differences between two groups in instant hemoglobin concentration and in 5～7 days (P＜0.05).There were no differences between two groups in transcutaneous bilirubin,the risk of anemia in three months,the risk of jaundice in 5～7 days and the need of blue-light therapy (P＞0.05).There were no differences between two groups of women in postpartum hemorrhage,the length of third stage of labor and the rate of breast feeding (P＞0.05).Conclusion Clamping the umbilical cord when cord pulsation has ceased does not have negative effects on delivery process and postpartum hemorrhage,but it increases the instant hemoglobin concentration and hemoglobin concentration after delivery in 5～7 days.Still it is unclear whether it will affect the risk of jaundice.

Objective With methods of literature review, item test, reliability and validity test, etc., to develop a fall risk assessment scale specific to the obstetrical department, which, after reliability and validity test, can be used for women health care before, at and after delivery.Methods Items in the fall risk assessment scale for pregnant and maternal women were determined and weights were assigned by reviewing relative risk factors in literature, referring to items in other fall risk assessment tools from home and aboard. After a preliminary scale was formed, 154 pregnant and maternal women hospitalized from May to December 2016 were assessed, so that reliability and validity of the scale were tested.Results In this fall risk assessment scale for the obstetrical department, test-retest reliability was 0.822, interrater reliability was 0.926, content validity of expertise was 0.927, and area under the curve (ROC) was 0.750 (P<0.05). When the dividing value was 3.5, sum of sensitivity (0.714) and specificity (0.714) reached the max.Conclusions The new fall risk scale, with favorable reliability and validity, can filter patients with high risk of fall, yet, it is still to be testified and perfected in future application.

OBJECTIVE: To investigate the allergens in patients with allergic rhinitis in Yichang, so that to find common allergens of Yichang and to provide statistic basis for a reasonable prevention and treatment to allergic rhinitis. METHOD: 1,979 patients with allergic rhinitis in Yichang were detected for allergens by skin prick test and the distribution of positive rates to inhaled allergens was compared between different genders and ages. RESULT: 1,545 (78.1%) of 1,979 suspected allergic rhinitis patients presented positive reaction. The positive rate in male was significantly higher than in female, and that in juvenile group was significantly higher than in adults. Among positive cases in inhalation group, the most common allergen was flour mite (80.4%), followed by house dust mite (64.9%), cockroach (13.3%) and artemisia pollen (8.2%). CONCLUSION The study shows that the flour mite and house dust mite are the most common inhaled allergens causing allergic rhinitis in Yichang. We should pay more attention to the prevention and treatment for the juvenile patients.

【Objective】To explore the therapeutic effect of TCM syndrome differentiation and treatment for immunoglobulin A(IgA) nephropathy.【Methods】The selected 123 patients with IgA nephropathy were divided into two groups(in the proportion of 3∶1) by number randomization.Group A(n=86) was given Tripterygium Glycosides tablets and differential treatment according to syndrome patterns,and Group B(n=37) was given routine western medicine including anti-inflammation drugs,drugs for controlling blood pressure and glucocorticoid hormone.The two groups received a 3-month treatment course and received one more course according to individual cases.The total therapeutic effect,effect for TCM syndrome patterns,and toxic and side effects were observed.The changes of TCM syndrome scoring were compared before and after treatment.【Results】In group A,symptoms were completely relieved in 29,markedly relieved in 30,relieved in 15 and un-relieved in 12 patients,the total effective rate being 86.05%,while respectively in 4,5,10 and 18 of patients in group B,the total effective rate being 51.35%.The total effect was better in group A than that in group B(P0.05).The improvement on TCM syndrome scoring in group A was superior to that in group B(P
