1.Supportive fibrous tissues of the nasal epithelium with special reference to the site-dependent difference

The nasal mucosa and submucosa likely contain both vascular beds against cold and dry air and resident immunoreactive cells against various antigens. Therefore, a specific fibrous structure seems to be necessary. Using histological specimens from 20 elderly cadavers, we examined the nasal mucosal and submucosal architecture. The ciliated columnar epithelium of the nasal mucosa was characterized by 1) a thick basal lamina, 2) few elastin-positive fibers beneath the epithelium, that was quite different from the nearby mucocutaneous junction area with a thick layer (0.3–0.8 mm) of elastic and oxytalan fibers corresponding to the skin dermis, 3) CD34-positive cells distributing diffusely in the submucosal tissue, and 4) few smooth muscle actin (SMA)-positive fibers beneath the epithelium. Some of submucosal fibrous structure appeared to express both elastin and CD34. CD34-positive arterioles were abundant beneath the ciliated epithelium, but they appeared negative for SMA antibody that cross-reacts with endothelium. Notably, the ciliated columnar epithelium was thin in the lateral wall of the nasal cavity, while the inferior concha carried the thick pseudostratified columnar epithelium.Strangely, the inferior or palatal wall of the nasal cavity was covered by the thick stratified epithelium. We found SMApositive mucosal venous plexus in the lateral wall of nasal cavity, but the submucosa was filled with glands in the inferior concha. Vascular beds might be replaced by glands in the nasal submucosa. The site-dependent difference in the mucosal morphology as well as the absence of vascular beds might be a result of secondary change with aging.
2.Why Patients in Institutions with Religious Background Achieve ‘Good Death’ ? : Findings from National Bereavement Survey of 127 Hospice and Palliative Care Units

Previous study reported that patients of institutions with religious background are more likely to achieve ‘good death’ than patients in non-religious institutions, although the reasons are unclear. This study aims to examine the reasons for this difference using a national survey of religious and non-religious institutions. We sent a self-administered questionnaire to 10,715 bereaved family caregiver and 133 hospice and palliative care units which cared for their family members prior to death. The questionnaire for bereaved families included the Good Death Inventory (GDI). The questionnaire for institutions included their religious affiliation and their subjective and objective provided care. In total, 7,286 bereaved families (68%) and 127 (95%) institutions including 23 religious end-of-life care institutions responded. We performed t-test and χ-square test to compare the GDI scores and provided care for religious and non-religious end-of-life care institutions. The GDI total score was significantly higher (p=0.01) in religious institutions compared to non-religious institutions. Items such as “providing many activities for seasonal events to the patients”, “providing more bereavement care”, “having facilities to practice their religion”, and “having regular visits from religious leaders”, were all reported more frequently in the religious institutions, and significantly associated with achievement of ‘good death’ (p<0.05 for all items). This study suggests that factors for achieving ‘good death’ are not only limited to providing religious care but also providing other forms of care, services, and activities that can enhance the patients’ ‘good death’.
3.Dual-Energy Subtraction Imaging for Diagnosing Vocal Cord Paralysis with Flat Panel Detector Radiography.

OBJECTIVE: To investigate the clinical feasibility of dual energy subtraction (DES) imaging to improve the delineation of the vocal cord and diagnostic accuracy of vocal cord paralysis as compared with the anterior-posterior view of flat panel detector (FPD) neck radiography. MATERIALS AND METHODS: For 122 consecutive patients who underwent both a flexible laryngoscopy and conventional/DES FPD radiography, three blinded readers retrospectively graded the radiographs during phonation and inspiration on a scale of 1 (poor) to 5 (excellent) for the delineation of the vocal cord, and in consensus, reviewed the diagnostic accuracy of vocal cord paralysis employing the laryngoscopy as the reference. We compared vocal cord delineation scores and accuracy of vocal cord paralysis diagnosis by both conventional and DES techniques using kappa statistics and assessing the area under the receiver operating characteristic curve (AUC). RESULTS: Vocal cord delineation scores by DES (mean, 4.2 +/- 0.4) were significantly higher than those by conventional imaging (mean, 3.3 +/- 0.5) (p < 0.0001). Sensitivity for diagnosing vocal cord paralysis by the conventional technique was 25%, whereas the specificity was 94%. Sensitivity by DES was 75%, whereas the specificity was 96%. The diagnostic accuracy by DES was significantly superior (kappa = 0.60, AUC = 0.909) to that by conventional technique (kappa = 0.18, AUC = 0.852) (p = 0.038). CONCLUSION: Dual energy subtraction is a superior method compared to the conventional FPD radiography for delineating the vocal cord and accurately diagnosing vocal cord paralysis.

4.A Case Report of the Interval Form of Carbon Monoxide Poisoning

A 56-year-old man was admitted to our hospital for the purpose of rehabilitation. Because this patient developed apallic syndrome 21 days after he recovered from his first coma, he was diagnosed with the interval form of carbon monoxide (CO) poisoning. On admission, he showed paralysis of all limbs, motor dysfunction due to joint contractures, pain, and higher brain dysfunction. These symptoms were consistent with magnetic resonance imaging (MRI) findings that the frontal white matter was mainly affected while the involvement in the basal ganglion was mild. Rehabilitation was continued, and his motor dysfunction was improved in a few months, although the higher brain dysfunction needed a longer term to improve. An interruption of his rehabilitation resulted in the exacerbation of the motor dysfunction. Subsequent complications of CO poisoning have been reported to be linked to the areas of the involvement revealed by MRI. Since CO poisoning can show various symptoms, a diversity of rehabilitation is required, according to the patients' symptoms. We think that rehabilitation is the most effective therapy for the subsequent complications of CO poisoning. Since the symptoms require a long time to improve, it is difficult to deal with them within the application of the existing health insurance and nursing care support regulations. We consider that rehabilitation for motor dysfunction and higher brain dysfunction are important, in addition to hyperbaric oxygen therapy in the acute stage of CO poisoning.
