1.Design and application of a mobile treatment vehicle for lower limb trauma patients

With the development of the economy in our country,trauma has already become a major medical burden in society,traffic injury has already become a major type of trauma,and the trauma of lower limbs is relatively much more.Since patients with lower extremity trauma have limited mobility,postoperative dressing changes need to be done at the bedside.At present,in the process of lower limb dressing change,it is necessary for family members or medical staff to help elevate the affected limb,which is unstable and difficult to adhere to.The dressing change process not only increases the pain of patients,but also affects the sight line and comfort of medical staff during operation,resulting in incomplete wound exposure and affecting the effect of dressing change.The washing solution and disinfectant in the dressing change process are easy to contaminate the sheets,which increases the difficulty of ward management.This dressing change method is unscientific,irregular and inconvenient to operate.Therefore,the medical staff of department of emergency of the First Hospital of Jiaxing City developed a mobile treatment vehicle for leg care for debridement and dressing change of patients with lower limb trauma,this device has been granted the National Utility Model Patent of China(patent numbeer:ZL 2021 2 0647636.0),which makes bedside debridement and dressing change more scientific,convenient and normative.The utility model relates to a mobile treatment vehicle for leg care,which is composed of a base,a support seat,a leg support plate,a support rod,a fixed plate and a smart trash can.The angle of the leg support plate can be adjusted 360 °by loosening the bolt,and the height can be adjusted by sliding up and down.Adequate rotation flexibility and high adjustment space can effectively solve problems of bedside debridement and dressing change for patients with lower limb trauma,provide scientific,simple and operable auxiliary tools for trauma medical staff,improve the comfort of patients and operators,and improve the quality of ward management in emergency trauma ward.

3.Prevalence and outcome of chronic hypertension and its complications with gestational diabetes

Objective:With the full liberalization of China's fertility policy,the gradual increase in maternal age during pregnancy,and the rising proportion of overweight and obesity among women of childbearing age,the number of pregnant women with chronic hypertension(CHTN)combined with gestational diabetes mellitus(GDM)is increasing,leading to a significantly increased risk of adverse pregnancy outcomes.This study aims to analyze the prevalence of CHTN and CHTN complications with GDM,and compare the adverse pregnancy outcomes between the 2 conditions,providing a basis for intervention measures. Methods:This study was a prospective cohort study.A total of 378 366 cases from a large cohort of pregnant women between January 1,2016 to December 31,2020 were screened to identify 1 418 cases of pregnant women with CHTN,among which 1 027 were cases of CHTN alone and 391 were cases of CHTN combined with GDM.SAS9.4 was used to statistically analyze the basic characteristics,clinical data,and pregnant outcomes of pregnant women and to analyze the risk factors affecting the pregnancy outcomes of patients with CHTN and its complications with GDM. Results:The prevalence rate of CHTN with pregnancy was 3.8‰,and the prevalence rate of CHTN combined with GDM was 1.0‰.Patients with CHTN combined with GDM accounted for 27.57%(391/1 418)of all pregnant women with CHTN.Maternal age,number of pregnancies,parity,previous cesarean section,systolic blood pressure,diastolic blood pressure,and mean arterial pressure at the time of enrollment were statistically significant differences between the 2 groups(all P<0.05).After adjusting for potential confounding factors such as maternal age,parity,and number of pregnancies,binary Logistic regression analysis showed that pregnant women with CHTN combined with GDM had a 1.348 times higher risk of cesarean section(OR=1.348,95%CI 1.043 to 1.741),a 2.029 times higher risk of placental adhesion(OR=2.029,95%CI 1.190 to 3.462),a 1.540 times higher risk of preeclampsia(OR=1.540,95%CI 1.101 to 2.152),and a 2.670 times higher risk of macrosomia(OR=2.670,95%CI 1.398 to 5.100)compared to pregnant women with CHTN alone. Conclusion:Pregnant women with CHTN combined with GDM have a high risk,and their pregnancy outcomes differ from those of pregnant women with CHTN alone in terms of cesarean section,placental adhesion,preeclampsia,and macrosomia.Prenatal care for this population,especially the management of blood pressure and blood sugar,needs to be given special attention.
4.Safety evaluation of thymectomy in elderly patients aged 65 years and over

