This paper provides a comprehensive review of the antibacterial properties of three different types of Malaysian honey, namely kelulut, tualang and acacia, in preventing wound infecting bacteria. The antibacterial activity of these honey is mainly attributed to the physicochemical and phytochemical, which have been shown to be effective against a broad range of bacterial pathogens. This review discusses the effectiveness of honey in inhibiting the growth of various wound-infecting bacteria, factors that contributed to the antibacterial properties of the honey, mechanisms of action of honey in inhibiting bacterial growth and their potential for future use in clinical practice. The findings of this review suggest that Malaysian honey has the potential to be used as an alternative and complementary therapy to conventional antibiotics in the prevention and treatment of wound infections.

Mullerian agenesis or Mayer-Rokitansky-Kuster-Hauser Syndrome (MRKH) Type-II is a
congenital defect in the Mullerian duct that results in the absence of a uterus in women. The
aetiology of this syndrome is unknown and has been considered a sporadic genetic disease.
MRKH, together with anorectal anomaly, is an extremely rare condition and has only been
reported in a few cases without any information on genetic analysis. This study investigated the mutational profile of a girl diagnosed with MRKH and anorectal anomalies with
rectovaginal fistula. The whole exome sequencing (WES) trio-genetic analysis of a 5-year-old
Malaysian girl diagnosed with MRKH (having anorectal anomaly with rectovaginal fistula)
was performed together with her normal parents, using the Ion AmpliSeq Exome RDY kit
(ThermoFisher Scientific, USA). Data were analysed using Torrent Suite v.5.0.4 and annotated
using ANNOVAR. Single nucleotide polymorphisms (SNPs) with an allele frequency >0.01
were excluded, and the remaining variants were filtered based on de novo mutations,
autosomal recessive, and autosomal recessive genetic traits. Related genes were analysed by
biological pathway analysis (g:Profiler) and protein-protein interaction (HIPPIE v.2.3,
STRING v.11.5, dan GeneMANIA). A total of 36 mutations were identified, and two of them,
the LHX5 (p.P358Q), inherited from the father, and CFTR (p.R1158X), inherited from the
mother. There were 28 de-novo mutations from 28 genes. All genes were involved in 27
biological processes that connected with 23 interactions, and are likely to cause MRKH
syndrome in this patient.

Teratomatous tumours of the head are rather uncommon. We report a 3-month-old child who presented with acute
signs of raised intracranial pressure, needing craniectomy and subtotal tumour removal. The patient was diagnosed
as intracranial immature teratoma grade 3, from the pathological study and elevated alpha-fetoprotein (AFP). Managing brain teratoma posed a challenge to the managing team due to the location of the tumour, the unavailability
of standardized chemotherapy protocol and the dilemma of commencing adjuvant chemotherapy in a very young
infant. The tumour was partially removed due to its critical location. And chemotherapy was delayed until the patient
achieved 7 months of age. After four rounds of chemotherapy, the patient remained in remission for more than three
years follow up.
5.Prevalence of Ultrasound-diagnosed Non Alcoholic Fatty Liver Disease Among Rural Indigenous Population in Malaysian and Its Association With Biochemical and Anthropometric Measures

Introduction: Non-alcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver disease worldwide, especially in the Western world and Asia-Pacific regions. This study was designed to determine the prevalence of NAFLD detected by sonographic assessment among the rural indigenous population in Peninsula Malaysia and its association with anthropometric and biochemical factors. Methods: A cross-sectional survey was carried out from January 2014-February 2016. Subjects were recruited among indigenous peninsula Malaysia population in rural villages, aged ≥18 years old. The survey was questionnaire-based followed by anthropometric and blood parameters measurements. All subjects underwent abdominal ultrasound assessment to screen for the presence of NAFLD. Semi-quantitative visual grading was performed to assess for mild, moderate or severe NAFLD. Results: A total of 270 subjects underwent the screening program (mean age 43.3 ±14.0 years). Approximately 53 subjects (19.6 %) were identified with NAFLD. Of those with NAFLD, approximately 83% had moderate grade of fatty liver and the remainder were diagnosed with mild grade. NAFLD was closely associated with age, body mass index (BMI), central obesity, hypertension, total cholesterol (TC), triglyceride/high density lipoprotein-cholesterol (TG/HDL-C) ratio. On multivariate logistic regression, a high BMI (≥23.0 kg/m2), central obesity, and raised TG/HDL-C ratio were independent risk factors for developing NAFLD. Conclusion: This pioneer study defines the prevalence of NAFLD among rural indigenous population in Peninsula Malaysia. Lifestyle-related diseases, such as NAFLD can affect both rural and urban communities with equal severity. High BMI, central obesity, and elevated TG/HDL-C ratio were independent risk factors for developing NAFLD.
6.Impact of Tibial Tubercle Osteotomy on Final Outcome in Revision Total Knee Arthroplasty:Our Experience and Technique in Pakistan

