1.Single-cell spatial profiling reveals immune-steroidogenic crosstalk in adrenals of patients with primary aldosteronism

Introduction:
Primary aldosteronism (PA), most commonly caused by aldosterone-producing adenomas (APAs), represents the leading
form of surgically curable secondary hypertension. While genomic studies have elucidated the mutational landscape
of APAs, the spatial organisation and functional role of immune populations across APAs, aldosterone-producing
micronodules (APMs), and adjacent adrenal cortex remain poorly defined at single-cell resolution.
Methodology:
Single-cell RNA sequencing (scRNA-seq) was integrated with spatial transcriptomics in APAs and paired adjacent adrenal
cortex, complemented by immunohistochemical (IHC) validation. Immune populations were spatially mapped using
canonical markers (CD14, CD68, CD163, HLA-DR, CD8A, and CD4) across defined adrenocortical regions.
Results:
The adrenal microenvironment in PA demonstrates structured immune organisation rather than passive infiltration.
CD14+ monocyte-lineage cells localize intraparenchymally within APAs (n = 10), intercalating between CYP11B2+
aldosterone-producing cells and forming a pattern distinct from perivascular immune niches. scRNA-seq further
identified a transcriptionally distinct CD14+ population within the zona reticularis (zR) that co-expresses steroidogenic
markers (CYB5A, SULT2A1, TSPAN12) while lacking canonical monocyte transcripts. IHC supported this observation,
demonstrating CD14 expression within adrenocortical zR parenchymal cells (n = 5). In parallel, CD4 and HLA-DRA
exhibited diffuse cytoplasmic staining within zR parenchymal cells in the absence of classical macrophage marker coexpression (CD68), suggesting non-canonical or context-dependent expression within steroidogenic compartments.
CD68+ and CD163+ macrophages were sparsely distributed across APA, APM and adjacent cortex, consistent with lowdensity tissue-resident populations, while CD8A+ cytotoxic lymphocytes were enriched in APAs and APMs with diffuse
parenchymal cytoplasmic staining of CD8A additionally observed within the zR.
Conclusion
These findings reveal a previously unrecognized spatially organised immune-steroidogenic interface within the adrenal
cortex. The presence of immune-associated transcriptional and protein signatures within zR cells suggests potential
functional plasticity of steroidogenic cells, possibly extending to antigen presentation-related pathways, warranting
further mechanistic investigation

2.Global and regional cardiac magnetic resonance feature tracking left ventricular strain analysis in assessing early myocardial disease in β thalassemia major patients

Background:
Cardiac magnetic resonance imaging (CMR) is the modality of choice for quantification of myocardial iron overload in β-thalassemia major patients using the T2* sequence. CMR feature tracking (FT) is a recent magnetic resonance imaging tool that gives an idea about myocardial fibers deformation; thus, it can detect early impairment in myocardial function even before the reduction in ejection fraction.
Methods:
This study aims to assess the ability of left ventricular CMR-FT in the early detection of systolic dysfunction in β thalassemia major patients and to correlate it with the degree of myocardial iron overload measured by CMR T2*.This prospective study enrolled 57 β thalassemia major patients who received long-term blood transfusion and 20 healthy controls. CMR was used to evaluate left ventricular volumes, ejection fraction, and the amount of myocardial T2*. A two-dimensional left ventricular FT analysis was performed. Both global and segmental left ventricular strain values were obtained.
Results:
The mean global circumferential strain (GCS) and global radial strain (GRS) values were significantly lower in patients compared to control (P = 0.002 and P = 0.006, respectively). No correlation was found between T2* values and ejection fraction; however, there was a significant correlation between T2* values and GCS and GRS (P = 0.012 and P = 0.025, respectively) in thalassemia patients. Regional strain revealed significantly lower values of GCS and GRS in basal regions compared to apical ones (P = 0.000).
Conclusions
Our study revealed that CMR-FT can play a role in the early detection of systolic impairment in thalas‑ semia patients.
3.Agmatine ameliorates diabetes type 2-induced nephropathy in rats

Objective: To assess the nephroprotective potential of agmatine in a rat model of streptozotocin-induced diabetic nephropathy. Methods: A single dose of streptozotocin (40 mg/kg) coupled with a fructose diet induced diabetes in Wistar rats. Agmatine (40 and 80 mg/kg) was administered to rats for 12 weeks. The body weight and fasting blood glucose were measured weekly. Insulin level, urine output, total protein, albumin, blood urea nitrogen, creatinine, and cystatin-C were also determined at the end of the experiment. Furthermore, superoxide dismutase, glutathione, interleukin-1β, interleukin-6, and tumor necrosis factor-alpha were evaluated in kidney tissue. Histopathological study was also performed using hematoxylin and eosin staining. Results: Agmatine at both doses significantly increased final body weight, and lowered fasting blood glucose, urine output, insulin, total protein, albumin, blood urea nitrogen, creatinine, and cystatin-C levels compared with the diabetic group (P < 0.05). Inflammatory markers and antioxidant effect were significantly improved in agmatine-treated rats. Moreover, the histopathological changes in renal structure were ameliorated by agmatine treatment. Conclusions: Agmatine alleviates diabetic nephropathy by improving renal functions and reducing inflammation and oxidative stress. The molecular mechanisms of its nephroprotective actions need to be investigated in future study.

