1.Investigation and analysis of medical radiation application frequency in Jinan City, China

Objective To investigate and analyze the resources and application frequency of radiological diagnosis and treatment in Jinan City in 2023 and provide a basis for the rational application of radiological diagnosis and treatment resources and strengthening radiological health protection management. Methods The health administrative department issued a work plan. A general survey was conducted on radiological diagnosis and treatment institutions (excluding dental clinics) in Jinan City using a questionnaire. The survey covered the basic information of the radiological diagnosis and treatment institutions, the distribution of the radiological diagnosis and treatment equipment, the number of radiological workers, and the frequency of radiological diagnosis and treatment. IgA nephropathy(IgAN)is a common primary glomerular disease in children,characterized by the deposition of IgA or IgA-dominant immune complexes in the glomeruli.The clinical manifestations are highly heterogeneous,with some children progressing to end-stage renal disease.The prognosis of IgAN is influenced by multiple factors,involving clinical,pathological,and laboratory examinations.Among the clinical factors,persistent proteinuria,reduced baseline estimated glomerular filtration rate(eGFR),hypertension during follow-up,and hyperuricemia are risk factors for poor prognosis.The predictive value of hematuria remains controversial.Pathologically,the Oxford classification highlights that segmental sclerosis,tubular atrophy/interstitial fibrosis,and crescents are significantly associated with disease progression,with tubular atrophy/interstitial fibrosis serving as an independent risk factor for poor outcomes.The prognostic significance of mesangial hypercellularity and endocapillary hypercellularity requires further validation.Among the biomarkers,serum galactose-deficient IgA 1 and complement-related markers(such as IgA/C3 ratio)show potential predictive value,but need to be supported by large samples studies.This article reviews the clinical,pathological,and biomarker-related risk factors influencing the prognosis of pediatric IgAN,aiming to provide basis for developing risk prediction models and guiding individualized treatment strategies.
3.Phenotypic and genotypic analysis of five fetuses with Harlequin ichthyosis due to variants of ABCA12 gene.

OBJECTIVE:
To explore the clinical and genetic characteristics of five fetuses with Harlequin ichthyosis (HI).
METHODS:
Five fetuses with HI diagnosed at Guangdong Women and Children Hospital between 2017 and 2024 were selected as study subjects. Clinical and laboratory data were collected and reviewed. Whole exome sequencing (WES) was carried out, and candidate variants were verified by bioinformatic analysis. This study was approved by the Medical Ethics Committee of the hospital (Ethics No.: 202401024).
RESULTS:
The five fetuses had presented with ectropion, eclabium and contracture and flexion of fingers and toes. WES revealed that all had harbored compound heterozygous or homozygous variants of the ABCA12 gene. Among the eight types of variants, five were unreported previously.
CONCLUSION
The compound heterozygous or homozygous variants of the ABCA12 gene probably underlay the HI in the five fetuses. Clinicians should be vigilant about the possibility of HI in fetus with ectropion, eclabium, and contracture and flexion of fingers and toes.

4.Experimental study on the impact of nucleic acid purification and vacuum concentration on DNA recovery rate

Objective To investigate the effect of a nucleic acid purifier combined with a vacuum concentrator on DNA recovery rate.Methods Using 39 tubes of 007 standard samples,two 30 μL samples were taken from each tube.Samples were purified using the PrepFiler ExpressTM BTA Purification Kit on the AutoMate ExpressTM purifier,with elution volumes of 30 μL and 200 μL.Samples with 200 μL eluent were concentrated to 30 μL using a trace DNA sample vacuum concentrator.DNA solution concentrations were measured,and recovery rates were calculated and compared.Results The measured DNA concentration of the 007 standard samples was 0.08~0.15(0.11±0.02)ng/μL,showing a significant difference from the kit-labeled concentration of 0.1 ng/μL(t=3.88,P<0.01).The DNA solution concentration in the purified and concentrated group(200 μL eluent concentrated to 30 μL)[0.05~0.13(0.08±0.02)ng/μL]was significantly higher than the purified group(30 μL eluent)[0.02~0.08(0.05±0.01)ng/μL](t=21.20,P<0.01).The DNA recovery rate of the purified and concentrated group[54.55%~92.86%(75.41±10.12)%]was substantially higher than the purified group[25.00%~54.55%(39.79±7.13)%](t=19.79,P<0.01).Conclusion Utilizing a large-volume elution system in nucleic acid purification can significantly enhance DNA recovery rate.Combining this method with a vacuum concentrator effectively increases DNA solution concentration,thereby improving DNA testing success rates.

