A chemical investigation of secondary metabolites (SMs) from Aspergillus nidulans resulted in the identification of five novel dioxopiperazine (DKP)-diphenyl ether hybrids, designated as diphenylemestrins A-E (1-5). These compounds 1-5 represent the first known dimers combining DKP and diphenyl ether structures, with compound 4 featuring an uncommon dibenzofuran as the diphenyl ether component. The structural elucidation and determination of absolute stereochemistry were accomplished through spectroscopic analysis and electronic circular dichroism (ECD) calculations. Notably, diphenylemestrin C (3) exhibited moderate cytostatic activity against NB4 cells, with a half maximal inhibitory concentration (IC50) value of 21.99 μmol·L-1, and induced apoptosis at higher concentrations.

3.AngioJet-assisted catheter-directed thrombolysis versus catheter-directed thrombolysis alone in the treatment of acute deep venous thrombosis of lower extremity: a mid-term controlled clinical study

Objective:To compare the mid-term clinical effects of AngioJet rheolytic thrombectomy assisted catheter-directed thrombolysis (ART+CDT) with catheter-directed thrombolysis (CDT) in the treatment of acute deep venous thrombosis of lower extremities.Methods:Ninety-one patients admitted to the Department from Jan 2016 to Dec 2017 were placed with inferior vena cava filters and divided into ART+CDT group (30 cases)and CDT group (61 cases). Total urokinase dosge, thrombolytic time, operative cost, length of hospital stay, detumescence rate, thrombus clearance rate, cumulative patency rate of lower limb veins, Villalta score at 2 years and 5 years, thrombosis recurrence rate and chronic venous insufficiency quality of life questionnaire were compared between the two groups.Results:The success rate of surgery was 100% in both groups, there was no mortality. There were significant differences in the short-term postoperative outcomes between the two groups in terms of total dosage of urokinase, thrombolysis time, total cost of surgery, length of hospital stay, detumescence rate, venous patency scores before and after treatment, and venous patency rate (all P<0.05). For the mid- and long-term postoperative outcomes of 2 and 5 years, there were no significant differences in the incidence of PTS, recurrence rate of thrombus, chronic venous function scale, and cumulative patency rate at 2 years (all P>0.05). Conclusions:ART+CDT has a significant advantage over CDT alone in terms of early efficacy and early reopening of blood flow in patients. Both ART+CDT and CDT have a low incidence of PTS and a low recurrence rate of thrombus in the mid-term follow-up, and both have satisfactory performance in the mid- and long-term efficacy of interventional treatment of deep venous thrombosis of lower limbs.
4.Effects of discharge preparation service based on timing it right on care readiness and benefit finding of caregivers for elderly dementia patients

Objective:To explore the effect of discharge preparation service based on timing theory on the care readiness and benefit finding of caregivers for elderly dementia patients.Methods:From January 2021 to December 2022, 100 caregivers of elderly dementia patients from the Neurology Department and Rehabilitation Department of Jingjiang People's Hospital were selected as the subject by convenient sampling. Caregivers were divided into a control group and a study group based on their enrollment time, with 50 cases in each group. The control group received routine nursing, while the study group was treated with discharge preparation services based on timing theory. The effect was evaluated using the Caregiver Preparedness Scale (CPS) and Caregiver Benefit Finding Questionnaire.Results:After intervention, the CPS score of the study group was (22.80±2.83), which was higher than the control group's (17.92±2.60), and the difference was statistically significant ( P<0.05). After three months of discharge, the score of the Caregiver Benefit Finding Questionnaire in the study group was (117.50±6.25), which was higher than the control group's (109.98±9.89), and the difference was also statistically significant ( P<0.05) . Conclusions:The discharge preparation service based on timing it right can improve the care readiness and benefit finding of caregivers for elderly dementia patients.

