Objective To investigate the microbial disinfection and sterilization effectiveness of an electron linear accelerator on the surface and interior of postal packages, and to design and conduct a penetrating radiation disinfection and sterilization experiment. Methods This experiment selected Bacillus pumilus E601 (ATCC 27142) as the indicator microbial strain for radiation. An experimental model of postal package was subjected to penetrating radiation with different dose gradients using high-energy electron beams generated by an S-band 10 MeV electron linear accelerator. The disinfection and sterilization effectiveness was assessed by culturing and counting B. pumilus after high-energy electron beam radiation treatment. A control group was established to analyze the influence of actual absorbed doses at different gradients on the disinfection and sterilization effectiveness of the electron linear accelerator. Results The actual absorbed doses of high-energy electron-beam radiation required to achieve 1-, 3-, and 6-log reductions of B. pumilus were 1.63, 5.07, and 10.22 kGy, respectively. Complete inactivation was achieved at an absorbed dose of 10.92 kGy, which met the processing requirements specified in the technical standard for ionizing-radiation disinfection. Conclusion The experimental data indicated that the actual absorbed dose for B. pumilus is linearly positively correlated with the log reduction of bacterial colonies killed by electron beam radiation. For disinfection and sterilization requirements in different scenarios, the required actual absorbed dose can be achieved by dynamically adjusting the operating speed of the under-beam transmission system.

BACKGROUND: Generalized pustular psoriasis (GPP), a rare and recurrent autoinflammatory disease, imposes a substantial burden on patients and society. Awareness of GPP in China remains limited. METHODS: This cross-sectional survey, conducted between September 2021 and May 2023 across 14 hospitals in China, included GPP patients of all ages and disease phases. Data collected encompassed demographics, clinical characteristics, economic impact, disease severity, quality of life, and treatment-related complications. Risk factors for GPP recurrence were analyzed. RESULTS: Among 127 patients (female/male ratio = 1.35:1), the mean age of disease onset was 25 years (1st quartile [Q1]-3rd quartile [Q3]: 11-44 years); 29.2% had experienced GPP for more than 10 years. Recurrence occurred in 75.6% of patients, and nearly half reported no identifiable triggers. Younger age at disease onset ( P  = 0.021) and transitioning to plaque psoriasis ( P  = 0.022) were associated with higher recurrence rates. The median diagnostic delay was 8 months (Q1-Q3: 2-41 months), and 32.3% of patients reported misdiagnoses. Comorbidities were present in 53.5% of patients, whereas 51.1% experienced systemic complications during treatment. Depression and anxiety affected 84.5% and 95.6% of patients, respectively. During GPP flares, the median Dermatology Life Quality Index score was 19.0 (Q1-Q3: 13.0-23.5). This score showed significant differences between patients with and without systemic symptoms; it demonstrated correlations with both depression and anxiety scores. Treatment costs caused financial hardship in 55.9% of patients, underscoring the burden associated with GPP. CONCLUSIONS The substantial disease and economic burdens among Chinese GPP patients warrant increased attention. Patients with early onset disease and those transitioning to plaque psoriasis require targeted interventions to mitigate the high recurrence risk.

Presbyopia is a physiological phenomenon related to age, in which there is a progressive difficulty in near vision due to decreased accommodation ability. It is one of the main reasons for the decline in visual related quality of life of people over 40 years old. The therapeutic objective is to improve symptoms, enhance functional near vision, and achieve the goal of restoring self-accommodation. A multitude of correction strategies are available, conventional spectacles is the simplest and safest method, which including monofocal, bifocal, trifocal, or gradient multifocal glasses. With the increasing demand for visual quality, corneal contact lenses and presbyopia correction surgeries(corneal refractive surgery, intraocular surgery, and scleral surgery)are becoming more and more accepted. The goal of these surgeries is to correct existing refractive errors while properly addressing the issue of decreased accommodation ability. At the same time, research on pharmacological interventions for presbyopia has been increasing year by year, including miotics and lens softeners, to improve the symptoms of presbyopia through different mechanisms. This article provides a review of relevant literature on presbyopia correction in recent years.

Objective:To develop a treatment-related quality of life scale for Chinese patients with multiple myeloma (MM) and to test its reliability and validity.Methods:The initial scale was constructed through a literature search, Delphi expert correspondence, and cognitive testing. This study conducted a preliminary survey of 379 patients with MM and a formal survey of 865 patients from the hematology departments of 155 hospitals nationwide from February 2024 to March 2024. The final scale was obtained after conducting item analysis and reliability and validity tests on the initial scale.Results:The constructed scale contains 36 items covering six domains: physiological, psychological, social, treatment side effects, general health, and others. In the preliminary survey, the Cronbach’s alpha coefficient of each item ranged from 0.597 to 0.939, and the test-retest reliability was 0.747 ( P<0.001). Exploratory factor analysis extracted eight common factors with a cumulative variance contribution of 60.058%. In the formal survey, the Cronbach’s alpha coefficient of each item ranged from 0.484 to 0.930, and the test-retest reliability was 0.835 ( P<0.001). Confirmatory factor analysis revealed a comparative fit index of 0.750, a root-mean-square error of approximation of 0.090, and a root-mean-square residual of 0.067. Conclusion:The treatment-related quality of life scale for Chinese patients with MM designed in this study exhibited good reliability and validity, reflecting the impact of treatment on the quality of life of patients. This scale can provide a reference to clinicians for assessing the disease status of patients.