Objective:To evaluate the safety of thymic surgery in patients aged 65 years and over.Methods:A total of 696 patients who underwent thymectomy/thymoma resection in the Department of Thoracic Surgery of Beijing Hospital from November 2011 to March 2022 were collected and divided into two groups according to the age of 65 years old.The preoperative course of disease, MG stage, dosage of pyridostigmine bromide, American College of Anesthesiologists(ASA)score, surgical method, intraoperative bleeding, postoperative drainage, postoperative complications, Clavien-dindo score(CDC), and myasthenic crisis were recorded and statistically analyzed.Results:A total of 696 patients were enrolled, including 364 males and 332 females, aged 15~86 years, with an average age of 49.1 years.There were 309 patients with thymoma, 565 patients with MG, and 178 patients with both.There were 124 cases in the elderly group(≥65 years old)and 572 cases in the non-elderly group(<65 years old). The incidence of thymoma was higher in the elderly group(54.8 % versus 42.1 %, χ2=6.664, P=0.010), while the incidence of MG was lower(67.7 % versus 84.1 %, χ2=17.827, P<0.001). The ASA score of the elderly group was higher than that of the non-elderly group( χ2=52.372, P=0.000), and the preoperative ventilation function FEV1 and FEV1/FVC were also significantly lower than those of the non-elderly group( z=8.187, 4.580, P=0.000 for all). The drainage volume in the first 3 days after operation and postoperative drainage tube time in the elderly group were significantly higher than those in the non-elderly group( P=0.018, P=0.003). The incidence of postoperative myasthenia crisis in the elderly group was higher than that in the non-elderly group( P=0.034). There was no significant difference in the incidence of postoperative complications between the two groups, but after Clavien-dindo classification, the score of the elderly group was higher than that of the non-elderly group( P=0.003). Although the ASA score and Clavien-dindo score of the elderly group were both higher than those of the non-elderly group, there was no correlation between the two. Conclusions:Although the preoperative ASA score and pulmonary function of elderly patients were poorer than those in the non-elderly group, while the incidence of postoperative myasthenia crisis was higher, and the incidence of postoperative complications was not higher, the Clavien-dindo classification, however, was higher in elderly patients than that of the non-elderly group.After careful preoperative evaluation and strengthening perioperative management, most elderly patients can receive thymus surgery safely with acceptable risks.
5.Clinical characteristics in elderly patients with thymic epithelial tumors and prognostic analysis of more than 3 years postoperative follow-up

Objective:To evaluate the clinical characteristics and the surgical safety in patients aged 65 years and over with thymic epithelial tumor, and analyze the prognosis of 3-10 years postoperative tumor and myasthenia gravy.Methods:A total of 228 patients diagnosed as thymic tumor and undergoing surgical surgery to remove the tumor in Beijing Hospital from Jan.2011 to Dec.2018 were retrospectively enrolled.Patients were divided into a young(≤ 65 yrs)and old(>65 yrs)groups.The operation time, intra-operative bleeding, drainage volume in the first 3 days after operation, days with drainage tube after operation, postoperative days of hospital stays, the diameter of the tumors, pathological classification, Tumor-Node-Metastasis(TNM)staging, Masaoka-Koga staging, whether or not complicated with myasthenia gravis and complications were compared between two groups.The patients were followed up by outpatient or telephone, and recurrence of thymoma, survival status and improvement of myasthenia gravis were tracked.Results:There were significant differences in pathological classification between the two groups( P=0.002). The postoperative days with drainage tube were longer in patients≥65 years old than in patients<65 years old[4(2-17)days and 3(1-9), Z=2.316, P=0.021]. Thymic atrophy was more common in patients ≥ 65 years old than in patients <65 years old(10.2% and 1.7%, χ2=5.937, P=0.015). Incidence of thymoma plus myasthenia gravis were higher in patients aged <65 years than those aged ≥65years(68.2% vs.40.8%, χ2=12.240, P<0.001), and incidence of thymic hyperplasia were higher in patients aged <65 years than those aged ≥65years(58.1% and 38.8%, χ2=2.316, P=0.016). The recurrence of thymoma was a poor prognostic factor affecting the survival of patients.Meanwhile, Masaoka-Koga stage Ⅲ and Ⅳ and TNM stage Ⅲ and Ⅳ were independent risk factors for postoperative recurrence of thymoma. Conclusions:Thymectomy is safe and effective in the patients aged 65 and over, and may have a better long-term prognosis.
6.Path analysis of the influence of professional values and self-concept of nursing professional on academic burnout of nursing students