Existing epidemiologic and clinical studies have demonstrated that obesity is associated with the risk of a variety of cancers. In recent years, an increasing number of experimental and clinical studies have unraveled the complex relationship between obesity and cancer risk and the underlying mechanisms. Obesity-induced abnormalities in immunity and biochemical metabolism, including chronic inflammation, hormonal disorders, dysregulation of adipokines, and microbial dysbiosis, may be important contributors to cancer development and progression. These contributors play different roles in cancer development and progression at different sites. Lifestyle changes, weight loss medications, and bariatric surgery are key approaches for weight-centered, obesity-related cancer prevention. Treatment of obesity-related inflammation and hormonal or metabolic dysregulation with medications has also shown promise in preventing obesity-related cancers. In this review, we summarize the mechanisms through which obesity affects the risk of cancer at different sites and explore intervention strategies for the prevention of obesity-associated cancers, concluding with unresolved questions and future directions regarding the link between obesity and cancer. The aim is to provide valuable theoretical foundations and insights for the in-depth exploration of the complex relationship between obesity and cancer risk and its clinical applications.

6.Identification and Expression Analysis of AP2/ERF Family in Stress Responses of Pinellia ternata Based on Transcriptome Data

ObjectiveTo identify the functions of the AP2/ERF family members in Pinellia ternata and promote the genetic improvement of P. ternata varieties. MethodWe identified and conducted a systematic bioinformatics analysis of the AP2/ERF family member genes in P. ternata based on the three generations of transcriptome data. Real-time polymerase Chain reaction (Real-time) PCR was employed to determine the expression pattern of AP2/ERF genes in different tissues and under different stress conditions. ResultA total of eight full-length AP2/ERF family members were identified from the transcriptome data, which were classified into three sub-gene families: AP2, ERF, and DREB. The deduced AP2/ERF proteins in P. ternata had the length of 251-512 aa, the theoretical pI of 5.29-11.72, the instability index of 45.90-82.41, subcellular localization in the nucleus, and conserved domains and motifs. AP2/ERF genes were expressed in different tissues of P. ternata, with high expression levels in the leaf. The stress response experiments showed that PtERF1 mainly responded to NaCl stress. The expression of PtERF2 and PtERF4 was significantly up-regulated under low temperature and polyethylene glycol (PEG)-simulated stress. PtERF3 responded to both low temperature and NaCl stress. The expression of PtERF5 was induced by high temperature, low temperature, NaCl and PEG stress. The expression of PtERF7 was up-regulated under high temperature, while that of PtERF8 under low temperature. ConclusionThe AP2/ERF genes in P. ternata can respond to stress and have the potential functions of regulating photosynthesis and improving root stress resistance.
7.Analysis of current situation and influencing factors of online health information seeking behavior in older patients with coronary heart disease

Objective:To explore the situation and influencing factors of online health information seeking behavior of older patients with coronary heart disease.Methods:From July to November 2021, a cross-sectional survey was conducted on 451 older patients with coronary heart disease in four districts of Qingdao City using the general information questionnaire, Patient Activation Scale, Social Support Scale and Online Health Information Seeking Behavior Scale.Results:Olderpatients with coronary heart disease had a score of (70.69 ± 9.19) for online health information seeking behavior. The results of multiple linear regression analysis showed that gender, education, internet use frequency, social support and patient activation were the main influencing factors of online health information seeking behavior ( R2=0.639, F=31.58, all P<0.05). Conclusions:Older patients with coronary heart disease have a moderate level of online health information seeking behavior, and is influenced by multiple factors. Targeted measures should be taken to make patients actively search for disease information online to prevent disease deterioration and promote healthy aging.
8.Long-term outcomes and influencing factors of idiopathic nephrotic syndrome in children

Pediatric idiopathic nephrotic syndrome (INS) is characterized by massive albuminuria, hypoproteinemia, edema and hyperlipidemia, with a long course and high probability of relapse and prolongation. Long-term complications caused by long-term usage of hormones and immunosuppressants in children with INS seriously affect their physical and mental health and quality of life. Most children with steroid-sensitive nephrotic syndrome can be cured before adulthood, while some of them relapse in adulthood. Long-term prognosis of children with steroid-resistant nephrotic syndrome is poor. There have been few studies in China followed the long-term outcomes and its related factors of children with INS over 10 years. The paper reviewed the literatures on the long-term outcomes of children with INS, including renal survival, growth, mental health, learning and work, marriage and fertility, disease recurrence and long-term related complications, to explore the factors related to the poor long-term outcomes of children with INS and to assist in clinical decision-making and follow-up management.
9.A survey of benefit finding of caregivers in senile dementia patients and analysis of influencing factors