Objective:To analyze the protective effect of a respiratory syncytial virus (RSV) vaccine based on bacterial outer membrane vesicle (OMV) in mice.Methods:The pre-fusion protein (preF) of RSV was linked to the surface of OMV through the transmembrane protein cytolysin A (ClyA) to form the nanovaccine OMV-preF. The morphological characteristics of OMV and OMV-preF were observed under a transmission electron microscope. OMV-preF was intramuscularly injected into BALB/c mice and the elicited humoral and cellular immune responses were evaluated. The protective effect of OMV-preF was assessed by challenging the immunized mice with RSV Long strain. One-way analysis of variance and Tukey test were used for statistical analysis.Results:The results showed that preF was stably expressed in OMV, and both OMV-preF and OMV exhibited a double-layer vesicle structures under the microscope. OMV-preF could significantly activate the cellular and humoral immune responses in mice, causing a significant increase in CD8 + T cells and CD19 + B cells as well as a significant increase in the serum level of specific IgG. The neutralizing antibodies produced in the immunized mice could significantly inhibit the replication of RSV Long strain in vivo. Conclusions:The nanovaccine OMV-preF can induce high-level humoral and cellular immune responses, and the antibodies produced following immunization can effectively inhibit viral replication. This study provides a new strategy for RSV subunit vaccines.

OBJECTIVE: To investigate the clinical phenotype and genetic characteristics of a family with Dentinogenesis imperfecta type Ⅰ(DGI-Ⅰ). METHODS: Clinical data were collected from a patient with DGI-Ⅰ admitted to the Reproductive Medicine Department of the Affiliated Hospital of Jining Medical University in March 2024. Clinical and familial data were retrospectively collected. Peripheral blood samples (5 mL each) were obtained from the proband and her family members for genomic DNA extraction, followed by whole-exome sequencing (WES) and Sanger sequencing validation. The pathogenicity of the detected variants was assessed according to the Classification Standards and Guidelines for Genetic Variants formulated by the American Society of Medical Genetics and Genomics (ACMG) (hereinafter referred to as the "ACMG Guidelines"). The study was approved by the Ethics Committee of the Affiliated Hospital of Jining Medical University (Ethics No. 2024-08-C012), and written informed consent for clinical research were obtained from all participants. RESULTS: The proband, a 35-year-old female, presented with translucent yellow primary teeth and progressive browning, darkening, and loss of permanent teeth, without skeletal abnormalities. Affected family members exhibited similar phenotypes. Genetic testing revealed a heterozygous COL1A2 variant (c.1503+1G>A) in the patient and other affected members, while unaffected family members all lacked this variant. Based on the ACMG Guidelines, this variant was classified as likely pathogenic (PM4 + PP1_Strong + PM2_Supporting). CONCLUSION The COL1A2 c.1503+1G>A heterozygous variant is the disease-causing mutation in this family. Above finding has expanded the mutational spectrum of the COL1A2 gene and provided a basis for genetic counseling and diagnosis in similar cases.

Objective:To explore the spatiotemporal evolution and convergence trend of the level of health resource supply in the Yangtze River Delta Region,and to provide quantitative support for the formulation of health resource supply policies.Methods:Using Dagum Gini coefficient,kernel density estimation,Markov chain,and spatial β convergence model to analyze the regional differences,dynamic evolution,and convergence of health resource supply levels in the Yangtze River Delta.Results:From 2010 to 2022,the level of health resource supply in the Yangtze River Delta maintained an increasing trend,with overall differences,intra regional and inter regional differences all showing a narrowing trend.Inter regional differences contributed the most,and Anhui Province's development was relatively balanced.There is a weak polarization phenomenon in the Yangtze River Delta as a whole,and the balance in Zhejiang has improved in the later stage.The spatial factors significantly affect the dynamic evolution process of the supply level of health resources in the Yangtze River Delta,with obvious positive spillover effects,and the supply level has β convergence characteristics.Conclusion:It is needed to promote the comprehensive development of regional economy,accelerate the improvement of the supply level of health resources in underdeveloped areas,and narrow the gap.At the same time,it needs to enhance spatial interaction between regions and pay attention to the positive spillover effects in areas with higher supply levels.