eToyBox is a learning management system for preschool teachers to improve their health literacy, which ultimately aims
to improve children’s obesity-related behaviour. As part of the development process of eToyBox, assessment on digital
literacy, acceptance of digitization of education materials, and perceived barriers in adopting online learning is needed.
Fifty-four preschool teachers under the Community Development Department (KEMAS) in Kuala Lumpur, Selangor,
and Sarawak, who participated in ToyBox Study Malaysia intervention in 2018, took part in this cross-sectional study.
An online self-administered questionnaire was used to assess sociodemographic background, use of communication
tools and media, and teacher’s views on adapting the ToyBox modules to digital education materials. Respondents were
contacted, and questionnaire link was shared through WhatsApp messages. Most participants (74.0%) were Malay
females aged 31 to 40 years old. Most participants had internet access (94.4%) and owned at least a smart phone,
laptop or tablet (94.4%). Participants perceived their computer skills to be average (75.0%). Majority of respondents
(65.0%) reported advanced and higher abilities in word processing and email, but only 22.0% in spreadsheet skills. The
main barrier to accessing online material was unstable internet connection (74.1%). Most respondents (90.0%) agree
that adapting effective modules to online learning will be beneficial for professional development and teaching practices.
In conclusion, most participants supported digitizing Toybox Study Malaysia educational content and were comfortable

with its implementation via an online learning platform. The findings from this study can advise future development of
online learning materials for preschool teachers in Malaysia.
5.CASE REPORT - A malnourished post-stroke man with multi-morbidity and sarcopenia risk in a long-term stroke clinic: A case report

A 78-year-old post-stroke man with multiple comorbidities who was activity of daily living-dependent developed aspiration pneumonia associated with nasogastric tube (NGT) blockage. He presented with malnutrition and risk of sarcopenia with hypoalbuminaemia, small calf circumference (CC), low body mass index and small mid upper arm circumference. He showed symptoms of moderate-to-severe vascular dementia with behavioural psychological stress disorder, resulting in carer stress. Psychoeducation among the carers and referral to a neuro-psychiatrist were ensued after outpatient-based team meeting discussion. Herein, we highlight the importance of screening for sarcopenia and nutritional status in post-stroke patients with the use of the CC and serum albumin level as well as the involvement of a multidisciplinary team in the primary care setting to improve patient outcomes. Percutaneous endoscopic gastrostomy tubes are more suitable than NGTs for post-stroke patients who require enteral feeding to improve the nutritional status.

6.Pectoral nerve blocks for transvenous subpectoral pacemaker insertion in children: a randomized controlled study

Background:
Postoperative pain management after pacemaker insertion routinely requires opioid agents, nonsteroidal anti-inflammatory drugs, or paracetamol. However, interest in opioid-sparing multimodal pain management to minimize postoperative narcotic use has increased recently. This study aimed to assess the pectoral nerve (PECS) block versus standard treatment on postoperative pain control and opioid consumption in pediatric patients after transvenous subpectoral pacemaker insertion.Method: In this randomized controlled study, 40 pediatric patients underwent transvenous subpectoral pacemaker insertion with either congenital or postoperative complete heart block. Patients were randomly assigned to two groups: Group C (control) received conventional analgesic care without any block and Group P (pectoral) received a PECS block. Demographics, procedural variables, postoperative pain, and postoperative opioid consumption were compared between the two groups.
Results:
In children undergoing transvenous subpectoral pacemaker insertion, the PECS block was associated with a longer procedure time; however, the cumulative dose of fentanyl and atracurium was reduced and the hemodynamic profile was superior in Group P compared with Group C intraoperatively. Postoperatively, the PECS block was associated with lower postprocedural pain scores, which was reflected by the longer interval before the first call for rescue analgesia and lower postoperative morphine consumption, without an increase in the rate of complications.
Conclusion
Ultrasound-guided PECS blocks are associated with a good intraoperative hemodynamic profile, reduced postoperative pain scores, and lower total opioid consumption in children undergoing transvenous subpectoral pacemaker placement.
7.Acceptance and effectiveness of the Healthier Choice Logo (HCL) among food industries in Malaysia

