1.Prevalence of hypertension and its influencing factors among the elderly in Qinghai Plateau

Objective To investigate the current status of hypertension in the old adults living in urban city and rural areas in Qinghai Plateau and analyze the related influencing factors in order to provide data and evidence for targeted formulation of preventive and control measures for the pop-ulation.Methods Cluster-random sampling was used to subject 1372 elderly people(aged ≥60 years)from 8 urban areas and 25 natural villages in Xining City,Qinghai Province.Questionnaires were used to collect their demographic data,body mass index(BMI),history of chronic diseases,and lipid-related indicators.According to complicated with hypertension or not,they were divided into a hypertension group(615 cases)and a non-hypertension group(757 cases).SPSS 26.0 soft-ware was employed to perform statistical analyses with descriptive analysis and multivarlate un-conditional logistic regression analysis.Results Among the 1372 elderly persons,615 participants had hypertension,and the overall prevalence was 44.8%,and that in urban area and rural area was 50.1%and 38.5%,respectively,with significant difference(P<0.01).Statistical differences were observed between those with and without hypertension in terms of age,BMI,and proportions of coronary heart disease(CHD),diabetes and stroke(P<0.05,P<0.01).In the urban populations,there were obvious differences in marital status,BMI,and proportions of CHD and diabetes be-tween those with and without hypertension(P<0.01).For the rural populations,notable differ-ences were observed in age and proportions of CHD and diabetes between those with and without hypertension(P<0.05,P<0.01).Multivariate unconditional logistic regression analysis revealed that urban areas,obesity,CHD and diabetes were risk factors for hypertension in the elderly living in the urban and rural areas(OR=1.622,95%CI:1.299-2.026,P=0.000;OR=0.564,95%CI:0.315-1.006,P=0.042;OR=0.604,95%CI:0.417-0.874,P=0.008;OR=0.472,95%CI:0.328-0.678,P=0.000;OR=0.474,95%CI:0.334-0.673,P=0.000).Obesity,CHD and diabetes were risk factors for hypertension in those in the urban areas(OR=0.553,95%CI:0.317-0.963,P=0.036;OR=0.506,95%CI:0.320-0.800,P=0.004;OR=0.458,95%CI:0.303-0.692,P=0.000),and CHD and diabetes were risk factors in those in the rural areas(OR=0.382,95%CI:0.219-0.666,P=0.001;OR=0.452,95%CI:0.253-0.807,P=0.007).Conclusion There is sig-nificant difference in the prevalence of hypertension between the elderly living in the urban city and rural areas in Qinghai Plateau.The old adults with overweight,obesity,and complication of CHD and diabetes are prone to developing hypertension.

3.Early recognition of coronary artery lesion in Kawasaki disease and its relationship with monocyte to HDL-C ratio

Objective:To investigate the early recognition of coronary artery lesions(CAL)in Kawasaki disease(KD)and its relationship with monocyte/high-density lipoprotein cholesterol(HDL-C)ratio(MHR).Methods:A total of 216 children with KD who were hospitalized in Affiliated Hospital of Nantong University from June 2019 to June 2022 were selected as the research subjects,and divided into training set(162 cases)and test set(54 cases).The clinical data of the children were collected,and the children in the training set were divided into the CAL group(45 cases)and the NCAL group(117 cases)according to the diagnostic results of echo-cardiography,and the differences in clinical data and laboratory test results were compared between the two groups;Logistic regres-sion analysis was used to analyze the risk factors of CAL in children with KD;Pearson was used to analyze the correlation between MHR and CAL in children with KD.According to the MHR quantile,the children in the CAL group were divided into low MHR group(≤0.28),medium MHR group(0.29~0.42)and high MHR group(≥0.43),and they were analyzed and compared.Cox regression model was used to analyze the relationship between MHR and CAL risk in children with KD,and a predictive model was constructed based on the independent risk factors of CAL in children with KD.Results:There were 162 KD children with fever,and summer was a high incidence period;compared with the NCAL group,the CAL group had statistically significant differences in age,gender,fever time,KD type,MHR,WBC,PLT,NLR,and CRP(all P<0.05);Pearson correlation analysis showed that MHR was positively cor-related with the degree of coronary artery dilatation in children with CAL(r=0.743,P=0.001).and the risk of CAL in the KD children in the high MHR group was significantly higher than that in the low MHR group(HR=2.857,95%CI:1.329~6.431,P=0.003);Logis-tic regression analysis showed that gender,fever time,MHR,WBC,NLR and CRP were independent risk factors for CAL in children with KD.A prediction model was constructed based on the independent risk factors of CAL:Logit(P)=1.342+0.359×gender+0.181×ever time+1.064×MHR+0.459×WBC+0.146×NLR+0.211×CRP,P=e logit(P)/1+e logit(P),the AUC of this model was 0.874(95%CI:0.799~0.892),compared with the test set(AUC was 0.881,95%CI:0.785~0.913),the difference was not statistically sig-nificant(P>0.05);the AUC of MHR for predicting CAL in children with KD was 0.796,the sensitivity was 0.896,and the specificity was 0.824,which could be used as an early predictor of CAL in children with KD.Conclusion:MHR has a certain predictive value in the diagnosis of CAL in children with KD,and can reflect the degree of CAL in children with KD to a certain extent.Therefore,it is necessary to pay attention to the changes of MHR in children with KD in clinical practice.
4.Directional screening and identification of potential cytotoxic components from Achnatherum inebrians by a combination of surface palsmon resonance and chromatography.