Objective:To investigate the status of caregivers' benefit finding of senile dementia patients in Shiyan city, Hubei Province, and to analyze its influencing factors, so as to provide basis for carrying out targeted intervention measures.Methods:Totally 252 caregivers of senile dementia patients who visited Taihe Hospital, Shiyan People′s Hospital and Sinopharm Dongfeng General Hospital in Shiyan City, Hubei Province from July 2019 to July 2021 were selected as the research objects by convenience sampling method. The general Information Questionnaire, the Caregiver benefit finding questionnaire, the General self-efficacy Scale (GSES) and the Perceived Social Support Scale (PSSS) were used. The Pearson correlation analysis method was used for the correlation between caregivers' self-efficacy, social support and perceived benefit. Catreg was used to analyze the influencing factors of caregivers' benefit finding in senile dementia patients.Results:The total score of caregiver benefit questionnaire for 252 caregivers of senile dementia patients was (104.74 ± 14.82), the item "made me realize the significance of my health to my family and society" received the highest score (4.70 ± 0.55), while the item "made me quit bad habits" received the lowest score (3.22 ± 1.17). The GSES and PSSS scores of 252 caregivers of senile dementia patients were (24.74 ± 7.02) and (54.21 ± 13.32), which were positively correlated with the benefit finding ( r=0.565, 0.459, both P<0.01). The catreg analysis showed that the age of the caregiver, the average monthly income of the family, self-efficacy and social support were the influential factors of the perceived benefit level of the caregiver of senile dementia patients ( F values were 3.30-104.92, all P<0.05), which could explain 42.4% of the variance. Conclusions:The benefit finding of caregivers of senile dementia patients still needs to be improved. Medical staff should pay attention to the evaluation of benefit finding of caregivers of senile dementia patients, and provide personalized intervention measures to relieve caregivers' care pressure, maintain their physical and mental health, and improve the care quality of caregivers of senile dementia patients.
10.Correlation between psychological resilience and self-neglect of community empty-nest elderly: the mediating effect of depression

Objective:To explore the correlation between psychological resilience, self-neglect and depression among community empty-nest elderly people, and to analyze the mediating role of depression between psychological resilience and self-neglect.Methods:This study was a cross-sectional study. From July to October 2020, a stratified multi-stage sampling was used to select 560 community empty-nest elderly people from 6 community health service centers in Qingdao as the research object. The survey was carried out using the General Information Questionnaire, the Chinese version of the Connor-Davidson Resilience Scale (CD-RISC) , the simplified version of the Geriatric Depression Scale, and the Geriatric Self-neglect Scale. Pearson correlation was used to analyze the correlation between variables. The structural equation model was constructed using AMOS 24.0. A total of 560 questionnaires were distributed, and 549 valid questionnaires were recovered, with the valid recovery rate of 98.04%.Results:Among 549 empty-nest elderly people, the total score of Chinese version of CD-RISC was (64.30±14.57) , and the average score of the Geriatric Self-neglect Scale was (3.72±2.67) . There were 174 elderly people (31.70%) with depressive symptoms. The total score and the scores of each dimension of the Chinese version of the CD-RISC in the community empty-nest elderly people were negatively correlated with the total score of the simplified version of the Geriatric Depression Scale ( P<0.01) . Except for the dimension of safety self-neglect, the scores of other dimensions and the total score of the Geriatric Self-neglect Scale were negatively correlated with the total score and the scores of each dimension of the Chinese version of the CD-RISC ( P<0.01) , and were positively correlated with the total score of the simplified version of the Geriatric Depression Scale ( P<0.01) . Depression played a mediating role between the psychological resilience and self-neglect of empty-nest elderly in the community, and the mediating effect accounted for 53.85% of the total effect. Conclusions:Psychological resilience of community empty-nest elderly can directly affect self-neglect, and can also indirectly affect self-neglect through depression. Attention should be paid to the improvement of the psychological resilience of the empty-nest elderly in the community, to reduce the occurrence of self-neglect, and to promote their healthy aging.