Introduction: The Healthier Choice Logo (HCL) was introduced in 2017 by the Ministry of Health Malaysia. This paper analysed acceptance of HCL, effectiveness of HCL in encouraging healthier product reformulation, and factors affecting reformulation among food industries. Methods: An online self-administered questionnaire consisting of four sections utilising multiple choice and 5-point Likert scale questions was distributed to food industries in Malaysia. Sample size calculation yielded 100 respondents. Results: Food industries had a higher acceptance of the processes and requirements involved in HCL implementation. HCL was highly effective in encouraging product reformulation among food industries in Malaysia. Meeting consumer demand, improving brand image, public health, more awareness around nutrition labelling, logo and national nutrition target, more technical knowledge and budget were found to motivate healthier product reformulation. However, product suitability, consumer acceptability, difficulties maintaining taste and shelf life, and limited budget were the challenges faced in product reformulation. There was no correlation between HCL acceptance and factors encouraging or inhibiting reformulation. Conclusion: These findings are expected to help relevant authorities or stakeholders make changes, if necessary, towards processes and requirements involved in HCL application to ensure wider HCL implementation. Future research should identify the relationship between HCL implementation and public health improvement among the Malaysian population.
8.Oncolytic effect of Newcastle disease virus (NDV) strain AF2240 and V4-UPM on hypoxic and normoxic osteosarcoma cells

Aims:
Hypoxia is believed to be one of the key components contributing to the clinical resistance of cancer therapies. Alternative strategies are under investigation to overcome this resistance and the oncolytic virus stands amongst the others. Newcastle disease virus (NDV) has been demonstrated to possess oncolytic activity against cancer cells. The present study investigated the effects of oncolytic NDV strain AF2240 and V4-UPM on osteosarcoma cells (Saos-2) under normoxic and hypoxic conditions.
Methodology and results:
Results showed that the NDV strain AF2240 and V4-UPM could infect and kill normoxic and hypoxic Saos-2 cells equally well by inducing hypoxia-independent apoptosis, and S-phase cell cycle arrest under the microscopy examination, cell viability assay, Annexin V apoptosis assay and cell cycle analysis experiments. However, the Velogenic NDV strain AF2240 excelled over the lentogenic NDV V4-UPM with increased oncolytic effects in Saos-2 cells.
Conclusion, significance and impact of study
In a nutshell, normoxia or hypoxia microenvironment has little effect on NDV-induced oncolysis of Saos-2 cancer cells which poses as a potential agent for the treatment of resistant cancer.
9.Bacterial contamination from selected catfish pond farm and its susceptibility to antimicrobial agents

Aims:
The aquaculture industry has been identified as one of the sources for the emergence of antimicrobial resistant bacteria. This study aimed to determine the level of bacterial contamination from selected catfish pond farming at Jasin, Melaka and its susceptibility to antimicrobial agents. The correlation between the sampling points’ Water Quality Index and the level of bacterial contamination was also determined.
Methodology and results :
Sampling was carried out at six sampling points of the river as well as selected catfish pond farming. Microbial counts were assessed using the membrane filtration and isolates were further identified by using the polymerase chain reaction (PCR) method. Antimicrobial susceptibility tests were carried out using the disk diffusion method. Results showed that the total bacterial count obtained was not significantly different (P>0.05) between sampling points with an average of 4.47 ± 0.41 log CFU/mL. A total of 27 different bacterial species were identified where 63% were identified as Gram-negative bacteria. A medium negative correlation (r=-6.16, P<0.01) was found between the sampling points’ Water Quality Index and the level of bacterial contamination. Among bacterial species identified, 18 species are pathogenic strains.
Conclusion, significance and impact of study
Antibiotic susceptibility test indicated that only 4 isolates showed resistance to antimicrobials tested and the study shows the potential cross contamination between fishponds and Kesang River, Melaka. Thus, public health concerns are tangible, and prevention of the cross contamination is crucial for the farm.
10.The Prevalence of Online Natural Health Products Purchase: A Systematic Review

Aim and design: This systematic review aimed to estimate the prevalence of online natural health products (NHPs)
purchases among consumers. Data Sources: Four databases (PubMed, Science Direct, Scopus and Web of Sciences)
were searched for articles published up to July 1, 2021. Review Method: Studies included were those reporting the
prevalence of online NHPs purchases, those excluded were case reports, commentaries, letters, editorials, review
articles, theses and non-English studies. The risk of bias of selected studies was assessed using the Joanna Briggs Institute’s checklist, and the pooled prevalence of online NHPs purchases was generated using the random-effect model.
Results: A total of 30 studies were included in the meta-analysis, representing a total of 40,535 respondents. The
pooled prevalence of online NHPs purchases was estimated at 7.60% (95% CI: 5.49, 10.01). Prevalence was higher
in studies conducted in recent years, among physically active populations and when sports nutrition was included
within the scope of NHPs. More than half of the selected studies have a moderate risk of bias, and considerable
heterogeneity was observed across the selected studies. Conclusion and Impact: Online purchases of NHPs are not
common among consumers, but they have become more common in recent years. A wide range of NHPs were investigated across studies, which may have contributed to the considerable heterogeneity reported in this review. It is
suggested that future studies investigating online NHPs purchases consider reporting individual prevalence statistics
specific to each NHP type, to facilitate meaningful comparisons between studies.