Objective:To explore the relationship between serum triglyceride to high-density lipoprotein cholesterol (TG/HDL-C) ratio, interleukin-17C (IL-17C), and C-C motif chemokine ligand-26 (CCL-26) with clinical symptoms and suicide ideation in patients with major depressive disorder.Methods:A total of 186 patients with major depressive disorder (major depressive disorder group) admitted to the Heze Third People′s Hospital from January 2021 to December 2024 were prospectively selected, including 66 males and 120 females, aged 19-68 (36.5±11.9) years old. In addition, 93 healthy and mentally healthy volunteers (control group) with matched gender and age were selected, including 38 males and 55 females, aged 19-65 (35.6±9.5) years old. According to the presence of suicidal ideation, patients with major depressive disorder were divided into a suicidal ideation group ( n=80) and a non-suicidal ideation group ( n=106). Serum TG/HDL-C ratio, IL-17C and CCL-26 levels of all participants were measured. Clinical symptoms were assessed using the 17-Item Hamilton Depression Rating Scale (HAMD 17), 14-Item Hamilton Anxiety Rating Scale (HAMA), and Pittsburgh Sleep Quality Index (PSQI). Spearman's rank correlation analysis was performed to evaluate associations between serum markers and clinical scores. Generalized additive models were used to assess nonlinear correlations. Multivariate unconditional logistic regression was conducted to identify independent risk factors for suicidal ideation. A nomogram prediction model was constructed. Model fit was evaluated with the H-L test, the area under the curve (AUC) of the receiver operating characteristic (ROC) curve assessed predictive ability, the C-index evaluated discrimination, calibration curves assessed agreement, and decision curve analysis assessed clinical utility. Results:Compared with the control group, the serum TG/HDL-C ratio, IL-17C level, HAMD 17 score, HAMA score and PSQI score increased in the severe depression group, while CCL-26 level decreased ( P<0.001). Serum TG/HDL-C ratio and IL-17C levels were positively correlated with HAMD 17, HAMA and PSQI scores ( r s=0.74, 0.74, 0.75, 0.78, 0.78, 0.79; all P<0.001), while CCL-26 levels were negatively correlated with these scores ( r s=-0.62, -0.63, -0.65, all P<0.001). There were nonlinear relationships between serum TG/HDL-C ratio, IL-17C, CCL-26 levels and the clinical symptom scores ( P<0.001). Among the 186 major depressive disorder patients, the incidence of suicidal ideation was 43.01% (80/186). History of attempted suicide, increased HAMD 17 score, increased HAMA score, increased PSQI score, elevated TG/HDL-C ratio and elevated IL-17C were independent risk factors for suicidal ideation in patients with severe depression ( OR (95% CI)=10.89 (2.27-52.25), 1.81 (1.19-2.74), 1.23 (1.08-1.40), 1.53 (1.15-2.02), 6.64 (2.71-16.24), 1.46 (1.18-1.81) respectively), elevated CCL-26 were independent protective factor( OR (95% CI)=0.22 (0.11-0.43)). Constructing a column chart prediction model for suicidal ideation in patients with severe depression based on the independent factors affecting suicidal ideation in patients with severe depression mentioned above, the AUC for predicting suicidal ideation in patients with severe depression is 0.93 (95% CI:0.88-0.96), and the C-index is 0.93 (95% CI:0.92-0.93). The decision curve showed that the predicted probability was consistent with the ideal curve, and when the threshold probability was greater than 0.10, the clinical net benefit was greater than 0. Conclusion:Elevated serum TG/HDL-C ratio and IL-17C levels, as well as decreased CCL-26 levels, are associated with depression, anxiety, sleep disorders, and suicidal ideation in patients with severe depression. The column chart prediction model based on serum TG/HDL-C ratio, IL-17C and CCL-26 levels has high predictive value for suicidal ideation.

Diseases caused by animal virus infection seriously restricts the healthy development of animal husbandry.In-depth study of the molecular mechanism of viral replication and pathogenesis will provide theoretical basis for screening vaccine and drug targets.N6-methyladenosine(m6 A)modification occurs extensively in viral and host transcriptomes and affects viral replication and pathogenicity by regulating gene expression,which acts as a novel regulator of gene expression in addition to DNA and protein modifications.Insight into the regulatory molecular mechanism of m6 A modification in virus infection is the research hotspots and frontiers.In recent years,there are re-ports of alternation of the m6 A modification-associated epitranscriptomes and related function a-nalysis during virus infection.Here,we summarize the alternation of the epitranscriptomes induced by African swine fever virus(ASFV),porcine reproductive and respiratory syndrome virus(PRRSV),porcine epidemic diarrhoea virus(PEDV),cestode virus(CSFV),porcine pseudorabies virus(PRV),Marek's disease virus(MDV),Newcastle disease virus(NDV),avian leukaemia virus(ALV)and duck hepatitis A virus(DHAV)infection,and the subsequent effects on viral replica-tion and pathogenicity.We also discuss the potential role and molecular mechanism of m6 A modification in animal virus replication and pathogenesis,which will contributes to the prevention and control for animal disease.