6.Prognosis of congenital bile duct cysts after cyst resection and risk factors analysis of postoperative biliary calculus

Objective:To study the prognosis of congenital bile duct cysts following cyst resection, and to analyze the risk factors associated with the development of postoperative biliary calculus.Methods:Clinical data of 149 patients with congenital bile duct cysts undergoing surgery in the First Affiliated Hospital of Nanjing Medical University from May 2004 to January 2022 were retrospectively analyzed, including 59 males and 90 females, with a median age of 32 (21, 47) years old. Patients were divided into two groups: the stone group ( n=51, biliary calculus occurred during the follow-ups after surgery) and non-stone group ( n=98). Clinical data such as gender, age, medical history, cyst type, biliary calculus, anastomotic stenosis and occurrence of cancer were compared. All patients were followed up via telephone consultations. A logistic regression analysis was used to identify the risk factors associated with the occurrence of biliary calculus after surgery. Results:The duration of the follow-ups was 120 (24, 211) months. The observed incidence of postoperative biliary calculus, anastomotic stricture, and cancer in the patients were 34.2% (51/149), 8.7% (13/149), and 4.7% (7/149), respectively. The logistic regression analysis indicated that incomplete cyst resection ( OR=3.332, 95% CI: 1.221-9.094) and postoperative anastomotic stenosis ( OR=13.300, 95% CI: 2.586-68.401) were associated with a higher risk of biliary calculus formation after cystectomy (all P<0.05). Conclusion:Patients with congenital bile duct cysts suffer a high risk of biliary calculus formation after cystectomy. The residual cyst and postoperative anastomotic stenosis are independent risk factors for biliary calculus after surgery.
7.Effect of iridoid glycosides from Boschniakia rossica on epithelial-mesenchymal transition of HepG2 cells induced by transforming growth factor-beta 1

ObjectiveTo investigate the effect of iridoid glycosides from Boschniakia rossica (IGBR) on epithelial-mesenchymal transition (EMT) of HepG2 hepatoma cells induced by transforming growth factor-beta 1 (TGF-β1). MethodsHepG2 hepatoma cells were induced by 10 μg/L TGF-β1 to construct an EMT model of hepatoma cells. The cells were divided into control group (treated with serum-free DMEM), model group (treated with 10 μg/L TGF-β1), and IGBR group (treated with 10 μg/L TGF-β1 and 500 mg/L IGBR), and all cells were cultured for 48 hours. Cell adhesion assay, wound healing assay, and Transwell chamber assay were used to observe the migration and invasion abilities of cells. RT-PCR and Western blot were used to measure the mRNA and protein expression levels of E-cadherin, N-cadherin, and vimentin in cells, and Western blot was used to measure the protein expression levels of Slug, Twist1, ZEB1, p-STAT3, and STAT3. A one-way analysis of variance was used for comparison of continuous data between multiple groups, and the least significant difference t-test was used for further comparison between two groups; the independent-samples t test was used for comparison between two groups. ResultsAfter TGF-β1 induction, HepG2 cells in the model group showed long spindle-shape changes, while those in the control group showed polygonal epithelia-like changes. Compared with the model group, the IGBR group had a significant reduction in cell adhesion rate and significant inhibition of cell migration and invasion abilities (all P<0.05), as well as significant increases in the mRNA and protein expression levels of E-cadherin (P<0.05), significant reductions in the mRNA and protein expression levels of N-cadherin and vimentin (all P<0.05), and significant reductions in the protein expression levels of Slug, Twist1, ZEB1, and p-STAT3 (all P<0.05). ConclusionIGBR can inhibit TGF-β1-induced EMT process in HepG2 cells, thereby attenuating cell adhesion, migration, and invasion abilities, and it can also upregulate E-cadherin, downregulate N-cadherin and vimentin, and upregulate the protein expression of Slug, Twist1, ZEB1, and STAT3, possibly by inhibiting the STAT3 pathway to downregulate the EMT transcription factors such as Slug, Twist1, and ZEB1.
8.Boschniakia rossica boschnaloside inhibition of epithelial-mesenchymal transition of hepatoma SMMC-7721 cells