OBJECTIVE:
To establish a method for directional screening of the cytotoxic components from the medicinal herb of Achnatherum inebrians by a combination of surface plasmon resonance (SPR) biosensor and chromatographic isolation technology.
METHODS:
Under the guidance of bioactive assessment based on binding abilities between objects and the α-Mannosidase (α-Man) target, the active components from different solvents extracts, different polar extraction parts and fractions were screened orderly and directionally using SPR. Components with a high binding ability to α-Man can be precisely oriented in a narrower fractions range and are easy to isolate. Three human cancer cells were used to evaluate the cytotoxic activity of component with the highest affinity to α-Man.
RESULTS:
Eight compounds were isolated and identificated from A. inebrians for the first time. Deoxyvasicinone possessed the highest affinity to α-Man among them. Moreover, deoxyvasicinone showed good effects on inhibited proliferation of human hepatoma cells HepG2 (IC50 = 5.7 μmol/L), human breast cancer cells MCF7 (IC50 = 7.21 μmol/L) and human lung cancer cells HCC827 (IC50 = 0.75 μmol/L), respectively. In particular, its inhibitory effect on HCC827 was stronger than the positive drug gefitinib (IC50 = 1.65 μmol/L).
CONCLUSION
A comprehensive strategy of directional screening potential cytotoxic components from herb based on biomolecular interaction and chromatography was established. Deoxyvasicinone as an effective anti-cancer component was initially isolated from A. inebrians. It is expected that this screening strategy could provide new perspectives for rapid screening and identification of active components from natural plants with the complex matrix.
5.Regulation of Fuxian Lotion on H1R, PAR-2/TRPV1 Itch Signal Pathway in Mice with Chronic Dermatitis Eczema Through T Cell-mediated Immune Balance

OBJECTIVE To explore the role of Th1/Th2 immune balance in the signal transduction of H1R, PAR-2/TRPV1 itch signal pathway and the intervention mechanism of Fuxian lotion. METHODS There were 60 BALB/c mice randomly divided into 6 groups after 3 days of adaptive feeding, with 10 mice in each group, which were blank control group, model control group, high, medium and low dose group of Fuxian lotion(36, 12, 4 mg·cm-2) and positive drug control group(1 mg·cm-2). Except for the blank control group, all other groups were sensitized with 7% and 0.5% 2,4-dinitrochlorobenzene acetone olive oil solution for multiple times. The abdomen and ears of mice in the blank group were coated with acetone solution for control. The drug was administered 24 hours after challenge, twice a day for 10 consecutive days. The ear thickness and the degree of mice ear swelling were measured and scored. The times of scratching the ears of mice were counted. The spleen index was calculated. HE staining was used to observe the pathological changes in mice ear tissue. The serum TNF-α, IL-1β, IL-31 levels were detected by ELISA, the mRNA level of the above immune factors was detected by realtime PCR, and the expression of H1R, PAR-2 and TRPV1 was detected by immunohistochemistry. RESULTS Compared with the blank control group, the ear thickness, ear allergic reaction score, scratching index, the spleen index of mice in the model control group was significantly increased(P<0.01), the TNF-α, IL-1β, IL-31 protein level and mRNA level were significantly increased(P<0.01), and the contents of H1R, PAR-2 and TRPV1 were significantly increased(P<0.01). There were obvious eczema like pathological changes in chronic dermatitis. Compared with the model control group, Fuxian lotion could effectively reduce the degree of auricle swelling, the score of ear allergic reaction, scratch index and spleen index, significantly reduce the levels of TNF-α, IL-1β, IL-31 protein level and mRNA level(P<0.01). The content of H1R in the high dose group of Fuxian lotion, the medium dose group of Fuxian lotion and the positive drug control group was significantly reduced(P<0.01), and the content of PAR-2 and TRPV1 in all dose groups of Fuxian lotion and the positive drug control group were significantly reduced(P<0.01). CONCLUSION Fuxian lotion can improve chronic dermatitis and eczema by regulating the Th1/Th2 immune balance, down regulating the characteristic immune factor TNF-α, IL-1β, IL-31 and interfering with the itch signal pathway H1R, PAR-2/TRPV1 transmission, so as to anti-itching and inhibit inflammatory response.
6.Current status of hospital infection management in psychiatric hospitals in the Ningxia Hui Autonomous Region