Background:
Due to extensive fibrosis during revision surgery, adequate exposure is essential and it can be achieved with several extensile approach options, such as tibial tubercle osteotomy. Information regarding surgical exposure during revision arthroplasty is limited in developing countries, such as Pakistan, due to the lack of adequate data collection and follow-up. Therefore, the purpose of this study was to evaluate the impact of tibial tubercle osteotomy on final outcome of revision total knee arthroplasty (TKA).
Methods:
A total of 231 revision TKAs were performed between January 2008 and December 2017. Twenty-nine patients underwent tibial tubercle osteotomy for adequate exposure during revision surgery. Of these, 27 patients with complete follow-up were included in our study. Factors examined include age at the time of revision surgery, gender, comorbidities, arthroplasty site (right or left), body mass index (BMI), and primary indications for the tibial tubercle osteotomy during revision TKA. Functional outcome was measured by using Knee Society score (KSS) at 3 months and the final follow-up. All statistical analysis was done using SPSS version 20.0 with a p-value < 0.05 considered significant.
Results:
Out of 27 patients, 6 patients (22.2%) were men and 21 patients (77.7%) were women. Right knee revision arthroplasty was performed in 15 patients (55.5%), left knee revision arthroplasty was performed in 12 patients (44.4%), and bilateral revision surgery was performed in only 1 patient (3.7%). The mean BMI was 29.2 kg/m 2 . We used a constrained condylar knee in 20 patients (74%), a rotating hinge knee in 5 patients (18.5%), and mobile bearing tray plus metaphyseal sleeves in 2 patients (7.4%).The KSS was 52.21 ± 4.05 preoperatively, and 79.42 ± 2.2 and 80.12 ± 1.33 at 3 months and 12 months, respectively. Radiological union was achieved in all patients at 3 months. Of 27 patients, only 1 patient (3.7%) had proximal migration of the osteotomy site at 6 months: the patient was asymptomatic and union was also achieved and, therefore, no surgical intervention was performed.
Conclusions
Tibial tubercle osteotomy during revision TKA can be a safe and reliable technique with superior outcomes and minimal complication rates.
7.Pathogenic Vibrio spp. identified for white syndrome coral disease in Tioman Island Marine Park, Malaysia

Aims:
Coral diseases have emerged over the last several decades, causing a loss of live coral cover in the Caribbean
and Indo-Pacific reefs. Hence, microbiological and disease cultural techniques are commonly used to investigate their
causative microbial agents. This is the first study to identify the potential of pathogenic Vibrio spp. isolated from
apparently white syndrome (WS) coral disease in Tioman Island Marine Park using biochemical and molecular
techniques.
Methodology and results:
The Vibrio colonies were isolated from 108 samples of WS infected corals (Acropora
cytherea and Montipora aequituberculata) including seawater, sediment and algae found adjacent to infected coral
colonies. A total of one hundred representative Vibrio isolates were characterized and most of them (n=50) were
identified as V. vulnificus, V. alginolyticus and Photobacterium damselae following biochemical analysis. The molecular
analysis revealed six Vibrio spp. (V. coralliilyticus, V. hepatarius, V. brasiliensis, V. tubiashi, V. campbellii, V.
ishigakensis) and one Photobacterium rosenbergii. Vibrio coralliilyticus isolated from all infected coral samples may be
highly responsible for the sign of WS disease.
Conclusion, significance and impact of study
The findings of this study provide baseline data and information on
potential coral pathogens identified in the coastal waters of Tioman Island. Etiological disease study is suggested to
validate their severity and virulence factors in the future.