Objective To investigate the inhibitory effect of Boschniakia rossica boschnaloside(BRBN)on the epithelial-mesenchymal transition(EMT),invasion,and migration of human hepatoma SMMC-7721 cells.Methods Transforming growth factor β1(TGF-β1)was used to induce an EMT model of SMMC-7721 cells.The cells were divided into a control group,model group,and BRBN group.Cell migration was detected with a wound healing test,and cell invasion was observed by Transwell chamber assay.The expression of E-cadherin,N-cadherin,Vimentin,Slug,Twist1,ZEB1,and signal transducers and activators of transcription 3(STAT3)were determined with the Western blot method.Results TGF-β1 resulted in spindle-shaped SMMC-7721 cells,down-regulated the expression of E-cadherin,and up-regulated the expression of N-cadherin and vimentin,suggesting that SMMC-7721 cells obtained a mesenchymal phenotype.Compared with the model group,the BRBN group SMMC-7721 cells'expression of E-cadherin was significantly increased,their expression of N-cadherin and vimentin were reduced,and their expression of Slug,Twist1,and ZEB1,as well as STAT3 phosphorylation,were down-regulated.Conclusions BRBN inhibits the invasion and migration of human hepatoma SMMC-7721 cells,possibly by reversing EMT through the STAT3 pathway.

10.Effect of exercise on cancer-related cognitive impairment in breast cancer patients:a Meta-analysis

Objective To evaluate the intervention effect of exercise on cancer related cognitive impairment of breast cancer patients,and to provide evidence-based evidence for the choices of rehabilitation programs for cognitive impairment.Methods The PubMed,Embase,Web of Science,Cochrane Library,Joanna Briggs Institute Library(JBI),CINAHL,CNKI,Wanfang,VIP and CBM were searched by computer for randomized controlled trials on the impact of exercise on cancer-related cognitive impairment in patients with breast cancer.The retrieval time limit was from the establishment of the database to October 2022.The quality of the included studies was evaluated according to Cochrane Handbook 5.1.0.RevMan5.4 was used for meta-analysis.Results A total of 18 publications including 1310 patients were included.The results showed that exercise could improve self-reported cancer-related cognitive impairment[SMD=0.33,95%CI(0.21,0.46),P<0.001],executive function[SMD=-0.27,95%CI(-0.42,-0.11),P<0.001]and attention[SMD=-0.37,95%CI(-0.60,-0.14),P<0.01],alleviate cancer-related fatigue[SMD=-0.56,95%CI(-0.79,-0.34),P<0.001],and reduce depression[SMD=-0.73,95%CI(-1.17,-0.30),P<0.001],but it has no significant effect on the memory of patients with breast cancer[SMD=0.05,95%CI(-0.16,0.27),P=0.64].Conclusion Exercise can improve the self-reported cancer-related cognitive impairment,executive function and attention of breast cancer patients,alleviate cancer-related fatigue,reduce depression,but it has no significant effect on improving memory,which still needs further verification.