ObjectiveTo investigate current status of hospital infection management in psychiatric hospitals in the Ningxia Hui Autonomous Region, so as to provide references for improving the level of hospital infection management of psychiatric hospitals. MethodsIn December 2020, on-site supervision was conducted on hospital infection management in all 9 psychiatric hospitals in the Ningxia Hui Autonomous Region, meantime, the self-compiled questionnaire on hospital infection management status was used for investigation. ResultsAmong the selected hospitals, nine (100.00%) psychiatric hospitals had the main hospital leaders in charge of hospital infection management, five (55.56%) hospitals had established a hospital infection management committee, six (66.67%) hospitals had established an independent hospital infection management department, and one (11.11%) hospital had developed all 13 systems mentioned in the questionnaire related to hospital infection management and job responsibilities. In terms of hospital infection management staff, there were 23 staff members in the nine psychiatric hospitals, including 3 in the specialty (13.04%) and 20 in the part-time setting (86.96%). The score of the implementation of the basic system of hospital infection management in nine hospitals was (3.28±2.22). ConclusionThe system specification related to hospital infection management in the Ningxia Hui Autonomous Region psychiatric hospitals needs to be improved and further strengthened, the professionalism of hospital infection management personnel needs to be improved.
7.Effects of aerobic exercise on the heart rate variability in health elderly people living in hypoxia areas at high altitude

Objective To assess the effect of aerobic exercise on the cardiovascular autonomic regulation in healthy elderly people living in hypoxia areas at high altitude,through observing the 24 hours of heart rate variability(HRV) and its changes in HRV before,during and after incremental exercise.Methods 60 healthy elderly(> 60 years old)living in Qinghai-Tibet plateau(altitude > 2 600 m)were selected and divided into aerobic exercise group(practicing Tibetan traditional dance in long term,n=30)and control group (lacking aerobic exercise,n =30).Blood pressure,heart rate (HR) and oxygen saturation (SO2) were measured.The time domain parameters of HRV were detected by 24 hour Holter test.The time domains of HRV before,during and after incremental exercise were observed by Marquette 2000 treadmill exercise test.Results Systolic blood pressure,diastolic blood pressure and heart rate were lower(t=2.475,2.295,2.099,all P<0.05),and SO2 and SO2/HR ratio was higher(t=2.907,2.210,P<0.05)in aerobic exercise group than in control group.As compared with control group,the aerobic exercise group showed that the changes in SDNN were increased significantly during and 10 minutes after incremental exercise(both P<0.05),and RMSSD and PNN50 were significantly decreased during and 10 minutes after exercise(both P<0.05).Conclusions Regular aerobic exercise in the elderly people living in hypoxia areas at high altitude can enhance oxygen uptake and utilization,increase the heart rate variability,reduce heart rate and blood pressure,and improve the cardiac autonomic function.
8.Analysis on correlation between semen parameters with spontaneous abortion in early pregnancy after in vitro fertilization assisted pregnancy operation