Objective:To investigate the academic burnout level of nursing interns, and to analyze the influence path of nursing professional self-concept and professional values on the academic burnout of nursing interns.Methods:From September to December 2021, 204 nursing students who practiced for 6 months in the First Affiliated Hospital of Anhui Medical University were surveyed using the College Student Learning Burnout Scale, the Nursing Professional Self-Concept Scale and the Nursing Professional Values Scale. Structural equation modeling using AMOS 23.0.Results:The total score of academic burnout was (52.37 ± 11.60), which was in the middle level. The results of correlation analysis showed that the nurses' professional values were negatively correlated with depression, misbehavior, and low sense of achievement ( r=-0.253, -0.256, -0.406), and the differences were statistically significant ( P<0.01). Nursing professional self-concept was negatively correlated with depression, misbehavior, and low sense of achievement ( r=-0.466, -0.414, -0.525), and the differences were statistically significant ( P<0.01). Intern nursing students' nursing occupational self-concept had a partial mediating effect on their professional values and academic burnout, and the mediating effect accounted for 48.57% of the total effect. Conclusions:Professional values of nursing students can directly affect academic burnout, and also indirectly affect them through nursing professional self-concept.
7.Dyschesia and changes of pubo-rectal angle as observed by 3D ultrasound

Objective To discuss the relativity of dyschesia with the change of puborectalismuscle.Methods 68 patients with dyschesia were compared with 68 healthy volunteers at lithotomy position undergoing 3D ultrasonography on resting,contracting and maximum exertion phase respectively.Three dimensionally reconstructed images were reconstructed and pubo-rectal angle and the thickness of puborectalis muscle at 6 o'clock position were measured.Results The pubo-rectal angle of the study group and control group had no significant differences at either resting [(86 ± 8) ° vs.(86 ± 8)°] or contracting phases [(88 ± 9) ° vs.(86 ± 7) °] (t =-0.145,t =0.434,P > 0.05).While at maximum exertion the differences were significant [(80 ± 6) °vs.(95 ± 5) °,t =-5.397,P < 0.05].The d-value of pubo-rectal angle between maximum and resting exertion statistically different [(6 ± 3) °,(-9 ± 7) °,t =5.551,P < 0.05].The thickness of puborectalis muscle between the two groups differed statistically significant only at maximum exertion phase [(4.60 ± 0.60) mm vs.(3.97 ± 0.32) mm,t =6.872,P < 0.05].The d-value of the thickness of puborectalis muscle between maximum and resting exertion were statistically different (t =-11.474,P < 0.05).Conclusions The pubo-rectal angle of the study group is smaller at maximum exertion than the control group.The thickness of puborectalis muscle at six o'clock (lithotomy position) in study group is larger at maximum exertion than the control group.And the severity of constipation changed with the variation of angle.
8.Sacral nerve stimulation for existence of c-kit positive cells of interstitial cells of Cajal in the subserosal layer of the STC rat

Objective:To evaluate the effect of sacral nerve stimulation (SNS) for STC by receptor tyrosine kinase c-kit.Methods:Wistar rats were fed with diphenoxylate to make slow transit constipation(STC) rat model.Sacral nerve stimulation(SNS) for the SNS Rat Group.The studied animals were allocated into three experimental groups:STC Rat Group;SNS Rat Group;Normal Rat Group;Every group included 10 rats.The c-kit of ICC in the subserosal layer of rats were analyzed by immunohistochemistry,western blot and RT-PCR.Results:The morphological characteristics of STC Rat Group were not comparable to those of the multipolar c-Kit positive ICC seen in the subserosa of colon of normal rat.In the colon of rat,c-kit protein and c-kit gene in SNS rat group and Normal rat group was significantly higher than STC rat group detecting by western blot.Statistical differences between STC Group and SNS Rat Group were found (P<0.05);statistical differences between STC Group and Normal Rat Group were also found (P<0.05).There was not statistical differences between SNS rat group and Normal Rat Group.Conclusion:SNS has effective treatment for the STC rat.
9.Expression of TFPI-2 gene and its promoter methylation in acute myeloid leukemia.