8.Linezolid-resistant Enterococcus casseliflavus and Enterococcus gallinarum isolated from poultry farms in Kelantan, Malaysia

Aims:
Linezolid has become a decisive therapy in treating infections with vancomycin-resistant Enterococcus (VRE).
Currently, the emergence of linezolid-resistant Enterococcus further complicates the therapeutic options and leads to
global health threat not only in hospital setting but in the community. The study aimed at antimicrobial pattern of
Enterococcus isolated from 6 poultry farms in Kelantan, Malaysia.
Methodology and results:
Between February and December 2019, 300 broiler cloacal swab sample (Gallus gallus
domesticus) were collected and screened for linezolid-resistant enterococci (LRE) using a standard biochemical and
antimicrobial susceptibility tests. Among all the samples, 32.3% (n=97/300) grew Enterococcus, 71.1% (n=69/97) of it
were identified Enterococcus casseliflavus by molecular identification, whilst remaining isolates 28.9% (n=28/97) were
further identified as Enterococcus gallinarum by 16S rRNA sequencing. None of the isolates were found to exhibit high-level resistance to vancomycin. However, 3/97 (3.1%) were exhibit resistance to high-level gentamicin based on Kirby-Bauer disk diffusion test. Whereas 48/97 (49.5%) of isolates were observed to be resistant to ampicillin, 28/97 (28.9%)
were resistant to penicillin. Surprisingly, among the two strains isolated, 18.6% (n=18/97) of it were resistant to linezolid.
Isolates showed resistance to linezolid by disk diffusion test were verified by VITEK-2 automated system (bioMérieux,
USA) with MIC ≥8 µg/mL. All antimicrobial susceptibility test and minimal inhibitory concentration (MIC) results were
interpreted according to Clinical and Laboratory Standard Institute (CLSI).
Conclusion, significance and impact of study
In conclusion, this study has reported the prevalence of linezolid
resistant Enterococcus (LRE) in highly intrinsic antibiotic resistant of E. casseliflavus and E. gallinarum in Malaysia
poultry farms, alongside with the truancy of vanA strains. The emergence of LRE strains is an alarming problem to the
animal husbandry and healthcare setting worldwide. This could lead to potentially untreatable and life-threatening
enterococcal infections. Even more worrying is the spread of LRE to geographical regions where these strains were
previously unreported, which may pose a global health threat. Antimicrobial surveillance in poultry husbandry is thus,
dimly necessary to prevent wide spread of multidrug-resistant bacteria.

9.Prevalence Of Type 2 Diabetes Mellitus And Its Association With Socioeconomic Status In Kandahar City: A Cross-Sectional Study

Diabetes mellitus is a chronic disease with a life long duration and generally of gradual progression. Beside the upward trend in the prevalence rate, diabetes and its complications also have a significant economic impact on countries and their health systems. This study was aimed to assess the prevalence of type 2 diabetes mellitus (T2DM) and its association with socioeconomic status (SES) in the urban population of Kandahar city. A population-based cross-sectional study was conducted from January 2019 to May 2019 in the urban area of Kandahar province. A total of 1308 subjects, 837 (64%) women and 471 (36%) men, were included in the study. Data on sociodemographic characteristics, medical history, dietary intake and physical activity were collected in a standardized questionnaire. Diagnosis of diabetes was established according to the WHO criteria. Mean age (SD) was significantly higher in men than in women (50.3 (14.4) vs. 44.9 (11.4) years). The difference in BMI was not significant between men and women (28.1 vs. 28.7 kg/m2). Majority of the study population was aged 40-54 years (46.3%), married (86.7%), illiterate (87.4%), and obese (39%). 6.6% were tobacco smokers, higher in men (13.6%) than in women (2.6%). The prevalence of diabetes mellitus, hypertension, obesity and central obesity was 15.2%, 25.5%, 39%, and 67.1%, respectively. In this study population, it could be suggested that socioeconomic factors are associated with T2DM and socioeconomic status is worth further investigation nationwide to properly understand the role of SES.