Objective To investigate the correlation between semen parameters with spontaneous abortion in early pregnancy after in vitro fertilization (IVF) assisted pregnancy operation .Methods 220 patients with successful pregnancy after IVF assisted pregnancy operation in the reproductive center of this hospital were selected as the research subjects .Among them ,40 cases of early spontaneous abortion served as the observa-tion group and 180 cases without early spontaneous abortion occurrence as the control group .The related pa-rameters of sperm hyaluronic acid unbinding rate ,sperm nucleoprotein immaturity rate and sperm morphology normal rate were measured and analyzed by adopting the computer assisted sperm analysis (CASA) .Results The sperm hyaluronic acid unbinding rate and sperm nucleoprotein immaturity rate in the observation group were significantly higher than those in the control group ,the difference was statistically significant ( P<0 .05);the sperm morphology normal rate in the observation group was significantly lower than that in the control group ,the difference was statistically significant (P<0 .05);the other semen parameters had no statis-tical differences between the two groups (P>0 .05) .The statistical analysis result showed that the sperm hy-aluronic acid unbinding rate ,sperm DNA fragmentation rate and sperm nucleoprotein immaturity had the posi-tive correlation between each other ,which had the negative correlation with the other indicators .After the stepwise analysis by the binary Logistic regression model ,the results showed that the high risk factors affect-ing spontaneous abortion in early pregnancy after IVF included sperm nucleoprotein immaturity rate and sperm hyaluronic acid unbinding rate .Conclusion The sperm nucleoprotein immaturity rate and sperm hyalu-ronic acid unbinding rate are two high-risk factors inducing early spontaneous abortion after IVF .
9.Analysis of DSPP gene mutation in a Chinese pedigree affected with hereditary dentinogenesis imperfecta.

OBJECTIVETo analyze the clinical phenotype of a Chinese pedigree affected with hereditary dentinogenesis imperfecta and mutation of dentin sialophosphoprotein (DSPP) gene.
METHODSAffected members underwent intraoral photography, dental film and panoramic radiography. Genomic DNA was extracted from peripheral venous blood samples. Coding regions of the DSPP gene were subjected to PCR amplification and Sanger sequencing. Functional effect of the mutation was predicted with SIFT and PolyPhen-2. The tertiary structure of wild type and mutant proteins were predicted by Swiss-Port.
RESULTSA heterozygous c.50C to T (p.P17L) mutation was identified in exon 2 of the DSPP gene in the proband and her father. The same mutation was not found among 200 unrelated healthy controls. The Pro-17 residues and its surrounding positions in DSPP are highly conserved across various species. The mutation was predicted to be damaging to the structure of DSPP protein.
CONCLUSIONThe c.50C to T (p.P17L) mutation of the DSPP gene probably underlies the disease in this pedigree. Above finding has expanded the spectrum of DSPP gene mutations and provided a basis for genetic counseling and prenatal diagnosis for this family.
10.Whole exome sequencing identifies an AMBN missense mutation causing severe autosomal-dominant amelogenesis imperfecta and dentin disorders.

Tooth development is a complex process that involves precise and time-dependent orchestration of multiple genetic, molecular, and cellular interactions. Ameloblastin (AMBN, also named "amelin" or "sheathlin") is the second most abundant enamel matrix protein known to have a key role in amelogenesis. Amelogenesis imperfecta (AI [MIM: 104500]) refers to a genetically and phenotypically heterogeneous group of conditions characterized by inherited developmental enamel defects. The hereditary dentin disorders comprise a variety of autosomal-dominant genetic symptoms characterized by abnormal dentin structure affecting either the primary or both the primary and secondary teeth. The vital role of Ambn in amelogenesis has been confirmed experimentally using mouse models. Only two cases have been reported of mutations of AMBN associated with non-syndromic human AI. However, no AMBN missense mutations have been reported to be associated with both human AI and dentin disorders. We recruited one kindred with autosomal-dominant amelogenesis imperfecta (ADAI) and dentinogenesis imperfecta/dysplasia characterized by generalized severe enamel and dentin defects. Whole exome sequencing of the proband identified a novel heterozygous C-T point mutation at nucleotide position 1069 of the AMBN gene, causing a Pro to Ser mutation at the conserved amino acid position 357 of the protein. Exfoliated third molar teeth from the affected family members were found to have enamel and dentin of lower mineral density than control teeth, with thinner and easily fractured enamel, short and thick roots, and pulp obliteration. This study demonstrates, for the first time, that an AMBN missense mutation causes non-syndromic human AI and dentin disorders.