The aim of this study was to detect the mRNA expression of tissue factor pathway inhibitor-2 ( TFPI-2) and its methylation in bone marrow mononuclear cells from acute myeloid leukemia (AML) patients and to explore its significance in AML. Bone marrow mononuclear cells were isolated from newly diagnosed AML patients (n = 33), complete remission AML patients (n = 19), relapsed/refractory AML patients (n = 12) and iron deficiency anemia patients (control group, n = 15). Expression of TFPI-2 mRNA was detected with real-time quantitative PCR (RT-PCR) and the methylation of CpG island in its promoter was detected with methylation-specific PCR (MSP). The results showed that the expression of TFPI-2 mRNA in newly diagnosed AML, complete remission AML and relapsed/refractory AML patients was much lower than that in the controls (P < 0.05). Furthermore, its expression in relapsed/refractory AML patients was lower than that in newly diagnosed AML patients (P = 0.006). Compared with complete remission AML patients, the expression of TFPI-2 mRNA in newly diagnosed AML patients was significantly reduced (P = 0.030). The percentage of TFPI-2 promoter methylation in AML patients was 64.63% (42/64). In newly diagnosed AML group, complete remission AML group and relapsed/refractory AML group,the percentages of TFPI-2 promoter methylation were 66.67% (22/33), 52.63% (10/19) and 83.33% (10/12) (P > 0.05), respectively. The optical density ratio of TFPI-2 mRNA expression was 0.165 (0.005-2.099) in methylated AML patients, and 0.597 (0.011-2.787) in unmethylated AML patients (P < 0.05). Methylation of TFPI-2 gene promoter was not detected in control patients. After 2 courses of chemotherapy, the level of TFPI-2 mRNA was much higher in the CR group than that in the non-CR group (P < 0.05). It is concluded that the down-regulation or silence of TFPI-2 gene potentially results from its promoter methylation, and the expression level of TFPI-2 and the methylation status of its promoter may be used as indicators of risk stratification and evaluation of disease progress.

10.Relationship between reactive aggression, proactive aggression and parenting practices among preschool children

Objective To investigate the occurrence status of reactive aggression and proactive aggression among preschool children in Shanghai,and explore the relationship between children's aggressive behavior and family factors such as parental caregiving and supervision.Methods In this investigation,1 234 preschool children in 10 kindergartens of Shanghai were rated by their teachers and parents using the Aggressive Behavior-Teacher's Checklist,Parent Behavior Inventory (PBI) and a general questionnaire.General information and status of aggressive behavior of the children,environmental characteristics of the families and manners of parental involvement,including supervision and caregiving were described by frequency,percentage,mean value and standard deviation.The relations among family characteristics,parenting styles and children's reactive and proactive aggression were analyzed by t test,variance analysis and multiple linear regression analysis.Results Mean scores of reactive aggression and proactive aggression among preschool children were 2.12 ± 0.93 and 1.39 ± 0.68,respectively.The score of reactive aggression was correlated to the score of proactive aggression significantly (r =0.683,P < 0.01).Correlation analysis showed that significant positive correlation existed between the scores of hostile/coercive parenting practices and reactive aggression (r =0.163,0.123,P < 0.01) ; and significant negative correlation existed between the scores of supportive/participatory parenting practices and reactive and proactive aggression (r =-0.066,-0.076,P < 0.05 or P < 0.01).Linear regression analysis indicated that hostile/coercive parenting practices,boys,younger mothers and lower education level of mothers were risk factors for reactive aggression (P < 0.01),while hostile/coercive parenting practices,boys,younger mothers and lower education level of fathers were risk factors for proactive aggression in preschool children (P < 0.01).Conclusions Parenting practices may be closely related to the occurrence and types of preschool children's aggressive behavior.Therefore family intervention measures should be provided based on the type of aggression and its related risk factors.
